1.Severe placental abruption complicated with disseminated intravascular coagulation: clinical analysis of 28 cases

Objective To analyze the clinical characteristic of disseminated intravascular coagulation(DIC)complicated by placental abruption,improving the diagnosis and treatment level.Methods A retrospective analysis of clinical data of placental abruption complicated by DIC was made in 28 cases.Results In 28 cases,hypertensive disorders in pregnancy( n =15) was the main cause,persistent abdominal pain and vaginal bleeding was the most common complaint.There were 6 cases delivery through vaginal,and 22 cases delivery by cesarean section,complicated by hemorrhagic shock in 9 cases,accounting for 32.1%,concurrent stillbirth in 20 cases,accounting for 71.4%.In 28cases,there were live births in 8 cases,after childbirth and newborn severe asphyxia in 6 cases,including 2 cases after the rescue of neonatal death,and another 4 patients after the rescue of neonatal survival.The occurrence of uterine subtotal or total hysterectomy was in 6 cases,accounting for 21.4%,acute renal failure in 2 cases,no maternal death.Conclusions Placental abruption complicated with DIC made a serious threat to maternal and child life and health,and the pregnancy should be terminated immediately after clinical diagnosis.DIC was not a hysterectomy indications,coagulation factors can help to correct the coagulation disorders,and to preserve the uterus and maintain the maternal physical and mental health.

3.Effect of labetalol in treatment of severe preeclampsia

Objective To investigate the effect of labetalol in treatment of severe preeclampsia clinical effect.Methods The control group of severe preeclampsia patients received routine clinical treatment,the study group were treated with labetalol,two groups of patients with severe preeclampsia were treated for 7 d.Results After treatment,the two groups of DBP,SBP,HR,and 24 HUP were significantly lower than before,the study group improved the above indicators better(P<0.05).The study group of patients with severe preeclampsia premature delivery rate,postpartum hemorrhage rate,neonatal asphyxia rate(10.20%,8.16%,6.12%)were significantly lower than the control group(28.57%,34.69%,20.41%)(P<0.05).Conclusion The application of conventional therapy combined with labetalol can significantly improve the hypotensive effect of severe preeclampsia,is conducive to the protection of maternal physical and mental health and life safety.
4.Changes and significance of serum interleukin-17 and matrix metalloproteinase-9 in children of henoch-schonlein purpura

Objective To investigate the chanegs and correla iton between IL-17 and MMP-9 in c hildren of henoch-schonlein purpura,and to explore the pathogenesis of teh IL-17 and MMP-9 in henoch-schonlein pur-pura /henoch-schonlein purpura nephritis.Methods The level of serum IL-17 andMMP-9 in 74 henoch-schon-lein purpura childr en in initial stage and 30 cases of healthy children( control group) were detetced by ELISA. Results (1) The level of plasma IL-17 was significantly higher in children of henoch-schonlein purpura thna tha t foh ealthy control group (86.59 ±35.50) fg/L vs.(62.38 ±14.65)fg/L,P<0.01].(2) The level of plasma MMP-9 was significantly higher in children of henoch-schonlein pupru ar than that o fhealt hy control group [(201.82 ±105.87) fg/L vs(89.27 ±27.99)fg/L,P <0.01].(3) Henoch-schonlein purpura children were followed up for6 months,dependin g on whether the kidney was involved,they were devided into non-he-noch-schonlein purpura nephritsi grou p nad henoch-s chonlein purpura nephritis group.In initial phase of HSP,the level of plasma IL-17 was significant lyhigher in henoch-schonlein purpura nephritis group than non-henoch-schonlein purpura nephrits group [(101.67 ±39.55) fg/L vs.(101.67 ±39.55)fg/L,P<0.05].The level of p lasma MMP-9 was significantly higher in henoch-schonlein purpura nephritsi group than no n-henoc h-schon-lein purpura nephritis group [(249.63 ±97.57) Pg/L vs.(185.30 ±104.39) pg/L,P<0.05 ].(4) IL-17 and MMP-9 correlation aanlysis didn't show sign ificant correlation(r=0.184,P>0.05).Conclusion IL -17 and MMP-9 are involved in the pathogenesis of henohc-schonlein purpura/henoch-schonlein pur pura nephritis.
5.Scan parameter optimization of renal artery with inflow-sensitive inversion recovery magnetic resonance angiography

