1.Effectiveness observation of early sequential nutrition support in the treatment of AECOPD patients with typeⅡ respiration failure

Objective To observe the clinical effectiveness of early sequential nutrition support (ESN)in the treatment of acute exacer-bation of chronic obstructive pulmonary disease (AECOPD)patients complicated with type Ⅱ respiration failure.Methods Eighty-four AE-COPD patients with type Ⅱ respiration failure were enrolled into our study and received the conventional therapies such as mechanical venti-lation,anti-infection,spasmolysis and asthma control.Patients were randomized assigned into the ESN group (n =42)and the total parenteral nutrition group (TPN,n =42)according to the nutritional care methods,and the clinical effectiveness were observed.Results Clinical symptoms were significantly improved in both of the two groups.However,the total effective rate,average length of stay,pulmonary function, triceps skinfold thickness (TSF)and serum albumin levels were significantly better in the ESN group,and the differences were ofstatistical significance (P <0.05).Conclusion The study comfirmed the necessity of ESN application on the AECOPD patients with type Ⅱ respira-tion failure,which can further improve the treatment effectiveness and nutrition status.
2.Application of thrombelastogram in choosing antiplatelet drug for patients with cerebral arterial thrombosis

Objective:To investigate the application value of thromboelastography (TEG) in choosing antiplatelet drug for patients with cerebral arterial thrombosis.Methods: 100 new patients with cerebral arterial thrombosis, who received single antiplatelet therapy after received dual antiplatelet therapy (aspirin and clopidogrel) in acute stage (14d), were randomly divided into observation group (50cases)and control group (50cases). According to different method of choosing drug, patients of control group randomly were implemented different single antiplatelet drug, while patients of observation group received more sensitive antiplatelet drug based on TEG results. The follow-up observation about recurrent situation of cerebral arterial thrombosis for the two groups were carried out in further study.Results: In one year, the recurrence rate of cerebral arterial thrombosis of observation group (6%, 3/50) was significantly lower than that of control group (20%, 10/50) (x2=4.332,P<0.05). The differences of recurrence rate between different antiplatelet drug in intra-group were not statistically significant.Conclusion: TEG can efficiently evaluate the therapeutic effect of antiplatelet drug, and guide the selection for drug so as to reduce the recurrence rate of cerebral arterial thrombosis.
3.Pharmacokinetic study of ferulic acid in Ruxian Pill in rabbits in vivo

Objective To study the pharmacokinetics of ferulic acid in rabbits in vivo after ig administering Ruxian Pill.Methods The concentrations of ferulic acid in plasma were determined by HPLC.Taking coumarin as internal standard,a mixture of methanol-acetic acid-water(38.0∶0.5∶61.5) was used as the mobile phase,and UV detection was performed at 320 nm.The analytical column was ODS Hypersil C_(18)(150 mm?4.6 mm,5 ?m).Results The calibration curve was linear over the range of 12.5—1 600.0 ?g/L and the measurable limit of plasma detection was 6.25 ?g/L.The recovery was 96.17%,99.80%,100.23%. The RSD of the intra-day and the inter-days was less than 6%.The plasma concentration-time curve was consistent with one-comparment model.The pharmacokinetic parameters were as follows: AUC_(0-4 h)(855.29?100.78) ?g/(L?h),AUC_(0-∞)(1 051.99?125.96) ?g/(L?h),C_(max)=(952.71?119.04) ?g/(L?h),t_(max)=(0.5?0.00) h,t_(1/2)=(1.61?0.21) h,K_e=(0.46?0.05)/h. Conclusion The method is accurate and precise with better reproducibility,which can be used for the determination of plasma level of ferulic acid and the study of its pharmacokinetics for Ruxian Pill.
4.Investigation on Clinical Effect of Huangqishengmai Oral Solution for Viral Myocarditis

