In the Health-Care system,the right of privacy is an important part for protecting benefits and the trust between doctors and patients.Most of the discussion about privacy in Taiwan is modeled after the West.They assume that every individual is exiting like an atom,and separate the individual from the community.In our society and culture,we look at the individual with a totally different attitude.This paper tries to introduce the western privacy theory and extends its meaning when applying into the relationship between doctors and patients.It analyses the problem of privacy in terms of the participation and status of family members in the signing of the surgical consent form and patient physician relationship and tries to make a comparative studies of the reasonableness feasibility of the exclusion of the participation of the family in the West.
2.Changes of biochemical indexes in the brain and spinal cord after shock and vibration damage

BACKGROUND: High-energy vibration is easy to damage non-cavity organs of the body and the damage effect is remarkable, but few studies concern the process of high-energy vibration-induced injury.
OBJECTIVE: To understand physiological, biochemical and pathological changes of animal bodies after high-energy vibration-induced injury.
METHODS: A total of 32 dogs were randomly divided into four groups. Dogs in the mild vibration injury group, moderate vibration injury group and severe vibration injury group received 700, 1 000, and 2 100 m/s2 vibration. Dogs in the control group were considered as normal controls. Within 14 days after vibration, serum K+, Ca2+, Zn2+, S100β, and neuron specific enolase concentrations were detected. Immunohistochemical staining was observed in the spinal cord and the brain.
RESULTS AND CONCLUSION: Serum K+, Ca2+, Zn2+concentrations showed a regular change in the three vibration injury group, and no evident change was found immediately after vibration injury. K+ concentrations reached a minimum at 0.5 day after vibration injury. Ca2+ concentrations reached a minimum at 1 day after vibration injury. Zn2+concentrations reached a minimum at 0.5 or 1 day. Above concentrations gradual y increased and became normal at 14 days. Serum neuron specific enolase and S100β expression increased at 0.5 day after vibration in the three vibration injury groups, peaked at 1 day, gradual y diminished, and recovered to a normal level or higher level at 14 days. In the three vibration groups, bleeding point of contact position and hedge position could be found in the spinal cord and brain. The degree of bleeding was more significant when kil ed instantly after vibration compared with that at 14 days. S100β, glial fibril ary acidic protein and neuron specific enolase expression increased in the spinal cord and brain.
3.Association of vitamin D receptor gene polymorphisms with autoimmune liver diseases risk

Objective To investigate the association of vitamin D receptor gene (Apa1,Bsm1,Taq1) polymorphisms with autoimmune liver diseases risk.Metiods Case control test documents were retrieved through Pubmed,Ovid,Medline,and Web of science databases,according to the inclusion and exclusion criteria included in this study.The design of experiments,the characteristics of the object of study,research results was excerpted,STATA version 12.0 software were used.The correlation intensity was demonstrated with odds ratio (OR) and 95% confidence interval (CI).Results A total of 6 publications containing 9 studies (7 studies about primary biliary cirrhosis,2 studies about autoimmune hepatitis) published from January 2000 to February 2012 were identified and 844 cases and 1 522 controls were included.The combined results based on all studies showed that there was a statistically significant link between Apa 1 and autoimmune liver diseases (OR =0.85,95% CI 0.74-0.96,P =0.058,for a vs.A; OR =0.75,95% CI 0.58-0.97,P =0.212,for aa vs.AA;OR =0.78,95% CI 0.63-0.98,P =0.235,for Aa vs.AA; OR =0.77,95% CI 0.63-0.94,P =0.231,for Aa/aa vs.AA),while the Bsm 1 and Taq 1 didn' t show the association with autoimmune liver diseases.Conclusion The current meta-analysis shows that Apal may be a low-penetrant risk factor for autoimmune liver diseases.Bsm1 and Taq1 don't show the association with autoimmune liver diseases.
4.Antimicrobial Activities of Traditional Chinese Materia Medica Extracts

OBJECTIVE To discuss the antimicrobial activity of extracts from 10 kinds of traditional Chinese materia(medica)(TCMM)in vitro.METHODS Extracts from 10 kinds of TCMM were prepared,and subjected to(bacteriostatic) tests in vitro by test tube continuously dilution in order to observe their minimal inhibitory(concentration)(MIC),to Staphylococcus(aureus),Escherichia coli,Pseudomonas aeruginosa and Candida albicans.RESULTS Among the 10 kinds of TCMM extracts,which did have antimicrobial activity against S.aureus,E.coli,P.aeruginosa and C.albicans in(different) degree;among them,the antimicrobial(activit)y of the extracts of Radix Paeoniae Rubra,Punica granatum,Schisandra chinensis and Coptis chinensis to resistant bacteria was the highest,the extracts of(Radix) Paeoniae Rubra showed strong inhibitory activities to the above 4 bacteria,especially to the resistant(organisms) with the MIC at 7.8,1.95,1.95 and 7.8mg/ml,(respectively).CONCLUSIONS The extracts of Radix Paeoniae Rubra,P.granatum,S.chinensis and C.chinensis have broad spectrum of antimicrobial activity to(resistant) bacteria.
5.The application of IS6110 restriction fragment length polymorphism and spoligotyping in identification of Mycobacterium tuberculosis strains

