1.Study on the deletions of SHOX gene and the correlation with corresponding phenotypes

Objective:To study the deficiency ofshort stature homeobox containing (SHOX)gene deficiency ofshort stature and the relationship between genotypes and its corresponding phenotypes. Methods:Microsatellite analysis and direct sequencing were selected to analyze the deletions and mutations of SHOX gene in 203 cases of short stature;in addition ,anthropometric measures were assessed and radiographs of the left forearms and wrists in patients were collected to observe their phenotypes. Results:one mutation and eleven deletions were found;Detailed examination revealed certain dysmorphics,such as short forearm and lower leg,et al. Conclusion:In our study,the prevalence of SHOX deficiency in patients with short stature was 5.9% ,and certain correlations had been found between genotypes and corresponding phenotypes。

Objective To study the chemical constituents from Curcuma longa.Methods The chemical constituents were isolated and purified by various chromatographic methods and their structures were elucidated by the analysis of spectral data and physicochemical properties.Results One new compound was isolated from the EtOH extract of C.longa,which was named curcumaone J.Conclusion Curcumaone J is a new compound.

Second primary malignancies are more likely to occur in the multiple myeloma patients .Most of them are Acute myeloid leukemia ( AML) or Myelodysplastic syndrome ( MDS) .The incidence in male or those multiple myeloma patients who is less than 60 years old are more than female or those multiple myeloma patients who are bigger than 60 years old.In recent years,studies of multiple myeloma secondary to leukemia and other tumors are gradually increasing ,but the mechanism and treatment are still unclear .These diseases have the follow-ing characteristics such as progress rapidly ,high mortality rate and low complete remission rate after chemothera-py.

5.Matrine sustained release tablet, capsule and injection: comparison of pharmacokinetics and relative bioavailability

Objective:To compare the pharmacokinetics and relative bioavailability between matrine sustained release tablet,matrine conventional capsule and matrine injection. Methods: Dogs were given single oral dose and multiple doses of matrine sustained release tablet, capsule or injection in a randomized crossover way. Matrine concentrations in dog plasma were determined by RP-HPLC method. Results: The results showed that the t max and c max were 300 min and (5.088?0.490) ?g/ml for matrine sustained release tablet, (85?12) min and (6.360?0.215) ?g/ml for the conventional capsule, and 10 min and (6.500?0.404) ?g/ml for the injection. The relative bioavailability of the sustained release tablet was (153.7?9.4)% compared with that of the conventional capsule; the absolute bioavailability of the sustained release tablet was (73.5?14.2)% compared with that of the injecetion. 6.Randomized Control Trial of Xuebijing Injection in Adjuvant Treatment of Children with Perforated Appen-dicitis

OBJECTIVE:To evaluate the efficacy and safety of Xuebijing injection in the adjuvant treatment of children with perforated appendicitis. METHODS:72 children with perforated appendicitis in the Dept. of Pediatric General Surgery of our hospi-tal were enrolled into a prospective clinical trial,and randomly divided into experimental group (n=36) and control group (n=36). Both groups received the emergency surgery and anti-infective treatment;experimental group had additionally given Xuebijing injection. The white blood cell count (WBC),C-creation protein (CRP) and procalcitonin (PCT),liver and kidney function and hospitalization time before and after treatment were collected and adverse reaction. RESULTS:The WBC in experimental group af-ter 3 and 6 d of treatment was respectively(13.6±3.4)×109 L-1 and(9.1±4.2)×109 L-1,CRP was respectively(52.7±13.6)mg/L and(8.5±3.3)mg/L,the PCT(0.3±0.2)ng/ml,and the hospitalization time after 3 d were lower than control group,with signifi-cant difference(P<0.05). There were no significant difference in the liver and kidney function(P>0.05). There was 1 case with itch of skin but with no severe ADR. CONCLUSIONS:Xuebijing injection in the adjuvant treatment of children with perforated ap-pendicitis is safe and effective.
7.Self-assembly of plasma protein as a nanocarrier for lipophilic drugs

