Objective To evaluate the effect of continuous renal replacement therapy (CRRT) on extracorporeal membrane oxygenation (ECMO)-induced expression of inflammatory factors in pig kidney.Methods Twenty-four adult pigs of either sex,weighing 25-32 kg,were randomly divided into 4 groups (n =6 each) using a? random number table:control group (group C),sham operation group (group S),ECMO group and CRRT group.Anesthesia was induced with ketamine,diazepam and atropine and maintained with ketamine and diazepam.The pigs were tracheotomized,intubated and mechanically ventilated.The left femoral arteries were cannulated for MAP monitoring.Heparin 150 U/kg was injected intravenously.Right femoral artery and left internal jugular vein were cannulated for blood-letting and fluid infusion.In ECMO and CRRT groups,ECMO was performed for 24 h starting from 1 h after cannulation,in addition CRRT was performed for 24 h simultaneously in ECMO group.The pigs were then sacrificed and kidney specimens were obtained for determination of the content of interleukin-1β (IL-1 β),IL-6 and tumor necrosis factor-α (TNF-α) by ELISA.Results There was no significant differ-ence in the content of IL-1β,IL-6 and TNF-α between C and S groups (P ＞ 0.05).Compared with C and S groups,the content of IL-1β,IL-6 and TNF-α was significantly increased in E and CRRT groups (P ＜ 0.01).Compared with group E,the content of IL-1β,IL-6 and TNF-α was significantly decreased in group CRRT (P ＜0.01).Conclusion CRRT can decrease ECMO-induced expression of inflammatory factors in pig kidney to some extent,indicating that it can alleviate the inflammatory responses in kidney.

Objective To analyze imaging of gastrointestinal stromal tumors(GIST),and to compare their imaging features with operational and pathological findings.Methods Clinical,imaging,and pathological data of 20 patients with GIST were collected.Results Imaging findings were endophytic or exophytic tumors with heterogeneous density or signal intensity,corresponding to hemorrhage,necrosis,and cystic changes.Imaging was correct for the location of the lesion in 11 of 16 primary GIST and 4 cases of relapsed tumors.Preoperational CT did not detect mesenteric,peritoneal,and omental metastasis in 5 cases. Hepatic metastases detected at CT (3 cases )were identified by operational findings.Conclusions GIST has some imaging features.CT is a useful tool in detecting and characterizating of lesions rather than detecting mesenteric,peritoneal,and omental metastasis.

Objective To evaluate the predictive effect of preoperative pressure-flow studies on the outcome of transurethral prostatectomy.Methods Sixty-three patients with benign prostatic hyperplasia(BPH) were evaluated by using pressure-flow studies,international prostate symptom score,uroflowmetry,and residual urine volume before and 3 months after surgery.The patients were divided into 3 groups(obstruction equivocal and unobstruction) according to the grade of pressure-flow.The outcome of transurethral prostatectomy were analyzed in each group with detrusor contractility(weak or normal).Results Treatment outcome could be predicted by using the grade of benign prostatic obstruction and detrusor contractility.The sensitivity of which was 73.5%,with a specificity of 88.5% and positive predictive value of 96.2%.Conclusions Urodynamic studies provide great predictive value of option in transurethral prostatectomy.

Coronary heart disease is a kind of heart disease that is caused by atherosclerosis. The lipid deposition in the vessel wall results in occlusion of coronary artery and stenosis, which could induce myocardial ischemia and oxygen deficiency. Intervention therapies like percutaneous coronary intervention (PCI) and coronary stent improve myocardial perfusion using catheter angioplasty to reduce stenosis and occlusion of coronary artery lumen. Accordingly, intervention therapies are widely applied in clinic to treat ischemic cardiovascular disease, arterial intima hyperplasia and other heart diseases, which could save the patients' life rapidly and effectively. However, these interventions also damage the original endothelium, promote acute and subacute thrombosis and intimal hyperplasia, and thus induce in-stent restenosis (ISR) eventually. Studies indicated that the rapid reendothelialization of damaged section determined postoperative effects. In this review, reendothelialization of implants after intervention therapy is discussed, including the resource of cells contributed on injured artery, the influences of implanted stents on hemodynamic, and the effects of damaged degree on reendothelialization.

