1.The Influence of Uncertainty in Illness in Compliance of Respiratory Function Exercise and Rehabilitation after Surgery in Patients with Coronary Artery Bypass Graft Surgery

Objective To explore the influence of uncertainty in illness in compliance of respiratory function exer-cise and rehabilitation after surgery in patients with coronary artery bypass graft (CABG) surgery. Methods Mishel uncer-tainty in illness scale and compliance of respiratory function exercise scale were used to collect data in 108 patients with CABG. The correlation of the uncertainty in illness and compliance of respiratory function exercise was analyzed. Patients were divided into three groups (high score, medium score and low score groups) according to the detection of uncertainty in illness. Values of compliance of respiratory function exercise, postoperative care time, postoperative ventilation time, postop-erative hospital stay duration, 6-min walking test (6MWD) and left ventricular ejection fraction (LVEF) were compared be-tween three groups. Results The uncertainty in illness was in medium level (95.51±9.48) in patients with CABG surgery, which was negatively correlated with compliance of respiratory function exercise (r=-0.926, P<0.01). The postoperative venti-lation time and postoperative care time were less in low score group than those of medium score group and high score group. The compliance of respiratory function exercise, 6-min walking test and postoperative LVEF were better in low score group than those of medium score group and high score group. Conclusion Medical personnel should take targeted interventions to reduce the uncertainty in illness in patients, enhance the compliance of respiratory function exercise, reduce the postopera-tive complications and promote early recovery of patients.
2.Correlation of Antibacterials Amount with Drug Resistance of Escherichia coli in Our Hospital from 2012 to 2014

OBJECTIVE:To study the correlation between antibacterials amount and drug resistance of Echerichia coli,and to provide reference for clinical use of antibacterials. METHODS:Retrospective review was used to calculate DDDs of antibacterials and resistance rate of Escherichia coli to 11 kinds of antibacterials each quarter. The correlation analysis was carried out using the SPSS 13.0 statistical software. RESULTS:The resistance rates of E. coli to piperacillin/tazobactam,cefoperazone/sulbactam and le-vofloxacin were with upward trends,and the others showed downward trends. The resistance rates of E. coli to meropenem and imi-penem/cilastatin appeared in 2014,increasing from 0 to 8.8% and 9.4%,respectively. DDDs of them were significantly correlated to drug resistance of E. coli,showing positive correlation(r=0.915,0.793,P<0.01). DDDs of piperacillin/tazobactam was signif-icantly correlated to resistance rate of E. coli(r=0.807,P<0.01),while that of ceftazidime was negatively correlated to resistance rate of E. coli(r=-0.672,P<0.05). There was no statistical significance in resistance rate of E. coli to other 7 kinds of antibacte-rials. CONCLUSIONS:There are some correlations between the DDDs and resistance rates. We should strengthen the monitoring of bacterial resistance and the management of rational application of antibacterials.

Objective To compare the tracing effects of radionuclide and barium sulfate on lactulose hydrogen breath test (LHBT), and to explore the value of LHBT combined with radionuclide imaging in the diagnosis of small intestinal bacterial overgrowth (SIBO) in patients with irritable bowel syndrome (IBS).Methods From November 2010 to November 2012, 89 patients (47 males, 42 females;mean age (45.7±12.9) years) with IBS and 13 healthy volunteers (9 males, 4 females;mean age (43.3±8.6) years) were enrolled in this prospective study.All the subjects underwent LHBT combined with radionuclide imaging.Recording the time when the increment of H2 value >0.005‰ and the OCTT of the radionuclide.Four healthy volunteers also underwent LHBT combined with barium sulfate 1 week after radionuclide imaging.The location of barium sulfate was recorded when H2 value increment >0.020‰.Patients with SIBO received rifaximin treatment, and the effect was observed.χ2 test, Pearson correlation analysis and Wilcoxon rank sum test were used to analyze the data.Results (1)In LHBT combined with barium sulfate test, barium sulfate was found still stagnating in small intestine by abdominal X-ray when H2 value increment >0.020‰ in 4 healthy volunteers, and barium sulfate didn′t reach the colon in delayed imaging in 1 patient.(2) The rates of SIBO detected by LHBT in IBS patients and healthy volunteers were significantly different (43.8%(39/89) vs 5/13;χ2=0.133, P=0.716), and those detected by LHBT combined with radionuclide imaging were also significantly different (39.3%(35/89) vs 1/13;χ2=4.970, P=0.026).(3)The time of H2 value increased >0.005‰ correlated well with OCTT in 13 healthy volunteers ((73±31) and (50±19) min;r=0.871, P<0.001) and 54 IBS patients without SIBO ((83±34) and (66±28) min;r=0.735, P<0.001), but there was no correlation in 35 IBS patients with SIBO ((36±30) and (75±30) min;r=0.304, P=0.076).(4)A total of 34 SIBO-positive patients received a rifaximin treatment, with a significant improvement in the frequency of abdominal pain and abdominal distension after the treatment according to Rome Ⅲ diagnostic criteria: 5(4, 6) vs 4(3, 5), 4(1, 6) vs 0(0, 4)(z values:-4.842 and-5.388, both P<0.001).Conclusion LHBT alone is not a valid test for SIBO, and LHBT combined with radionuclide imaging is a good candidate for SIBO diagnosis.
5.Langerhans cell infiltration in laryngeal squamous cell carcinoma and its clinical significance

