ARDS is a type of acute diffuse lung injury, characterized by inflammation leading to increased pulmonary vascular permeability and loss of aerated lung tissue, the clinical manifestations are refractory hypoxia, progressive respiratory distress and non cardiac pulmonary edema.ARDS mortality rate is very high, especially in patients with severe ARDS, about 27%-45%. In order to improve diagnostic specificity, a revised definition was published in JAMA in 2012, but no new biomarkers or parameters were adopted.Along with the progress in understanding the pathophysiology of ARDS, a variety of humoral factors by inflammation and molecules derived from injured tissues or activated cells may be potential biomarkers for clinical.

Objective To observe the effect of bcl-2 antisense oligodexyonucleotide (ASODN) by phosphorothiote-modification on the expression of bcl-2 mRNA and protein in human melanoma A375 cells line. Methods With the phosphorothiote-modification and liposome-encapsulation of ASODN, A375 cells were divided into ASODN group, nonsense oligodexyonucleotide (SODN) group and control group. Bcl-2 mRNA and protein were detected by RT-PCR and immunohistochemistry, respectively. Results The expression of bcl-2 protein was remarkably decreased in ASODN group than that in SODN group and control group (53.14 ±4.26 vs 138.22 ± 8.45, 53.14 ± 4.26 vs 141.08 ± 7.83, both P < 0.01 ). The level of bcl-2 mRNA was significantly lower than that in SODN group and control group (0.38 ± 0. 11 vs 0.96 ±0.13, 0. 38 ± 0.11 vs 0. 97 ± 0. 14, both P < 0. 01 ). Conclusion Bcl-2 antisense oligodexyonucleotide could down-regulate the bcl-2 level and block its protein expression.

Objective To detect the PML-RARα fusion gene expression of APL patients with FISH and fluorescent quantitative PCR and discuss the sensitivity and specificity of two techniques. Methods The detection of the PML-RARα fusion gene expression of 75 APL patients with FISH and fluorescent quantitative PCR were carried out simultaneously. Results Eighty eight patients of primary and relapse or remission phases were examinated and total conformity rate was 96.59 %. Fourteen primary patients were detected and conformity rate was 100 %. Seventy four relapse or remission patients were detected and conformity rate was 95.95 %. Stem cell essays were detected for six times and conformity rate was 100 %. Conclusion The sensitivity of FISH and fluorescent quantitative PCR is identical for primary APL patients and FISH is more sensitive. But the sensitivity of FISH is weaker than that of fluorescent quantitative PCR for detection of residue disease.

Objective To evaluate the application of health education in patients with cholecystectomy. Methods From October 2009 to July 2010, 93 patients in our hospital with cholecystectomy were randomly divided into the control group (45 cases) and the observation group(48 cases). The control group was given routine nursing care, the observation group received perioperative health education in addition to the routine nursing care, then compared the nursing effect of two groups. Results In the observation group, the patients' knowledge and awareness rate of their own disease was 95.8%, and the control group was 80.0%, the difference was statistically significant between two groups. The recovery of gastrointestinal function in the observation group was significantly better than the control group, the difference was statistically significant between two groups. Conclusions Health education can enable patients with cholecystectomy correct understanding of their disease, eliminate negative emotions, and help the recovery of gastrointestinal function in patients, which is worthy of attention.

Objective To assess the efficiency of total laminectomy and transpedicular screw placement techniques and a thoroughly decompression with a three column fixation device on the early cervical spine cord injury. Methods 27 patients were operated successfully with total cervical laminectomy through transpedicular screw techniques.Precise measurements of all cervical spines were made by postoperative CT scan of pedicle dimensions, angulation, and offset relative to the lateral mass boundaries. And meanwhile, cortical integrity and neurovascular injury were then assessed by obtaining postoperative computed tomography scans of each patient. Results Using manual techniques,220 pedicles were instrumented and all the cases were followed up between three to twenty-four months. The postoperative gradations were obviously better than the preoperative evaluation.Regardless of the technique used, the vertebral artery was the structure most likely to be injured. Conclusion Insufficient correlation between different surgeons' assessments of surface landmarks attests to the inadequacy of screw insertion techniques in the cervical spine based on such specific topographic. At the early stage of cervical spinal cord injury, using total laminonectomy decompression through cervical screws techniques to reconstruct spinal alignment could improve the prognosis of patients.

Objective To explore the application value of Carbon Nanoparticles followed by microscale Methylthioninum Chloride in sentinel lymph node biopsy for breast cancer. Methods Two hundred and eight breast cancer patients were selected in the First Affiliated Hospital of Zhengzhou University during recent two years who performed sentinel lymph node biopsy. They were randomly divided into five groups according to different lymphatic tracer injected:Sequential group (A group,128 patients),Carbon Nanoparticles group (B group,44 patients) and Methylthioninum Chloride group (C group,36 patients).According to different Methylthioninum Chloride doses, sequential groups were randomly divided into low dose group (A1 group,48 patients),middle dose group (A2 group,42 patients),high dose group (A3 group,38 patients),If sentinel lymph nodes were negative no longer performed axillary lymph nodes dissection.Receive the five groups statistics of detection rate, the condition of lymphatic tracer, the incidence of local skin irritation and so on.Results (1) The detection rate of A,B and C three groups had no obvious difference. The incidence of lymphatic tracer dyed not ideal in B group was higher than A and B two groups, the incidence of local skin stimulation in C group was higher than A and B two groups.(2) A1,A2,A3,three groups had no difference of the detection rate and lymphatic tracer, but the incidence of local skin stimulation in A3 group is higher than A1,A2 two groups.3.5 groups of SLN detection rate had no relationship with patients' age,BMI,tumor size, tumor location, tumor molecular classification factors (P > 0.05).Conclusions Carbon Nanoparticles followed by microscale Methyhhioninum Chloride detection rate was similar with two separate application, but can reduce the lymphatic vessels not developing situation when used Carbon Nanoparticles alone and reduce the local irritation caused by large dose of Methylthioninum Chloride,so it can be a good choice of SLNB in clinical.

Objective To monitor the expression patterns of bcr-abl in chronic myeloid leukemia (CML) patients during treatment with imatinib mesylate and evaluate the detection of MRD by RQ-PCR method. Methods The ABI Prism 7500 Sequence Detection System using Taqman fluorogenic probes was used to quantify target gene. bcr-abl mRNA was detected by RQ-PCR in 106 CML patients. The normalized quotient (NQ) of bcr-abl mRNA was calculated as followings: NQ=bcr-abl mRNA copy numbers/abl mRNA copy numbers. Results The NQ of BCR-ABL mRNA was well correlated with the progression of disease and the number of Ph+ cell (r =0.9824 and 0.9346, respectively). The NQ was decreased rapidly in 62 patients and kept in low level for a long time, and only 2 of them were relapsed. For 8 patients, after treatment the NQwere decreased initially and increased sharply, 7 of them were relapsed after 5-9 months. After treatment the NQ of 31 patients were still>0.1, 11 patients were relapsed after a short remission and 7 were ineffective or progression. Out of 5 patients whose NQ were fluctuated and had little regularity, but all of them had a continuing remission. Conclusion RQ-PCR is a more sensitive technique in the detection of bcr-abl fusion gene.It is an important method to monitor the tumor cell during the treatment with imatinib mesylate in CML patients.
