Objective To evaluate the short-term effect of open and laparoscopic cholecystectomy surgery on body.Methods Forty patients with cholecystectomy were divided into open surgery group and laparoscopic surgery group by random digits table method with 20 cases each.The clinical index was compared between two groups.The immunoglobulin G(IgG) and procalcitonin (PCT) before surgery and 4 d after surgery were determined.Results The operative blood loss,anal exhaust time,length of stay was (135.80 ±26.74) ml,(1.6 ±0.9) h,(6.7 ±3.2) d in open surgery group,which was better than that in laparoscopic surgery group [(63.41 ± 15.06) ml,(0.4 ± 0.3) h,(2.8 ± 1.9) d],and there was significant difference (P ＜ 0.05).The incidence of heat was 15％(3/20) in open surgery group,which was higher than that in laparoscopic surgery group (0),and there was significant difference (P ＜0.05).There was no significant difference in the IgG and PCT between two groups before surgery (P ＞ 0.05).The IgG and PCT at 4 d after surgery was (13.93 ± 1.98) g/L,(2 153.2 ± 1 452.1) mg/L in open surgery group,which was higher than that in laparoscopic surgery group[(9.23 ± 2.87) g/L,(768.0 ± 237.0) mg/L],and there was significant difference (P ＜ 0.05).Condmion Laparoscopic cholecystectomy has smaller short-term effects on body,while the body of the humoral and cellular immune function in mild,conducive to the body's recovery.

Objective To discuss the effects of vitamin C in vitro,on human cancer HeLa cell multiplication and apoptosis, in order to promote vitamin C in the tumor treatment application. Methods the morphology changeof cervical cancer Hela cell was observed under the inverted microscope;the effects of vitamin e on Hela cell by MTT assay; cell cyele and weakly perishing rate was analysed rate was analysed by Flow CytoMeter;applying the agarose gelatin electricity swims to analyse weakly perishes in the process the cell gene group DNA change. Results Vitamin C hinders the HeLa cell by the density and the time dependence way to the G0/G1 period, suppresses it to grow and to induce weakly perishes. Conclusions Vitamin C may suppress the HeLa cell to grow, causes the cell to hinder to the G0/G1 period and induces cell weakly perishing.Its reason possibly was HPV E6 protein expression surrenders to restoring the function of P53 gene.

Aim To study tumor suppressor gene OPCML,DAPK methylation changes during the process of CasKi cell apoptosis induced by trichosanthin, and to explore the correlation and the role between cervical cancer cell apoptosis and tumor suppressor gene methylation so as to find new demethylation drugs.Methods ① MTT was applied to assay the inhibition of TCS to CasKi cell proliferation and flow cytometry was used to analyze cervical CasKi cell apoptosis induced by trichosanthin; ② Methylation-specific PCR(MSP) technology was applied to detect cervical cancer and during cell apoptosis process OPCML and DAPK gene promoter methylation status of CpG islands.Results In CasKi cervical cancer cells,OPCML and DAPK gene promoter region showed a high degree of CpG island methylation status, by trichosanthin treatment,the growth of CasKi markedly was inhibited, and flow cytometry analysed the characteristic sub-G1 peak,OPCML and DAPK gene promoter region showed no CpG island methylation of performance.Conclusions During the process of CasKi cell apoptosis induced by trichosanthin,OPCML and DAPK gene demethylated significantly,accordingly, trichosanthin might be a new methylation inhibitor,and there might be some correlation between cell apoptosis and tumor suppressor gene methylation.And OPCML and DAPK gene methylation tests might become new clinical indicators for the early detection of cervical cancer.

[Objective] To discuss the effect of the community chronic disease management mode which combines physical identification and“preventive treatment”in control of GLU and TG of diabetes. [Method] 300 patients with T2 DM were randomly divided into the control group and intervention group with 150 patients in each group. The intervention group were physical y identified, and the management mode based on theories of“preventive treatment”including health education, diet control, exercise and medication used in the intervention group according to the result of physical identification, while the general management was used in the control group. 1 year later, changes of patients ’GLU and TG were observed. [Result] The community chronic disease management mode which combines physical identification and “preventive treatment”can improve T2 DM patients’GLU and TG, and reduce the GLU and TG of patients with QiXu, YangXu, and ShiRe types of physique. [Conclusion] The community chronic disease management mode which combines physical identification and“preventive treatment”is proved to be effective, and it improves the management of diabetes in communities.

