1.Study on TLC Identification and Content Determination of Compound Yantongxiao Spray

Objective To establish the methods for TLC identification and contents determination of Compound Yantongxiao Spray.Methods Flos Lonicera,Borneolum Syntheticum and Menthol in the spray were identified by TLC,and the content of chlorogenic acid was determined by HPLC.Results The TLC characteristic spots of Flos Lonicera,Borneolum Syntheticum and Menthol can be identified.For content determination,the linear range of chlorogenic acid was in 0.08~0.28 ?g(r=0.999 0),the average recovery of chlorogenic acid was 102.34 %(n=6)and RSD was 2.03 %.Conclusions The methods are simple,accurate,specific and reproducible,can be used for the quality control of Compound Yantongxiao Spray.
2.Expressions of human fibrinogen-like protein 2 in peripheral and cerebrospinal fluid white blood cells in patients with cryptococcal meningitis

Objective The purpose of this study was to explore the expression of human fibrinogen-like protein 2 (hFgl2) in patients with cryptococcal meningitis (CM).Methods The expression of hFgl2 in the peripheral blood leukocytes from 20 CM cases and 20 healthy individuals,and the expression of hFgl2 in cerebrospinal fluid (CSF) leukocytes in 11 pairs of CM patients and matched controls was detected using immunohistochemical methods.Medical image analysis system (MIAS) was used to analyze the expression of hFgl2 gray values,which were negatively correlated to the expression of hFgl2 protein in both peripheral blood and CSF.Results The average gray values for hFgl2 protein in peripheral monocytes of CM patients and healthy controls were 114.49± 11.82 and 137.56± 10.64,respectively,and those in peripheral neutrophils were 111.71 ± 8.62 and 143.56±12.57,respectively.Compared with the healthy control group,expressions of hFgl2 in peripheral blood monocytes and neutrophils were both significantly increased in CM patients (t =21.21,t=27.19; P<0.01).The average gray values for hFgl2 protein in CSF monocytes from CM patients and negative controls were 115.44±8.67 and 149.25±8.57,respectively,and those in CSFneutrophils were 111.71±8.62 and 143.56 ± 12.57,respectively.The expressions of hFgl2 in CSFmonocytes and neutrophils were significantly increased in CM patients (t =29.95,t=28.87; P<0.01),while the expression intensity of hFgl2 was similar when peripheral blood monocytes and neutrophils compared with their CSF counterparts (t=0.924,t=0.607; both P>0.05).Conclusions hFgl2 can be expressed in both peripheral and CSF monocytes and neutrophils in CM patients.
3.Expression of matrix metalloproteinase-2 and-9 in the PBMC of patients with polymyositis and dermatomyositis

Objective To investigate the relation of the expressions of matrix metalloproteinase-2(MMP-2)and-9(MMP-9)with polymyositis/dermatomyositis(PM/DM).Methods Blood samples were obtained from 8 patients with PM,12 patients with DM and 10 normal human controls.Real-time quantitative RT-PCR and Western-blot were applied to examine the mRNA and protein expressions of MMP-2 and MMP-9 respectively in the peripheral blood mononuclear cells(PBMC)of these patients.Results The mRNA expression levels of MMP-2 and MMP-9 in the PBMC of patients with PM or DM were 2.41 and 1.66 folds as high as those of the normal controls,respectively;these difierences were significant between the two groups.Increased expressions of MMP-2 and MMP-9 were also noted in the patients with PM or DM.Conclusion MMP-2 and MMP-9 may play a part in the development of polymyositis and dermatomyositis.

7.Therapeutic and antimicrobial efficacy of macrolides,minocycline, and tosufloxacin against macrolide-resistant mycoplasma pneumoniae pneumonia in pediatric patients

Objective To observe the differences in the therapeutic efficacies of macrolides,minocycline,and tosufloxacin against macrolide-resistant Mycoplasma pneumoniae(MRMP).Methods A total of 188 children with M.pneumoniae pneumonia confirmed by culture and PCR were analyzed.Of these,150 patients had a strain with an MR gene and 134 had one with an A-to-G mutation at position 2063 of M.pneumoniae 23S rRNA domain V.Azithromycin(n=27),clarithromycin(n=23),tosufloxacin(n=62),or minocycline(n=38)was used for definitive treatment of patients with MR M.pneumoniae.Among the 188 patients,the other 38 patients with macrolide-sensitive Mycoplasma pneumonia (MSMP)were grouped into azithromycin(n =16)and clarithromycin groups(n =22)for observing whether there is differences with respect to efficacy under parallel treatment between patients with MRMP and MSMP. Results Defervescence within 48 h after the initiation of antibiotic therapy was observed in 41%of the patients in the azithromycin group,48% of those in the clarithromycin group,69% of those in the tosufloxacin group,and 87% of those in the minocycline group.The average number of days of fever after the administration of antibiotic treatment was lower in the minocycline and tosufloxacin groups than in the macrolide groups(azithromycin and clarithromycin groups).The decrease in the M.pneumoniae burden,as estimated by the number of DNA copies,after 48 to 96 h of treatment was more rapid in patients receiving minocycline(P=0.016)than in those receiving tosufloxacin(P=0.049),azithromycin(P=0.273),or clarithromycin(P=0.107).Conclusion We found that the clinical and bacteriological efficacies of macrolides against MR M.pneumoniae pneumonia was low.Our results indicated that minocycline rather than tosufloxacin can be considered the first-choice drug for the treatment of M.pneumoniae pneumonia in children aged >8 years.
8.THE EFFECT OF AGING ON THE EXCITABILITY OF THE PERIPHERAL NERVES IN RATS

