1.Diagnostic accuracy of serum interleukin-8 in predicting severe acute pancreatitis: a Meta-analysis

Objective To investigate the diagnostic accuracy of serum interleukin-8 (IL-8) in predicting severe acute pancreatitis (SAP) by conducting a Meta-analysis.Methods Publications addressing the diagnostic accuracy of serum IL-8 in the diagnosis of SAP were selected from Pubmed,Embase,Cochrane library,CBM,Wanfang,VIP,and CNKI databases.Data analysis was conducted by STATA 9.2software.Results According to the inclusion criteria,five diagnosis tests were retrieved,including 299 SAP patients and 2956 healthy controls.Meta-analysis results showed that the pooled sensitivities of IL-8 were 65.8%and 70.9% for days 1 and 2,respectively; the pooled specificities were 66.5% and 91.3% for days 1 and 2,respectively; the pooled positive predictive value were 54.7 and 86.2 for days 1 and 2,respectively; the pooled negative predictive value were 75.9 and 80.3 for days 1 and 2,respectively; the area under curve (AUC) were 0.73 (95% CI:0.21-0.97) and 0.91 (95% CI:0.85-0.92) for days 1 and 2,respectively; and the diagnostic odds ratio (DOR) were 6.1 (95% CI:2.8-13.2) and 23.4(95 % CI:8.1-63.7) for days 1 and 2,respectively.Conclusions Serum IL-8 on day 1 and 2 seems to perform high accuracy in predicting SAP,but diagnosis accuracy of serum IL-8 on day 1 was superior than that on day 1.Serum IL-8 might be an important tool for early prediction and diagnosis of SAP.
3.Combined determination of serum C-reactive protein,lipase,interleukin-1β,and intercellular adhesion molecule 1 in early identification of severity of acute pancreatitis

Objective Toevaluatethevalueofcombineddeterminationofvariousindicesintheclinicaldiagnosisofacutepancreatitis (AP)through dynamic measurements of the levels of peripheral serum C-reactive protein (CRP),lipase (LPS),interleukin-1β(IL-1β),andintercellularadhesionmolecule1(ICAM-1),aswellaschangesintheirlevels,inpatientswithAP.Methods Atotalof86 patients with AP who were treated in Fourth Affiliated Hospital of China Medical University from January 2010 to December 2012 were select-ed,among whom there were 39 patients with severe acute pancreatitis (SAP)and 47 patients with mild acute pancreatitis (MAP).Serum sam-ples were collected from these patients on days 1,3,5,and 7 of admission.Twelve healthy subjects were selected as controls.Enzyme-linked immunosorbent assay was applied to measure the levels of CRP,IL-1β,and ICAM-1 ,and the dry slides method was applied to measure the concentration of serum LPS.The t test and χ2 test were applied for comparison of continuous data and categorical data between groups, respectively.Results Onday1ofadmission,thepatientsinMAPgrouphadsignificantlyhigherlevelsofCRP,IL-1β,andLPSthan those in control group (t=-74.126,-60.135,and -364.153,respectively;all P<0.001);on day 1 of admission,the patients in SAP group had significantly higher levels of CRP,ICAM-1,IL-1β,and LPS than those in control group (t=-121.355,-25.728,-89.422,and -415.840,respectively;all P<0.001).The peak concentration of each index appeared on day 1 or 3 of admission,and decreased progressively in the course of therapy.The patients in SAP group had significantly higher levels of serum CRP,LPS,IL-1β,and ICAM-1thanthoseinMAPgroup(allP<0.05).Conclusion CombineddeterminationofserumCRP,LPS,IL-1β,andICAM-1 levels has a certain clinical value in early identification of severity of AP.
4.Predictive Study on Pharmacological Effects of Herbal Medicine based on Support Vector Machine

