1.Clinical diagnostic value of serum β-hCG and AFP in mediastinal germ cell tumor

Objective To explore the clinical diagnostic value of serum human chorionic gonadotropin beta subunit (β-HCG) and alpha fetoprotein ( AFP) in mediastinal germ cell tumors .Methods A retrospective analysis was conducted on the patients who were definitely diagnosed as mediastinal tumors or mediastinal neoplastic lesions .A total of 133 patients were included for analysis between January 2008 and May 2014, divided into two groups.42 cases of mediastinal germ cell tumor patients were marked as case group while 91 cases of other mediastinal tumor or mediastinal neoplastic lesion patients were marked as control group ( including 31 cases of thymoma , 10 cases of mediastinal neurogenic tumor , 2 cases of intrathoracic goiter , 25 cases of mediastinal cyst , 2 cases of mediastinal lipoma , 11 cases of mediastinal lymphoma and 10 cases of thymic carcinoma ) .AFP was detected by chemiluminescence detection , and -HCG was detected by electrochemical luminescence .K-S test was performed to investigate normality of data , non-normally distributed data were described as Median ( interquartile range ) .Mann-Whitney U test was done for measurement of data between two groups .Logistic regression analysis was performed as multivariate analysis.Receiver operating characteristic curve ( ROC) was used to determine the cut-off values.Results The levels of serum AFP and β-HCG in case group were 13.26 (2.39-48.09) ng/ml and 1.99 (0.10-15.7) IU/L, respectively, significantly higher than those in control group [AFP:2.47 (1.78-3.16) ng/ml,β-HCG:0.10 (0.10-0.55) IU/L].The difference of levels of AFP and β-HCG between the case group and the control group were statistically significant ( P=0.000 ) .There were no significant difference when it comes to β-HCG between the case group and intrathoracic goiter patients in control group .Apart from it, the difference of levels of AFP and β-HCG between the case group and every single control group were statistically significant .Cut-off values of AFP and β-HCG for distinguishing mediastinal germ cell tumors from mediastinal tumors were 5.07 ng/ml and 2.32 IU/L.In this scenario, for AFP and β-HCG, sensitivity were 57.1%and 50%, specificity were 97.8%and 96.7%, accuracy were 54.9%and 46.7%, area under the curve ( AUC ) were 0.773 and 0.755, positive likelihood ratios were 26.00 and 15.17respectively.Parallel experiments contributed to increase the sensitivity to 71.4%. Predictive probability (P) =1/[1+exp ( -0.319AFP-0.253HCG+2.850)] was obtained by logistic regression model.When cut-off value of predictive probability ( P ) was 0.30, specificity, AUC, and positive likelihood ratio were increased to 98.9%, 0.835 and 65.00respectively, negative likelihood ratio was decreased to 0.29, positive predictive value and negative predictive value were increased also (96.8%and 88.2%respectively).Conclusion Serum β-HCG, AFP and predictive probability ( P ) is valuable in the diagnosis of mediastinal germ cell tumor .
2.Evaluation of clinical pathway introduced in children with bronchiolitis

Objective To investigate the efficacy of the clinical pathway introduced in children with bronchiolitis.Methods Based on a retrospective study,the duration of hospital stay,hospital expenses,antibiotics usage rates,curative rate and nosocomial infection rate were compared between 181 bronchiolitis patients (the clinical pathway group) managed according to clinical pathway and other 122 bronchiolitis patients (the control group).The variation of clinical pathway was analyzed in the clinical pathway group as well.Results The duration of hospital stay in clinical pathway group [4.96 (4.00,6.00) days] was significantly shorter than that in the control group [5.81 (4.82,7.00) days],and the difference was significant (Z =3.137,P < 0.05).The hospital expenses [3 701.23 (3 124.50,4 396.19) yuan] in clinical pathway group were significantly lower than that in the control group [3 954.22 (3 325.07,4 679.66) yuan],and the difference was significant (Z =2.042,P < 0.05).The antibiotics usage rate (20.44%)(37/181 cases) in clinical pathway group was significantly lower than that in the control group (40.16%) (49/122 cases),and the difference was significant (,x2 =13.945,P < 0.05),and the curative rate (93.92%) (170/181 cases)in clinical pathway group was significantly higher than that in the control group (86.89 %) (106/122 cases),and the difference was significant (x2 =4.447,P < 0.05).The nosocomial infection rate (4.42%) (8/181 cases)was also lower than that in the control group (10.66%,13/122 cases),and the difference was significant (x2 =4.393,P <0.05).The variation rate of clinical pathway was 48.62% (88/181 cases) in clinical pathway group.Conclusions The curative rate is improved and the duration of hospital can be shortened if the clinical pathway is introduced in bronchiolitis children,with less the hospital expense.Moreover,the antibiotics usage rates and the nosocomial infection rate are reduced in the implementation process of clinical pathway which is worthy to be put in use.However,there is a high variation rate in the clinical pathway.It should be interposed and administrated appropriately in dinical treatment for clinic.
3.The relationshio between diet habits and constitution type in the general population 8448 adults cases from nine provinces or municipalities of China

