1.Protective effect and antioxidative mechanism of α-lipoic acid in rats with acute pancreatitis

Objective To investigate the protective effects of α-lipoic acid in rats with acute panereatitis(AP)and its potential mechanism.Methods Wistar rats were randomly divided into four groups according to random number table:sham operation(SO)group,AP group,normal saline(NS)group and α- lipoic acid group with 30 rats in each group.AP model was induced by retrograde iniection of 3.5%sodium taurocholate into the pancreatobiliary duct.Rats in α-lipoic acid group immediately received α-lipoic acid intra- peritoneal injection at the dose of 1 mg/kg.Rats in NS group received sanle amount of normal saline.The rats were sacrificed at 1,3,6,9 and 12 h after AP induction.The serunl levels of amylase.TNF-α and ICAM-1 were measured.Pancreatic histological changes were observed.The activities of pancreatic SOD and MDA were measured. Results In rats of AP group,optical microscopy showed pancreatic edema,adhesion and necrosis. The semm amylase,TNF-α,ICAM-1 and MDA levels in pancreatic tissue 6h after operalion were(2211±547)U/L,(174.8±7.9)ng/ml,(49.3±8.0)ng/ml and(32.2±5.9)U/mg prot,respectively,in AP group;which were significantly increased when compared with those of SO group(P<0.05).Pancreatic SOD activity was(38.5±9.5)U/mg prot,which was signifieandy lower than(56.7±6.7)U/mg pint of SO group (p<0.05).The serum amylase,TNF-α,ICAM-1 and MDA levels in pancreatic tissue 6 h after operation in α-lipoic acid group were(1478±642)U/L,(164.8±6.2)ng/ml,(37.5±3.9)ng/ml and(20.2 ±8.4)U/mg prot,respectively;which were significantly decreased when compared with those of AP group(P<0.05).Pancreatic SOD activity was(66.0±8.6)U/mg prot,which were significantly hisher than(38.5±9.5)U/mg prot of AP group(P<0.05).Condusiors The pathogenesis of AP wag associated with oxiddative stress,and α-lipoic acid as an antioxidant played a role in the treatment of AP.the possiblemechanismsincludedinhibitedproduction of TNF-α and ICAM-1.

3.Clinical effect of magnesium isoglycyrrhizinate on moderate and severe nonalcoholic steatohepatitis and its mechanism analyse

Objective To observe the clinical effect of magnesium isoglycyrrhizinate on moderate and severe nonalcoholic steatohepatitis(NASH) and analyse its mechanism. Methods 42 cases with moderate and severe nonalcoholic steatohepatitis were selected in our study. All patients were divided into observation group and control group randomly. Control group were received simvastatin while the observation group were received simvastatin combined with magnesium isoglycyrrhizinate treatment. The course was 6 weeks.The changes of NASH classiifcation, clinical symptom, liver function, lipid levels and liver ifbrosis items in two groups before and after treatment were observed and recorded. Results All patients were received 6 week treatment, none of them dropped out. The clinical symptoms were improved in both two groups. There were 5 severe NASH improved to moderate NASH, 8 moderate NASH improved to mild NASH in observation group while only 3 severe NASH improved to moderate NASH in control group. The difference of NASH classiifcation between two groups was signiifcant(P<0.05). Compared to pre-treatment, the AST, ALT, TBIL,γ-GT were decreased in both two groups. But the liver function items in observation were lower than control group(P<0.05). The lipid level were decreased in both two group and there were no signiifcant differences between two groups after treatment. The level of PC III, HA, C-IV were decreased in observation group while had no changes in control group. Conclusion The magnesium isoglycyrrhizinate could decrease the AST, ALT and lipid level, improve the classiifcation of liver ifbrosis, and had low rate of side effect during treatment.
4.Effects of mangiferin on tissue factor expressionin vascular endothelial cells and the underlying mechanisms

Aim To explore the effects of mangiferin on tissue factor(TF) expression in human umbilical vein endothelial cells(HUVECs) and the underlying mechanisms.Methods HUVECs were isolated and primarily cultured in vitro.After the treatment with mangiferin and oxidized low density lipoprotein(oxLDL), TF expression was determined in HUVECs with real-time PCR and Western blot.Results oxLDLinduced the mRNA and protein expression and pro-thrombotic activity of TF in HUVECs.However, the inductive effects of oxLDL were blocked significantly by mangiferin.Furthermore, mangiferin modified TF expression and activity in a dose-dependent manner.Mangiferin was demonstrated to enhance the activity of peroxisome proliferator-activated receptor gamma(PPARγ).In contrast, GW9662, an antagonist of PPARγ, reversed at least partially the suppressive effects of mangiferin on TF.Conclusion Through activating PPARγ, mangiferin suppresses the expression of TF serving pro-thrombotic functions in endothelial cells.
5.Pathogen distribution and susceptibility profile of fungal isolates from bloodstream infections during the period from 2013 through 2015

