1.Prediction of cognitive decline among elderly patients with type 2 diabetes mellitus

Objective:
To predict the risk of cognitive decline among elderly patients with type 2 diabetes mellitus (T2DM), so as to provide insights into the early identification of cognitive decline in elderly patients with T2DM.
Methods:
The elderly patients with T2DM hospitalized in the Qingpu Branch of Zhongshan Hospital Affiliated to Fudan University were selected as subjects. General information was collected by a questionnaire survey, including demographics and nutritional status, and cognitive decline was assessed using the frailty Phenotype Scale, the Montreal Cognitive Assessment and the Clinical Dementia Rating. Multivariable logistic regression model was used to analyze the influencing factors of cognitive decline among elderly patients with T2DM. A nomogram was established, and was verified with Bootstrap resampling method and receiver operation characteristic curve.
Results:
A total of 270 questionnaires were sent out and 262 valid questionnaires were collected, with an effective rate of 97.04%. There were 137 males (52.29%), 146 patients at ages of 60 to 69 years (55.73%), 179 married patients (68.32%), and 85 patients with cognitive decline (32.44%). Multivariable logistic regression analysis identified gender (female, OR=3.118), age (70 to 79 years, OR=3.218; 80 years and older, OR=3.058), educational level (high school or technical secondary school, OR=0.335; junior college, OR=0.130; college and above, OR=0.300), regular exercise (OR=0.083), memory (decreased, OR=29.723), nutritional status (risk of malnutrition, OR=16.307; malnutrition, OR=39.469), activity of daily living (decreased, OR=6.804) and depressive symptoms (OR=8.609) as factors affecting cognitive decline among elderly patients with T2DM (all P<0.05). The nomogram based on the above factors fit well and indicated strong predictive ability.
Conclusion
Gender, age, educational level, regular exercise, memory, nutritional status, activity of daily living and depressive symptoms can effectively predict the risk of cognitive decline among elderly patients with T2DM.
2.Application of shortcut nursing procedure in first-aid treatment of patients with severe head trau-ma

Objective To improve level of first-aid treatment of severe head trauma and reduce fa-tality. Methods Clinical nursing procedures were applied to patients.Patients need first-aid treatment (105 cases) were divided into the control group(50 cases) and the experimental group(55 cases).The con-trol group adopted routine first-aid procedure,the experimental group was given first-aid nursing with self-designed shortcut nursing procedure for severe head trauma.The effect of application of shortcut nursing procedure was observed by comparing the fatality and time consumption of first-aid treatment between the two groups. Results The fatality and time consumption of first-aid treatment in the experimental groupwas lower than that of the control group, P<0.05. Conclusions Application of shortcut nursing proce-dure in first-aid treatment can not only improve level of first-aid treatment and reduce fatality, but also can instruct nurses how to rescue patients and improve their professional and vocational level.
3.Establishment of pepsinogen reference intervals of healthy population in Changsha area

Objective To establish the reference intervals of serum pepsinogen(PG)in healthy adults in Changsha area,and to explore their distribution as well as related factors.Methods Serum PGⅠand PGⅡ levels were determined by a latex-enhanced turbidimetric immunoassay method in 1 279 subjects from Changsha area.Then PGⅠ/PGⅡ was calculated.Results The PGⅠ, PGⅡ and PGⅠ/PGⅡ levels of 1 279 healthy adult residents showed a skew distribution.The levels of serum PGⅠand PGⅠ/PGⅡ in males were 62.0 ng/mL and 4.3,which in females were 59.4 ng/mL and 4.1,significantly lower than those in males(P<0.05).Age was significantly positive correlated with serum concentrations of PGⅡ(r= 0.278,P<0.01),and was negative correla-ted with PGⅠ/PGⅡratio(r= -0.173,P<0.01).The normal reference ranges of serum PGⅠ,PGⅡ and PGⅠ/PGⅡ in males were≥56.8 ng/mL,≤22.6 ng/mL,and ≥3.3,respectively,and in females were ≥51.4 ng/mL,≤23.1 ng/mL,and ≥3.0,respec-tively.Conclusion The serum PG levels of healthy adult residents significantly show skew distributions in Changsha area,which are affected by gender and age.The establishment of reference intervals of PG contributes to clinical screening and diagnosis of gas-tric-related deseases.

5.Comprehensive assessment indexes of nursing process quality in hospitals

Objective To set up assessment indexes of nursing process quality in hospitals that are scientific, practical and in keeping with the times so as to provide hospital departments in charge of nursing with the basis for reasonable assessment and scientific decision-making. Methods Typical investigations were conducted to understand the content and methods of the current assessments of nursing process quality in hospitals; the basic scope of the assessment indexes was determined through meeting discussions; the indexes were selected, verified, and established by means of brainstorming and the Delphi method; field surveys were conducted to assess and analyze the nursing process quality in 25 departments. Results A system of comprehensive assessment of nursing process quality in hospitals was established. It covered four categories, viz. nursing services, nursing control, nursing skills and nursing safety, and decomposed into assessment indexes of different levels, with the overall structure consisting of 1 overall goal, 4 first-level indexes, 12 second-level indexes, 5 third-level indexes and 290 subindexes. Conclusion The new system of indexes, which stresses the idea of nursing services, highlights the quality control principle of focusing on the patients,and is characterized by clear-cut structure and easy operability, provides a reliable guarantee for scientifically and rationally assessing nursing process quality.
6.Quantitative comparison of parathyroid hormone 1 receptor between human periodontal ligament stem cells and human periodontal ligament cells

