1.Clinical and pathological features of esophageal and gastric junction adenocarcinoma and current status of postoperative adjuvant chemoradiotherapy

The incidence of adenocarcinoma of the esophagogastric junction ( AEG) is rising rapidly in recent years. AEG as an independent disease different from squamous cell carcinoma of the esophagus and gastric adenocarcinoma is getting acknowledged for most of scholars, with the distinctive anatomy location and involute recurrence and metastasis style. A higher rate of relapse and poor prognosis after resection become conscious since most of patients have been diagnosed with advanced disease, and adjuvant chemoradiotherapy needs to be valued further. But so far, there is no clinical data and subgroup analysis aimed at the value of postoperative treatment for AEG specially, leading to controversy is remaining in patients could get benefit from postoperative treatment, and delineation of the radiation target, etc. The design of the irradiation target should in terms of the clinical?pathological characteristics of the AEG, Siewert'''' s subtypes, the lymph node metastasis mapping and recurrence characteristics after surgery.
2.The Role of Macrophages in the Induction of Antitumor Immune Responses of the Mice Immunized with MIF Gene-Modified Tumor Vaccine

Objective: To investigate the mechanisms involved in the induction of antitumor immune responses by MIF gene- modified tumor vaccine. Methods: The changes of the number and the functions of macrophages from the force immunized with MIF gene-modified FBI3 tumor vaccine were analysed.Results: The number,the expression of surface molecules associated with immune responses, the secretion of TNF-?and NO, the phagocytosis, the cytototicity and the antigen presenting activity of peritoneal macrophages from the mice immunized with MIF gene-modified tumor vaccine were all enhanced. Conclusion: The functional enhancement of macrophages is related to MIF secreted by MIF gene-modified tumor vaccine in vivo and the activation of macrophage may play an important role in the antitumor immune responses induced by MIF gene-modified tumor vaccine.
3.Preparation and characterization of diclofenac sodium layered double hydroxide nanohybrids

Aim: To prepare diclofenac sodium-layered double hydroxide nanohybrids(DS-LDH) and investigate its properties. Methods: The diclofenac sodium anions were hybridized with LDH by co-precipitation. The synthetic nanohybrids were characterized by various methods, such as laser particle sizer, XRD, IR, and TEM. Paddle method according to the Chinese Pharmacopoeia was used to determine the dissolution. The anti-inflammation of nanohybrids and diclofenac sodium were compared by the effect on auricular tumescence induced by xylene in mice. Results: The acidic negative ion of layered double hydroxides were replaced by the diclofenac anion to form DS-LDH. Release studies showed that the nanohybrids could serve as controlled release systems for DS. Zn-Al-NO3-LDH was proved to have character of neutral pH which is more suitable for orphthalmic preparation. The DS-LDH nanohybrids could keep the effect of anti-inflammation. Conclusion: Layered double hydroxide nanohybrids can be used as sustained-release carrier of anionic drugs. A novel type of drug-inorganic nanohybrids was prepared successfully which has a potential value of clinical application.
4.Characterization of drug resistance and serotyping of 117 isolates of Streptococcus pneumoniae from a hospital in Shanxi Province

Abstract: Objective This study aims to investigate the drug resistance, resistance mechanisms, and serotype epidemiological features of clinically isolated Streptococcus pneumoniae (SP) in Shanxi Province. Methods SP strains were isolated between January 2021 and July 2023 from Shanxi Provincial People's Hospital. An automatic microbiological identification and drug sensitivity analysis system was used to identify strains and conduct antibiotic susceptibility assay. The species identification was also carried out using a MALDI-TOF mass spectrometer, optochin assay, and amplification of the CPSA gene. Finally, the polymerase chain reaction (PCR) technique was implemented for drug-resistant and virulence gene detection and serotyping. Results A total of 117 strains were isolated including 115 strains (98.3%) being erythromycin-resistant Streptococcus pneumoniae (ERSP). The ERSP was composed of 114 macrolide-lincosamide-streptogramin B (MLSB) resistant strains, carrying the ermB (114/114) and mefA (42/114) gene, and one M-resistant strain absent of ermB and tetM. The most common pneumococcal six serotypes in ERSP strains were 19F, 3, 34, 19A, 6A/6B, and 23F, respectively, totaling 71 strains (61.7%). Compared with pediatric and adult, serotype 19F in geriatric patients was the dominant type with a significant difference (χ2=6.315, P=0.043). Compared with adult patients, the strains isolated from pediatric and elderly patients showed a higher rate of insensitivity (resistance and intermediate) to oral penicillin (χ2=7.277, P=0.026). Serotype 19F strains showed high insensitivity to oral penicillin (81.0%), compound sulfamethoxazole (100.0%), tetracycline (94.7%), and ceftriaxone (9.5%). The serotypes 19A and 23F strains also showed high insensitivity to penicillin (>70%), tetracycline (100%) and compound sulfamethoxazole (>85%). However, serotype 3 strains had low insensitivity to oral penicillin (8.3%), ceftriaxone (0%), and compound sulfamethoxazole (27.3%). There were 42 strains positive for ermB+mefA, of which serogroup 19 was the main type, accounting for 64.3% (27/42). Of these strains, 88.1% (37/42) carried ply, lytA, and HylA genes simultaneously. Conclusion Drug resistance to ERSP still to be a serious problem for human health. There are differences in serotype distribution in terms of age, resistance genes, and drug resistance. Therefore, the vaccination of pneumococcal conjugate vaccine 13 (PCV13) is an essential and effective measure to prevent SP infection in this region.
5.Exercise-sensitive Indices Screening from Electrocardiogram Based on Rest-workload Alternating Pattern.