Objective To investigate the pathogen distribution and susceptibility profile of fungal isolates from bloodstream infections,and valuate the clinical utility of G test in diagnosis of fungal infections for the purpose to improve antifungal therapy.Methods A retrospective analysis was carried out to analyze the fungal pathogens isolated from bloodstream infections in the First Affiliated Hospital of Nanjing Medical University during the period from January 2013 through December 2015 and their antimicrobial susceptibility.Results A total of 114 fungal strains were isolated from bloodstream infections during the 3-year period,most of which were Candida (99/114,86.8%),especially Candida albicans (30.7%).About 41.2% (47/114) of the fungal strains were isolated from Department of Thoracic Surgery (10,5 and 4 strains in 2013,2014 and 2015),Hematology (11 strains in 2014),and ICU (7 strains in 2014).Antimicrobial susceptibility testing showed that all the fungal strains (100%) were susceptible to amphotericin B,but 83.5% susceptible to itraconazole (the lowest).G test was positive before the result of blood culture in 13 of the 54 patients who received G test.Conclusions Candida was the most common fungus in fungal bloodstream infection.Amphotericin B is the most active antifungal agent in vitro.Blood culture combined with serological test can provide clinicians an earlier and reliable diagnosis.
6.Construction of recombinant adenovirus vectors carrying proapoptotic gene bimS.

In order to further study the anti-tumor activity of bimS and the feasibility of using adenovirus vector for gene therapy,we constructed a recombinant adenovirus vector of pro-apoptotic factor bimS(pAdEasy-CMV-bimS). Human bimS gene was amplified from HL-60 leukemic cell by RT-PCR and it was identified by sequencing. Then bimS was cloned into the shuttle vector pAdTrack-CMV that carried a green fluorescence protein (GFP) gene to generate a recombinant plasmid pAdTrack-CMV-bimS. This plasmid and adenovirus backbone plasmid pAdEasy-1 were linearized and electroporated into E. coli BJ5183 host bacteria to mediate homologous recombination. The positive clone was identified by restriction endonuclease digestion. The recombinated adenovirus pAdEasy-CMV-bimS was transferred into HEK293 cell for packaging and amplification. The virus titre was determined and the insert of bimS gene was verified by PCR method. Finally, HEK293 cell was infected by recombinated adenovirus pAdEasy-CMV-bimS (MOI=100) and bimS protein was detected by western blot. GFP expression in transfected HEK293 cells could be observed at 48-72 hours. bimS gene was detected in cultural supernatant of infected HEK293 cell by PCR and there was typical cytopathic effect(CPE) in HEK293 cell 7 days after infection. Western blot showed bimS protein expression in infected HEK293 cells was markedly higher than that in uninfected HEK293 cells. The result indicated that human bimS recombinant adenovirus was constructed successfully, which could therefore provide a sound base for anti-tumor gene therapy with bimS in vivo and in vitro.

7.Feasibility of school health management model based on Internet technology

Objective:
In order to understand and supervise the current situation of school health in Shanghai, the feasibility of internet-technology (IT) based school health management model is exarnined.
Methods:
Questionnaire survey, The feasibility analysis of IT based school health management model is discussed.
Results:
Principal directors from educational departments and health supervision centers more optimistic about the school health supervision model than school teachers(80.0%, 95.5%, 52.0%;73.3%, 90.9%, 55.1%). However, the three departments all in the publishing the information of school health(0, 13.9%, 6.3%).
Conclusion
IT based school health management model will become one of the most important supervision methods in the future. School Health management model based on IT is feasible.
8.Efficacy comparison of lymph node dissection patterns of the reverse and the cabbage in hand-assisted laparoscopic D2 radical gastrectomy.

OBJECTIVETo compare the clinical efficacy of the lymph node dissection patterns of the reverse and the traditional cabbage in hand-assisted laparoscopic D2 radical gastrectomy (HALG).
METHODSFrom December 2010 to October 2013, 194 patients with HALG in Chengdu Military General Hospital were enrolled in this study. According to the pattern of lymph node dissection, 108 patients were performed with the reverse procedure which took spleen as starting point, from left to right, and 86 patients were performed with the traditional cabbage procedure which took the abdominal cavity as the center, from both sides to middle. A retrospective comparative analysis was made on the intra- and post-operative data between the two groups.
RESULTSAll the patients were successfully performed with HALG, and no peri-operative death occurred. There were no significant differences in the incision length [(7.0 ± 0.2) cm vs. (6.9 ± 0.3) cm], the operative time [(170.9 ± 33.8) minute vs. (174.6 ± 22.4) minute], dissected lymph node number (17.6 ± 7.5 vs. 17.1 ± 5.8) and post-operative complications [(6.5%(7/108) vs. 8.1%(7/86)] between the reverse group and cabbage group (all P>0.05). However, less blood loss [(204.6 ± 98.2) ml vs. (259.1 ± 122.6) ml, P<0.01] and shorter postoperative hospital stay [(9.0 ± 1.7) day vs. (10.5 ± 4.0) day, P<0.01] were observed in reverse group as compared to cabbage group. During 1 to 6 months follow-up, no death case was found in reverse group, while 1 case died due to upper gastrointestinal bleeding 48 days after operation in cabbage group.
CONCLUSIONEfficacy is similar between the two HALG procedures in lymph node dissection, while reverse procedure has certain advantages, such as less blood loss and faster recovery.