Objective: To investigate the clinical effect of Huangqishengmai Oral Solution for Viral myocarditis. Methods: 128 patients in the therapeutic group were treated with Huangqishengmai Oral Solution. 72 patients in the control group were treated with routine treatment. The changes of heart rate, heart function, electrocar-diogram ST-T, arrhythmia, SOD and MDA before and after treatment were observed. Results: (a) The heart rates of both groups slowed down after treatment, and there were obvious differences in various markers of heart function between before and after treatment. (b) ST-T change of patients in the therapeutic group was obvious. The effective rate of the therapeutic group was higher than that of the control group. (c) SOD level of patients in the therapeutical group rised. MDA level decreased. (d) No obvious difference in treating arrhythmia was observed between two groups. Conclusion: Huangqishengmai Oral Solution is effective for viral myocarditis and should be widely applied in clinic.
5.Effects of Umbilical Therapy by Rheum palmatum Combined with Early Colon Irrigation on the Quality of Life in Elderly Patients with Colon Stoma

OBJECTIVE:To explore the effects of umbilical therapy by Rheum plamatum combined with early colonic irriga-tion on the quality of life in elderly patients with colon stoma. METHODS:80 patients underwent radical resection of abdominoperi-neal rectal carcinoma were randomly divided into control group and observation,with 40 cases in each group. Control group was given early colonic irrigation;observation group was additionally given umbilical therapy by R. plamatum. Treatment course of 2 groups lasted for one month,and both were followed up for 6 months after treatment. The quality by life in 2 groups was evaluated by using QLQ-C30,and therapy costs were calculated before and after treatment. RESULTS:Compared with before treatment, function score and total health score of 2 groups improved significantly in 1,3,6 months after treatment,while symptom score and single item score decreased significantly,with statistical significance(P<0.05). In 1,3,6 months after treatment,role func-tion,social function,cognitive function,emotional function and total health score of observation group were significantly higher than those of control group,and insomnia and pain score were significantly lower than control group,with statistical significance (P<0.05). 3 and 6 months after treatment,diarrhea and economic difficulty score of observation group were significantly lower than those of control group,with statistical significance(P<0.05). 1 months after treatment,loss of appetite score of observation group were significantly lower than those of control group,with statistical significance (P<0.05). 6 months after treatment, fa-tigue,nausea and vomiting score of observation group were significantly lower than those of control group,with statistical signifi-cance (P<0.05). 6 months after treatment,nursing cost,drug cost and total cost of observation group were significantly lower than those of control group,with statistical significance(P<0.05). CONCLUSIONS:The umbilical therapy by R. plamatum com-bined with early colonic irrigation can improve the quality of life in elderly patients with colon stoma significantly and reduce treat-ment cost.
6.Effects of Jiantai Liquid on Progesterone Receptor(PR)and PRmRNA in the Endometrium of Mice at Nidation Stage

Objective:To study on effect of Jiantai Liquid on progesterone receptor(PR)in the uterus at the nidation stage in the mouse with disturbance of blastocyst nidation.Methods:The gestational mice were randomly divided into a normal group,a model group and a Chinese drug group.Mifepristone was used for inducing disturbance of blastocyst nidation in the mice,which were interfered with Jiantai Liquid.At the eighth week of pregnancy,the mice were killed and morphological changes of the endometrium were observed,and PR expression in the uterus was detected with immunohistochemical method and PR and PRmRNA expressions were detected with Western blot and PT-PCR methods.Results:The endometrium showed maldevelopment in the model group and normal development in the Chinese drug group,similar to the normal group;the gland,the PR expression range and intensity of PR expression in the interstitial substance in the Chinese drug group increased as compared with the model group(P0.05);expressions of PR protein and gene in the Chinese drug group were significantly higher than those in the model group(P0.05).Conclusion:Jiantai Liquid can promote development of the endometrium and improves blastocyst nidation via regulating expressions of PR and PRmRNA in the uterus in the mice with disturbance of blastocyst nidation.
7.Role of nucleolin on myocardial hypertrophy and fibrosis in type II diabetic cardiomyopathy mice