Objective To assess the application of IS6110-restriction fragment length polymorphism (RFLP)and Spoligotyping in the epidemiology of tuberculosis and to discuss the characteristic of the Mycobacterium tuberculosis in different regions in China.Methods 158 isolates of Mycobacterium tuberculosis strains, were verified by IS6110-RFLP and spoligotyping.Results The discriminatory power of IS6110-RFLP was higher than that of spoligotyping. The result of spoligotyping was compared with the international spoligotype database. Fourteen types belonged to the shared types, in which the type 1 was epidemic and widely,called Beijing genotype. 6.The relationship between serum leptin and fibrosis in patients with chronic hepatitis B incorporate alcoholic liver disease

Objective To study the relationship between serum leptin and fibrosis in chronic hepatitis B incorporate alcoholic liver disease(CHB + ALD) patients.Methods Select 17 male patients with CHB+ALD,15 male patients with liver cirrhosis(LC) ,19 male patients with chronic hepatitis B(CHB) and 12 male healthy persons as normal control(NC).Serum leptin is detected by ELISA in 63 patients.Serum levels of fibrosis markers(HA,LN,PC HI ,IV-C) are determined at the same time.Results Serum leptin level in CHB +ALD are significantly higher than those of the NC(6.79±24.12) μg/L vs (4.27±7.18 ) μg/L (P<0.05).There is no significant difference between patients with CHB and the NC.The four fibrosis markers in CHB + ALD patients are significantly higher than that of men both in the CHB and the NC.There are significant correlations between both serum leptin level and quadrinomial fibrosis markers(P<0.05 or P<0.01 ).Conclusion That serum leptin level is one aecelerate gene in the fibrosis formation in patients with CHB+ALD.
7.Epidemiological survey on NAFLD and its risk factors in enterprises employees of Rizhao region

Objective To investigate the prevalence and its major risk factors of NAFLD among enterprises employees of Rizhao region. Methods 8 259 employees aged over 18 were selected by multi-stage stratified random cluster sampling method in the enterprises of Rizhao region. The data were analyzed, such as height, weight, blood pressure,blood lipids,blood sugar,hepatitis series,and ECG,abdominal ultrasound,chest χ ray were conducted in the fasting state. The diagnosis of NAFLD was in line with diagnostic criteria adopted by China Institute of Liver Disease and alcoholic liver disease group, and the prevalence and major risk factors of NAFLD were investigated. Results The prevalence of NAFLD was 18. 1% (male 23.0% ,female 7. 6%;the ratio of male to female:3. 03:1) ,The prevalence of NAFLD reached a peak from 40 to 50 in male and after the age of 70 in female,and there was no statistically difference after the age of 50 between male and female. The close correlation was demonstrated by Logistic regression analysis between sex, hyperuricemia, HDLC, overweight or obesity, hyperglycemia, hypertriglyceridemia and NAFLD. Conclusion The prevalence of NAFLD was high, especially in older persons in enterprises employees of Rizhao region, and sex,hyperuricemia,HDLC,overweight or obesity,hyperglycemia and hypertriglyceridemia were closely associated with NAFLD.
8.Ghrelin protects against hippocampal injury after global cerebral ischemia/reperfusion and regulate glutamic acid/γ-aminobutyric acid sensitive neuron discharge

