2.Association between thyroid-associated antibodies and ovarian reserve in infertile women:a clinical study of 526 cases

Objective To compare the positive rate ofthyroid autoantibodies in infertile women with different ovarian reserve function,and investigate the immune factors of diminished ovarian reserve.Methods A cross-sectional study was conducted on infertile women admitted to Department of Reproductive Medicine of Beijing Gynaecology and Obstetrics Hospital from June to December,2020.The levels of anti-Müllerian hormone (AMH ),thyroid stimulating hormone (TSH ),thyroglobulin antibody (TGAb ) and thyroid peroxidase antibody (TPOAb)were detected in the 526 enrolled infertile patients.According to their AMH level,they were divided into normal ovarian reserve group and diminished ovarian reserve group.After they were stratified according to their age,the differences of TSH,TGAb and TPOAb levels were compared in the different age groups to analyze the related factors for diminished ovarian reserve.Results Univariate analysis showed that the diminished ovarian reserve group had significantly higher positive rates of TPOAb (18.8% vs 11.1%,P=0.024)and TGAb (18.8% vs 8.0%,P=0.001 )than the normal ovarian reserve group.Multivariate logistic regression analysis indicated that age and TGAb positivity were related to diminished ovarian reserve[OR=1.083(95% CI:1.021~1.150),P=0.008;OR=1.159(95% CI:1.034~1.301 ),P=0.011].Subgroup analysis suggested that the positive TGAb and TPOAb were significantly correlated with AMH level in the 36-~40-year-old group (P<0.05).Conclusion The infertile women with diminished ovarian reserve have higher TGAb and TPOAb levels,and the diminishment in those aged 36~40 years might be related to the positive TGAb and TPOAb.
3.A study on the impact of problem-based simulation training video feedback combined with hierarchical management on the core competence of elderly care personnel

Objective:To analyze the impact of problem-based simulation training combined with hierarchical management on the core competencies of elderly nursing staff.Methods:A total of 150 elderly patient nurses who worked in Sichuan Science City Hospital from June 2021 to March 2022 were subjected to a 6-month problem-based simulation training combined with hierarchical management since March 2022. The professional nursing abilities, core nursing abilities, nursing quality, and satisfaction of nursing staff with teaching were compared before and after training. Paired t-test was performed using SPSS 23.0. Results:The theoretical scores and skill operations of nursing staff for elderly patients were higher after the training than before the training ( P<0.05). The clinical nursing ability, critical thinking in scientific research, leadership ability, interpersonal relationships, professional development ability, educational guidance, ethics, legal practice score, and overall core competency score of nursing staff for elderly patients were all higher than before training ( P<0.05). The basic nursing care, nursing service attitude, nurse patient communication, document writing, and nursing operation scores of nursing staff for elderly patients were higher than before training ( P<0.05). The scores of humanistic care, dietary guidance, life care, and rehabilitation guidance all increased ( P<0.05). Conclusions:The problem-based simulation training combined with hierarchical management can improve the professional ability, core nursing ability, and nursing quality of nursing staff for elderly patients. Both patients and nursing staff acknowledge the effectiveness of this nursing approach.
4.Transapical approach under 3D printing guidance for myectomy in apical hypertrophic cardiomyopathy: A case report

We reported a 26-year-old male who was diagnosed with apical hypertrophic cardiomyopathy with left ventricular aneurysm. The location of the hypertrophic myocardium and the extent of resection were accurately assessed preoperatively using 3D modeling and printing technology. Myectomy was performed via transapical approach, and the intraoperative exploration was consistent with the description of the preoperative 3D modeling. The patient underwent the surgery successfully without any complications during the hospitalization, and the cardiopulmonary bypass time was 117 min, the aortic cross-clamping time was 57 min, and the hospital stay time was 7 d. The postoperative echocardiography demonstrated left ventricular cavity flow patency. This case provides a reference for the management of patients with apical hypertrophic cardiomyopathy.
6.Survey on psychosocial services in northeast Sichuan

ObjectiveTo investigate the current status of psychosocial services in various institutions as well as the mental health status of residents in Northeast Sichuan, so as to provide references for the further construction and implementation of psychosocial services in this area. MethodsA total of 148 institutions in Tongjiang county of Bazhong city, Lizhou district of Guangyuan city and Dazhu county of Dazhou city were surveyed through self-compiled questionnaires covering the construction status of psychosocial service system and the implementation of mental health service in each institution. Meantime, the mental health status and psychological service needs of 21 505 residents in pilot areas of three cities were investigated using the Patients' Health Questionnaire Depression Scale-9 item (PHQ-9), Generalized Anxiety Disorder Scale-7 item (GAD-7) and the self-designed mental health service needs questionnaire. ResultsAmong the 148 institutions in the pilot areas, 81 (54.7%) of which had dedicated mental health service, and 58 (39.2%) were equipped with full-time or part-time mental health service personnel. In 2019, 95 (64.2%) institutions conducted mental health services for employees, and 104 (70.3%) conducted mental health propaganda activities. Of the 75 educational institutions, 67 (89.3%) conducted mental health education for students, and 47 (62.7%) achieved full coverage of the mental health education curriculum among students. The detection rates of depression and anxiety among the residents were 36.8% and 30.8%, respectively, and 83.7% of the residents had the mental health service needs, mainly in the aspects of personal growth, marriage and family, children's education and stress management. ConclusionThe psychosocial services in the pilot areas of the three cities in northeast Sichuan are well conducted, while the guarantee of workplace, funds and personnel remains to further strengthen. Furthermore, residents have prominent emotional problems such as depression and anxiety, and have a high demand for mental health services.
7.Social support status and influencing factors among schizophrenics in remission in Northeast Sichuan