Objective Human serum albumin ( HSA) is a kind of plasma protein , which can transport various small molecu-lar substances to the body.A disulfide bond breaking strategy is used to fabricate HSA-curcumin nanoparticles(NPs-CCM) in order to improve the solubility and tumor targeting capability of curcumin . Methods NPs-CCM were fabricated with dithiothreitol as the re-ducing agent .The size and shape of the nanoparticles were observed with high resolution electron microscopy .We explored the change of hydrophobic region during the process of degeneration of albumin , the releasing behavior in the solution and the tumor targeting capa-bility in vivo and vitro of NPs-CCM. Results The diameter of NPs-CCM was about 150 nm.The determination of surface hydropho-bicity showed that ANS fluorescence intensity decreased gradually as the extension of degeneration time , at the time 0 min, 2 min, 5 min and 10 min, ANS relative fluorescence intensity was 5492(5432-5673), 2486(2319-2878), 1998(1904-2317), 1503 (1280-1584) respectively.The hydrophobic region of albumin gradually reduced during the degeneration time .The curcumin re-leased from NPs-CCM amounted to about 50% within 40 h, and the NPs-CCM can be absorbed by tumor cells and accumulation in tumor tissue. Conclusion Albumin can improve the solubility and tumor targeting capability of curcumin , and it has potential clinical application value.
8.The effects of respiratory training on pulmonary function in patients with chronic obstructive pulmonary disease

Objective To observe the effects of respiratory training on pulmonary function in the patients with chronic obstructive pulmonary disease(COPD). Methods A total of 74 patients with COPD were recruited and randomly divided into a treatment group and a control group with 37 subjects in each.The treatment group re-ceived respiratory training including lip-contracting breathing,abdominal breathing and breathing exercises for 6 months in addition to routine medical treatment.The control group received only routine medical treatment.Pulmona-ry function was evaluated before and after treatment. Results After treatment,both groups improved in terms of all the pulmonary function measures,showing increased vital capacity(VC),forced expiratory volume after 1 s(FEVl),peak forced expiratory flow rate(PEFR),as well as forced expiratory flow at 25%of Vcmax(V25),de-creased residual volume and residual volume as a percentage of total lung capacity(RV/TCL).The difference be-tween the two groups was significant with regard to the extent of improvement. Conclusion Respiratory training can improve the pulmonary function of COPD patients.
9.Assay of serum growth arrest-specific gene product 6 and stromal cell-derived factor isoform-1 in patients with diabetic retinopathy

Background Diabetic retinopathy (DR) is a progressive vision-threatening complication of diabetes mellitus (DM),but its pathogenic mechanism is still unclear.Researches showed that growth arrest-specific gene product 6 (Gas6) /TAM system participates in pathogenesis and development of DR,and stromal-derived factors (SDF) vary in 2-type DM patients.However,whether Gas6/TAM and SDF-1 are associated with DR is below understood.Objective This study was to determine the relationship between the staging of DR and the levels of serum Gas6,SDF-1α and SDF-1β in DM patients.Methods A prospective cohord study was designed in this study.Ninety 2-type DM patients were included in the 3rd Affiliated Hospital of Southern Medical University Hospital from January to August in 2013.The patients were grouped into the non-diabetic retinopathy (NDR) group,background DR group (BDR) and proliferative DR (PDR) group,with 30 for each group.Thirty normal volunteers were enrolled in the same hospital and same period.The periphery blood 2 ml was collected from all the subjects under the consent inform.The levels of serum Gas6,SDF-1α and SDF-1β were assayed by ELISA,and leukocytes,neutrophils,plasma triglycerides (TG),total cholesterol (CHOL),high density lipoprotein cholesterol (H DL-C) and low density lipoprotein cholesterol (LDL-C) levels were detected and compared among the 4 groups.The correlations of serum Gas6,SDF-1 α and SDF-1 β changes with blood inflammatory cells and blood lipid were analyzed.Results The plasma CHOL concentrations were 4.93(4.14,5.44),5.02(4.35,5.69),4.54(3.85,5.93) and 5.99(5.11,6.89)mmol/L in the normal control group,NDR group,BDR group and PDR group,respectively,and the blood CHOL concentrations were significantly higher in the PDR group than those of the normal control group,NDR group,BDR group (P =0.002,P =0.007,P =0.006).White blood cell counts in the normal control group,BDR group were higher than those of the PDR group (P =0.034,P =0.015),neutrophil counts in the BDR group were higher than those of the PDR group (P =0.024),HDL-C in the NDR group was higher than that in the PDR group (P =0.032).LDL-C in the PDR group was higher than that in the normal control group.Compared with the normal control group,serum Gas6 levels were significantly lower in the NDR group and BDR group (P =0.048,P =0.006),and the serum Gas6 level showed an insignificant increase in the PDR group in comparison with BDR group (P =0.297).Serum SDF-1α levels in the PDR group was significantly higher than that in the BDR group (P =0.033) ;serum SDF-1β levels in the PDR group,BDR group were significantly higher than that in the NDR group (P =0.011,P =0.008) and normal control group (P =0.030,P =0.002).Weaker positive correlation was observed between the serum Gas6 and CHOL,TG,LDL-C levels (r=0.285,r=0.200,r=0.241,all at P<0.05),between SDF-1α and SDF-1β (r=0.190,P<0.05) as well as between SDF-1β and white blood cell (r=0.183,P<0.05).Serum Gas6 served as dependent variable,while white blood cell,neutrophil,CHOL,TG,HDL-C,LDL-C,SDF-1α,SDF-1β served as independent variables,multiple stepwise regression analysis showed Gas6 =170.791 + 5.283CHOL (F =5.021,P =0.027).Conclusions Serum Gas6,SDF-1α and SDF-1β probably participate in the development of DR in 2-type diabetic patients.Gas6,SDF-1 α,SDF-1 β may play roles by affecting blood glucose level,angiogenesis,inflammatory cells and blood lipid metabolism.
10.Optimum ratio of medicine dosage for dexmedetomidine mixed with sufentanil used for PCIA after Nuss procedure in pediatric patients with pectus excavatum