9.The effect of phased goal oriented liquid therapy on tissue perfusion and cognitive function in lung cancer patients undergoing radical surgery

Objective:To explore the effects of phased goal directed fluid therapy (GDFT) during anesthesia surgery on tissue perfusion and cognitive function in patients undergoing radical lung cancer surgery.Methods:A total of 108 lung cancer patients were prospectively selected and randomly divided into a control group and a study group using a random number table method. The control group received classical restrictive liquid therapy, while the study group received staged GDFT. We compared the surgical time, intraoperative blood loss, colloid fluid dosage, crystalloid fluid dosage, total output, and urine volume between two groups of patients; Two groups of patients were compared in terms of oxygenation index (OI), respiratory index (RI), central venous oxygen saturation (ScvO 2), lactate (Lac), central venous arterial carbon dioxide partial pressure difference (Pcv-aCO 2), oxygen supply index (DO 2I), and oxygen uptake rate (O 2ERe) before anesthesia induction (T 0), before single lung ventilation (T 1), 1 hour of single lung ventilation (T 2), immediate resumption of dual lung ventilation (T 3), 30 minutes of dual lung ventilation (T 4), and after surgery (T 5); The Mini Mental State Examination (MMSE) was used to evaluate the cognitive function scores of two groups of patients 1 day before surgery and 1 and 3 days after surgery, while recording the incidence of cognitive dysfunction (POCD) and pulmonary complications (including pulmonary infection, acute lung injury, pulmonary embolism, pulmonary edema, atelectasis, etc.) within 3 days after surgery. Results:The amount of crystal fluid and urine output in the research group was significantly lower than that in the control group, while the amount of colloidal fluid was significantly higher than that in the control group (all P<0.05). The OI of the study group T 1-T 5 was significantly higher than that of the control group, while the RI of T 2-T 5 was significantly lower than that of the control group (all P<0.05). The ScvO 2 of the study group T 1 to T 5 was significantly higher than that of the control group, and the Lac was significantly lower than that of the control group (all P<0.05); The MMSE scores of both groups of patients were significantly lower than those before surgery on day 1 and 3 after surgery, and the MMSE scores of the study group were significantly higher than those of the control group on day 1 and 3 after surgery (all P<0.05). The incidence of POCD within 3 days after surgery in the study group was 16.67%(9/54), lower than 37.04%(20/54) in the control group (χ 2=5.704, P=0.017); The incidence of pulmonary complications in the study group was lower than that in the control group (5.56% vs 22.22%, χ 2=4.955, P=0.026). Conclusions:The application of staged GDFT during anesthesia in patients undergoing radical lung cancer surgery can further improve tissue perfusion, improve microcirculation and oxygen supply-demand balance of systemic organs and tissues, including the brain, alleviate perioperative brain function damage, and reduce the occurrence of postoperative POCD compared to conventional liquid therapy.
10.Relationship between perceived social support and family function in parents of pediatric liver transplant recipients: a chain mediation effect analysis

Objective:To investigate the chain mediation effect of psychological flexibility and dyadic coping between perceived social support and family function in parents of pediatric liver transplant recipients.Methods:Totally 320 parents of pediatric liver transplant recipients who were treated at the Department of Pediatric Organ Transplantation, Tianjin First Central Hospital from April to October 2023 were selected by convenience sampling. The participants were surveyed using a general information questionnaire, the Perceived Social Support Scale (PSSS), the Acceptance and Action Questionnaire-Ⅱ (AAQ-Ⅱ), the Dyadic Coping Inventory (DCI), and the Family APGAR Index (APGAR). Pearson correlation analysis was used to examine the relationships between perceived social support, psychological flexibility, dyadic coping, and family function in these parents. Structural equation modeling (SEM) was performed using Amos 26.0 to analyze the chain mediation effect of psychological flexibility and dyadic coping between perceived social support and family function, with the Bootstrap method used for model testing.Results:A total of 320 questionnaires were distributed, with 312 valid responses, yielding a response rate of 97.50% (312/320). The scores for the 312 parents were as follows: PSSS (59.29±15.64), AAQ-Ⅱ (20.35±9.07), DCI (124.64±32.65), and APGAR (6.98±2.74). Family function was positively correlated with perceived social support and dyadic coping ( P<0.01), and perceived social support was positively correlated with dyadic coping ( P<0.01). Psychological flexibility was negatively correlated with family function, perceived social support, and dyadic coping ( P<0.01). SEM results showed that psychological flexibility and dyadic coping had a significant chain mediation effect between perceived social support and family function, with a mediation effect value of 0.059. The chain mediation effect of psychological flexibility and dyadic coping accounted for 13.81% of the total effect (0.059/0.427) . Conclusions:Perceived social support directly affects family function in parents of pediatric liver transplant recipients and also indirectly influences family function through the chain mediation effect of psychological flexibility and dyadic coping.