Heart rate is the most common index to directly monitor the level of physical stress by comparing the subject's heart rate with an appropriate "target heart rate" during exercise. However, heart rate only reveals the cardiac rhythm of the complex cardiovascular changes that take place during exercise. It is essential to get the dynamic response of the heart to exercise with various indices instead of only one single measurement. Based on the rest-workload alternating pattern, this paper screens the sensitive indices of exercise load from electrocardiogram (ECG) rhythm and waveform, including 4 time domain indices and 4 frequency domain indices of heart rate variability (HRV), 3 indices of waveform similarity and 2 indices of high frequency noise. In conclusion, RR interval (heart rate) is a reliable index for the realtime monitoring of exercise intensity, which has strong linear correlation with load intensity. The ECG waveform similarity and HRV indices are useful for the evaluation of exercise load.

6.Mental disorder and its risk factors in first degree relatives of patients with type 2 diabetes mellitus

Objective To investigate the incidence and the risk factors of anxiety or depression and in the first degree relatives (FDRs) of patients with type 2 diabetes mellitus ( T2DM ). Methods A total of 102 T2DM patients who visited our hospital from May to July 2010 and their FDRs completed SAS,SDS,and life satisfaction evaluation. The medical history of T2DM patients was collected and compared with their FDRs with or without mental disorders. The correlations of risk factors with anxiety or depression were analyzed, t test or X2 test were used for statistical analysis and a P value less than 0. 05 was considered statistically significant. Results The incidence of anxiety or depression of FDRs was 31.4% and 17.6%,respectively. The life satisfaction score of FDRs with mental disorders was significantly decreased ( t = 4. 26 and 4. 09, P<0. 05 ). Fasting glucose level of T2DM patients with anxious or depressive FDRs was significantly increased ( t = -2. 48 and -2. 15, P<0. 05 ). Higher medical cost and multiple treatment strategies were positively correlated with SAS and SDS score of the FDRs. Conclusion Higher incidence of mental disorders and lower satisfaction scores could be found in the FDRs of T2DM patients. Better control of blood glucose and reduced treatment cost and strategies may be useful in improving metal condition and life satisfaction of the FDRs of T2DM patients.
7.The mediating effect of resilience between mindfulness and perceived stress in psychiatric nurses

Objective:To explore the influence of mindfulness on perceived stress in psychiatric nurses and the mediating effect of resilience.Methods:A total of 427 psychiatric nurses from a psychiatric hospital in Beijing were selected.Five-facet mindfulness questionnaire(FFMQ), Chinese-version perceived stress scale (CPSS) and Connor-Davidson resilience scale (CD-RISC) were used in this survey.Pearson correlation analysis and multiple stepwise regression analysis were performed using SPSS 24.0.And mediating effect was performed using AMOS 24.0.Results:The total score of psychiatric nurses' perceived stress was (37.89±7.16). Among all psychiatric nurses, 66.74% were under high pressure and 20.84% were under great pressure.The perceived stress was negatively correlated with mindfulness(123.69±10.82, r=-0.65, P<0.01) and resilience(85.40±17.87, r=-0.67, P<0.01), and mindfulness was positively correlated with resilience ( r=0.50, P<0.01). The direct effect of mindfulness on perceived stress was -0.15, the indirect effect was -0.34, and the total effect was -0.49. Conclusion:Resilience plays a partial mediating role between mindfulness and perceived stress psychiatric nurses.
8.A Novel Method for Extraction of Heart Sounds Envelope Based on Interpolation of Local Peak and Valley Points

