Clinical islet transplantation is currently being explored as a treatment for its superiorities of lesser in vasive,lower risk,avoiding or preventing chronic complications.Because of the difficulty in isolation,purification and transplantation,the yield of islet is lower than needed;shortage of donor,difficulty in obtaining adequate islet cells for sustaining B cell mass and function over time,and immune rejection reactions are the hurdels to widespread application of islet transplantation.
2.Therapeutic efficacy of transcather arterial chemoembolization for hepatic carcinoma and its influence on hepatic function: a retrospective study of 92 patients

Objective To evaluate the therapeutic efficacy of transcather arterial chemoembolization for hepatic carcinoma and its influence on hepatic function. Methods Patients with HCC were divided into three groups according to repeated TACE times. The therapeutic efficacy was assessed with survival rate, car-cionma size and biologic response (AFP). Hepatic function was evaluated according to some hepatic param-eters(ALT,AST,ALB,DB,TB,PT). Results All three groups responses rates were 4.3%, 23.1% and 31.6% ;srespectively,liver function in group 1 and group 2 returned to its pretreatment level(P <0.05) ,but in group 3, liver function did not return to its pretreatment level associated with more repeated TACE times. The 1-,2-and 3-year survival rates of group 1 were 27.0%, 10.8%, 5.4% ; respectively, while those of group 2 were 57.1%, 33.3%, 19.0% ; respectively, which were not significantly different from those of Team 3 ,which was 62.5%, 37.5%, 18.8%, respectively. Conclusions (1) three to four repeated times TACE was an effective palliative treatment that prolongs survival of patients with HCC,while 1 to 2 re-peated times had a limited benefit; (2) Irreversible hepatic impairment induced by TAEC could affect its therapeutic efficacy.
3.Management of patients with acute billiary pancreatitis

Objective To investigate the treatment options of acute biliary pancreatitis.Methods A retrospective review was performed in 134 consecutive patients who presented to a single tertiary care institution from 2000 to 2010 with acute biliary pancreatitis.Results Of the 134 patients,30 had a persistent (commonbile duct,CBD)stone.Following multivariate analysis,at admission CBD size on ultrasound,alkaline phosphatase(ALP),total bilirubin(TB),significantly correlated with persistent CBD stone.Receiver operator curve analysis and linear regression were applied to obtain optimal and equitable predictive values,and variables combined.Optimal values were.,CBD≥10 mm;AP≥150 U/L;and TB≥51.3μmol/L.Presence of three variables had an associated odds ratio(OR)of29.5(P<0.001)for presence of persistent CBD stone.Zero variables conferred asignificantly decreased probability of CBD stone,OR 0.07(P<0.001).Conclusions Presence of three variables significantly correlated with persistent CBD stone.Biliary evaluation by endoscopic retrograde cholangiopancreatography(ERCP)is suggested.In the absence of any positive predictive variables,cholecystectomy may be sufficient.Decisions regarding patients with one to two be made vailables should occur on a case-to-case basis.
4.Manipulative reduction and lateral percutaneous K-wire fixation for treatment of supracondylar humerus fractures in 128 children.

OBJECTIVETo explore the clinical effect of manipulative reduction and lateral percutaneous K-wire fixation on supracondylar humerus fractures.
METHODSFrom Feb. 2004 to Jun. 2010,128 cases of supracondylar humeros fractures in children (96 boys and 32 girls) were treated by manipulative reduction and lateral percutaneous K-wire fixation. The average age of the children was 8 years old ranging from 2 to 15 years. Among them, 112 cases were extension fractures, 16 were flexion type; 102 cases belonged to ulnar deviation, and 26 cases belonged to radial deviation. After treatment, the elbow flexion range and carrying angle of the children were measured under the Flynn evaluation standard while considering the postoperative complications status to analyze the clinical effect on manipulative reduction and lateral percutaneous K-wire fixation.
RESULTSAll these children were followed up from 2 to 36 months (16 months on average). According to Flynn evaluation standard,the result were excellent in 116 children (90.6% of the total patients), good in 11 (8.6%), fair in 1 (0.8%). No infection, no ischemic muscular atrophy and no nerve damage had been found during the treatment.
CONCLUSIONThe manipulative reduction and lateral percutaneous K-wire fixation of supracondylar humerus fractures in children has small wound, is stable and reliable, easy to be operated, safe and effective and low cost. What's more, it can also avoid the complication caused by conservative treatment and operation. It is a good treatment of supracondylar humerus fractures in children.

5.Management and opportunity of operation in obstructive acute biliary pancreatitis

Objective To investigate the management and timing of operation in obstructive acute biliary pancreatifis.Methods A etrospective review was performed of seventy-six consecutive patients who presented to a single tertiary care institution from 2000 to 2010 with obstructive acute biliary pancreatitis.Results Of the seventy-six patients,thirty-six patients underwent operations,with early(after onset within 2 weeks)operations in twenty cases,delayed operationsin sixteen cases and one case dead,no one experienced recurrent pancreatitis during follow-up.Fifteen patients were treated by endoscopy,ERCP/EST in eleven cases,ERCP/ENBD in one case and ERCP only in three case.All patients were cured,only one patient experienced recurrent pancreatitis.Twenty-five patients received conservative treatment,one patient died,six patients experienced recurrent panereatitis.Conclusions Surgery is essential in the management of acute biliary pancreatitis.In cases of obstructive acute biliary pancreatitis,early operation or endoscopic therapy should be performed to remove biliopancreatic obstruction after aggressive conservative treatment.
6.Isolation and purification of pig islet cells