Objective To observe the infiltration situation of Langerhans cells(LC)in laryngeal squamous cell carcinoma tissue and to investigate its clinical significance.Methods Seventy-two cases of laryngeal squamous cell carcinoma were selected and the tumor tissue in all cases was operatively resected.The immunohistochemical SP method was adopted to detect the infiltration situation of LC in laryngeal squamous cell carcinoma tissue.Then the relationship between the LC infiltration degree with tumor stage,relapse,metastasis and prognosis was analyzed.Results Among 72 cases of laryngeal squamous cell carcinoma,27 cases(37.5%)were LC infiltration degree of +,37 cases(37.5%)were ++ and 18 cases(25.0%)were +++.The LC infiltration degree in laryngeal carcinoma tissue was correlated with the cervical lymph node metastasis,recurrence,disease free survival,3-year survival and 5-year survival(P<0.05),and had no correlation with the age,classification,TNM stage and postoperative radiotherapy(P>0.05).Conclusion LC infiltration can be seen in the tumor tissue of the patients with laryngeal squamous cell carcinoma.The LC infiltration degree is higher,indicating that the prognosis would be better.
6.Serum-and glucocorticoid-regulated kinase 1 suppresses inflammatory responses mediated by toll like receptor 4 via nuclear factor-κB

Objective To investigate the role of serum and glucocorticoid regulated protein kinase (SGK) 1 in the inflammatory responses mediated by toll like receptors.Methods Mice were injected with lipopolysaccharide (LPS,1 mg/kg) 2 h after the pretreatment of EMD638683 (10 mg/kg) or phosphate buffered saline (PBS) as control.At the time points of 3 and 24 h,pro-inflammatory cytokines (interleukin [IL]-6,IL-12 and tumor necrosis factor [TNF]-α) in serum were measured using enzymelinked immunosorbent assay (ELISA).Livers and lung were harvested at 6 h and 24 h after the injection of LPS,embedded by optimum cutting temperature (OCT) and then stained with hematoxylin and eosin (HE).Peripheral blood mononuelear cell (PBMC) were isolated and stimulated by LPS with or without the pretreatment of EMD or LY294002.Cytokines (IL-6,IL-12 and TNF-α) were measured using ELISA.IKKα/β,IKBα and nuclear factor (NF)-κB p65 were detected by Western bolt.Data were analyzed by one way analysis of variance.Results In the model of LPS-induced endotoxin sepsis,inhibition of SGK1 induced secretion of pro-inflammatory cytokine (IL-6 [t=3.007,P<0.05],IL-12[t=4.413,P<0.05] and TNF-α[t=5.403,P<0.05]),increased inflammatory cells infikration into the liver and lung within 6 h,and induced serious multiple organ damage with collapse of alveoli and fatty degeneration of liver.After 24 h,pharmacological inhibition of SGK1 with EMD638683 increased proinflammatory cytokine (IL-6 [t=18.540,P<0.01],IL-12[t=16.520,P<0.01] and TNF-α[t=34.880,P<0.01]) production in human PBMC upon LPS stimulation and inhibited the phosphorylation of IKKα/ β/IKBα and nuclear factor (NF)-κB p65.Conclusions SGK1 suppresses the toll like receptor 4 mediated inflammatory responses via NF-κB.
7.Study on the application of health education in patients with cholecystectomy and effect analysis