Objective To investigate the expression and significance of Toll-like receptors-4 in peripheral blood and placental tissue in gestational diabetes mellitus (GDM) patients.Methods From February 2013 to February 2015.a total of 30 cases of gestational diabetes mellitus patients(GDM group) and 30 cases of normal pregnant people(health group)were selected as research objects.Peripheral blood and placental tissue of two groups were collected.The expression of TLR4 mRNA in peripheral blood mononuclear cells was detected by RT-PCR,the expression of TLR4 protein in placenta tissue was detected by immunohistochemistry.Results The expression of TLR4 mRNA in peripheral blood mononuclear cells of the observation group was significantly higher than that of the health group[(0.63±0.12) vs.(0.32±0.07)],the difference was statistically significant(t=12.223,P<0.05).The expression of TLR4 in villous trophoblast cells,decidual cells and amniotic epithelial cells in GDM group was significantly higher than that in the health group(Z=2.325,2.374,2.162,P<0.05).Conclusion The expression of TLR4 in peripheral blood mononuclear cells,villous trophoblast cells and decidual cells in gestational diabetes mellitus patients significantly enhanced,suggesting that TLR4 might be related to the e gestational diabetes mellitus.

Objective To observe the response of and recurrence in patients with localized eczema after treatment with topical glucocorticoid alone or in combination with topical antibacterials.Methods Totally,217 outpatients with acute or subacute localized eczema were enrolled and randomly classified into two groups to be treated with topical corticosteroids combined with mupirocin cream (experiment group,n=191) or topical corticosteroids alone (control group,n =190) for two weeks.The patients who were cured at the end of the 2-week treatment were followed for 13 weeks for the observation of recurrence.Chi-square test and Student's t test were conducted to compare the cure and recurrence rates between different groups of patients.Results After two weeks of treatment,the cure rate in the experiment group was significantly higher than that in the control group (80.6％ vs.64.8％,x2 =6.576,P =0.01).Increased cure rate was also observed in patients with acute and subacute eczema receiving glucocorticoidCaspase14antibiotic combination therapy compared with those receiving corticosteroids alone.During the 13Caspase14week follow up,a lower recurrence rate was noted in the experiment group compared with the control group (15.6％ vs.33.8％,x2 =6.455,P ＜ 0.05).Further more,the patients experiencing relapse of eczema showed a higher proportion of hand eczema cases (29.4％ vs.13.0％,x2 =4.982,P ＜ 0.05)compared with those without relapse.Adverse reaction was mild in these patients,and no statistical difference was observed between the experiment group and control group (5.71％ vs.4.46％,x2 =0.176,P ＞ 0.05).Conclusion The combination with topical antibacterials may enhance the therapeutic effect of topical glucocorticoids on and reduce the recurrence of localized eczema.

BACKGROUND:With the introduction of related policies on organ procurement and distribution, the organ procurement and standards of the organ procurement have a clear operational criteria, as wel as how to al ocate organs. But the work processes and organizational positioning on organ procurement organization are not enough elaborated and normal. OBJECTIVE:To analyze the experience of Guangzhou General Hospital of Guangzhou Military Command as the organ procurement organization units in quality management, timely review working progress and explore areas for improvement, which provide the corresponding countermeasures for enhancing quality management and establishing the efficient and professional organ procurement organization. METHODS:A computer-based online search in CNKI database and PubMed database was performed with the keywords of“organ donation, organ transplantation, Organ Procurement Organization, quality management”in English and Chinese by screening titles and abstracts. RESULTS AND CONCLUSION:According to relevant laws and regulations, Guangzhou General Hospital of Guangzhou Military Command formulates a standard evaluation standard and sets up the team, thus achieving the reunification of the responsibilities and powers. At the same time, they improve the relevant organ donation,acquisition, preservation, registration, and design different quality management plans according to these different aspects. In addition, In addition, the hospital hold regular coordinator training and make it standardization. Through a series of quality management measures, the hospital organ procurement organization has gradual y regularized and achieved some results. It is recommended to further strengthen scientific research, achieve the unification of efficiency and effectiveness of scientific research;improve the awareness of the hospital leaders, consummate the mechanism of organ procurement;focus on personnel training, improve the professional quality of the organization members;enhance information management, achieve multi-disciplinary col aboration;build quality evaluation system, achieve the whole process of quality management;emphasize on medical ethics, and promote the harmony between the hospital and the patient.

