1.Effects of Collaboration Intervention on Self-efficacy and Self-perceived Burden in Stroke Patients

Objective To explore the effects of collaboration intervention on self-efficacy and self-perceived burden in stroke patients. Methods 92 stroke patients hospitalized in the neurology department from January to June, 2014 were divided into control group (n=46) and intervention group (n=46). The control group received routine nursing and the intervention group received collaboration intervention in addi-tion. They were assessed with Self-Efficacy for Managing Chronic Diseases and Self-perceived Burden Scale before and after intervention. Results There was no difference in the scores of self-efficacy and self-perceived burden between groups before intervention (t<0.585, P>0.05). The score of self-efficacy was more in the intervention group than in the control group (t=3.483, P<0.001) after intervention, and was less of self-perceived burden (t=-5.698, P<0.001). Conclusion The collaboration intervention can improve self-efficacy and relieve self-per-ceived burden in stroke patients.
2.Application of the standardization management form of the totally implanted vascular access devices

Objective To strengthen the standardized management of totally implanted vascular access devices (TIVAD) in order to prevent and reduce the complications.Methods The unified TIVAD maintenance record form was adopted.The form contained the embedded and maintenance information.The maintenance and propaganda handbooks were handed out to the patients.The incidence of complications between before and after the application of the maintenance record form was analyzed statistically.Results The incidence of infection and obstruction fell from 2.1%(20/942) and 8.3%(78/942) to 1.0%(13/1 258) and 2.8%(35/1 258),the difference was statistically significant.Conclusions The application of TIVAD maintenance record form can reduce the occurrence of complications and prolong the service life of TIVAD.
3.Association between pulse pressure variation of radial artery and pulse pressure variation of dorsal pedal artery in neurosurgery operation

Objective To discuss the agreement between pulse pressure variation of radial artery and pulse pressure variation of dorsal pedalartery in neurosurgery.Methods Twenty-five patients undergoing selective craniotomy under general anesthesia were enrolled.The following data were monitored and recorded respectively after tracheal intubation general anesthesia under different time:radial artery pulse pressure variability (PPV1) and dorsalis pedis pulse pressure variation (PPV2).Tidal volume was set to 8 ml/kg.Bland-Altman plots were created to assess agreement between PPV1 and PPV2.Results The mean differences and the limits of agreement between PPV1 and PPV2 are 20 min after induction of anesthesia 0.5% (-1.9%-2.8%), boneless flap instantly-0.5% (-3.8%-2.9%), Cut the dura mater instantly-0.1% (-3.2%-3.0%), and bone flap 0.1% (-2.4%-2.6%).Conclusion Dorsal pedal artery pulse pressure variation in neurosurgery craniotomy has certain guiding significance to the monitoring and management.
4.Time to Start Rehabilitation after Mastectomy for Affected Limb Function

Objective To explore the effect of rehabilitation training start time after mastectomy on the recovery of the affected limb function. Methods 90 patients who received modified radical mastectomy were divided into groups A, B and C randomly. All the patients accepted the unified rehabilitation program, but started 1, 3 and 10 d after the operation respectively. They were evaluated with the Japanese Orthopaedics Association (JOA) assessment for affected shoulder 1, 3 and 6 months after operation. Results The score of JOA was the least in the group C (P<0.01) 1 month after operation, and was the most in the group A 3 months after operation (P<0.01). At the end of the 6th month, there was no difference among these groups (P>0.05). Conclusion The rehabilitation training can be started as early as possible.
5.Study on the Actions of Seabuckthorn Oil Emulsion Against Chronic Atrophic Gastritis and Gastric Ulcer in Rats

