Objective:To evaluate the changes of the left atrial systolic function in patients with acute myocardial infarction(AMI)using quantitative tissue velocity imaging(QTVI).Methods:The systolic velocities of the middle of left atrial lateral wall, anterior wall, inferior wall and atrial septum were measured with QTV1 in 45 patients with AMI.The left atrial volume and active atrial emptying fraction(AA-EF)were measured using single-plane Simpson method.Results:(1)Compared with the control group(30 normal subjects),the diameter and volume of the left atrium,as well as AA-EF,increased obviously in patients with AMI(P < 0.01).(2)Compared with the control group, the systolic velocities of the middle of left atrial lateral wall, anterior wall, inferior wall and atrial septum,as well as the average value, increased significantly in patients with AMI (P < 0.05 or P < 0.01).In addition, the average value of left atrial systolic velocity was closely correlated with AA-EF(r=-0.906,tr=14.001,P < 0.01).Conclusion:QTVI could be used to evaluate the left atrial systolic function accurately in patients with AMI.

Objective To study the level and influencing factors of sense of coherence (SOC) among elderly patients with Parkinson's disease (PD).Method Totally 118 elderly patients with PD were investigated by a self-designed demographic questionnaire, sense of coherence-13 (SOC-13) and social support rating scale (SSRS),and to explore the level and influencing factors of SOC among the patients. Results The total score of SOC-13 was (52.27±9.30),and it was at a low level.Multiple regression analysis showed that educational level, severity of the disease, family economic status and social support were the influencing factors of SOC.Conclusion SOC of elderly patients with PD is at a low level, which is influenced by individual factors, nurses should take targeted measures to improve the patients' SOC status by the influencing factors.

Objective To know the change of medical care and health services utilization of Yayuncun community residents in Beijing. Methods Using the prospective study, residents above 15 years old were selected to answer the Beijing Typical Community Resident Questionnaire designed by the local researchers. In 2004 and 2005, the baseline survey was carried. From 2006 to 2008, same questionnaire was used to follow-up survey, and 1412 residents were selected into the last study. Results The proportion of people who should see a doctor but not in past month were decreased, the differences among the three years were significant (P < 0. 01 ); The top three reasons that who should see a doctor but not were the "medical care costs too high", "self- felt the sickness light", "self-treatment". The residents who went to the community health agencies increased from 2006 to 2008. The medical costs ( drugs, medical fees, inspection fees) decreased from 2006 to 2008, the differences were significant (P <0. 01 ) ;The satisfaction increased from 2006 to 2008, the differences were significant ( P < 0. 01 ). Conclusion Under the comprehensive promotion of community health services in Beijing, the satisfaction and the utilization of medical care and health services were improved.

OBJECTIVE:
To assess the efficacy and safety of aromatase inhibitors (AIs) combined growth hormone in treatment of adolescent boys with short stature.
METHODS:
One hundred and fifty-one short stature pubertal boys with age of 10-14 years and bone age of 13-15 years, who were admitted to the Department of Pediatrics, the First Affiliated Hospital, Zhejiang University School of Medicine, were included in this trial. According to their own or parents' intention, the children were divided into recombinant human growth hormone (rhGH)+AI group ( =108) and rhGH group ( =43). All children were injected subcutaneously with rhGH 0.15-0.2 IU·kg ·d , and those in rhGH+AI group were additionally given 2.5 mg/d letrozole or 1 mg/d anastrozole, orally for 12 months or longer. The children were followed-up every 3 months. During the follow-up visit, the predicted adult height (PAH), sex hormone level, glucose and lipid metabolism, and other indicators were measured, and adverse reactions were monitored.
RESULTS:
After intervention, there were significant differences in ΔBA(bone age)/ΔCA(chronological age), ΔHtSDS (height standard deviation score based on bone age)and ΔPAH between rhGH+AI group and the rhGH group( < 0.05 or < 0.01). During follow-up, 63.9%of the children in the rhGH+AI group had elevated uric acid and 51.9%had decreased high-density lipoprotein (HDL); 25.9%showed severe acne, excitement, hyperactivity and irritability, 11.1%had knee pain; 4.6%had fracture; 2.8%had mild renal dysfunction; 1.9%had inactivity, drowsiness, memory loss and performance decline; 1.9%showed mild abnormal liver function; 0.9%showed impaired fasting glucose; 0.9%showed granulocytopenia. In the rhGH group, 11.6%of the children presented with knee pain and 2.3%with impaired fasting glucose.
CONCLUSIONS
AI combined with rhGH can delay the growth of BA and effectively improve the PAH of adolescent boys with larger bone age. However, the occurrence of adverse reactions of AI should be closely monitored during treatment.

Objective To explore the effects of rapid response first-aid case on bedside emergency gastroscopy hemostatic treatment. Methods A total of 52 patients with esophageal-fundal varices in liver cirrhosis from January 2014 to June 2015 were divided into control group (n=25) and experimental group (n=27). The patients of control group received the routine nursing; then we summarized the experience and mistakes of operation cooperation, optimized process;next we designed rapid response first-aid case and used in the experimental group. The blood transfusion time, goods preparation time, blood loss, blood transfusion volume were compared in two groups. Results The data of the experimental group in average bleeding to transfusion time (90. 22 ± 36. 47)min, materials preparation time (14. 66 ± 3. 48) min, the average amount of bleeding (369. 62 ± 158. 90) ml, average blood transfusion amount (322. 40 ± 117. 82) ml were lower than those of the control group [ average bleeding to blood transfusion time ( 123. 60 ± 51. 87 ) min, materials preparation time ( 22. 44 ± 2. 59 ) min, average bleeding volume ( 660. 20 ± 181. 82 ) ml, average blood transfusionamount(458.00±140.63)ml](P<0.05).Conclusions Theimplementationofrapidresponse first-aid case can effectively shorten the rescue time, reduce bleeding and blood transfusion time, the blood loss, the wastage of human resources and medical resources, improve the efficiency of nursing work in the department and the success rate of rescue.