AIM: To investigate the effect of nucleolin on diabetic cardiomyopathy in mice.METHODS: A type II diabetic cardiomyopathy mouse model was prepared using a cardiac-specific nucleolin-overexpressing transgenic mice.The mice were divided into wild-type mouse control group, nucleolin transgenic mouse control group, wild-type mouse diabetes group and nucleolin transgenic mouse diabetes group.Wheat germ agglutinin (WGA) fluorescent dye, Masson staining and PowerLab system detection were used to further clarify the role of nucleolin on cardiac hypertrophy, fibrosis and cardiac function in type II diabetic cardiomyopathy mice.RESULTS: Compared with wild-type mouse control group, no significant increase in blood glucose level was found, while genetical myocardial cell hypertrophy was significantly attenuated in nucleolin transgenic mouse diabetes group.The collagen fibers were also significantly reduced, and hemodynamic indexes ± dp/dtmax, left ventricular end-diastolic pressure, left ventricular systolic pressure and heart rate were also improved.The above differences were statistically significant.CONCLUSION: Nucleolin may reduce the occurrence of myocardial hypertrophy and fibrosis, thus improving the cardiac function of diabetic cardiomyopathy mice.
8.Multivariate analysis of clinical characteristics and prognosis in pulmonary tuberculosis patients complicated with diabetes mellitus

Objective To study and analyze the clinical features of pulmonary tuberculosis complicated with diabetes,and the risk factors affecting the prognosis of pulmonary tuberculosis patients complicated with diabetes by multivariate analysis.Methods From January 2012 to January 2015,200 tuberculosis patients complicated with diabetes in the Second Hospital of Yuncheng were selected in the research.The clinical data of 200 patients were retrospectively analyzed,the clinical characteristics of pulmonary tuberculosis with diabetes (age,gender,lung cavity etc) were summarized,the 2-year follow-up data were analyzed to analyze the recurrence.The patients were divid-ed into good prognosis group and poor prognosis group.The clinical data were compared between the two groups,multi-variate analysis and logistics regression analysis were used to analyze the risk factors affecting prognosis of pulmonary tuberculosis patients with diabetes.Results The distribution of pulmonary tuberculosis patients with diabetes in males was higher than females (P<0.05),the age distribution was mainly over 60 years old,the incidence rate of lung cavity was 67.50%,the positive rate of sputum was 64.00%.After 2 years of follow-up,there were 42 cases of recurrence.Univariate analysis results showed that there were statistically significant differences in age,blood glucose control effect,drug resistance,drug compliance and other aspects between the poor prognosis group and the good prog -nosis group (all P<0.05),and there were no statistically significant differences in gender,lung cavities,sputum posi-tive aspects between the two groups (all P>0.05).The logistics linear regression analysis showed that the effect of blood glucose control,drug resistance and compliance were the risk factors affecting prognosis of pulmonary tuberculosis patients with diabetes.Conclusion Pulmonary tuberculosis complicated with diabetes occurred in elderly male popu-lation,the prognosis is often affected by effect of blood glucose control,drug resistance,medication compliance and other factors,clinical intervention should be carried out according to the prognostic risk factors.
9.Predicting value of plasma homocysteine concentrations for diabetic retinopathy

Objective To explore the relationship between plasma homocysteine levels and diabetic retinopathy (DR).MethodIn 40 DR patients and 63 non-DR patients,the indexes including plasma homocysteine concentrations,duration of diabetes,ages,blood pressure,preprandial blood glucose(PBG),plasma GHbA,e,plasma lipids,serum folie acid and semm vitamin B12 were analyzed.ResultsPlasma homocysteine level was significantly higher in DR patients (12.9μmol/L) than that in non-DR patients (7.8μ.mol/L) (P<0.01),and the relationship remained significant correlation after adjusted for duration of diabetes,GHbA1c,ages,serum folic acid and serum vitamin B12(OR = 1.23,P<0.05).ConclusionHyperhomocysteinimia is a risk factor and an independent predieator of DR.
