1.Analysis of distribution characteristics and drug resistance change of acinetobacter baumannii in primary hospital during 10 years

Objective To analyze the distribution characteristics and drug resistance change of acinetobacter baumannii in prima‐ry hospital during 2005-2014 to provide reference for clinical rational drug use .Methods The infection characteristics of acineto‐bacter baumannii in primary hospital during 10 years and its resistance to 10 kinds of common antibacterial drugs was analyzed .Re‐sults A total of 576 strains of acinetobacter baumannii were isolated during 2005-2014 ,accounting for 31 .44% of all Gram nega‐tive bacteria ,which was significantly higher than that of Escherichia coli ,pseudomonas aeruginosa and klebsiella pneumonia bacillus (P<0 .05);446 strains were mainly originated from the sputum specimens (77 .43% ) and 290 strains(50 .35% ) from ICU ;the re‐sistant rate was 44 .44% for CSL ,62 .24% for MIN and more than 70 .00% for 8 kinds of antibacterial drugs of IPM ,MEM ,etc .;which to IPM ,CAZ ,SXT showed the declining trend year by year ,while which to MEM ,AMK ,LEV ,MIN showed the rising trend year by year .Conclusion The isolated acinetobacter baumannii strains in primary hospital are rised year by year ,and generally have resistance to commonly used antibacterial drugs ,the clinical doctors should rationally select antibacterial drugs according to the drug susceptibility test results for preventing the occurrence of acinetobacter baumannii infection .
2.Effects of Lycium barbarum polysaccharide on tumor microenvironment T-lymphocyte subsets and dendritic cells in H22-bearing mice

OBJECTIVE: To study the effects of Lycium barbarum polysaccharide (LBP) on tumor microenvironment T-lymphocyte subsets and dendritic cells in H22-bearing mice and the mechanisms for intervention of tumor immune escape by LBP. METHODS: H22-bearing mice were given LBP orally for two weeks. T-lymphocyte subsets and the phenotypes of dendritic cells in tumor-infiltrating lymphocytes (TIL) were detected by flow cytometry (FCM). RESULTS: LBP could significantly increase the numbers of CD4(+) and CD8(+) T cells in TIL as compared with those in model control group (P<0.05). In model control group, the number of dendritic cells in tumor microenvironment decreased markedly, while in LBP-treated group, the increased number of dendritic cells and B7-1 expression were observed, but there were no significant differences between these two groups. CONCLUSION: LBP has anti-tumor effect probably by increasing the numbers of CD4(+) and CD8(+) T cells in TIL to relieve the immunosuppression and enhance the anti-tumor function of the immune system. But whether LBP can recover the phenocyte and function of dendritic cells in H22-bearing mice should be further studied.

The amounts of iron and amino acids in dried pig powder were determined. It contained a large quantity of iron (230-260m/100g) and all essential amino acids. The blood fortified wheat flour biscuit containing 15mg Fe/100g was made and 30?2g of the biscuit was given to 65 primary school students (age 8-10) per capita per day during school day break for 40 days from Sep to Oct in 1984. For the control group, equal amount of the same biscuit without blood fortification was given to 34 subjects of the same age. Objective To evaluate the effect and tolerance of late-course three dimensional conformal radiotherapy(LC3DCRT) combined with concurrent chemotherapy for stage Ⅲ non-small cell lung cancer(NSCLC).Methods From May 2000 to May 2003,48 such patients were entered into this study.The patient's characteristics were: 38 male and 10 female,with median age of 62 years(range 40 to 74);Karnovsky performance score ≥70;stage ⅢA 16 and ⅢB 32,squamous cell carcinoma 38 and adenocarcinoma 10.The treatment regimen consisted of conventional radiotherapy first(40Gy/20f/4W),followed by 3DCRT(24-30Gy/4-5f/2W) combined with concurrent chemotherapy.Conventional irradiation field encompassed the primary lesion,ipsilateral hilum and mediastinal lymph drainage region.LC3DCRT focused on the primary lesion only,with the 80%-90% isodose curve covering the planning target volume(PTV) and the target dose was prescribed to PTV.Supraclavicular metastatic lymph node was treated by mixed 6MV X-ray and electron beam to a total dose of 65-70Gy.Chemotherapy treatment regimen consisted of isophosfomide(25mg/m~2,d1、8,iv) and cisplatin(30mg/d,d1-3,iv) in the 1st and 5th week.Results Before the end of the second month after treatment,the complete response(CR)and partial response(PR) rate was 16.7% and 75.0%,respectively,with a CR+PR rate of 91.7%.The 1-,2-and 3-year local control and overall survival rates as monitored by the Kaplan-Meier method was 87.5%,50.0%,35.7% and 87.5%,46.7%,28.6%,respectively.All patients completed the planned treatment without interruption.Hematological toxicity and radiation-induced pneumonitis as shown by the WHO staging system were the most common acute toxicities but they were tolerable,with 8.3% of grade 3 leukopenia and 4.2% of grade 3 radiation-induced pneumonitis.The severity of the other acute toxicities such as nausea,fever,hemoglobin decrease,and radiation-induced esophagitis were mainly grade 1 or grade 2.Conclusions Late course three dimensional radiotherapy combined with concurrent chemotherapy shows a promising results with tolerable acute toxicities.Long-term survival and late toxicities need further observation.
6.Investigation on Mild Cognitive Impairment among Elderly in Urban Community of Xi'an

Objective To investigate the actuality and risk factors of mild cognitive impairment among the elderly in Xi'an. Methods The elderly from 10 urban communities in Xi'an were selected using random cluster sample method. Mini-Mental Status Examination and cognitive function questionnaire self-designed were assessed. Results and Conclusion There were 396 (25.02%) elder people with cognitive impairment in 1583 cases. The morbidity of mild cognitive impairment was inverse to the educational level, and was related to age, gender, job category, marriage and social support (P<0.05).
7.Chemical constituents of Saxifraga stolonifera (L.) Meeb.