Objective To extract envelope of heart sounds exactly,for the purpose of the further analysis of its characteristics.Methods The way that envelope extraction of heart sounds based on key-points was given.The points of local peak and valley were calculated firstly,and then heart sound envelope was gotten by the interpolation of these points.Results Compared with the envelope extracted by Hilbert-transform and mathematical morphology,respectively,the outline of heart sounds was extracted more accurately,and its time-domain characters were acquired by this method.Conclusion The envelope of heart sound is extracted correctly by this method,which is useful for the further analysis.
9.The effect of preoperative carbohydrate preconditioning on postoperative insulin resistance and inflammatory response in patients after gastroenteric tumor resection

Objective To assess the effect of preoperative carbohydrate preconditioning on postoperative insulin resistance and inflammatory response in patients after gastroenteric tumor resection.Methods Sixty patients with elective gastroenteric tumor resection were divided into preoperative carbohydrate preconditioning group and control group by random digits table with 30 cases each.Preoperative carbohydrate preconditioning group was given oral containing 50 g glucose carbohydrate 300 ml 2 h before surgery,and control group was given traditional method,fasting 12 h before surgery and water deprivation 6 h before surgery.The blood samples were collected to measure the level of fasting blood glucose (FBG),fasting insulin (FINS),interleukin (IL)-6 and C-reactive protein (CRP),3 h before surgery and 1,3,7 d after surgery respectively.Homeostasis model assessment insulin resistance index (HOMA-IR) was computed.Results There were no statistical differences in FBG,FINS,IL-6,CRP and HOMA-IR 3 h before surgery between the 2 groups (P > 0.05).The FBG,FINS,IL-6,C RP and HOMA-IR in the 2 groups at 1,3 d after surgery were significantly higher than those 3 h before surgery,but those except for FBG in control group were significantly higher than those in preoperative carbohydrate preconditioning group [1 d after surgery:(39.67 ± 10.37) mU/L vs.(25.78 ± 12.43) mU/L,(98.67 ± 12.42) μg/L vs.(65.36 ± 16.72)μg/L,(121.74 ±11.32) mg/L vs.(82.18 ±4.36) mg/L,19.07 ±5.49 vs.11.67 ±6.32;3 d after surgery:(24.34 ±6.78) mU/L vs.(16.23 ±7.56) mU/L,(116.43 ± 18.57) μg/L vs.(78.53 ± 10.38) μg/L,(151.30 ± 10.46) mg/L vs.(129.29 ± 10.24) mg/L,8.56 ±2.87 vs.5.12 ±2.11],there were statistical differences (P <0.05).There were no statistical differences in FINS and HOMA-IR in preoperative carbohydrate preconditioning group between 7 d after surgery and 3 h before surgery (P > 0.05),but there was statistical difference in control group (P< 0.05).There were no statistical differences in FBG,IL-6 and CRP in the 2 groups between 7 d after surgery and 3 h before surgery (P > 0.05).There was no aspiration during anesthetic stage.Conclusion Preoperative carbohydrate preconditioning may shorten the insulin resistance time after gastroenteric tumor resection,reduce the intensity of insulin resistance,and improve inflammatory response,thus contributing to the rehabilitation of patients.
10.Reasons for perioperative death in treatment of acute myocardial infarction by emergent percutaneous coronary intervention

ObjectiveTo analyze the dead reasons in treatment of acute myocardial infarction (AMI) by emergent percutaneous coronary intervention (EPCI) during perioperation. Methods The clinical characteristics of the dead patients,results of EPCI and reasons of death were retrospectively analyzed in 473 patients with AMI who received EPCI treatment during March 2004 to March 2011. ResultsAmong the 18 cases(3.8%)dead patients,there were 8 case with three-vessel lesions,5 cases with two-vessel lesions, 2 cases with single vessel lesion and 3 cases with left main lesion. Nine patients were accompanied with hypertensions,5 cases with diabetes mellitus, 1 case with old myocardial infarction, 2 cases with old cerebral infarction, 1 case with chronic obstructive pulmonary disease complicated by severe pneumonia, and 1 case with chronic renal insufficiency. Nine patients died of cardiogenic shocks , 4 cases (22. 2%) of heart ruptures, 2 cases (11.1%) of noreflow,1 case (5.6%) of massive hemorrhage of the upper alimentary tract, 1 case (5.6%) of respiratory failure and 1 cases (5.6%) of refractory ventricular fibrillation.ConclusionsPrimary reasons for perioperative death were cardiac shock and heart rupture in treatment of acute myocardial infarction by EPCI. Multi-vessel lesion, complications with hypertensions and diabetes mellitus may serve as succumbed factors of death.