BACKGROUND:Human periodontal ligament stem cels are a kind of mesenchymal stem cels that have self-renewal and multidifferentiation potential. Previous studies have showed that human periodontal ligament stem cels can differentiate into osteoblast-like cels or adipocyte-like cels under appropriate induction. Yet few studies have focused on the expression level of parathyroid hormone 1 receptor which wil affect the osteogenic potential of Human periodontal ligament stem cels. OBJECTIVE:To examine the expression level of parathyroid hormone 1 receptor between human periodontal ligament stem cels and human periodontal ligament cels and to discuss the role of parathyroid hormone 1 receptor in osteogenic differentiation. METHODS:By using magnetic-bead cel sorting, we separated and identified the human periodontal ligament stem cels and human periodontal ligament cels. We examined and compared the mRNA expression level of parathyroid hormone 1 receptor in human periodontal ligament stem cels and human periodontal ligament cels by Real-Time PCR. Osteoblastic differentiation was examined throughin vitro matrix mineralization by alizarin red staining and alkaline phosphatase assay. RESULTS AND CONCLUSION: Positive immunomagetic sorted cels were positive for STRO-1, CD146, Vimentin, indicating that they were periodontal ligament stem cels. Parathyroid hormone 1 receptor was expressed in human periodontal ligament stem cels and mainly located in cel membrane and cytoplasm which were similar to human periodontal ligament cels and MG63 cels. The expression of parathyroid hormone 1 receptor in human periodontal ligament stem cels was 3.7 times higher than that in human periodontal ligament cels, which was similar to that in MG63 cels. After osteogenic induction, human periodontal ligament stem cels showed a higher expression of parathyroid hormone 1 receptor and osteoblast-related genes as wel as the activity of osteoblast alkaline phosphatase and mineralization compared to human periodontal ligament cels. Our data showed that parathyroid hormone 1 receptor was higher in human periodontal ligament stem cels than human periodontal ligament cels and the expression was related with osteogenic differentiation, suggesting that human periodontal ligament stem cels display a higher potency of osteogenic differentiation and act as seed cels with a vast application prospect in oral tissue engineering.

8.Constructing a clinical practice patterns for intensive care nursing masters with professional degree based on competence based education

Objective Constructing a scientific and practical clinical practice patterns for intensive care nursing masters with professional degree.Methods Based on the competence based education,a clinical practice patterns for intensive care nursing master with professional degree was developed by literature review,questionnaire survey,unstructured interview and Delphi technique.Results 16 experts from 10 third-level and first-class hospitals and 1 Nursing College were consulted,the retrieving rates of questionnaire were 93.75%,100.00%,respectively.The authoritative coefficients were 0.94 and the coordination coefficients were 0.257.The index system consisted of 6 first dimensions,47 second-dimensions and 112 items.Conclusions The positivity and authority of experts in this study were in high degree,the concentration and harmonization of experts' opinions were relatively high.The results had high reliability and feasibility.It can be used to provide objective and quantitative reference for intensive care nursing master with professional degree.
9.Influence of esmolol on plasma NT-proBNP, heart-type fatty acid binding protein and cardiac function in acute cerebral infarction patients with myocardial enzyme abnormalities

Objective To analyse the influence of esmolol on plasma N terminal pro-brain natriuretic peptide ( NT-proBNP) , heart-type fatty acid binding protein ( H-FABP) and cardiac function in acute cerebral infarction patients with myocardial enzyme abnormalities.Methods 86 acute cerebral infarction patients with myocardial enzyme abnormalities from September 2010 to July 2014 in our hospital were selected and divided into experimental group and control group randomly (43 cases in each group).All patients were accompanied by myocardial enzyme abnormalities, patients in control group were given basic treatment, while patients in experimental group were given esmolol hydrochloride intravenous injection on the basis of control group.Plasma NT-proBNP, H-FABP levels and cardiac function were analyzed in two groups.Results Myocardial enzyme levels had no significant difference between two groups on admission.Plasma NT-proBNP levels also had no statistical differences after admission hospital of 6 h, 15 h and 3 d , and experimental group was significantly lower than control group ( P<0.05 ) after admission hospital of 5 day and 7 days.H-FABP levels had no statistical differences after admission hospital of 6 h, 15 h, and the experimental group was significantly lower than the control group (P<0.05) after admission hospital of 3, 5, 7 days.Cardiac function with Killip classification showed no significant difference when on admission and after admission hospital of 3 days, and the experimental group was significantly better than control group (P<0.05) after admission hospital of 5, 7 days.Conclusion Acute cerebral infarction patients with myocardial enzyme abnormalities given esmolol on the basic treatments significantly reduce NT-proBNP and HAFBP levels, and improve cardiac function, which is worthy of promotion in clinical applications.
10.Metabonomics-a useful tool for individualized cancer therapy.

Metabonomics has developed rapidly in post-genome era, and becomes a hot topic of omics. The core idea of metabonomics is to determine the metabolites of relatively low-weight molecular in organisms or cells, by a series of analytical methods such as nuclear magnetic resonance, color spectrum and mass spectrogram, then to transform the data of metabolic pattern into useful information, by chemometric tools and pattern recognition software, and to reveal the essence of life activities of the body. With advantages of high-throughput, high-sensitivity and high-accuracy, metabolomics shows great potential and value in cancer individualized treatment. This paper introduces the concept,contents and methods of metabonomics and reviews its application in cancer individualized therapy.