Objective To evaluate the application of health education in patients with cholecystectomy. Methods From October 2009 to July 2010, 93 patients in our hospital with cholecystectomy were randomly divided into the control group (45 cases) and the observation group(48 cases). The control group was given routine nursing care, the observation group received perioperative health education in addition to the routine nursing care, then compared the nursing effect of two groups. Results In the observation group, the patients' knowledge and awareness rate of their own disease was 95.8%, and the control group was 80.0%, the difference was statistically significant between two groups. The recovery of gastrointestinal function in the observation group was significantly better than the control group, the difference was statistically significant between two groups. Conclusions Health education can enable patients with cholecystectomy correct understanding of their disease, eliminate negative emotions, and help the recovery of gastrointestinal function in patients, which is worthy of attention.
9.Suprachiasmatic nucleus slices induce molecular oscillations in fibroblasts

Objective To study whether suprachiasmatic nucleus (SCN) slices are able to induce the molecular oscillations in NIH/3T3 fibroblast. Methods SCN slices from 10-day-old SD rat and NIH/3T3 cells were co-cultured in a serum-free condition. 24h mRNA profiles of Per1 and Rev-Erbα were measured in NIH/3T3 cells using real-time PCR. Results After co-cultured for 6 days, ten SCN slices can induce the significant daily oscillation of Per1 and Rev-Erba mRNA expression in NIH/3T3 cells (P<0.01). The peak time Rev-erbα and Per1 were at CT5 and CT11 respectively. Rev-Erbα oscillations were significant even with two SCN slices and 2 days co-culture (P<0.05). In contrast, Per1 expression fluctuation was not observed until more than 6 days of co-culture and with six SCN slices (P=0.031). Conclusion Diffusible signals release from SCN slices can regulate molecular rhythms in cultured fibroblasts. Rev-Erbα and Per1 don't start to oscillate at the same time, and Rev-Erbα is more sensitive to SCN signal.
10.Diagnosis value of T lymphocyte subgroup, lymphocyte and C-reactive protein in patients with acute and chronic Brucellosis

Objective To investigate the variation of lymphocyte subgroup,lymphocyte and C-reaction protein in patients with acute and chronic Brucellosis,for auxiliary reference in differential diagnosis of acute and chronic Brucellosis.Methods All the blood samples were come from clinical laboratory of Heilongjiang Agricultural Reclamation Bureau General Hospital,which including 35 acute patients(acute group),62 chronic patients(chronic group) and 30 healthy people(control group).Flow cytometric method was used to determine the T lymphocyte subgroup(CD3+,CD19+,CD3+/CD4+,CD3+/CD8+) and the lymphocyte.Application of rate scattering turbidimetric method was used for determination of C-reactive protein.Results Percentages of CD3+ cells in acute and chronic groups were increased compared with that of control group(%:74.71 ± 6.79,70.61 ± 8.99 vs.65.00 ±7.50; F =12.40,P < 0.05),and the acute group was higher than the chronic group(P< 0.05).Percentages of CD19+,CD3+/CD4+ cells in acute and chronic groups were decreased compared with that of control group(CD19+,%:7.79 ± 3.93,9.94 ± 4.53 vs.11.50 ± 3.25; CD3+/CD4+,%:30.56 ± 6.38,35.56 ± 8.22 vs.39.00 ± 4.00; F=18.62,13.03,all P< 0.05),and the acute group was lower than the chronic group (P< 0.05).Percentage of CD3+/CD8+ cells in the acute group was higher than those of the chronic and control groups(%:4.06 ± 8.35,29.50 ± 7.25 vs.28.72 ± 8.30; F =12.06,P < 0.05).The lymphocyte counts in acute and chronic groups were higher than that of the control group(%:34.29 ± 10.91,39.12 ± 12.08 vs.30.84 ± 6.45; F=13.78,P < 0.05),but the acute group was lower than the chronic group(P < 0.05).C-reactive protein levels in acute and chronic groups were increased compared with that of the control group(mg/L:27.43,15.87 vs.0.60; H =19.42,P < 0.05),and the acute group was higher than the chronic group(P < 0.05).C-reactive protein positive rates in acute and chronic groups were increased compared with that of the control group(65.7%,61.3% vs.3.3%; x2=27.54,23.16,all P < 0.01).The difference between groups was statistically significant(x2 =9.94,P < 0.01).Conclusion Lymphocyte subgroup,lymphocyte and C-reactive protein are changed differently in patients with acute and chronic Brucellosis which have a certain reference value in differential diagnosis and treatment of acute and chronic Brucellosis.