Objective To evaluate the optimum ratio of medicine dosage for dexmedetomidine mixed with sufentanil used for patient?controlled intravenous analgesia ( PCIA) after Nuss procedure in pedi?atric patients with pectus excavatum. Methods Sixty pediatric patients diagnosed with pectus excavatum, aged 5-12 yr, weighing 18-50 kg, of ASA physical statusⅠorⅡ, scheduled for elective Nuss procedure under general anesthesia, were randomly divided into 3 equal groups using a random number table:different ratios of medicine dosage while dexmedetomidine was added to sufentanil groups ( SD1-3 groups) . Postopera?tive analgesia was as follows: group SD1 received sufentanil 1 μg∕kg + dexmedetomidine 2 μg∕kg; group SD2 received sufentanil 1 μg∕kg + dexmedetomidine 3 μg∕kg; group SD3 received sufentanil 1 μg∕kg +dexmedetomidine 4 μg∕kg. A mixture of tropisetron 0?1 mg∕kg and dexamethasone 0?1 mg∕kg ( in 100 ml of normal saline) was added in each group. The PCA pump was programmed to deliver 0?5 ml with a lockout interval of 15 min and background infusion at 2 ml∕h. The PCA pump was connected immediately after the end of operation, and sufentanil with a dosage of 0?1μg∕kg was used as a rescue analgesic within 48 h post?operatively. The VAS score was maintained below 4. The requirement for rescue analgesics was recorded. The Ramsay sedation scores was recorded at 4, 8, 12, 24 and 48 h postoperatively, and the occurrence of adverse reactions such as nausea and vomiting, bradycardia, over?sedation, respiratory depression, agitation and shivering was recorded within 48 h after surgery. Results No pediatric patients developed nausea and vomiting, respiratory depression, bradycardia, over?sedation, and shivering. No pediatric patients required rescue analgesics in SD2 and SD3 groups. Compared with group SD1 , the requirement for rescue analgesics and incidence of agitation were significantly decreased, and Ramsay sedation scores were increased at 4 and 8 h after operation in SD2 and SD3 groups. Ramsay sedation scores were significantly higher at 4 h after oper?ation in SD3 group than in SD2 group. Conclusion Dexmedetomidine 3 μg∕kg mixed with sufentanil 1μg∕kg is the optimum ratio of medicine dosage when used for PCIA after Nuss procedure in pediatric patients with pectus excavatum.
