Objective To study the imaging features and pathological characteristics of MCRCC in order to improve its diagnostic accuracy. Methods The multi-phase SCT and contrast-enhanced ultrasound were performed in 18 cases of MCRCC proved by the pathology before their operation.The imaging features of MCRCC in 18 patients were reviewed. Results All tumors appeared as well-defined round or oval multilocular cystic masses composed of serous fluid with regular thin cystic wall and an irregular renal shape. There were thin coarse septum(6 cases), thick septum(8 cases) and septum with nodule(4 cases) of all the tumor. Dynamic SCT presented enhancement from slight to moderate degree in thin septum cases, obviously homogeneous enhancement in septum with nodule cases and two different previously enhancement in thick septum cases, so did CEUS. The degree of enhancement correlated well positively with the dynamic SCT and CEUS in 18 cases of MCRCC(P <0.05). Conclusion The typical imaging features of MCRCC are multilocular cystic masses composed of serous fluid with regular thin cystic wall, thin coarse septum, thick septum and septum with nodule. There are enhancement from slight to moderate degree in septum cases by SCT and CEUS. The imaging features of SCT and CEUS are integrated in MCRCC, which may improve the accurate diagnosis of MCRCC and provide strong guidance for the clinical treatment.
3.Atlantoaxial pedicle screw system for treatment of unstable atlantoaxial dislocation post traction

Objective To explore the technique and clinical outcome of the atlantoaxial pedicle screw system in the treatment of the unstable atlantoaxial dislocation post traction.Methods The study involved 32 patients with atlantoaxial dislocation(type T2 of TOI classification)admitted from March 2005 to September 2009.There were 17 patients with fresh odontoid fracture and two with old odontoid fracture,five with traumatic disruption of the transverse atlantal ligament and eight with congenital odontoid dysplasia.JOA scores of neurological function before operation was at a range of 5-13(average 8.38).The average of space available for the cord(SAC)was 9.15 mm.Before the atlantoaxial pedicle screw system was carried out,the skull traction was performed in all the patients preoperatively.Results A total of 128 pedicle screws were inserted safely,with mean operation time and perioperative blood loss for 1.5 hours and 300 ml,respectively.No injury to the vertebral artery or spinal cord was observed.All the patients were followed up for 12-24 months,which showed that JOA scores one year after operation was increased to 10-17(average 14.56),with the improvement rate of 71.70%,and that the SAC was average 14.86 mm.The X-ray and SCT scans verified the proper position of the screws,with no internal fixation failure or atlantoaxial redislocation.After 3-6 months,all the patients except for one patient achieved a solid bone fusion.One year after operation,the one patient with no bone graft fusion was removed of the internal fixation system and obtained satisfactory restoration of the rotational function.Conclusions Atlantoaxial pedicle screw system is an effective method for the treatment of the unstable atlantoaxial dislocation post traction,for it has the advantages of stable three-dimension fixation,direct screw placement,intraoperative reduction and high fusion rate.

5.The expression of seven cytokeratins in skin epithelial tumors

Objective To observe the expression of 7 cytokeratins in skin epithelial tumors and to assess their diagnostic value. Methods The expression of 7 cytokeratins, including cytokeratin 7(K72.2), cytokeratin 8(C-51), cytokeratin 10(DE-K10), cytokeratin 14(LL002), cytokeratin 17(E3), cytokeratin 18(DC10)and cytokeratin 19(KS19.1), was assessed by immunohistochemical staining (S-P method) in 54 cases of different skin epithelial tumors and 20 normal skin specimens. Results Of 54 cases studied, 10 were squmous cell carcinoma (SCC), 10 basal cell carcinoma (BCC), 19 hair follicle tumor, 2 sebaceous carcinoma, and 13 sweat gland tumor. The expression patterns and distribution of the 7 cytokeratins were different in different skin epithelial tumors. Most of the tumor cells stained diffusely for cytokeratins in SCC, BCC and hair follicle tumor; different cytokeratins expressed in different parts of each of the subtypes of sweat gland tumors. Conclusions Analysis of a selected group of cytokeratins may be helpful in the diagnosis and differential diagnosis of SCC, basal cell carcinoma and skin adenexal tumor.

8.Clinicopathological characteristics of 24 gastrointestinal stromal tumor cases with concurrent carcinoma