To study the chemical constituents of Saxifraga stolonifera (L.) Meeb., chromatographic techniques were applied to separate and purify the compounds, and their structures were confirmed on the basis of physicochemical properties and spectral data. Ten compounds were isolated and identified as 5-O-methylnorbergenin (1), 3, 4-dihydroxyallylbenzene-4-O-beta-D-glucopyranoside (2), (7R, 8S)-4, 9, 9'-trihydroxyl-3-methoxyl-7, 8-dihydrobenzofuran-1'-propylneolignan-3'-O-beta-D-glucopyranoside (3), quercetin-3-O-beta-D-xylopyranosyl-(1 --> 2)-beta-D-galactopyranoside (4), kaempferol-3-O-alpha-L-rhamnopyranoside (5), (3S, 5R, 6R, 7E, 9R)-3, 5, 6, 9-tetrahydroxy-7-megastigmane (6), benzyl-O-alpha-L-rhamnopyranosyl-(1 --> 6)-beta-D-glucopyranoside (7), p-hydroxyacetophenone (8), pyrogallic acid (9) and p-hydroxyphenol (10). Compound 1 is a new compound. Compounds 2-10 were isolated from this plant for the first time.
8.Analysis of acute radiation injury in non-small cell lung cancer treated by three-dimensional conformal radiotherapy

Objective To analyze the influencing factors with radiation induced injury of nonsmall cell lung cancer(NSCLC)treated by three-dimensional conformal radiotherapy,in order to supply he reference criteria for optimize the treatment planning of NSCLC, further to improve the local control rate and quality of life of NSCLC patitents.Method From Aug 2000 to Dec 2004,107 NSCLC patients received three-dimensional conformal radiotherapy were enrolled in this study.In all the patients,48 patients accepted three dimensional conformal radiotherapy(3D-CRT),the other 59 patients accepted conventional radiotherapy(CRT)before later-course of 3D-CRT.All of the patients received the prescription dose between 60-78 Gy and the median dose 66 Gy.Results In the 107 patients,the rate of radiation induced injury was 62.6%.and the rate of≥2 grade was 38.3%.23 cases were classified in grade 2,14 cases in grade 3,4 cases in grade 4.Univariate analysis showed chronic obstructive pulmonary disease,number of beams field,lung mean dose,lung V5-V40 were important influencing factors on radiation induced injury.Further more,lung mean dose,lung V20 and chronic obstructive lung disease were likely to be the independent factors of radiation induced injury by Logistic model.Conclusions lung mean dose,chronic obstructive lung disease and lung V20 were the independent influencing factors on irradiated induced injury.
9.Dosimetry analysis on radiation-induced acute esophagitis after three-dimensional conformal radiotherapy for non-small cell lung cancer

Objective To analyze the related factors with radiation-induced esophagitis after threedimensional conformal radiotherapy for non-small cell lung cancer (NSCLC) ,in order to explore the predictors for optimizing the treatment planning of NSCLC.Methods From Aug 2000 to Dec 2004,104 NSCLC patients received radiotherapy and were eligible for this study,45 cases squamous cell carcinoma,20 cases adenocarcinoma,33 cases carrying with cancer cells by test and 6 case with no definitive pathologic feature.46 patients were treated with three dimensional conformal radiotherapy(3DCRT) ,the other 58 patients conventional radiotherapy (CRT) before later-course 3DCRT.All the patients received the prescribed dose between 60-78 Gy and the median dose 66 Gy.The correlation of the variables were evaluated by Spearman relationship analysis.The morbidity of radiation-induced esophagitis was analyzed by X~2 test.The multivariate effect on radiation-induced esophagitis was statistically processed by Logistic regression model.Results In 104 patients,the morbidity of radiation- induced esophagitis was 46.2% ,including 32 cases at grade 1,15 cases at grade 2,1 case at grade 3.Univariate analysis showed the maximal and mean dose of esophagus,the volume of esophagus irradiated,the values of V_(40) ,V_(45),V_(50) ,V_(55),V_(60) ,LETT_(45),LETT_(50) ,LETT_(55),LETT_(60) for the esophagus were correlated with radiation-induced esophagitis.Logistic regression model showed that the maximum dose received by the esophagus was the independent factor of ≥2 grade radiation-induced esophagitis.Conclusions The maxmal dose of esophagus received might be the important factor of radiation-induced esophagitis.
10.Investigation and analysis of the basic medical security system in Guangxi

Objective To understand the performance of the basic medical security system in Guangxi for decision-making support on the development planning of the system during the 12th FiveYear Plan in Guangxi.Methods Descriptive statistics were made to analyze the annual report data of Guangxi's basic medical security system during 2009 to 2011,regarding the basic medical security for urban workers and that for urban residents,as well as those of the new rural cooperative medical system.Results The survey found that the three basic medical security systems in Guangxi have almost achieved full coverage; the compensation ratio keeps rising as the inpatient expenses of urban residents was up to 62.21%,64.96% and 66.96% respectively from 2009 to 2011,and that for urban residents up to 34.26%,39.96% and 49.68% respectively; the ratio for those covered by the new rural cooperative medical system has been maintained at 45% for the three years; the coverage of the systems keeps expanding.Conclusion Recommend to integrate pilots of the three security systems and carry out serious diseases pilot programs; encourage reform of the payment system at institutions at or above county level; reform the risk-pooling of disease management of at primary medical institutions; strengthen the information systems of basic medical insurance system,in an effort to ensure the sound development of the basic medical insurance system.