ObjectiveTo explore the relationship between diet habits and constitution type of 8448adults from 9 provinces and municipalities of China.MethodsA total of 8448 participants from Jiangsu,Anhui,Gansu,Qinghai,Fujian,Beijing,Jilin,Jiangxi and Henan Province were enrolled in this study.The clinical information of constitution type,diet habits and demographic characteristics was collected.Logistic regression analysis was used to evaluate the relationship between diet habits and constitution type.ResultsLogistic regression analysis showed that gentleness type and Qi-deficiency were correlated with sweet food ( OR =1.22,P < 0.05 ). There was a significantly positive correlation between Yangdeficiency and hot food intake ( OR =2.89,P < 0.05 ),as well as between Yin-deficiency and cold food intake ( OR =1.56,P < 0.05).Phlegm-wetness was mainly caused by fatty food consumption ( OR =2.07,P < 0.05 ).Barbecue food was positively correlated with Wet-heat type ( OR =1.64,P < 0.05 ),Bloodstasis type ( OR =1.37,P < 0.05 ),and Qi-depression type ( OR =1.35,P < 0.05 ).Special diathesis type was related to dietary preference of sweet food ( OR =1.29,P < 0.05 ).ConclusionDifferent type of constitution may be related to specific lifestyle or diet habits,and diet habit might play a positive role in Gentleness type or health condition.
4.Effects of endometriosis on the reproductive ability of mice

Objectives The aim of the study was to establish a mouse model of endometriosis , and to observe the effects of endometriosis on the reproductive ability in the mice .Methods The mouse models of endometriosis was estab-lished by subcutaneous and intraperitoneal injection of heterogenous endometrium .The pregnancy rate and live embryo number in the experimental , sham operation and blank groups were observed and compared , and the effects of endometrio-sis on reproductive ability of the mice were evaluated .Results Two weeks after the establishment of the mouse model of endometriosis , there were lesions in the peritoneal cavity and subcutaneous tissue of the mice .Compared the pregnancy rates among the model group , sham operation group and blank group , the differences were statistically significant ( P<0.05).Compared the live embryo number in the three groups , there were also significant differences (P<0.05).Con-clusions A mouse model of endometriosis is successfully established , and it demonstrates that endometriosis may affect the reproductive ability of mice .
5.Effect of Berberine on Mice RAW264.7 Macrophages Polarization

Objective To investigate the effect of berberine on the polarization of mice RAW264.7 macrophages induced separately by lipopolysaccharide (LPS) and interleukin-4 (IL-4). Methods Mice RAW 264.7 macrophages cultured in vitro were divided into model group, medication group, and blank control group. Both model group and medication group were given either LPS (in final dose of 100 ng/mL) or IL-4 (in final dose of 10 ng/mL). Additionally, the medication group was treated with berberine in final dose of 20 μmol/L. The blank control group was given the same volume of phosphate buffered saline ( PBS). Real-time fluorescence quantitative polymerase chain reaction (PCR) was used to detect the mRNA expression of arginase-1 (Arg-1), inducible nitric oxide synthase ( iNOS) , suppressor of cytokine signaling2 ( SOCS2) and SOCS3. Enzyme-linked immunosorbent assay (ELISA) was used to determine the contents of tumor necrosis factor alpha (TNF-α) and IL-10. Results The content of TNF-αand the mRNA expression levels of iNOS and SOCS3 in macrophages induced by LPS were increased, and then were down-regulated by berberine (P<0.05 or P<0.01) . The content of IL-10 and the mRNA expression level of Arg-1 in macrophages induced by IL-4 were increased, and then were down-regulated by berberine ( P<0.05), but berberine had no effect on the mRNA expression level of SOCS2 ( P>0.05). Conclusion Berberine has an effect on inhibiting the M1 and M2 polarization of macrophages in vitro, suggesting that berberine may play a regulatory role in the dynamic balance of M1/M2.
6.Genetics of sporadic cerebral small vessel disease