Objective Non-contrast enhanced magnetic resonance angiography ( MRA) has become the focus of research at home and abroad .The aim of the article was to optimize the scan parameters of renal artery with inflow-sensitive inversion recovery ( IF-IR) MRA. Methods 30 cases of renal artery patients into two groups were collected from December 2013 to May 2014 in our hospi-tal.They were divided into stenosis group(n=12) and non-stenosis group(n=48).IFIR-MRA sequences were carried out on each patient by using various BSP-TI with 1200,1300 and 1400 ms, all other parameters were identical .The image signal to noise ratio, a renal artery branch and venous artifact were evaluated by two experienced radiologists , and the best BSP-TI was obtained from different groups. Results BSP-TI=1400 ms was optimal for renal artery stenosis group while 1200 ms was the most suitable for non-stenosis group. Conclusion IFIR-MRA of renal artery can achieve better image quality of renal artery and its branches and it is feasible in clinical routine practice , especially for those patients with renal insufficiency .
6.Study on the HPLC Fingerprints of Dangua Yangmu Cream

OBJECTIVE:To establish the HPLC fingerprints for Dangua yangmu cream. METHODS:HPLC was performed on the column of Kromasil C18 with mobile phase of acetonitrile-0.026% phosphoric(gradient elution)at flow rate of 0.8 ml/min,the detection wavelength was 270 nm,the column temperature was 25 ℃ and the injection volume was 20 μl. The chromatographic peak of aurantio-obtusin was used as reference peak to determine the 10 batches of Dangua yangmu cream,and Similarity Evalua-tion System for Chromatographic Fingerprint of TCM(version 2.0)was conducted to identify common peaks and evaluate similari-ty. RESULTS:There were totally 25 common peaks in the 10 batches of Dangua yangmu cream,and the similarity was not lower than 0.921. The validation results showed the fingerprints of 10 batches of Dangua yangmu cream had good consistency with the ref-erence fingerprints. COMCLUSIONS:The established method is specific and reliable,and can provide basis for the quality evalua-tion and control of Dangua yangmu cream.
7.The application of hemoperfusion in severe juvenile dermatomyositis

Objectives To investigate the clinical effect of hemoperfusion in severe juvenile dermatomyositis. Meth-ods Two patients with severe juvenile dermatomyositis, who accepted hemoperfusion treatment, were retrospectively analyzed. Results In these 2 patients, the hemoperfusion process were smooth. The muscle pain was reduced and the muscle enzymes were decreased. After hemoperfusion, one patient refused to continue treatment and the disease recurrent. The other patient con-tinued to accept the immunosuppressive therapy and eventually improved. Conclusions For juvenile dermatomyositis, hemo-perfusion was a safe and effective auxiliary treatment. It could be applied when the general treatment was invalid.
8.Effect of CDR1/CDR2 or MDR1 genes overexpression on oxidative stress in Candida albicans