Objective To establish effective method for large-scale purification of islet cells from pig pan-cress. Methods Pig pancreas tissue was digested with collagenase P followed by purification in a HCA-Fi-coil dis continuous gradient using Cobe2991 cell separator. After isolation, the islet cell yield and purity were evaluated with light microscope with DTZ staining, and the islet function assessed by insulin release as-say in vitro. Results The number of the islets coll ected from each pancreas averaged (275 000±20 895)islet equivalents (IEQ) before purification, and (230 350±26 679) IEQ after the purification with discon-tinuous gradient centrifugation. From each gram of the pancreatic tissue, (2710±229) IEQ were obtained with an average purity of (50.2±1.95) %. The purified islets responded well to high-concentration (16.7 mmol/L) glucose stimulation with a 4. 74-fold increase of insulin secretion over the basal level (3.3 mmol/L, P <0.001). Conclusion The established method can be applicable for large-scale purifi-cation of fully functional islet cells from pig pancreas.
7.Combination effects of pulsed electric field and low temperature plasma on H22 mouse liver cancer cell

Objective:To investigate the effect of pulsed electric field (PEF) combined with low temperature plasma (LTP) on mouse liver cancer cell.Methods:H22 mouse liver cancer cells were divided into liver cancer group, PEF treatment group, LTP treatment group, combined group A (first PEF treatment immediately after LTP treatment), combined group B (first LTP treatment immediately after PEF treatment), combined group C (same as combined group A, but 20 minutes interval) and combined group D (same as combined group B, but 20 minutes interval). Cell viability was detected by cell counting, apoptosis was detected by flow cytometry, intracellular reative oxygen species (ROS) was marked by fluorescence and counted. Twenty healthy female Kunming mouse aged 4-6 weeks without specific pathogens were subcutaneous injected with liver cancer cells, and then were randomly divided into model group, PBS control group, PEF experimental group, LTP experimental group and combined group (LTP+ PEF, no interval) ( n=4). Tumor relative volume and tumor inhibition rate were measured. Results:The survival rates were liver cancer cell group (98.3±0.9)%, PEF treatment group (66.8±4.4)%, LTP treatment group (62.1±3.9)%, combined group A (43.7±3.7)%, combined group B (31.0±1.4)%, combined group C (46.8±2.9)%, combined group D (39.0±2.3)%. Compared with liver cancer cell group, the cell survival rate of all treatment groups was decreased, and the cell survival rate of the four combined treatment group was lower than that of PEF treatment group and LTP treatment group, the differences were statistically significant (all P<0.05). The survival rate of combined B group was the lowest. The results of apoptosis detection were consistent with those of cell survival rate. Under fluorescence microscope, the ROS fluorescence of cells in the combined group B was significantly increased, and the ROS fluorescence of cells in the LTP treatment group was more than that in the PEF treatment group, and the percentage of ROS positive cells in the combined group B was higher than that in the LTP treatment group and the PEF treatment group, with statistical significance (all P<0.05). Tumor relative volume and tumor inhibition rate in the combined group were better than those in the PEF and LTP groups, and the differences were statistically significant (all P<0.05). Conclusion:LTP combined with PEF has a better killing effect on H22 cells than PEF or LTP treatment, which is expected to be a new tumor therapy.
8.Effects of miR-195 targeting SALL4 on the migration, invasion and epithelial mesenchymal transition of gastric cancer cells

Objective:To investigate the effects of miR-195 on the migration, invasion and epithelial mesenchymal transition (EMT) of human gastric cancer cell line (HGC-27) and its mechanism.Methods:HGC-27 cells were cultured in vitro and they were randomly divided into control group, miR-195 negative control (NC) group and miR-195 mimics (mimics) group; the expressions of miR-195 and SALL4 mRNA were detected by real-time fluorescence quantitative PCR (RT-qPCR) ; The changes in cell morphology of each group were observed under a microscope; the expressions of SALL4, E-cadherin, N-cadherin and Vimentin in HGC-27 cells were detected by Western blot; MTT method was used to detect the change of HGC-27 cell survival rate; the abilities of migration and invasion in HGC-27 cells were detected by scratch test and invasion test (Transwell) ; and the targeting relationship between miR-195 and SALL4 was confirmed by double Luciferase Report.Results:Compared with those in control group and miR-195 NC group, the miR-195 expression level, apoptosis rate and E-cadherin protein expression level of HGC-27 cells in miR-195 mice group were significantly higher ( P<0.05) , the cell survival rate, scratch healing rate, invasion number, SALL4 mRNA and SALL4, N-cadherin, Vimentin protein expression levels were significantly lower ( P<0.05) , the morphology of HGC-27 cells returned to normal, and the epithelial mesenchymal behavior was significantly reduced; in addition, there was a binding site between miR-195 and SALL4 mRNA 3'UTR region, and compared with that in SALL4-3'UTR-WT + miR-195 NC group, the luciferase activity of SALL4-3'UTR-WT + miR-195 MICs group was lower ( P<0.05) . Conclusion:MiR-195 may inhibit the migration, invasion and epithelieal-mesenchymal transition of human gastric cancer cells by targetingly inhibiting the expression of SALL4.

Objective:
To investigate the clinical application value of symmetrical three-hole laparoscopic sleeve gastrectomy (LSG).
Methods:
The retrospective and descriptive study was conducted. The clinical data of 232 patients with obesity who were admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from April 2017 to December 2018 were collected. There were 40 males and 192 females, aged (32±8)years, with a range of 17-63 years. All the 232 patients underwent symmetrical three-hole LSG.
