1.Mild hypothermia and beating heart bypass in patch therapy for 92 cases of atrial septal defect

Objective To explore the advantage of mild hypothermia and beating heart bypass in patch therapy for atrial septal defect. Methods 92 patients with atrial septal defect were carried out surgical repairing by mild hypothermia and beating heart bypass. Results There was no death. There was 1 case complicated by cerebral air embolism;The consciousness recovered after 3 days of operation. There were 2 cases of pneumonia. Conclusion The patch therapy of mild hypothermia and beating heart bypass for atrial septal defect had small influence on internal environmental,fewer complications,and could be easy to carry out,and could be the first choice.
2.Preparation and release mechanism of gestodene reservoir-type intravaginal rings.

This study taking gestodene (GEST) as a model, investigated the factors affecting reservoir-type intravaginal ring (IVR)'s drug release. This paper reported a gestodene intravaginal ring of reservoir design, comprising a gestodene silicone elastomer core encased in a non-medicated silicone sheath, separately manufactured by reaction injection moulding at 80 degrees C and heating vulcanization at 130 degrees C is reported. The test investigated the factors affecting drug release through a single variable method, taking the drug release rates of 21 days as standards. When changing the thickness of the controlling sheath outside, the ratio of the first day of drug release and mean daily release (MDR), named the relatively burst effect, is closing to 1 with the thickness of controlling sheath increasing, while the 1.25 mm sheath corresponding to 1.04 controlled the burst release effectively; a positive correlation (r = 0.992 2) existed between the average drug release (Q/t) and drug loading (A) within a certain range. The C6-165 controlling sheath with high solubility of GEST is easier to achieve controlled release of the drug; GEST crystalline power is more effective to implement controlled release of drugs among difficent states of the drug. A 1/4 fractional segment core gives a relatively burst effect of 1.76, while the 1/1 and 1/2 are 1.93 and 1.87 separately, at the same drug loading, concluding that use of a fractional segment core would allow development of a suitable GEST reservoir IVR. In summary, GEST reservoir-type IVR could be adjusted by the thickness of controlling sheath, the loading of drug, the material properties of controlling sheath, the dispersion state of drug, the additive composition and structure of intravaginal ring, to control the drug release behavior and achieve the desired drug release rate.
3.Inhibition control characteristics in college students with different mobile phone addiction

Objective:To investigate the inhibition control characteristics in college students with mobile phone addiction,then to design tactics to enhance their inhibition control ability.Methods:Totally 150 college students were surveyed with the Mobile Phone Addiction Index (MPAI)to filter out the high and low mobile phone addiction.Thirty students with more than eight affirmative answers to the seventeen MPAI items were selected as the mobile phone addiction group while another 30 students with less than eight affirmative answers were selected as non addicts group of MPAI.All participants were asked to complete task of stop signal.Results:In college students with mobile phone addiction,the stop-signal reaction times were longer than that in the students without mobile phone addiction [(292.0 ±89.4)ms vs.(241.1 ±95.7)ms,P <0.05],while the non-signal reaction times were shorter than that in those without mobile phone addiction [(494.2 ±103.8)ms vs.(597.6 ±108.9 )ms,P<0.01].Conclusion:It suggests that the college students with mobile phone addiction may have poorer inhibitory control.
4.Risk factors of the complications following enteral nutrition in ICU critically ill patients

Objective To investigate the enteral nutrition(EN)complications in ICU critically ill patients and their risk factors.Methods The relevant information of 376 EN patients from three tertiary hospitals in Qingdao from September 2015 to April 2016 were retrospectively analyzed.The incidences of EN complications and their risk factors were analyzed.Results In these 376 patients,the complications included diarrhea(n=117,31.12%),constipation(n=89,23.67%),and reflux(n=102,27.13%).The incidence of diarrhea showed significant difference in terms of the application of antibiotics(OR=3.55),age(OR=1.03),fasting(OR=4.07),oral potassium preparations(OR=2.91),nasal feeding rate(OR=1.03),and nasal feeding volume(OR=1.002).The incidence of constipation showed significant difference in terms of nasal fluid type(OR=7.79),analgesics(OR=10.34),age(OR=1.06),nasal feeding rate(OR=0.95),and nasal feeding volume(OR=0.97).The incidence of reflux showed significant difference in terms of the use of analgesics(OR=2.143),mechanical ventilation(OR=2.071),nasal feeding pathway(OR=1.838),age(OR=1.025),nasal feeding rate(OR=1.042),and nasal feeding volume(OR=1.001).ConclusionApplication of antibiotics,increased age,fasting,oral potassium preparations,increased nasal feeding rate,and increased daily feeding volume are risk factors of EN-related diarrhea;short peptide EN solution,application of analgesics,and increased age are risk factors of EN-related constipation for enteral nutrition patients;and mechanical ventilation,application of nasogastric tube,increased age,increased nasal feeding rate,and increased nasal feeding are risk factors of EN-related relux in enteral nutrition patients.
5.Comparison of infrared mrker-based positioning system and electronic portal imaging device for the measurement of setup errors