B ased on Chinese medicine pharmacological literature data , the property , flavor , toxicity , meridian tropism, efficacy, and clinical application of Chinese herbal medicine were used as a set of attributes. The sup-port vector machine ( SVM ) was used in the establishment of predictive models of 187 pharmacological effects of Chinese herbal medicine respectively. And the cross-validation method was used to determine the accuracy of predictive models . After that , the predictive models with the predictive accuracy rate greater than 90% were used to predicate pharmacological effects of 624 herbals recorded in the Chinese Pharmacopoeia(2010 edition). It was found that the accuracy rate of 108 models was greater than 90%, and the accuracy rate of antibacterial effect predictive model was 99.76%. The highest predictive value of Chinese herbal medicine was the anti-oxi-dation effect of Menispermi Rhizoma.
5.Clinical observation on the relation between tongue petechia and gynecologic tumor and dysmenorrhea

Objective To discuss the relation between gynecologic tumor and dysmenorrheal and tongue petechia, make clear the diagnostic value of tongue petechia in gynecology department tumor and dysmenorrheal, and provide theoretic basis for diagnosing gynecologic diseases by TCM. Methods A total of 169 patients with gynecologic tumor and dysmenorrheal were set as a treatment group, and a total of 211 patients who excluded the tongue signs of gynecologic tumor and dysmenorrheal were set as a control group. Tongue petechias were identified and compared between the two groups. Results ①Probability of tongue petechia showed a significantly higher value in the treatment group than the control group. (P<0.01) ; ②Probability of tongue petechia showed a significantly higher value of patients with tumor in the treatment group than patients with other gynecologic diseases in the control group (P< 0.05) ; ③Probability of tongue petechia showed a significantly higher value of patients with dysmenorrhea in the treatment group than patients with other gynecologic diseases in the control group (P<0.01). Conclusion ①Relevant researche of gynecologic tumor, dysmenorrheal and tongue petechia at the tip of tongue was based on TCM theory, diagnostics and clinical experiences. The data of this study has proven this diagnostic method.②Compared with other kinds of diseases, gynecologic tumor and dysmenorrheal showed a higher probability in the appearance of tongue petechia. Therefore, petechia at the tip of tongue can be used as one of diagnostic references for gynecologic tumor and dysmenorrheal.
6.Clinical research on the effect of patient positioning in the evaluation of great saphenous vein reflux elicited by the pneumatic cuff method

Objective To investigate the effect of patient positioning on the Duplex ultrasound evaluation of great saphenous vein reflux elicited by the pneumatic cuff method. Methods FFifty great saphenous veins (GSV) with relfux (relfux group) and iffteen with no prior history of venous disease (healthy group) were examined by duplex scanning in the supine, 20 degrees reverse-trendelenburg (RT-20), 40 degrees reverse-trendelenburg (RT-40) and standing position. Each GSV was assessed for relfux at three venous sites:two centimeter below the sapheno-femoral junction (SFJ), the greater saphenous vein in the mid thigh (MGV) and the greater saphenous vein in the upper calf (CGV). Pneumatic cuff compression pressure of conifned 100 mmHg (1 mmHg=0.133 kPa) was used onto the calf to elicit relfux. The incidence of positive venous relfux was calculated. The statistical differences of the peak relfux velocity and duration of relfux in four positions were analyzed. Results TThe relfux elicited in the standing position was set as the gold standard. In healthy group, there was no false positive results of relfux in supine, RT-20 and RT-40 positions. In relfux group, false negative results were found at all venous sites when limbs were examined in supine position [false negative rate:59%(19/32), 22%(11/50), 24%(12/50)]. At RT-20 and RT-40 positions, the incidence of venous relfux reached 100% at MGV and CGV, and false negative cases were only detected at SFJ [false negative rate:12%(4/32), 12%(4/32)]. The relfux time in standing, supine, RT-20 and RT-40 positions were (7.75±3.23) s, (5.27±3.66) s, (8.67±3.72) s, (8.55±3.93) s respectively. There were signiifcant differences among different positions in reflux time (F=56.9, P<0.01). In detail, no significant differences were identified between standing position and RT-20 or RT-40 position (q=1.51, 1.33 respectively, both P > 0.05), except for supine position (q=4.11, P<0.01). Peak relfux velocity in standing, supine, RT-20 and RT-40 positions were (55.26±22.24) cm/s, (22.87±12.03) cm/s, (38.46±16.30) cm/s, (45.13±19.21) cm/s respectively. There were also signiifcant differences among different positions in peak relfux velocity (F=13.7, P<0.01). Comparing the supine, RT-20 and RT-40 positions with standing position, differences of the peak relfux velocity between them were all statistically signiifcant (q=12.71, 6.59, 3.98 respectively, all P<0.01). Conclusions When GSV reflux was examined by pneumatic cuff compression, false negative rate was higher in the supine position. RT-20 and RT-40 position were effective to detect GSV relfux, espically for GSV at mid-thigh and upper calf.
7.Effects of pre-existing antibody on seroconversion rate after influenza vaccination

