1.Analysis of factors influencing the quality of life of elderly patients with esophageal cancer

Objective To study the effects of different factors influencing the quality of life of elderly esophageal cancer patients who returned to the community after surgery.Methods A total of 216 elderly esophageal cancer patients with complete clinical data who had returned to the community after surgery were followed up by questionnaires,including the European Organization for Research and Treatment of Cancer Quality of Life rating (EORTC QLQ-C30) and esophageal cancer supplementary (QLQ-OES18) scales.Results The overall quality of life score of the patients was (35.2±22.1),which was lower than that in the Norwegian norm data.Scores for cognitive function,constipation,diarrhea,shortness of breath and loss of appetite were similar to those in the norm data.Scores for physical function,emotional function,role function and were lower than those in the norm data,while scores for pain,fatigue,nausea and vomiting,sleeplessness and were higher than those in the norm data.The overall quality of life score was higher in male patients than in female patients (F =5.12,P=0.029),in patients with spouses than in those without spouses (F=5.61,P=0.016),and in patients without complications than in those with complications (F=5.48,P=0.002).The overall quality of life was higher with longer survival times after surgery (F=3.68,P=0.003).No significant difference in the overall quality of life score was found between different age-groups (F=4.23,P=0.212).Conclusions The overall postoperative life quality is poor in community elderly patients with esophageal carcinoma.Gender,marital status,postoperative survival time,and complications are among the factors influencing postoperative quality of life of community elderly patients with esophageal carcinoma.
2.Evaluation of the body fluid mode on Sysmex XN-2000 for detecting of tumor cells in ascites and pleural effusion

Objective To evaluate the capability for detecting of tumor cells in ascites and pleural effusion by Sysmex XN‐2000 hematology analyzer .Methods Determination of 84 samples of ascites and pleural effusion specimens with Sysmex XN‐2000 hema‐tology analyzer humoral model to analyze the study parameters highly fluorescent cells absolute value (HF‐BF # ) and percentage (HF‐BF% ) of ascites and pleural effusions screening of tumor cells ,and related data for statistical analysis .Results In 84 cases of pleural effusion and ascites samples ,cytology found 14 cases of malignant cells ,cancer cells are not found in 70 cases .14 cases of malignant cases ,HF‐BF # average of 169 /μL(0-2 001) ,HF‐BF% with an average of 29 .7% (0% -261% );70 cases of benign cases ,HF‐BF # an average of 39 /μL(0-524) ,HF‐BF% with an average of 4 .2% (0% -27 .8% ) ,both groups were statistical differences in the mean significance(P<0 .05) .ROC curve analysis with high absolute value and percentage of fluorescent cells to tumor cells detection function ,HF‐BF # area under the curve(AUC) was 0 .493 ,the area(AUC)% HF‐BF under the curve was 0 .222 .Conclusion Sysmex XN‐2000 Pattern of humoral parameters of HF‐BF # and HF‐BF% ,although the tumor cells may pro‐vide some screening information ,but ineffective ,so we believe that in our daily work can′t be over‐reliance on the instrument HF‐BF# ,HF‐BF% and other parameters should be performed on every sample smear staining ,thus improving the detection rate of ma‐lignant cells .
3.Costimulatory blockade with anti-inducible costimulator antibody in combination with CTLA4Ig on prevention of islet allograft rejection

Objective To study the effects of costimulatory blockade with anti-inducible costim- ulator antibody(ICOS mAb)in combination with CTLA4Ig on prevention of islet allograft rejection. Methods Experimental animals were randomly divided into 4 groups(10 rats in each group).CT- LA4Ig + ICOS mAb group(group A):intraperitoneal injection of CTLA4Ig on day 0,2,4 and ICOS mAb on day 1,3,5 after islet transplantation;ICOSmAb group(group B):intraperitoneal injection of ICOS mAb on day 1,3,5 after islet transplantation;CTLA4Ig group(group C):intraperitoneal injection of CTLA4Ig on day 0,2,4 after islet transplantation;control group(group D):simple islet transplantation.The islet allograft survival and pathological changes in the transplanted islets after transplantation were observed.By using RT-PCR,the expression of IL-2 and IL-10 mRNA in the transplanted islets was detected.The expression of CD4~+ and CD8~+ T cell was detected by flow cy- tometry.Results In group A,the survival time was obviously prolonged as compared with other three groups and the transplanted islets were near normal under a light microscope.As compared with other three groups,the expression of IL-2 mRNA was significantly decreased in group A(P0.05).The expression of CD4~+ and CD8~+ T cell was not obviously up-regulated on the day 21 after transplantation.Conclusion The blockade of costimulatory signals with ICOS mAb in combination with CTLA4Ig has a favorable effects to restrain the rejection of islet transplantation.
4.Study on radiation dose caused by 18F-FDG in PET/CT examination