The changes of rheobase(Rh), chronaxie(Chr), standard electrical quantity(a) and mtensity-duration curve(i. t. c) were studied for understanding the effect of aging on the excitability of sciatic and caudal nerves in Wistar rats of different age. The results show that before grown-up excitability of the peripheral nerves increases progressively with age, after that it tends to decrease. Chronaxie as an index can not reflect the change of excitability of the peripheral nerves correctly.
9.Comparison of effect of different dose of dexmedetomidine on postoperative analgesia and sedation

Objective To compare the effects of different doses of dexmedetomidine on postoperative analgesia and sedation.Methods From January 2014 to November 2015,60 cases who needed postoperative analgesia and sedation in our hospital were selected.According to the dosage of dexmedetomidine,they were divided into control group and observation group,30 cases in each group.Two groups of patients were given analgesia pump for analgesia, with 0.5μg·kg-1·h-1,1.0μg·kg-1·h-1 two doses of dexmedetomidine,and analgesia 48 h.At different time points,the VAS pain score,Ramsay sedation score of the two groups were compared,the calm satisfaction in both two groups was recorded,the average incidence of delirium and delirium score in the process of the treatment and the occurrence of adverse reactions were observed.Results With the extension of time of postoperative analgesia,VAS scores in each group decreased,VAS scores in the observation group after 4h,8h,12h were (3.01 ±0.53)points, (1.95 ±0.58)points,(1.52 ±0.35)points,which were lower than those of the control group[(3.92 ±0.32)points, (2.86 ±0.67)points,(2.25 ±0.78)points],the differences were statistically significant (t=3.42,4.11,2.43,all P<0.05).After 24h,48 h,VAS score between the two groups had no significant difference(P>0.05).Postoperative 4h Ramsay score between the two groups had no significant difference (P >0.05 ).With increased postoperative analgesia time,Ramsay scores in two groups decreased,postoperative 8h,12h,Ramsay scores in the observation group [(2.95 ±0.83)points,(2.22 ±0.55)points]were lower than the control group[(3.76 ±0.78)points,(2.98 ± 0.89)points,t=3.45,2.38,all P<0.05].Postoperative 24h,48 h,the Ramsay scores between the two groups had no significant difference(P>0.05).The sedation satisfaction of the observation group (96.67%)was higher than the control group (86.67%),the incidence rate of delirium and delirium average score [3.33%,(15.11 ± 2.03)points]were lower than the control group[13.33%,(19.23 ±2.21 )points],the differences were significant between the two groups (t =4.32,4.32,3.27,all P<0.05 ).After treatment,the adverse reactions were mainly bradycardia,nausea,vomiting,drowsiness,respiratory depression,urinary retention.The overall incidence rate of adverse reactions between the two groups had no significant difference (P >0.05 ).Conclusion The effect of 1 .0μg·kg-1 · h-1 concentration dexmedetomidine within 24h of postoperative analgesia sedative is best,it can reduce the incidence of postoperative delirium,and without obvious drug side effects.
10.Effects of desflurane on membrane permeability of alveolar capillaries in rats with acute lung injury caused by endotoxin

Objective To investigate the effects of desflurane inhalation on pulmonary alveolar-capillary membrane permeability and the inflammatory cell counts in broncho-alveolar lavage fluid (BALF) during endotoxin-induced acute lung injury in rats. Methods Forty-eight Wistar rats weighing 200-290 g were anesthetized with intraperitoneal pentobarbital 100mg?kg-1 , tracheotomized and mechanically ventilated. PETCO2 was maintained at 35-45 mm Hg (VT 8 ml?kg-1 , RR 65-70 bpm). Right carotid artery and left femoral vein were cannulated for BP monitoring and fluid and drug administration. The animals were randomly divided into four group with 12 animals in each group : (1) control group received normal saline (NS) 1.2 ml i.v. followed by 4 h mechanical ventilation; (2) LPS group received LPS (055:B5, Sigma) 5 mg?kg-1 followed by 4h mechanical ventilation; (3)Desflurane group A and B received LPS 5 mg? kg-1 followed by desflurane inhalation for 4 h at 1 MAC (desflurane group A) or 1.5 MAC (desflurane group B) . Six animals in each group received Evans blue 50 mg?kg-1 at the beginning of the experiment for determination of pulmonary alveolar-capillary membrane permeability at the end of the experiment. The animals were sacrificed by exsanguinations at the end of 4h mechanical ventilation. Blood was collected for determination of total plasma protein concentration. Lungs were removed. The right lung was lavaged and BALF was collected for determination of protein content, and differential inflammatory cell counts. The left lung was used for microscopic examination. The morphologic changes were scored 0-3 (0 = normal, 3 = severe morphologic changes). In addition wet/dry lung weight ratio, pulmonary permeability index (BALF protein concentration /total plasma protein concentration?10-3 ) and mortality rate were also determined. Results In group 3 (desflurane 1 MAC) W/D lung weight ratio and lung water content significantly increased compared with those in LPS group. In group 4 ( desflurane 1.5 MAC) W/D lung weight ratio, lung water content, pulmonary permeability index, Evans blue content in lung tissue, morphological change scores and mortality rate were all significantly increased compared with LPS group (group 2). There was no significant difference in total and differential inflammatory cell counts in BALF between group 2 and 4.Conclusion Desflurane inhalation is detrimental to the lungs acutely injured by endotoxin in a dose-dependent manner.