OBJECTIVE:To study the actions of seabuckthorn oil emulsion against chronic atrophic gastritis and gastic ulcer in rats.METHODS:Chronic atrophic gastritis and gastric ulcer models were established in rats and mice and the antiulcer and anti-chronic atrophic gastritis actions of seabuckthorn oil emulsion were determined.RESULTS:Seabuckthorn oil emulsion could obviously prevent gastric ulcer and chronic atrophic gastritis induced by deoxycholic acid and stress and could decrease the index of acute gastric ulcer caused by ligature of pylorus in rats and could oppose the ulcer induced by indomethacin-ethanol significantly in mice.CONCLUSION:The results suggest that the antiulcer and anti-chronic atrophic gastritis actions of seabuckthorn oil emulsion may be related with its protection of the gastric mucosal barrier and neutralization of acid.
6.Suppressing PI3K/PKB signal pathway to reverse drug resistance of gastric carcinoma cell line SGC7901/VCR:efficiency and mechanism

Objective To investigate the reversing effects on drug resistance of gastric carcinoma by suppressing PI3K/PKB signal pathway,and explore its implicated mechanism.Methods MTT assay was used to test the inhibitory effects of VCR alone or VCR in combination with PI3K/PKB inhibitor,LY294002 on SGC7901/VCR cells.The SGC7901 treated with or without LY294002 served as control.The protein levels of PKB and phospho-PKB in the above VCR cells were determined by Western blot analysis.The mRNA expressions of MDR1,Bax and Bcl-2 were evaluated by semi-quantitative PCR with ?-actin as the inner control.The apoptosis was detected by flow cytometry.Tumor volume on the tumor-bearing mice transplanted by SGC7901/VCR cells or cells treated by VCR,LY294002 and their combination respectively was measure for the in vivo effect of LY294002.Results LY294002 enhanced the sensitivities of SGC7901/VCR cells to VCR significantly,and promoted the apoptosis rate induced by VCR prominently,with their corresponding drug resistant index decreasing from 40.45 to 8.50.The protein level of phospho-PKB was reduced,and the mRNA expression levels of MDR1 and Bcl-2 were inhibited(P
7.Effects of Oscillator on the Cleanliness of Class 100 Clean Bench in Pharmacy Intravenous Admixture Ser-vices

OBJECTIVE:To study the influence of oscillator on the cleanliness of class 100 clean bench in PIVAS. METH-ODS:Using sedimentated bacteria and the number of dust particle as index,in common drug configuration room,antibiotics con-figuration room and risk drugs configuration room including biological safety cabinet and horizontal laminar flow,the cleanliness of class 100 clean bench were monitored when oscillator was set at clean bench and different positions in work and non-working state. RESULTS & CONCLUSIONS:In working and non-working state of oscillator,there was no difference in sedimentated bacteria and the number of dust particle which was in line with the requirements of 2010 edition of GMP,i.e. the application and location of oscillator didn't influence the cleanliness of class 100 clean bench. From a view of safety,it is suggested to place the oscillator in the left(or right)posterior wall of clean table when biological safety cabinets is used to dispense antibiotic and risk drugs.
8.Immunophenotypic analysis and identification of hematogones and acute B lymphoblastic cells by flow cytometry