Objective:To investigate the effects of pre-existing antibody on seroconversion rate after influenza vaccination.Methods:This study recruited 1 900 healthy volunteers to receive influenza split vaccines in Xinjiang Uygur Autonomous region and Yunnan Province from September 2009 to October 2018. Hemagglutinin agglutination inhibition assay was used to detect the titers of specific antibodies in blood samples collected before vaccination and 28 d after vaccination and the effects of pre-existing antibody on the seroconversion to different influenza vaccine components were analyzed.Results:Trend analysis showed that with the increasing titer of pre-existing antibody, the seroconversion rates to A/H1N1, A/H3N2, B/Victoria and B/Yamagata vaccine components were gradually decreased (χ 2=121.76, P<0.001; χ 2=67.58, P<0.001; χ 2=45.25, P<0.001; χ 2=54.55, P<0.001). After adjusting for factors such as region, gender and age, multivariate logistic regression showed that pre-existing antibody titer equal to or higher than 40 was an independent factor that affected the seroconversion to A/H1N1, A/H3N2 and B/Victoria vaccine components, and the adjusted OR (95%CI) values were 2.50(2.00-3.13)、1.64(1.35-2.00) and 2.50(1.79-3.45), respectively. Conclusions:The seroconversion rate to each vaccine component was negatively correlated with the pre-existing antibody titer. The factor that pre-existing antibody titer equal to or higher than 40 was detrimental to the seroconversion to A/H1N1, A/H3N2 and B/Victoria vaccine components, but had no significant influence on B/Yamagata seroconversion.
8.The role of Nods like receptors in the patients with allergic rhinitis.

OBJECTIVE:
To explore role of Nods (nucleotide-binding oligomerization domain Nod Like receptors) kind of pattern recognition receptors (PRR) in patients with allergic rhinitis.
METHOD:
The mRNA and protein of Nod1, Nod2 of Nalp3 were analyzed in the turbinate mucosa of patients with allergic rhinitis, nasal septum deviation (NSD) nasal mucosa of patients and nasal polyp mucosa with Real-Time RT-PCR, Western blot and immunohistochemistry respectively, and Nod1 expression changes was explored in PBMC with wad explored Western-blot and then the level of IL-4, IL-6, IL-10, IFN-γ were detected in serum of AR after desensitization treatment.
RESULT:
These Nods like receptors, mainly found in nasal mucosa epithelial cells, glandular epithelium and inflammatory cells (e. g. plasma cells, eosinophils), were expressed in the nasal mucosa tissues. In AR group, Nod1 (mRNA and protein) expression were lower than NSD group (P<0.05), Nalp3 expression were higher than (P<0.05), while, there was no significant difference of Nod2 (mRNA and protein) between groups. After 6 months desensitization therapy, the change of Nod1 in PBMC was negatively correlated with the change of IL-10 in the peripheral blood, r=-0.88, P<0.05; while, change of Nod1 was positively correlated, with the change of IL-6, r=0.57, P>0.05.
CONCLUSION
Nod1, Nod2 and Nalp3 expression were seen in the two groups,and the Nod1 expression in allergic rhinitis group was lower than other two groups, while, the Nalp3 was higher than other two groups. It showed Nod1, Nalp3 may be involved in the pathogenesis of allergic rhinitis. Expression of Nod1 in PBMC reduced after sublingual desensitization treatment. Besides, the change of Nod1 was negatively correlated with the change of IL-10 in PBMC. So,it seemed that Nod1 may regulate IL-10 changes and be involved in sublingual desensitization therapy.