Objective To analyze the drug resistance and homology of Acinetobacter baumannii (Ab) isolated from patients with explosive injury ,so as to explore the characteristics of drug resistance and prevalence of infection .Methods A total of 61 strains of AB isolated from clinical specimens of patients with explosive injury were collected .The antimicrobial susceptibility of these iso‐lates was detected by using K‐B test .All the strains were gene typed by using the pulsed field gel electrophoresis .Results The re‐sults of antimicrobial susceptibility test shown that the 61 isolates of Ab had high resistance rate ,and were multi‐drug resistant to common antibacterial agents ,except for tigecycline (the resistante rate was 11 .5% ) and minocycline (the resistante rate was 48 .0% ) .The 61 isolates of Ab were divided into 8 kinds of genotypes ,among which type A was the most prevalent one (25 strains) .Other genotypes were type B(10 strains) ,type C(6 strains) ,type D(4 strains) ,type E(8 strains) ,type F(3 strains) ,type G(4 strains) and type H(1 strain) .The isolates of Ab were with high homology .Conclusion Multi‐drug resistance is observed in strains of Ab isolates from patients with explosive injury .Clonal strains of AB may be disseminates among regions ,which indicates that high attention should be paid to these strains .

Objective To discuss the influence of Saccharomyces boulardii on serum apolipoprotein levels in liver cirrhosis patients with spontaneous bacterial peritonitis (SBP).Methods 74 liver cirrhosis patients with SBP were selected and randomly divided into the observation group and control group.Patients in the two groups were given routine medical treatment such as liver protection and diuretic effect,correction of hypoproteinemia,anti-infection etc.Patients in the observation group were orally given Saccharomyces boulardii 0.5g for one time,two times daily for 4 weeks.The changes of serum apolipoprotein A1 and B levels in two groups were observed and compared,and the curative effect and safety were also observed.Results After treatment for 4 weeks,the serum apolipoprotein A1 and B levelsintwogroupswereobviouslyincreasedcomparedwithbeforetreatment (t =2.46,2.3 7,3.04,2.87,P ＜ 0.05or P ＜0.01),and the increase of observation group was much higher than that of control group(t =2.29,2.23,all P ＜ 0.05).Meanwhile,the total effective rate of the observation group (91.89％) was significantly higher than that of control group (72.97 ％) (x2 =4.57,P ＜ 0.05).3 cases and 5 cases of adverse reactions appeared in control group and observation group,the symptom was light and no serious drug adverse reaction(DAR) observed.The incidence rate of DAR between the two groups had no obviously statistical difference (x2 =0.14,P ＞ 0.05).Conclusion Saccharomyces boulardii has reliable curative effect and high security in the treatment of liver cirrhosis patients with SBP,the mechanism may be related with increasing the synthesis of apolipoprotein A1 and B in the liver and intestine,and increasing serum levels of apolipoprotein A1 and B.

Objective To understand the occurrence of healthcare-associated infection(HAI) in intensive care units (ICUs) in secondary and above hospitals in a province, and provide basis for HAI management.Methods HAI management quality control center in a province performed targeted monitoring on HAI in ICUs in secondary and above hospitals in a province from July 1, 2010 to June 30, 2015.Results ICUs in 176 hospitals were monitored, the incidence of HAI in ICUs for 6 consecutive years was 7.23％, case incidence of HAI was 9.72％;incidence of HAI showed a decreased trend in 6 years(P<0.001).The adjusted rate of HAI per 1 000 patient days in the general hospital ICUs was higher than that of special hospital ICUs (4.30‰ vs 3.53‰).Incidence of HAI in surgical ICUs and general ICUs were both relatively higher, adjusted rates of HAI per 1 000 patient days were 4.79‰ and 4.21‰ respectively.Respiratory tract, urinary tract, and bloodstream infection were major HAI in ICUs, accounting for 68.64％, 14.45％, and 10.09％ respectively, the utilization rates of central venous catheter, ventilator, and urinary catheter were 49.86％, 39.16％, and 81.95％ respectively, incidence of three catheter-associated infection were 1.74‰, 13.77‰, and 2.08‰ respectively, incidence of three catheter-associated infection decreased year by year (P<0.001).The utilization rates of ventilator, central venous catheter, and urinary catheter in different ICUs were not correlated with the incidence of infection(all P>0.05).A total of 36 223 strains of pathogens were isolated, the top 6 isolated pathogens were Acinetobacter baumannii(22.77％), Klebsiella pneumoniae (11.96％), Pseudomonas aeruginosa(11.94％), Staphylococcus aureus (8.08％), Candida albicans (5.63％), and Escherichia coli(5.55％).The isolation rates of carbapenem-resistant Klebsiella pneumoniae showed a increased tendency for 6 years(P<0.05).Conclusion Continuous targeted monitoring on HAI in ICUs, regular analysis on risk factors for HAI, and timely implementation of intervention measures can effectively decrease the incidence of HAI.