Objective To discuss the effectiveness of community-based intensified health management on type Ⅱ diabetes patients.Metbods From June 2010 to December 2011,1 024 type Ⅱ diabetes patients in Mishi Lane community were enrolled and assigned to the intervention group (n=501,obtaining intensive health management) and the control group (n=523,obtaining routine community health management).Fastingbloodglucose (FBG),2-hour postprandial blood glucose (PBG2h),HbA1c,blood pressure (BP),triglyceride (TG) and body mass index (BMI) were tested and recorded before and after the intervention.Student's t test or Chi-square test were used for data analysis.Results Differences of age (t=0.124,P＞0.05) and gender (x2=2.0,P＞0.05) were not statistically significant (both P＞0.05).Following the intensive health management,the control rates of HbA1c,BP,TG and BMI were increased to 75.4％,30.4％,85.6％ and 49.0％,respectively (x2 values were 4.54,4.71,6.54 and 3.67; all P＜0.05).TG,FBG and PBG2h of the intervention group were (1.5 ± 0.9),(6.2 ± 1.3) and (8.3 ± 2.4) mmol/L,respectively,which were significantly lower than those of the control group (all P＜0.05).Conclusion Community-based intensive health management could be effective for diabetes patients.

Objective To discuss the effectiveness of knowledge-attitude-practice(KAP)-based health management on type 2 diabetes in community residents.Methods From June 2010 to December 2011,930 type 2 diabetes patients living in Mishi Lane Community were enrolled and assigned to the intervention group(n=494) and the control group(n=436).The KAP mode was used for the intervention group,and the routine management mode was applied to the control group.Fasting blood glucose(FBG),PBG2 h,HbAlc,blood pressure(BP),triglyceride(TG) and body mass index(BMI) were tested and recorded.Student's t test or Chi-square test was used for data analysis.Results Age(t=0.124,P＞0.05),gender(x2=2.0,P＞0.05) and other demographics showed no statistically significant difference between the two groups(all P＞0.05).After one-year KAP-based health management,HbA 1 c of the intervention group reduced to(6.1 ± 1.2)％,FBG and PBG2 h reduced by 1.7 and 3.23 mmol/L,respectively(t=4.926,P＜0.05; t=4.306,P＜0.05; t=4.523,P＜ 0.05).Diet,physical exercises,medication,regular inspection,unhealthy habits and questionare test awaveness of the intervention group were improved(86.03％,82.19％,85.63％,70.45％ 76.11％ and 88.46％,respectively.Drinking,dietary structure,overweight and physical exercises of the intervention group were also improved(14.17％,15.79％,70.65％ and 68.83％,respectively; all P＜0.001).Conclusion The KAPbased health management is proved to be effective in the control of community diabetic patients.

Objective To study the effect of nursing intervention on preoperative anxiety of patients with gynecological carcinoma.Methods 60 patients with gynecological carcinoma in our hospital who were to receive operations were randomly distributed into the control group and the intervention group with 30 in each group.The control group was given routine nursing,while the intervention group received nursing intervention based upon routine nursing.Indexes such as preoperative anxiety,sleeping quality and satisfaction degree to nursing care were evaluated within them.Results Through targeted nursing intervention preoperative anxiety of patients in the intervention group alleviated,their confidence enhanced and satisfaction degree to nursing care improved compared to those of the control group.Conclusions Targeted nursing intervention can reduce preoperative anxiety of patients with gynecological carcinoma and increase satisfaction degree to nursing care.