ObjectiveTo measure the setup errors with infrared marker-based positioning system (IM-BPS) and electronic portal imaging device (EPID) for patients with esophageal carcinoma and lung cancer and investigate the accuracy and practicality of IM-BPS. MethodsFrom January 2007 to January 2008, 40 patients with esophageal carcinoma and 27 patients with lung cancer received three-dimensional conformal radiotherapy or intensity-modulated radiotherapy, setup errors during the treatment were measured with IM-BPS and EPID, and the data of setup errors were compared with paired t-test and agreement with x2-test. ResultsIt takes 10 - 12 mins to complete the validating for each patient by EPID) system, while IMBPS system only needs 2 -5 mins. The mean setup errors along x, y and z-axis for patients with esophageal carcinoma measured by IM-BPS and EPID were 3.49 mm, 3. 19 mm, 3.31 mm and 4. 03 mm, 3.41 mm, 3.43 mm, respectively. For the patients with lung cancer, the setup errors were 4. 23 mm, 3.51 mm, 3. 39mm and 4. 85 mm, 3. 53 mm, 3.74 mm, respectively. The difference of setup errors meanured by the two systems was within 1 mm for 65% esophageal carcinoma patients ( x2 =51.09, P =0. 000), and 55% lung cancer patients ( x2 =53. 35, P =0. 000).Conclusions The measurement results of setup errors for patients with esophageal carcinoma and lung cancer show that IM-BPS is mostly better than EPID. Though validating for patients can be measured accurately and be well quality controlled, IM-BPS is used easily because of macroscopic, homely,spare time and real-time monitoring.

7.Surgical treatment of shoulder joint posterior dislocation secondary to internal rotation contractnre deformity in brachial plexus birth palsy

ObjectiveTo observe the functional recovery of shoulder joint and the reduction of posterior dislocated humeral head in children with shoulder joint internal rotation contracture and humeral head posterior dislocation secondary to brachial plexus birth palsy treated by a modified surgical procedure through the anterior combined posterior approach of the shoulder. MethodsNinteen patients,ranging in age from 2.5 to 8.5 years (average 5 years),suffered posterior dislocation of the shoulder joint secondary to internal rotation contracture in brachial plexus birth palsy. The gleno-humeral joint deformity was confirmed by X-ray and CT examination and classified as type Ⅳ in 15 eases and typeⅤin 4 cases according to the modified water's criteria.The surgical procedure was as follows:the contracture soft tissue around the anterior of shoulder joint was released firstly through the anterior approach, and the posterior-inferior capsule of the shoulder was exposed and separated with the pseudoglenoid through the posterior approach,the humeral head was reduced by external rotation the arm,then the posterior-inferior capsule was retighten.A plaster cast was used to fix the shoulder at the neutral position of 0° for 4 weeks. ResultsAfter 12 to 36 months follow up(average of 20 months), the Mallet score of the shoulder was from 11.4 ± 1.7 (range 7-16)preoperative to 15.5 ± 1.8(range 13-19) postoperative,the difference was significantly (P < 0.05).The central relocation of humeral head was achieved in 16 patients, but the humeral head was still dislocated to posterior in 3 cases.ConclusionsThe posterior-inferior capsule was separated with the pseudo-glenoid and retighten through the posterior approach,and reduction of the humeral head by soft tissue releaseing through the anterior approach can recover the concentric relationship of gleno-humeral joint and improve the function of shoulder joint with posterior dislocation secondary to internal rotated contracture deformity in brachial plexus birth palsy.
8.Effects of the crude and processed Epimedium Sagittatum Maxim. on castrated mice