9.Features of full field electroretinogram responses in children with early type 1 diabetes

Objective To observe the features of the full field electroretinogram (FF-ERG) in type 1 diabetes (T1D) children without diabetic retinopathy (DR).Methods Retrospective case study.Forty-one T1D children and 25 age-matched normal controls underwent a complete ophthalmic examination,including best-corrected visual acuity,refraction,intraocular pressure,slit lamp,fundus photography,indirect ophthalmoscopy,and spectral domain optical coherence tomography to exclude DR.All FF-ERG tests were performed by an experienced technician.The ERG series includes six protocols:dark-adapted 0.01 ERG (r-b 0.01);dark-adapted 3 ERG (mix-a 3.0,mix-b 3.0);dark-adapted 10 ERG (mix-a 10.0,mix-b 10.0);dark-adapted oscillatory potentials (OPS);light-adapted 3 ERG (c-a 3.0,c-b 3.0);light-adapted 30 Hz flicker (30 Hz FP) ERG.To compare the amplitudes and implicit times of the FF-ERG between the T1D and control group children.Results Compared with the control subjects,the FF-ERG amplitudes decreased and the implicit times increased in T1D.Except for r-b 0.01 (t =-0.228,P>0.05),the amplitudes of other FF-ERGs were all significantly attenuated (t =-1.664,-3.645,-4.324,-6.123,-5.846,-12.9,-14.4,-5.23;P<0.05) in T1D children.The implicit times of mix-b 3.0,mix-b 10.0,c-b 3.0 and OP2 significantly increased (t=5.242,2.879,5.378,3.506;P<0.05).The implicit times of r-b 0.01,mix-a 3.0,mix-a 10.0,c-a 3.0 and 30Hz FP changes were not significantly (t=2.331,1.677,0.557,0.84,0.064;P > 0.05).Conclusion The FF-ERG amplitudes decreased and implicit times increased in T1D children compared with the control normal subjects.
10.Effect of bone marrow-derived mesenchymal stem cells on TGF-β1/Smad signaling pathway during acute lung injury in rats

Objective To evaluate the effect of bone marrow-derived mesenchymal stem cells (BMSCs) on transforming growth factor-β1 (TGF-β1)/Smad signaling pathway during acute lung injury (ALI) in rats.Methods Healthy clean-grade adult male Sprague-Dawley rats,aged 4-5 weeks,weighing 100-120 g,were selected,and BMSCs were prepared and cultured in vitro.Eighty-four healthy clean-grade adult male Sprague-Dawley rats,aged 4 weeks,weighing 170-190 g,were selected and divided into 4 groups (n=21 each) using a random number table method:control group (group C),group ALI,ALI plus BMSCs group (group ALI + BMSCs),and ALI plus phosphate buffer solution (PBS)group (group ALI+PBS).ALI was induced by intraperitoneally injecting 5 mg/kg lipopolysaccharide 0.5 ml in anesthetized rats.In group ALI+BMSCs,1×104 cells/ml BMSCs 0.5 ml (in PBS) was injected via the tail vein after intraperitoneal injection of lipopolysaccharide.PBS 0.5 ml was injected via the tail vein after intraperitoneal injection of lipopolysaccharide in group ALI+PBS.Arterial blood samples were collected at 6,24 and 48 h after injecting BMSCs for blood gas analysis and for determination of wet to dry weight ratio (W/D ratio),pathological changes (using haematoxylin and eosin staining),and expression of TGF-β1,Smad2 and Smad3 in lung tissues (by Western blot).Results Compared with group C,PaO2 was significantly decreased,PaCO2 and W/D ratio were increased,the expression of TGF-β1,Smad2 and Smad3 in lung tissues was up-regulated (P<0.05),and the pathological changes of lung tissues were accentuated in ALI,ALI+BMSCs and ALI+PBS groups.Compared with group ALI,PaO2 was significantly increased,PaCO2and W/D ratio were decreased,the expression of TGF-β1,Smad2 and Smad3 in lung tissues was down-regulated (P<0.05),and the pathological changes of lung tissues were significantly reduced in group ALI+BMSCs.Conclusion The mechanism by which BMSCs mitigates ALI may be associated with inhibiting TGF-β1/Smad signaling pathway in rats.
