1.Nosocomial Infections in ICU: Current Status and Intervention Strategy

OBJECTIVE The status and risk factors of nosocomial infections in intensive care unit(ICU) were investigated to make the intervention strategy. METHODS Use prospective monitoring and retrospective investigation to analyze the 198 nosocomial infections patients. RESULTS The lower respiratory tract infection was the main infection in ICU,about 69.4%.The risk factors were old ages,long hospitalization time,the use of many antibiotics,severe underlying illness,invasive operation manipulation and so on. CONCLUSIONS General intervention strategy should be adopt in the control of ICU nosocomial infections to low the infection rate and raise the rate of rescue.

AIM: To investigate the relationship between high density lipoprotein (HDL) subclasses and the binding activities of hepatic cell and extra-hepatic cell HDL receptor and intracellular protein kinase C (PKC). METHODS: Using purified pre-?_1 HDL and apoE-deficent HDL_3 to react with human smooth muscle cells (SMC) and HepG2 cells, then the activities of pre-?_1HDL and apoE-deficent HDL_3 binding to SMC and HepG2 cells, and the effects of this two HDL subclasses on PKC activities in SMC and HepG2 cells were observed . 3.Investigation and analysis of the subjective life quality of hospitalized children

Objective To investigate the subjective life quality of hospitalized children and analyze its influencing factors for improving their life quality.Methods 48 hospitalized children who were admitted between May to August 2011 were investigated in convenience using inventory of subjective life quality (ISLQ).Results Patients investigated had a higher score than the national norm on Family Life Dimension.The satisfaction level of 20% ~ 30% children were below the average on Family Life and other eight dimensions,the cognitive components of 16 cases (33.3%) and emotional component of 11 cases (22.9%) were below the average.15 cases (31.3%) were not satisfied with their subjective quality of life.Female patients had higher scores than male patients on peer exchanges,body feelings and subjective quality of life satisfaction; the satisfaction level of experience depression for rural children was lower than urban children.Conclusions The subjective life quality of vast majority of hospitalized children is about the general level,but some patients still experience serious anxiety,depression and other negative psychological emotion.Nurses should guide the patients positively and specially,considering the influence of their gender and living places while providing caring.Based on the health education of patients and their family,we promote them to face the disease.
4.A study of the treatment of delayed emergency percutaneous coronary intervention on the prognosis of patients with ST elevation acute myocardial infarction

Objective To compare the prognosis of ST elevation acute myocardial infarction(STEMI) of delayed emergency per‐cutaneous coronary intervention(PCI) and elective PCI following the thrombolysis with reteplase ,and to investigate the clinic value of the former solution .Methods One hundred and twenty STEMI patients were reviewed retrospectively and were divided into 2 groups according to PCI .Eighty two cases were divided into emergency group :thrombolysis with reteplase offered within 6h was followed by elective PCI;38 cases were divided into delayed group:PCI was done after 12-24 h after ATEMI′occurring .The clini‐cal features and CTG changes were recorded ,TIMI class of IRA was conducted before and after PCI ,and their prognosis were com‐pared .Results There was no statistical differences in the class of thrombolysis of myocardial infarction(Class 0 -1 ,2 -3) of in‐farction relative artery (IRA)(4 .88% ,95 .12% vs .5 .26% ,94 .74% ) after PCI(P>0 .05) .There was no statistical difference in LVEF ,LVEDVI and LVESVI 3 months after attack between two groups(P>0 .05) .There was statistical difference in the severe cardiac failure and malignancy arrhythmia between two groups(P<0 .05) ,while no statistical difference exist in angina after infarc‐tion as well as cardiac mortality after attack between two groups(P>0 .05) .Conclusion Delayed emergency PCI to remove the ob‐struction in the coronary artery has no significant difference with elective PCI following thrombolysis in the incidence of composite end point events in STEMI .