Objective The aim of this study was to investigate the radiation dose caused by 18 F-fluorodeoxyglucose (FDG) in PET/CT examination and to optimize the concerned radiation protection. Methods Thirty patients from our conventional PET/CT examination were simple randomly selected, and they all underwent whole body PET/CT imaging. The radioactive dose of injected 18F-FDG was recorded. The internal radiation dose was calculated and the external radiation dose from patients was measured with the 451P-DE-SI ion chamber survey meter. The staff's dose was recorded with thermoluminescent detector (TLD). All dosimetry data were processed and analyzed statistically with Excel 2003. Results The injected radioactive dose of 18F-FDG was (432.9±51.8) MBq, and effective dose equivalent received per patient was (8.23±0.99) roSy. The correlation coefficient (r) of the dose equivalent rate and distance was-0.994 by power function curve fitting, and that of dose equivalent rate and time was - 0.988 by exponential curve fitting. The staff's dose was lower than the annual dose limit. Conclusions The patient's internal radiation dose caused by 18F-FDG in PET/CT examination is low, nonetheless, the clinician should always consider optimizing and minimizing the necessary radiation received by the patients. The patients having been injected with 18F-FDG should stay in one place to decrease their radiation to the public. From the medical point of view in optimizing radiation exposure, there may still be a potential to lower the injected 18F-FDG activity.
5.Assessment of left ventricular function of pulmonary hypertension patients by real-time three dimensional echocardiography

Objective To assess the changes of left ventricule and analyze the influential factors in pulmonary hypertension (PH) by comparing the changes of left ventricular volume and function using single cardiac cycle real-time three dimensional echocardiography ( sRT-3DE ) and two-dimensional echocardiography (2DE).Methods In case group,there were 30 inpatients diagnosed as PH,systolic pulmonary artery pressure(PASP)≥40 mmHg estimated by echocardiography,excluding left heart disease or congenital systemic-pulmonary circulation shunt.There were 30 healthy people as control group.Conventional 2DE and sRT-3DE were performed in both groups and obtained 2D and 3D parameters.Results ①In case group,the 2D-LVEF were significantly larger than 3D-LVEF (P < 0.01 ).There was less agreement of 2DE and 3DE in case group than in control.②Comparing with the control group,left ventricular EDV,ESV and SV in ease group decreased ( P <0.01 ),so as to 3D-LVEF ( P <0.05).The left ventricular spherical index(SIs,SId)and eccentric index (EId,EIs ) in case group were significantly larger than those in control group ( P <0.01 ).③LVEDV in case group was negatively correlated with PASP (r =- 0.47),and positively correlated with RVSV and RVEF ( r =0.84,0.66) ;3D-LVEF in case group was negatively correlated with PASP ( r =- 0.54),and positively correlated with RVSV and RVEF ( r =0.58,0.53); there was no significant correlation between LVEDV and RVEDV.LVEDV was negatively correlated with SIs and SId ( r =-0.65,-0.61),so as to EId and EIs ( r =-0.67,0.67).LVEF was negatively correlated with SIs and SId ( r =-0.64,-0.61),so as to EId and EIs ( r =-0.66,-0.68),and the corresponding P all above were less than 0.01.Conclusions PH can result in reductions of left ventricular volume and systolic function,and the variations of which have the correlations with the severity of PH and the increase in SI and EI.sRT-3DE can provide a method to get more accurate information of left ventricle.
6.Synthesis and Anti-HIV-1 Activity of One Schiff Base Derivative with Glucosamine of Gossypol

Objective: To confirm the structure and preferential conformation of the Schiff base of gossypol with 1, 3, 4, 6-tetra-O-acetyl-β-D-glucosamine and explore its anti-HIV-1 activity.Methods: The Schiff base of gossypol with 11, 3, 4, 6-tetra-O-acetyl-β-D-glucosamine was synthesized and identified by FT-IR, NMR spectroscopy and the PM6 semi-classical calculation.The inhibitory activity of the novel compound against the laboratory-adapted HIV-1IIIB strain was examined using the HIV-1IIIB/TZM-bl indicator cell culture system.Results: The 1H and 13C-NMR signals of the new Schiff base were assigned.The PM6 semi-classical calculation indicated that enamine-enamine tautomeric form of the new Schiff base was more stable,which was stabilized by the intramolecular hydrogen bonds.The anti-HIV-1 test showed that the compound could block the entry of HIV-1IIIB into the target cells.Conclusion: The Schiff base of gossypol with 1, 3, 4, 6-tetra-O-acetyl-β-D-glucosamine exhibits enamine-enamine tautomeric form in solution, which shows potential anti-HIV-1 activity.

