1.Progress in perioperative liquid therapy

Liquid therapy is a vital method of perioperative treatment.Choosing correct kinds of transfusion and reasonable therapy strategies can significantly reduce the perioperative complications and shorten days in hospital and improve the critical patients prognosis.The historical development of perioperative liquid therapy is a forward process,which is about crystalloids vs.cilloid,dry vs.wet,and other controversial problems continuing to be summarized,explored and discussed.In this process,there are some specific perioperative treatment strategies with the physician having a deeper cognitive,such as goal-directed therapy,early goal directed therapy and fast track surgery which have significant clinical curative effect.This article summarizes the advance of perioperative liquid therapy.
2.Research Progress of Cerebral Structure and Functional Network Change in Patients with Mild Cognitive Im-pairment (review)

This article reviewed the application of different kinds of imaging methods combined with the analysis of brain connectome in mild cognitive impairment. Previous studies demonstrated that the brain worked as the small-world network. In patients with Alzheimer's disease or mild cognitive impairment, the structure of small-world of network might be changed. According to the different methods, the trend of those changes might be different. This paper reviewed the current situation of the study in mild cognitive impairment through brain network analysis, and emphasized its importance and prospects.
3.Importance of Doctor-patient Communication Courses in Clinical Medicine

By analyzing the present condition of communication courses in some medical colleges and universi -ties and the clinical practice situation , this paper emphasized the significance of the clinical professional courses :it is beneficial to promote the reform of medical education , is advantageous to the medical students′humanistic spirit , to improve clinical teachers′humanistic quality , is helpful to reduce medical disputes .And on the future develop-ment of clinical communication course put forward the following suggestions: extended content , the integration of related courses , reform teaching methods , intensify communication course value .
4.Safety Management of Hemodialysis Equipment

Objective To discuss the safety management of hemodialysis equipment for safe and effective treatment to patients.Methods The management methods were summarized in aspects of the installation,utilization and maintenance of hemodialysis equipment.Results The hemodialysis patients received the safe and effective treatment through the strict management and the standardized system.Conclusion The standardized management can not only reduce the unnecessary failures of the equipment,and prolong the service life,but also increase the utilization rate of the equipment,and provide the best,safest and most satisfactory treatment for patients.
5.Lymphocyte cells subtypes in hypertrophic adenoid in children with otitis media with effusion

Objective To study the frequency of lymphocyte cells in hypertrophic adenoid in children with otitis media with effusion. Methods The study group consisted of 45 children suffering from hypertrophic adenoid coexisting with otitis media with effusion (HA/OME). The control group consisted of 34 children with adenoid hypertrophy(HA),but without otitis media. Adenoids excision was implemented. Cells were stained with fluorescently labeled protein-specific antibody. The lymphocyte of adenoids was determined by flow cytometry. Results Lymphocyte cells(CD3+CD4+CD69+T cells(1.29%),CD3+CD4+HLA-DR+T cells(0.61%),CD3+CD8+CD69+ T cells (1.39%),CD3+CD8+HLA-DR+ T cells (0.09%),CD19+CD69+ B cells (2.07%) and CD19+CD62L+B cells(0.21%)in HA/OME group were higher than in HA group:(CD3+CD4+CD69+T cells(1.94%), CD3+CD4+HLA-DR+T cells(0.84%),CD3+CD8+CD69+T cells(2.08%),CD3+CD8+HLA-DR+T cells(0.18%), CD19+CD69+ B cells (3.98%),CD19+CD62L+ B cells (0.46%). In addition ,there are the higher percentage lymphocyte cells(CD3+CD4+ T cells(8.97%),CD3+CD8+ T cells(4.27%),CD3+CD8+ Fas+ T cells(1.24%) and CD19+Fas+ B cells (3.23%) in HA/OME group than those in HA group:(CD3+CD4+ T cells (6.37%), CD3+CD4+CD69+ T cells (1.29%),CD3+CD4+HLA-DR+ T cells (0.61%) and CD3+CD8+ T cells(3.61%). Conclusions The local immune dysfunction resulting from hypertrophic adenoids may have a relationship with otitis media.
6.Comparison of Therapeutic Efficacy and Safety of Dexamethasone with Different Administration Routes in the Treatment of Chronic Secretory Otitis Media

OBJECTIVE:To compare therapeutic efficacy and safety of dexamethasone with different administration routes in the treatment of chronic secretory otitis media. METHODS:A total of 150 patients with chronic secretory otitis media were random-ly divided into control group(75 cases)and observation group(75 cases). Control group was given Dexamethasone sodium phos-phate injection 5 mg by auripuncture under endoscope. Observation group was given Dexamethasone sodium phosphate injection 5 mg via eustachian tube under endoscope. Both groups were given relevant medicine every 2 days,for 3 times in total. Clinical effi-cacies of 2 groups were observed as well as bone conduction threshold under 1,2,4,8 kHz,the levels of water channel protein 1 and water channel protein 4,recurrence,the occurrence of tympanic cavity effusion before and after treatment. The occurrence of ADR was recorded. RESULTS:The total response rate of observation group(93.33%)was significantly higher than that of control group(80.00%);recurrence rate and the incidence of tympanic cavity effusion of observation group were significantly lower than those of control group,with statistical significance(P<0.05). Before treatment,there was no statistical significance in bone con-duction threshold,the levels of water channel protein 1 and water channel protein 4 between 2 groups(P>0.05). After treatment, the levels of bone conduction threshold in 2 groups were significantly lower than before;the observation group was significantly lower than the control group. The levels of water channel protein 1 and water channel protein 4 in 2 groups were significantly high-er than before;the observation group was significantly higher than control group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:The administration of dexa-methasone via eustachian tube under endoscope is significantly better than auripuncture in terms of therapeutic efficacy for chronic secretory otitis media,relieving clinical symptom,improving bone conduction threshold,up-regulating water channel protein level, reducing the risk of recurrence and tympanic cavity effusion. The two routes of adimistration have similar safety.
7.Fuzzy Grouping Analysis on Herbs for Promoting Blood Circulation to Remove Blood Stasis