Objective To discriminate morphology and immunophenotype differences between hematogones and lymphoblast to provide some references for the correct identification of hematogones and minimal residual leukemia cells.Methods Immunophenotypes were detected by flow cytometry in a total of 132 bone marrow from 58 patients with acute B lymphoblastic leukemia during diagnosis,remission and relapse.Hematogones were identified based on combination of CD34/CD10/CD19/CD45 or CD34/CD10/CD45/CD19/CD20/CD38.Results Among 132 specimens,45 (34 %) were identified hematogones,the detection range was 0-36 %.Three specimens appeared in diagnosis patients,one in relapse,and the remaining 41 cases in remission.The detection rate of hematogones was 62 % (41/66) in the remission cases.More than 5 % leukemia cells of nucleated cells were detected in diagnosis and relapse,and less than 5 % residual leukemia cells was in 24 specimens from remission patients.In 28 specimens,the co-existence of hematogones and leukemia cells was found,including three in diagnosis,one in relapse and the remaining 24 in remission.Hematogones were characterized in term of variable expression of CD45 and very low side scatter.The early hematogones expressed CD34.With maturation increasing,hematogones gradually lacked CD34.CD19 and CD10 were presented in whole hematogones stage.Early hematogones had expression of CD10.Lymphoblasts showed maturation arrest and more homogeneous populations.SSC values of hematogones were higher than that of normal B cell progenitors.Antigen overexpression or underexpression was not found in normal hematopoietic progenitor B cells,and hematopoietic progenitor B cells did not appear cross-lineage markers,CD20+ cells exhibited continuous distribution from negative to weak positive for normal hematogones.Conclusions Hematogones were present in diagnosis,remission and relapse cases with acute B lymphoblastic leukemia,especially abundant in bone marrow after chemotherapy.It should be careful to diagnose and discriminate the malignant cells from benign cells.By comprehending continuous and complete maturation spectrum of antigen expression for normal hematogones,knowing phenotype of leukemia cells drift change patterns and using multiparameter flow cytometry and optimal antibody combination,it is significant in identifying residual lymphoblasts from hematogones and improving the detection accuracy in minimal residual disease.
9.Pathogenic bacteria distribution and clinical analysis of ventilator-associated pneumonia in infants and young children with congenital heart disease after surgery

Objective To investigate pathogen bacteria distribution and clinical analysis of ventilator-associated pneumonia in infants and young children with congenital heart disease after surgery for guiding the rational clinical use of antimicrobial drugs.Methods The clinical data of ventilator-associated pneumonia in infants and young children with congenital heart disease after surgery were retrospectively analyzed for microbiologically documented infection.Bacterial identification was performed in an automatic VITEK2Jr expression system and antimicrobial susceptibility testing by Kirby2Bauer method.The results of antimicrobial susceptibility testing were performed in WHONET 5.0 system software.Results There were 16 strains of pathogenic bacteria in 2010,Gram-negative bacteria 9 stains (Klebsiella pneumoniae 18.75%,Acinetobacter baummannii 12.5%),gram positive coccus 6strains (Streptococcus 18.75%),Fungi 1 strain.There were 32 strains of pathogenic bacteria in 2011,Gram-negative bacteria 24 stains (Acinetobacter baummannii 31.25%,Klebsiella pneumoniae 25%,Pseudomonas aeruginosa 6.25%),gram positive coccus 8 strains (Enterococcus 12.5%,Streptococcus 6.25%).There were 42 strains of pathogenic bacteria in 2012,Gram-negative bacteria 38 stains (Acinetobacter baummannii 26.19%,Klebsiella pneumoniae33.33%,Pseudomonas aeruginosa 21.42%),gram positive coccus 4 strains (Enterococcus 4.76%).Acinetobacter baummannii remained highly sensitive to Amikacin and Levofloxacin,highly resistant to Meropenem,Imipenem,Klebsiella pneumonia remained highly sensitive to Amikacin and Levofloxacin,highly resistant to Cefoperazone/Sulbactam,Meropenem,Imipenem,trimethoprim-sulfamethoxazole,Ciprofloxacin.Pseudomonas aeruginosa only remained sensitive to Levofloxacin and Piperacillin/Tazobactam.Gram-positive cooci proportion in pathogenic bacteria decreased year by year,Streptococcus and Enterococcus had major proportion in Gram-positive cooci,Staphylococci decreased,There was no Staphylococcus,Enterococcus and Staphylococci resistant to vancomycin.Conclusions Gram-negative bacteria was the major pathogenic bacteria in ventilator-associated pneumonia with congenital heart disease postoperative infants and young children,Acinetobacter baummannii Klebsiella pneumoniaeand Pseudomonas aeruginosa had major proportion in gram-negative bacteria and drug resistance increased.Rational use of antibacterials was very important to reduce drug resistant strains.