AIM: To observe the supplementation action of the crude and processed Epimedium Sapittatum Maxim. METHODS: The effects of different extracts of Epimedium Sagittatum Maxim. on the castrated mice were observed. RESULTS: The aqueous and alcohol extracts of the crude and processed Epimedium Sagittatum maxim. showed same marked inhibition on atrophy of castrated mice. CONCLUSION: The crude and processed Epimedium Sagittatum maxim. both have the supplementation action and show no marked difference in action.
9.Application of Different Blocking Ways at the First Hepatic Portal in Precise Hepatectomy

Objective To investigate the differences in the safety of the operation of different hepatic vascular exclusion for liver surgery. Methods Sixty patients with liver resection were grouped by different hepatic blood flow blocking methods, and given pre-operative assessment prior to surgery. Results On the first day after surgery, the average levels of ALT and AST were (395.0 ± 220.2) U/L and (415.3±311.0) U/L in patients who received Pringle’s method (110.2±53.0) U/L and (125.6±78.5) U/L in patients who received regional hepatic vascular exclusion, (98.9±32.2) U/L and (96.2 ±66.5) U/L in patients who didn't receive hepatic vascular exclusion, respectively. Postoperative liver function damage was more serious in patients who received Pringle's method than patients who received regional hepatic vascular exclusion or patients who didn't receive hepatic vascular exclusion, the difference was statistically significant (P<0.05) .Conclusion Regional hepatic vascular exclusion or not can not only reduce the incidence of postoperative complications, but also expand the indications for liver resection.
10.Effects of Bcl-2 associated with athanogene-1 gene silencing on heat shock protein 70 expression and human neuroblastoma cell apoptosis induced by hypoxia

Objective To investigate the protective effects of Bcl-2 associated with athanogene-1 (BAG-1) gene on human neuroblastoma cells (SH-SY5Y) injury induced by hypoxia/reoxygenation,and its influence on heat shock protein 70 (HSP70) expression.Methods The SH-SYSY cells at logarithmic growth phase were collected.Lenti virus mediated RNA interference (RNAi) technology was used to suppress the BAG-1 expression.The cells screened out can be divided into four groups:the cell control group with no lentivirus infection,lentivirus control group (containing only fluorescein protein lentivirus infection),BAG-1 siRNA group (BAG-1 siRNA silencing group),including BAG-1 siRNA-α group and BAG-1 siRNA-β group with lentivirus containing fluorescein protein (GFP) but at different BAG-1 siRNA target sites of silencing.Western Blot was used to detect the protein expression of BAG-1 and HSP70 in target cells after infectious recombination lentivirus for 72 hours;the Cell Counting Kit-8 (CCK-8) was used to detect the activity of four different group cells after hypoxia;the flow cytometry was used to detect the cell apoptosis;the HSP70 mRNA transcription level were detected by real-time fluorescent quantitative reverse transcription-polymerase chain reaction (RT-qPCR) respectively in each group.Results After lentiviral infection for 72 hours,the Western Blot results showed that in the two BAG-1 siRNA silencing groups,the interference effect of BAG-1 siRNA-β group was superior to that of BAG-1 siRNA-α group.The cell viability of each group showed an increase followed by a decrease with the prolongation of hypoxia time,and reaching the peak at 8 hours.After hypoxia for 8 hours being given,the cell viability in BAG-1 siRNA-β group was significantly lower than that of the cell control group,lentivirus control group and BAG-1 siRNA-α group (A value:0.59 ±0.09 vs.0.94±0.12,0.90± 0.11,0.91± 0.14,P < 0.01);the cell apoptosis rate was obviously higher in BAG-1 siRNA-β than that in the above three groups [(34.63 ± 3.46)% vs.(14.83 ± 3.75)%,(19.93 ± 6.49)%,(16.40± 1.18)%,all P < 0.01].There were no statistically significant differences in the HSP70 protein level and mRNA transcription level between BAG-1 siRNA-3 grroup,and the cell control group,lentivirus control group and BAG-1 siRNA-α group respectively (all P > 0.05).Conclusion BAG-1 gene can play a role in protection of hypoxia nerve cells,reduce the apoptosis,and its protective effect can be independent of HSP70 gene.