OBJECTIVE:To classify the herbs for promoting blood circulation to remove blood stasis.METHODS:Database of 33 commonly used herbs for promoting blood circulation to remove blood stasis was established by means of SPSS10.0 software package;and the nature,tastes,meridian attributions and actions of the herbs were collected as variables showing the characteristics of classification,and the names of the herbs served as variables for measurement to proceed Q Cluster in Hierarchical Cluster.RESULTS:When the herbs were classified into 4 groups,the first group included the herbs for promoting blood circulation to restore menstrual flow,the second the herbs for promoting blood circulation to arrest pain and dissipate tumor,the third the herbs of herbs for activating blood circulation to dissipate blood stasis and kill pain,the fourth the herbs for severely activating blood circulation to dissipate blood stasis.When the herbs were classified into 5 groups,the fifth group included herbs for activating blood circulation to cure wound.When the herbs were classified into 6 groups,the third one included the herbs for activating blood circulation to exclude wind and the sixth the herbs for activating blood circulation to stanch blood.CONCLUSION:Fuzzy grouping analysis can be applied to classify herbs of different kinds yet with the same efficacy and to classify the same group of herbs according to their drug actions.
8.Acupuncture and Moxibustion in Taiwan

In the past, education of acupuncture/ moxibustion in Taiwan depended on the masters-disciples relation. The knowledge of acupuncture/ moxibustion was transmitted to disciples individually.
Since its establishment in 1955, The Taipei Society Acupuncture and Moxibustion improved educational pro- gram to more systematic one. In the following 20 years varieties of studies in this field were conducted, people also turned their eyes to these traditional remedies
Though acupuncture and moxibustion had begun to be accepted by people, the new medical law was en- forced in 1975, which disqualified the acupuncturists for medcial practice. As a result, many of the acupuncturists and moxibustionists left Taiwan for overseas. Unlike many other nations, Taiwan has no system for certification of acupuncturists/ moxibustionists. The traditional Chinese doctor in Taiwan, after passing the examination for Chinese Medicinal exam can practice the Chinese herbal medicine and the acupuncture/ moxibustion. It seems an inconceivable system.
It is only after 1989 that the subjects on acupuncture/ moxibustion medicine were included in the exams for the traditional Chinese doctor in Taiwan. But it still doesn't include tests of practical skills. This is more of a hindrance than a contribution to acupuncture and moxibustion in Taiwan.
Though there has been the legalization movement for acupuncturists/ moxibustionists during the past 30 years, it met opposition from both of the health bureau and the traditional Chinese doctor trade union. The license system for acupunturist is still left uncertain.
Now in Japan, the high tech device like electronic microscope, micro inductor and micro-electric are introduced in the studies of acupuncture/ moxibustion medicine. And in the field of scientific research of the acupuncture and moxibustion, Japanese seem to have more grant view than other country. The health government of Taiwan should turn their eyes to the reality and change its policies towards this effective medicine.
Now acupuncture and moxibustion medicine has become one of the international research topics. In recent years WFAS academic congresses are held every year in the different nations. Here I would like to suggest that in advanced country like Japan, a new university specializing in acupuncture/moxibustion medicine should be established and it should accept and educate foreign student from overseas with international curriculum. Only the continuous and faithful exchange of the young scholars would recover the true friendship between Taiwan and Japan.
10.The influence factors of cognitive impairment in epilepsy children with less than six years old

Objective To investigate the influence factors of cognitive impairment in epilepsy children with less than 6 years old. Methods Eighty-nine epilepsy children with less than 6 years old were selected. The neuropsychological detection was tested by Gesell developmental diagnostic scale (GDDS), and head imaging examination (CT or MRI) and ambulatory electroencephalogram (EGG) were checked. Results The scores of adaptive behavior, big sports behavior, fine motor behavior, language behavior and personal- social behavior were (82.98 ± 14.02), (85.86 ± 13.79), (83.14 ± 13.44), (84.75 ± 3.29) and (84.99 ± 14.37) scores, the average development quotient of the children with epilepsy were greater than 75 scores, and there were no statistical differences among them (P > 0.05). According to the test results of adaptive behavior, big sports behavior, fine motor behavior, language behavior and personal-social behavior, the rates of developmental delay children were 21.35% (19/89), 19.10% (17/89), 16.85% (15/89), 24.72% (22/89) and 22.47% (20/89), and the rates of moderate to severe developmental delay were 3.37% (3/89), 3.37% (3/89), 4.49% (4/89), 3.37% (3/89) and 3.37%(3/89). Multivariate analysis results showed that first onset age of epilepsy and head imaging performance were correlated with adaptive behavior, big sports behavior, fine motor behavior and personal-social behavior (P < 0.05). Conclusions The first onset age of epilepsy and head imaging performances are the major factors in cognitive performance in epilepsy children with less than 6 years old, so clinical attention should be paid to improve the prognosis of cognitive outcome of epilepsy children, and improve their life quality.