This article was aimed to study the mechanism of interaction between human serum albumin (HSA) and zinc ions, in order to provide the information on the secondary structure modification of HSA and the thermodynamics parameters using circular dichroism (CD) and isothermal titration calorimetry (ITC). CD and ITC were applied in the study. The concentration of HSA were 0.025 mmol·L-1, 0.05 mmol·L-1, 0.1 mmol·L-1, and 0.2 mmol·L-1, respectively. The concentration of zinc ion was 5 mmol·L-1. The CD analysis revealed that the secondary structure modification of HSA differed depending on the concentration of HAS. And the content ofα-helix was inversely proportional to the concentration of HSA. When the concentration of HSA was 0.2 mmol·L-1, the content ofα-helix was special. A series of thermodynamics parameters including association constants (Kb), stoichiometry (N), entropy (ΔS) and enthalpy (ΔH) were investigated by ITC analysis. And two types of binding sites were observed when ZnSO4(mmol·L-1)?HSA (mmol·L-1)= 5?0.2. The secondary structure modification of HSA interacting with zinc ions depended on the concentration of HSA, a dramatic reduction ofα-helix was detected when the concentration of HSA attained 0.2 mmol·L-1. And the protein hydrophobicity reduction and peptide chains expansion were equally observed at this concentration. The ITC analysis also revealed endothermic and exothermic binding sites in the ZnSO4-HSA interaction when ZnSO4 (mmol·L-1)?HSA (mmol·L-1)= 5?0.2, indicating that the endothermic sites were specific but not preferential for zinc ions interactions. These results provided theoretical supports for the application of Zn2+ to open the endothermic sites of HSA.

AIM: To investigate lipoprotein lipase gene HindⅢ polymorphism and its relationship with serum lipids and apolipoprotein, serum HDL subclasses in patients with hyperlipoidemia. METHODS: Lipoprotein lipase gene HindⅢ polymorphism was assayed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). AIM: To investigate apolipoprotein A-Ⅰ gene (Apo A-Ⅰ) polymorphism and its relationship with serum HDL subclasses in patients with hyperlipidemia (HL). METHODS: Apo A-Ⅰ genotype was assayed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). The subclasses of serum HDL in 118 patients with hyperlipidemia and 109 healthy subjects were determined by two-dimensional gel electrophoresis conjunction with immunodetection method. RESULTS: Both in HL group and the control group, G/G and C/C genotypes were the most frequent at -78 bp and +83 bp of Apo A-Ⅰ gene, respectively. The frequency of rare A allele at -78 bp in HL group was significantly higher than that in control group. In HL group, subjects with G/A mutation had higher serum levels of TG, Apo C-Ⅲ, pre ?_1-HDL and HDL_ 3a , and lower levels of HDL_ 2a and HDL_ 2b compared to the subjects with G/G genotype. CONCLUSION: The G/A transition in the -78 bp position of the Apo A-Ⅰ gene promoter in patients with hyperlipidemia is associated with HDL subclasses. There is a general shift toward smaller sized HDL, which, in turn, indicates that HDL maturation might be abnormal.

10.Evaluation the liver reservation functions of patients with severe hepatopath of nonage treated with plasma exchange by 13C-methacetin breath test

Objective To judge the effect of plasma exchange (PE) to the patients with severe hepatopath of nonage according to evaluating the change of the liver function of reserve with 13C-methacetin breath test. Methods There are two groups: the case group and the control group. Each group has 30 patients. The patients in the case group were treated by PE. All the patients received 13C-methacetin breath test at before or one week after treatment. MVmax40, CUM40 and CUM120 were present. At the same time, clinical symptoms, glutamate-pyruvate transaminase (ALT), total bilirubin (TBiL) and prethrombin active (PTA) were observed. Results MVmax40, CUM40, CUM120 and PTA were higher, ALT and TBiL were lower in the case group after treatment (t=4.679, 4.752, 5.048, 5.413, 6.208, 7.413, P=0.000,P<0.01). After a week, MVmax40, CUM40, CUM120 and PTA were higher, ALT and TBiL were lower in the case group than that in the control group (t=2.260, 2.247, 2.476, 4.017, 3.250, 3.658, P<0.05). The total effective rates in the case group and the control group were 83.3 % (25/30) and 43.3 % (13/30),which are significant different(χ2 10.335,P<0.01). Conclusion PE can im-prove the liver reservation functionin the severe hepatopath of nonage.