8.The effects of rehabilitation training on cardiac function in cerebral infarct patients

Objective To study the effects of rehabilitation(RT)on cardiac function in cerebral infarct (CIF)patients with cardiac insufficiency(CIS).Methods Fifty-nine CIF patients with CIS were randomly divid- ed into a treatment group(T group,n=29)and a control group(n=30),and all patients were treated with routine pharmacotherapy for 2 months.In addition,RT was administrated in the T group at the same time.The grading of the New York Heart Association(NYHA)and the changes in cardiac function associated index were observed in both groups before and after treatment.Results Compared with the control group,NYHA grades,left ventricle ejection fraction(LVEF),the levels of brain natriuretic peptide(BNP)in the blood plasma,and the 6min walking range of the T group patients were all significantly improved after treatment(P<0.05).Conclusion RT can improve car- diac function in CIF patients with CIS.
9.The optimal dose reduction level in chest CT with CT volume scan mode using iterative reconstruction in piglet model

Objective To determine the feasibility and the optimal dose reduction of AIDR 3D on piglet chest CT which can provide image quality (IQ) comparable to filtered back projection (FBP).Methods Twenty-nine normal pigs with the weight of 3-12kg underwent 640-slice MDCT chest CT (Aquilion one,Toshiba) for 5 times with 80 kvp and various mAs.SureExposure3D technique were used and the index of noise were set to SD 10.0,12.5,15.0,17.5,20.0(Group A,B,C,D,E) to reduce dose successively.Group A were reconstructed with FBP,Group B,C,D,E were reconstructed using AIDR 3D (strong level).Quantitative image noise and signal to noise ratio(SNR) were measured in each group.Two radiologists graded subject image quality on both lung images (artifacts,central airway,lung tissue) and mediastinal images (mediastinal structure) and overall image quality using a 5-point scale in a blinded manner.Analysis of variance (ANOVA) and LSD test were used for comparisons of objective evaluation indices (CT value,noise,SNR)and radiation dose(CTDIvo1,DLP,ED) among the five groups.The Friedman test and Wilcoxon test were used for comparisons of demographic data and for detection of differences in subjective evaluation of IQ among groups.A receiver-operating characteristic (ROC) analysis was performed to establish a radiation reduction threshold up to which comparable IQ(score≥4) was maintained.Results Compared with Group A[noise(15.25 ± 3.14)HU,SNR 3.23 ± 1.07],Group B,C had significant lower noise and higher SNR [noise (12.11 ± 2.75),(13.18 ± 3.16)HU; SNR 4.13 ± 1.38,3.80 ± 1.20;F =7.38,3.11,P <0.05].Group D,E showed no significantly different noise and SNR with Group A[noise (15.14±4.51),(15.79 ±4.17)HU;SNR 3.40± 1.56,3.45 ± 1.70;P>0.05].Group B,C had significant better subject image quality compared with Group A (P <0.01).Group D had no significantly different subject image quality compared with Group A (P > 0.05).Group E had 5 pigs with overall image scores of < 3.The ROC curve of IQ established SD 17.5 (Group D) as optimal cut-off point (AUC 0.75,95% CI 0.58-0.92).Group D provided equivalent subjective image score and objective IQ measurements compared with FBP images in Group A.The ED of group D was 59% lower than that of group A[CTDIvol (1.14±0.27) mGy vs (0.47 ±0.16)mGy,F=183.83,P<0.01].Conclusion Using AIDR 3D technique,80 kvp with SureExposure3D (SD 17.5) can provide comparable IQ compared with routine dose with FBP reconstruction,and reduce 59% dose in piglet model.
10.Visuospatial-visuoconstructional ability in mild cognitive impairment

Objective To investigate visuospatial-visuoconstructional ability in patients with mild cognitive impairment(MCI).Methods The subjects were divided into three groups:122(51 males and 71 females)in the normal control group; 205(95 males and 110 females)in the MCI group,including 133 amnestic MCI(aMCI)and 72 non-amnestic MCI(naMCI),and 75(36 males and 39 females)in the Alzheimer' s disease(AD)group.The subjects were assessed for visuospatial-visuoconstructional ability by three tests:Rey-Osterrieth Complement Fixation Test(CFT),Clock Drawing Task(CDT),and Stick Test.Mini-mental State Examination(MMSE)was also performed in all groups.SPSS 11.5 statistical software was used for statistic analysis; Chi-square test was used to analyze enumeration data; One-way analysis of variance was used in pairwise comparison between the groups,and Bonferroni and LSD methods were used for multiple comparison.Results(1)The average MMSE score in the normal control group,MCI group and AD group was 28.24 ± 1.74,27.39 ± 1.83 and 19.98 ±3.23,respectively.(2)In the normal control group,either CFT imitating score or total CDT score had no obvious correlation with age or education,while Stick Test-Rotating part(STR)was correlated with age(r =-0.179,P < 0.05),but had no significant correlation with education.(3)There were correlations between CFT imitating score and total CDT score (r =0.337),CFT imitating score and STR(r =0.232),and total CDT score and STR(r =0.235).The correlations were statistically significant(P < 0.01).(4)CFT imitating and total CDT showed the greatest correlation with Trail Making Test B and Stroop Color Word Test Card C time consumption,an indicator of executive function.STR had greatest correlation with Auditory Verbal Learning Test,an index of memory.There were significant differences in CFT imitating,total CDT score and STR among the normal,MCI and AD groups(P < 0.01).(5)STR is a more informative test to be used in diagnosis of MCI than CFT imitating and total CDT score,as shown by comparison of these tests in three subjects groups.Conclusion Visuospatial-visuoconstructional ability impairment is one of the symptoms of MCI.In detection of MCI,the assessment of space rotating ability is better than CFT imitating and total CDT score.