Objective To observe the protective effect of ghrelin on hippocampal injury induced by global cerebral ischemia/reperfusion (I/R) and explore its effect mechanisms.Methods The male Sprague-Dawley (SD) rats were randomly divided into four groups,namely sham group,I/R group,normal saline (NS)+I/R group and Ghrelin+I/R group,with 42 rats in each group.The model of I/R was reproduced by clipping bilateral carotid artery of rats 15 minutes and then releasing them for 60 minutes.There were no challenges for rats in sham group,just exposed their carotid artery.Ghrelin+I/R group and NS+I/R group were challenged by injecting 1 μ.L ghrelin or NS into lateral ventricle before I/R.Some of brain tissue in the rats was harvested after experiment to determine the levels of malonaldehyele (MDA),myeloperoxidase (MPO) and glutathione (GSH) in hippocampus by using chemical colorimetry and observe infarct sizes and histopathology.Single extracellular neuron discharge in other rats was recorded to observe the activity of glutamic sensitive neurons (Glu-N) and γ-aminobutyric acid (GABA) sensitive neurons (GABA-N) in hippocampus CA1 region of rats suffered I/R.Results Compared with sham group,the levels of MDA and MPO in hippocampus of rats in the I/R group were raised markedly,the level of GSH was decreased significantly,the infarct sizes was increased significantly and pycnosis neurons were increased markedly.All sorts of indexes between NS+I/R group and I/R group showed no significantly statistical significance.Compared with NS+I/R group,the levels of MDA and MPO in hippocampus of rats in the Ghrelin+I/R group were decreased significantly [MDA (nmol/g):16.4 ± 4.2 vs.24.5 ± 6.7,MPO (nmol/g):6.4 ± 1.8 vs.10.2 ± 2.9,both P < 0.05],the activity of GSH was risen remarkably (μmol/g:2.65 ± 0.72 vs.1.66 ± 0.50,P < 0.05),the infarct sizes of hippocampus were reduced markedly [(43.9 ± 9.5)% vs.(77.0 ± 12.7)%,P < 0.01],the number of pycnosis neuron was reduced markedly (cells:36.2±4.5 vs.47.1 ±6.1,P < 0.01).The results of electrophysiology showed that the discharge frequency of Glu-N and GABA-N in hippocampus CA1 region of rats in I/R group increased markedly as compared with sham group,and no significant difference in the discharge frequency of Glu-N and GABA-N between NS+I/R group and I/R group.Compared with NS+I/R group,injected ghrelin could make the discharge frequency of Glu-N in hippocampus CA1 region of rats decreased markedly (Hz:3.81 ±0.67 vs.4.98±0.33 at ischemia,3.01 ±0.37 vs.3.77 ± 0.41 at reperfusion,both P < 0.05),and the discharge frequency of GABA-N increased markedly (Hz:5.62 ± 0.54 vs.3.62±0.39 at ischemia,4.81±0.48 vs.3.71±0.21 at reperfusion,both P < 0.05).Conclusion Ghrelin might protect hippocampal neuron after I/R iniury,and neuron excitability decrease might be related.
9.Comparison of Contents of Deoxyschizandrin and Schisantherrin A in Fructus Sch isandrae Chinensis

Objective To establish a HPLC method for the determination of deoxys chizandrin and schisantherrin A in Fructus Schisandrae Chinensis. Methods The as say was conducted on a YWG-C18 column with methonal-water (77 ∶23) as mobile phase. The flow rate was 1.0 mL/min and detection wavelength was 230 nm. Results Satisfactory linearities of deoxyschizandrin and schisantherrin A were shown in the range of 0.125~1.25 ?g and 0.08~0.8 ?g,and the recovery was 100.1 %an d 100.0 %(RSD being 0.15 %and 0.28 %。Schisandra sphenanthcra),and 99.7 %an d 101.2 %(RSD being 0.32 %and 0.67 %。Schisandra chinensis) (n=5). Conclusion The HPLC method was simple,curate,highly sensitive and reproducible. It may b e used for the quantitative determination of deoxyschizandrin and schisantherrin A in Fructus Schisandrae Chinensis.
10.Simultaneous Determination of Four Constituents in Roots and Knotty Rhizome of Glycyrrhiza uralensis by HPLC

This study was aimed to establish an HPLC method for the determination of liquiritin, isoliquiritin, liquiritigenin and glycyrrhizic acid in roots and knotty rhizome of Glycyrrhiza uralensis. The analysis was performed on a Diamonsil C18 column (250 mm í 4.6 mm, 5 μm) by using a gradient elution with mobile phase of water, phosphoric acid, acetonitrile at the flow rate of 1.0 mL·min-1. The detection wavelength was 276 nm (0~18 min), 360 nm (18~24 min), 276 nm(24~30 min), and 250 nm (30~65 min). The column temperature was set at 30℃. The results showed that the linear range of iquiritin, isoliquiritin, liquiritigenin, glycyrrhizic acid was 0 . 108 5~1 . 085、0 . 016 8~0 . 168、0 . 0049 4~0 . 049 4、0 . 407~4 . 07μg , respectively . The average recoveries of four constituents were 96.61%~100.89%, with RSD ≤ 0.81%. The contents of four constituents in roots of five batches were 0.513%, 0.072 9%, 0.048 4%, and 1.945%, respectively. Contents of four constituents in knotty rhizome from two batches were 0.456%, 0.063 6%, 0.036 2%, and 1.630%, respectively. It was concluded that there was good linear relationship between the response and concentration. Contents of four constituents in knotty rhizome were basically the same as those in the roots. The knotty rhizome can be used as raw material for the extraction of active components.