ObjectiveTo investigate the social support status and influencing factors of schizophrenics in remission in Northeast Sichuan, and to provide ideas for improving their social support. MethodsFrom May to September 2020, a total of 533 patients who met the diagnosis criteria of the International Classification of Diseases, tenth edition (ICD-10) for schizophrenics in remission at the mental health institutions in Guangyuan, Bazhong and Dazhou cities were selected for the survey, and patients were assessed by self-made demographic and clinical data inventory and Social Support Rating Scale (SSRS). Then the social support status of schizophrenic in remission and influencing factors were analyzed, meantime, the impact of the second round reimbursement policy of medical insurance benefits on their social support was addressed particularly. Results①The SSRS total score, objective support, subjective support, and utilization of support scores of schizophrenics in remission were lower than those of the national norm (t=5.065~30.382, P<0.01). ②Univariate analysis showed that SSRS score was relatively high among patients with female gender (t=-3.632), retired status (F=5.951), married status (F=5.951), spouse as primary caregiver (F=23.841), annual household income >5 000 yuan (F=15.892), patient's economic income (t=4.083), and outpatient or online follow-up (F=3.954), with statistically significant differences (P<0.05 or 0.01). ③The total and dimensional scores of SSRS in patients with access to the second round medical insurance reimbursement were significantly higher than those without (t=10.195~25.103, P<0.01). ④Multiple linear regression analysis denoted that gender, work status, marital status, primary caregivers, annual family income, economic income, follow-up visits and the second round medical insurance reimbursement were the factors influencing social support status of schizophrenics in remission (β=0.201~2.115, P<0.05 or 0.01). ConclusionThe social support of schizophrenics in remission in Northeast Sichuan is below the national average, furthermore, their social support levels are affected by the gender, work status, marital status, primary caregivers, annual family income, economic income, follow-up visits and the second round medical insurance reimbursement, and the second round medical insurance reimbursement may ameliorate the social support status of patients.

9.Meta-analysis of application of palliative care based on multidisciplinary cooperation in patients with heart failure

Objective:To evaluate the application effect of palliative care based on multidisciplinary cooperation in patients with heart failure.Methods:Randomized controlled trials of palliative care in patients with heart failure were searched by computer in PubMed, Embase, CINAHL, The Cochrane Library, China Biology Medicine disc, CNKI, Wanfang Database, VIP and other Chinese and English databases. The search time limit was from the establishment of the databases to December 31, 2020. Two researchers independently screened the literature, extracted the data and evaluated the quality. RevMan 5.4 software was used to conduct a meta-analysis of the included studies.Results:A total of 12 studies were included, including 1 402 patients. Meta-analysis results showed that compared with conventional management, palliative care based on multidisciplinary collaboration could improve quality of life of patients with heart failure ( SMD=0.72, 95% CI: 0.32-1.11, P<0.01), reduce the burden of symptoms on patients ( MD= -12.90, 95% CI: -20.25--5.54, P<0.01), and reduce depression level ( MD=-0.38, 95% CI: -3.69--0.08, P<0.05), anxiety level ( MD=-0.35, 95% CI: -0.66--0.05, P<0.05) and rehospitalization rate of patients ( RR=0.70, 95% CI: 0.57-0.86, P<0.01). There was no statistically significant difference in mortality between patients ( RR=1.02, 95% CI: 0.63-1.63, P=0.94) . Conclusions:Palliative care based on multidisciplinary collaboration can improve the quality of life of patients with heart failure, reduce the burden of symptoms, depression level and anxiety level and reduce the rehospitalization rate of patients, but it has no significant effect on reducing the mortality rate of patients. Given that palliative care is still in its infancy in China and is limited by the quality and quantity of included studies, the above conclusions need to be verified by more large-sample and high-quality studies.
10.Functional discovery and production technology for natural bioactive peptides.

Bioactive peptides play important roles in promoting human health, such as lowering blood pressure, blood sugar and blood lipid, anti-obesity, and anti-cancer. Thus, exploring functional bioactive peptides and developing efficient production technologies are of crucial importance. Herein, we review the development of function discovery and production technology for natural bioactive peptides. Presently, the top-down and bottom-up approaches are mainly used for the function discovery and production of natural active peptides. The top-down approach includes the direct extraction and identification for functional discovery, and the direct extraction, enzymatic hydrolysis and microbial fermentation for production. The bottom-up approach includes the polypeptide modification and database mining for functional discovery, and the chemical synthesis, enzyme synthesis, recombinant expression and cell-free synthesis for production. The top-down approach is usually associated with complicated process, lower efficiency, higher cost, harder quality control, and uncertain functionality, while the bottom-up approach is more suitable for the development of peptide drugs but difficult to be used for functional foods. With the technology development of sequencing and mass spectrometry, it is easier to obtain the proteomic information of various organisms at the molecular level. Based on the proteomic information, the top-down and bottom-up approaches can be combined to overcome the disadvantages of using these two approaches alone, thus providing a new strategy for the rapid development and production of natural active peptides.