Objective:To observe the clinicopathological features of gastrointestinal stromal tumor (GIST) cases with concurrent carcinoma. Methods:Patient data of 24 GIST cases with concurrent carcinoma were collected from the 157 GIST cases reported be-tween 2002 and 2012. The clinicopathological features of the GIST cases with concomitant carcinoma were studied. The expression of CD117, CD34, and SMA by the tumors was assayed using the immunohistochemical EliVision method. In particular, the expression of the proliferation marker Ki-67 was studied. Results:GIST cases with concurrent carcinoma accounted for 15.3%of the total GIST cas-es studied. The GIST patients with concurrent carcinoma included 14 males and 10 females. The male-female ratio of these patients was 1.4∶1. The age of the patients ranged from 41 years to 66 years, with a median age of 55 years. Lesions at the inferior segment of the esophagus were found in 7 of the 24 selected GIST cases;lesions at the gastric wall and in the intestines were observed in 15 and 2 cas-es, respectively. The diameter of the GIST cases with concurrent carcinoma ranged between 0.6 and 3.8 cm, with an average of 1.50 ± 0.85 cm. Slight dysplasia was observed in 4 of the 24 cases; no heteromorphism was present in the remaining 20 cases. The mitotic counts of GIST cases with concurrent carcinoma ranged from 0/50 HPF to 5/50 HPF, with an average of (0.79±1.83)/50 HPF. The pro-liferative index of Ki-67 in the GIST cases with concurrent carcinoma ranged between 0 and 7.72, with an average of 2.16 ± 3.26. The concurrent carcinoma cases included 5 cases with esophageal carcinoma, 2 with cardiac carcinoma, 15 with gastric cancer, and 2 with intestinal cancer. In contrast to the GIST cases with concurrent carcinoma, the GIST cases without carcinoma complications included 74 males and 59 females. The male-female ratio was 1.25∶1. The age of the patients without concurrent carcinoma ranged from 43 years to 71 years, with a median age of 54 years. Among the 133 GIST cases without cancer complications, gastric, intestinal, and esophageal lesions were found in 114, 13, and 6 cases, respectively. The diameter of GISTs without cancerous complications ranged from 2.4 cm to 15.5 cm, with an average of 6.11 ± 7.09 cm. Different degrees of dysplasia were seen in 82 of the 133 cases. The mitotic counts in the GIST cases without cancer complications ranged from 0/50 HPF to 53/50 HPF, with an average of (3.81±23.67)/50 HPF. The prolifera-tive index of Ki-67 for these cases ranged from 0 to 39.21 and averaged at 6.22 ± 16.96. The male-female ratio of the GIST cases with cancer complications was higher compared with the GIST cases without. The average diameter of GISTs with complications was small-er compared with that of GISTs without complications. The mitotic counts and the proliferative index of Ki-67 were significantly lower in the GIST cases with cancer complications than in those without (t=1.981, P<0.05 vs. t=1.993 5, P<0.05). Conclusion:Concurrent car-cinomas were found in 15.3% of the total GIST cases. No special clinical symptoms were observed in most GIST cases with cancer complications, as revealed when the carcinomas were examined. The proliferative index of Ki-67 in the GIST cases with concurrent car-cinoma is significantly lower compared with that of the GIST cases without complications.
9.Correlation between tumor angiogenesis and imaging features of multiphasic spiral computed tomography in renal cell carcinoma

Objective To evaluate the correlation between imaging features of spiral computed tomography(SCT) and pathology, MVD, and the expression of VEGF in renal cell carcinoma(RCC). Methods Thirty patients with RCC diagnosed by pathology underwent SCT examinations.MVD and the expression of VEGF were examined immunohistochemically using SABC techniques. Results The detection and characterization rate as well as accuracy of staging before operation about 30 RCC of multiphasic SCT scans were 100 %, 100 % and 83.3 %, respectively. In 30 cases of RCC, the mean MVD was 36±12.4. The positive expression rate of VEGF were 73.33 %. There were no correlation between the MVD and the expression of VEGF in RCC (r =0.057, P >0.05). The features of SCT: on multiphasic SCT scans, the pathological nuclear grade, the MVD and positive expression of VEGF in groups of RCC with diameter larger than 3.0 cm, central necrosis, pseudocapsule and intravenous tumor emboli were obviously higher than those of the contrast group (P <0.05, P <0.05, P <0.05, respectively). The pathological nuclear grade and MVD in groups of RCC with lymph node metastasis were higher than the contrast group (P <0.05, P <0.05, respectively). There were the positive correlation between the enhanced Hounsfield' s units in the CMP and the MVD in 30 cases of RCC(r =0.784, P <0.05). The degree of enhancement correlated well positively with the MVD in 30cases of RCC (P < 0.05). Conclusion Multiphasic enhanced SCT scan is a reliable technique in the detection, pathologic characterization of RCC. Some SCT features are closely correlated with MVD and expressions of VEGF in RCC, which could be a noninvasive method in predicting aggressiveness and metastasis.
10.A survey of fluorine content in drinking water of Yi County in Hebei Province in 2012

Objective To find out fluorine content in drinking water in Yi County,and to provide ascientific basis for prevention of endemic fluorosis and children's dental caries.Methods According to FluorideDistribution Survey Program in Drinking Water of Hebei Province,an administrative village was taken as a unit,one drinking water sample was randomly collected from every village with centralized water supply.For villages withdecentralized water supply,five drinking water samples were randomly collected according to directions (east,west,south,north and centre) if there were more than five water sources in the village;samples from all the water sourceswere collected if there were less than five.Fluorine content was determined with drinking water standard testmethods (GB/T 5750.2-2006).Analysis and comparison of fluoride content in drinking water of different landscapes,water types and different well depth were carried out.Results The range of water fluoride content was 0.00-18.39 mg/L,and water fluorine median was 0.24 mg/L in 1 024 water samples in 27 towns in the county.Watersamples of 490,505 and 29 copies were collected in the hills,plains and mountains,and water fluorine medianwas 0.26,0.22 and 0.21 mg/L,respectively.Concerning water fluorine content in drinking water,mountains werehigher than hills and plains (x2 =71.71,17.74,all P < 0.01).There was no significant difference between waterfluoride content in hills and plains (x2 =2.48,P > 0.05).Water samples of 742,228 and 54 copies were collected,respectively,in household well water,tap water and mountain spring water,and water fluoride median was 0.22,0.24 and 0.33 mg/L,respectively.Concerning water fluoridation content in drinking water,spring water were higherthan tap water and household well water (x2 =53.16,50.85,all P < 0.01).There was no significant differencebetween water fluoridation content in tap water and household well water (x2 =2.97,P > 0.05).Water samples of583 and 441 copies were collected,respectively,in well depth < 40 m and well depth ≥40 m,and water fluoride median was 0.24 and 0.23 mg/L,respectively,and there was no significant difference between water fluoridation content in the well depth < 40 m and the well depth ≥40 m (Hc =0.17,P > 0.05).Conclusions Fluoride content of drinking water in Yi County is not high,endemic fluorosis disease does not occur.