To investigate the effect of CDR1/CDR2 or MDR1 genes overexpression on oxidative stress in Candida albicans,we evaluated the effect of H2O2 on cell viability in C.albicans overexpressing genes CDR1 /CDR2 or MDR1 and their parent strains.After establishing an oxidative stress model with H2O2,we detected reactive oxygen species (ROS),mitochondrial membrane potential (Δψm) and the expression of oxidative stress response-related genes (CAP1 and GRP2) and ROS clearance related-genes (SOD2 and SOD5).The results showed that C.albicans growth were inhibited by 100% after the treatment of 5 mmol/L H2O2.HeO2 caused more ROS accumulation and Δψm reduction in parent strains than in CDR1/CDR2 or MDR1 genes overexpressed strains (P<0.05).Compared to parent strains,the up-regulated expression of CAP1 and GRP2 were relatively less in CDR1/CDR2 or MDR1 genes overexpressed strains,moreover,the down-regulated expression of SOD2 and SOD5 were also relatively less in CDR1/CDR2 or MDR1 genes overexpressed strains (P<0.05).In conclusion,the overexpression of CDR1/CDR2 and MDR1 genes could reduce the oxidative stress response and enhance the adaptability of C.albicans to oxidative stress.
9.Seroprevalence of neutralizing antibody among SARS-CoV-2 infected individuals

Objective:
To investigate the seroprevalence and influencing factors of serum neutralizing antibodies among SARS-CoV-2 infected individuals, so as to provide the evidence for developing the health management and COVID-19 vaccination strategy among SARS-CoV-2 infected individuals.
Methods:
Recovered SARS-CoV-2 infected individuals from January 1st, 2020 to February 10th, 2021 in Zhejiang Province were recruited in March 2021. Participants' demographics, underlying diseases, date of definitive diagnosis and severity of clinical symptoms were collected using questionnaire surveys, and serum neutralizing antibody against SARS-CoV-2 was detected using a fluorescent immunoassay. In addition, factors affecting the seropositivity of neutralizing antibody against SARS-CoV-2 were identified using a multivariable logistic regression model.
Results:
A total of 559 SARS-CoV-2 infected individuals were enrolled, including 480 confirmed cases and 79 asymptomatic carriers, with an median (interquartile range) age of 47.00 (22.00) years, and all participants had never received COVID-19 vaccination. The median (interquartile range) duration from diagnosis to serum sampling was 387.00 (11.00) days, and the seroprevalence of neutralizing antibody against SARS-CoV-2 was 83.90%. The serum neutralizing antibody against SARS-CoV-2 was all positive 9 months after diagnosis, and the seroprevalence of neutralizing antibody against SARS-CoV-2 appeared no tendency towards a decline with time within 14 months after diagnosis (P>0.05). Multivariable logistic regression analysis showed that women were 1.892 times (95%CI: 1.169-3.064) more likely to produce serum neutralizing antibodies against SARS-CoV-2 than men, and mild, common and severe/critically ill SARS-CoV-2 infected cases were 2.438 (95%CI: 1.305-4.557), 4.481 (95%CI: 2.318-8.663), and 23.525 (95%CI: 2.990-185.068) times more likely to produce serum neutralizing antibodies against SARS-CoV-2 than asymptomatic carrier, respectively.
Conclusions
The seroprevalence of neutralizing antibody was 100.00% among SARS-CoV-2 infected individuals within 9 months after diagnosis. Individuals' gender and severity of clinical symptoms correlate with the seroprevalence of neutralizing antibody against SARS-CoV-2.
10.CTNS gene mutation leads to cysteine nephropathy combined with corneal crystal in young child

Objective To explore the diagnosis of cystinosis.Methods The clinical and biochemical information, and gene detection results in a child with cystinosis was retrospective analyzed.Results Four-year-old female presented with photophobia and corneal crystal was found by ophthalmic examination at 2 years old, bilateral kidney stone was found, accompanied by development delay and rickets at 3 years old. Gas chromatography analysis in urine showed that a variety of amino acids were increased, and urine sugar and urinary micro-protein were also increased, which were in accordance with fanconi syndrome. The blood free carnitine was decreased, ester acyl carnitine spectrum was normal, and multi-amino acids such as lysine, valine and arginine were decreased. Gene analysis showed a homozygous mutation of c.696C>G (p.323 N>K) inCTNS gene, which was a known mutation. Both her parents were carrier of heterozygous mutation of c.696C>G inCTNS gene.Conclusion Child with kidney stone, renal damage, combined by multi-system damage such as eyes, bone, and thyroid should be paid attention to identify the cystinosis.