Cerebral smal vessel disease (CSVD) can be divided into sporadic and hereditary CSVD. The exact pathogenesis of sporadic CSVD is unknow n. Genetic factors may also play an important role, except for environmental and vascular risk factors. As a complicated disease, sporadic CSVD has the characteristics of multigenetic susceptibility. Therefore, investigating the related genetic factors may contribute to understanding the pathogenesis of sporadic CSVD. This article review s the advances in research on the genetics of sporadic CSVD.
7.Effects of a Chinese traditional medicine, Huoxue Xiaoyi Fang, on embryo development after in vitro fertilization in mouse models of endometriosis

Objectives To evaluate the effect of a Chinese traditional medicine , Huoxue Xiaoyi Fang , on the embryo development after in vitro fertilization ( IVF ) in mouse models of endometriosis .Methods To observe the quantity of oocytes including the average number of superovulation , fertility rate, 2-cell and 4-cell embryo cleavage rates at different stages in vitro fertilization, and morulation rate among the traditional Chinese medicine group (TCM group), endometriosis model group ( EM model group ) and sham-operated group .To explore the influence of endometriosis and the effect of Huoxue Xiaoyi Fang on the quantity of oocytes and embryonic development after IVF in the mouse models of endometriosis.Results All the average number of superovulation , fertility rate, 2-cell and 4-cell embryo cleavage rates in the three groups showed significant differences (P<0.05), and the average number of superovulation was in an order of sham operated group >TCM group>model group .There were no significant differences in the morulation rates in the three groups (P>0.05).There were significant differences among the IVF rates of the three groups (P<0.05), and to be in an order of sham operated group >TCM group>model group .There were significant differences in the final morulation rate among the three groups (P<0.05).Conclusions Endometriosis may influence the quality of embryonic development of EM model mice, including reduced average number of superovulation , fertility rate, and embryo cleavage rate .The Chinese traditional medicine Huoxue Xiaoyi Fang can improve the the quantity of embryonic development after IVF .
8.Role of gut microbiota and its metabolites in severe acute pancreatitis-related acute respiratory distress syndrome

Acute respiratory distress syndrome (ARDS) is a common complication of severe acute pancreatitis (SAP) and a leading cause of early death in SAP patients, but its pathogenesis is still unclear. In recent years, the role of gut microbiota and its metabolites in regulating SAP-related ARDS has attracted more and more attention, and in-depth studies on the pathogenesis of “intestine-lung axis” may provide new ideas for the research and development of drugs for SAP-related ARDS. This article summarizes the recent research advances in gut microbiota and its metabolites in SAP-related ARDS.
9.Community-based Rehabilitation Service Based on the Internet and Information Technology

Community-based rehabilitation (CBR) is positioned to provide various strategies for people with disabilities for achieving the embracing development of the community. CBR is also the terminal of the three-level rehabilitation service system. This paper analyzed the status of domestic community-based rehabilitation based on the document, discussed the possible problems referred to the international concepts about CBR, and suggested a new service pattern of CBR based on the internet and information technology.
10.Preoperative systemic chemotherapy plus regional intraarterial chemoembolization for patients with locally advanced gastric cancer

Objective To investigate the efficacy and safety of preoperative systemic chemotherapy combined with regional intraarterial chemoembolization in the treatment of locally advanced gastric cancer.Methods Clinical data of 158 patients of locally advanced gastric receiving neoadjuvant chemotherapy cancer from January 2008 to July 2012 were retrospectively analyzed.Patients were divided into two groups:those who received preoperative systemic chemotherapy plus regional intraarterial chemoembolization (group A,n =78) and those who received preoperative systemic chemotherapy (group B,n =80).Radical resection was perfomed after 3 to 4 weeks.Results The overall satisfactory rate was significantly higher (60%) in group A compared with 42% in group B (x2 =6.136,P <0.05).The incidence rate of toxicity reaction (except nausea) and postoperative conplications such as anastomotic leakage,intestinal obstruction,poor wound healing,abdominal infection and pulmonary infection were all lower in group A than in group B (all P < 0.05),while the incidence rate of nausea was higher in group A than in Group B (x2 =16.458,P < 0.01).There was no perioperative mortality related to neoadjuvant therapy in two groups.Conclusions Preoperative systemic chemotherapy combined with regional intraarterial chemoembolization was associated with better efficacy,and fewer toxicity reactions and postoperative complications in the treatment of locally advanced gastric cancer.
