1.An investigation on the effect of the post-operative self-nursing training of the affected arms in patients with breast cancer

BACKGROUND:The tissue structure of the affected arms of the breast cancer patients is seriously damaged after operation, which often causes the disorder of functional activity and directly affects the lives and work of Breast Cancer patients.OBJECTIVE: To promote the functional recovery of affected-side upperlimb in patients with breast cancer after operation by three-stage functional training and health guidance.DESIGN: Clinical observationSETTING: Department of Nursing, First Affiliated Hospital, China Medical UniversityPARTICIPANTS:Totally 168 patients,to be all female,aged from34 to72 years with the average of 49.65 years, diagnosed breast cancer and received operation in the First Affiliated Hospital of the China Medical University from May 2002 to May 2004, were recruited.METHODS:Self-training was performed on the upper-arm at the affectedtion ( one or two days after operation): Do active and passive exercises of Off-bed stage (3 to 14 days after operation): Do exercises of shoulder joint,hospital( 3 to 6 months), functional exercise and guidance training on the upper arm, 2 to 3 times per day , 15 to 20 minutes per time. Self-training effect of the patients was investigated through further consultation, once every 2 months after the self-training.cises status and the abnormal status of their affected arms of the patients.RESULTS :Totally 168 patients completed the study,all their date was enbetter recovery with their affected arms and could do some simple work 75.0% (126/168)of the patients persisted in the functional training of their affected arms over half a year after they discharged from the hospital;25.0% (42/168)of the patients had different degrees of lymphatic edema of the affected arms.CONCLUSION:We have got an obvious effect in the aspects of strengthening the guidance of the functional training of their affected arms of the breast cancer patients and telling the patients and their family members the significance of persisting in training their main points of self nursing of their affected arms.
2.Effects of different body temperatures on neuromuscular block induced by different doses of cisatracurinm in rabbits

Objective To investigate the effects of different body temperatures on the neuromuscular block induced by different doses of cisatracurium in rabbits. Methods Seventy-two healthy New Zealand white rabbits of both sexes weighing 2.0-2.2 kg were randomly divided into 3 groups (n=24 each): hypothermia group (L), normothermia group (N) and hyperthermia group (H). The rectal temperature was set at 34.5℃, 38.5℃ and 41.8℃ in group L, N and H respectively. Each group was further divided into 4 subgroups (n=6 each) receiving intravenous cisatracurinm 0.33, 0.66, 0.99 and 1.32 mg/kg respectively after rectal temperature was stabilized for 20 min. The animals were anesthetized with iv 20% methane 1 g/kg, tracheostomized and mechanically ventilated (VT 12 ml/kg, RR 60 bpm, I:E = 1:2;FiO2 100% ). TOF was monitored. The onset time, the duration of action (from the end of cisatracurium injection to the recovery of T1 to 5%, 25% and 95% of control height) and recovery index (recovery of T1 from 25% to 75% of control height) were recorded. MAP, HR and rectal temperature were recorded at 5 min before alteration of the body temperature, at 2 min before and 5, 10, 30, 60 and 90 min after administration of cisatracurium. Results (1) With the same dose of cisatracurium the onset time and duration of action were significantly shorter in group H, longer in group L and recovery from N-M block faster in group H, slower in group L as compared with group N. (2) At the same body temperature the onset time was significantly shortened and the duration of action and recovery were significantly prolonged as the dose of cisatracurium increased. (3) There was significant difference in the effect of interaction between different doses of cisatracurium and different temperatures on the onset time. (4) With the same dose of cisatracurium MAP was significantly decreased in group H, increased in group L, while HR was significantly increased in group H, decreased in group L. Conclusion The neuromuscular blocking effect of cisatracurium is decreased when the body temperature is increased and vice versa. The neuromuscular blocking effect of cisatracurium (0.33-1.32 mg/kg) is dose-dependent at the same body temperature. There is interaction between the two factors.
3.Observation of clinical efficacy of silybin combined with nucleotides in the treatment of viral hepatitis with nonalcoholic fatty liver disease

Objective To evaluate the clinical efficacy of silybin combined with nucleotides in the treatment of viral hepatitis associated with nonalcoholic fatty liver disease (NAFLD).Methods 88 viral hepatitis patients with NAFLD were randomly selected into the clinical research,and they were divided into groups according to the order of treatment.Among them 44 cases of single number were the control group treated with nucleoside(acid) drugs,44 cases of double number were the observation group treated with silibinincombined silybin combined with nucleoside(acid).The therapeutic effect after the end of the course of treatment were was evaluated.Results The effective rate in the observation group was 90.91%,which that in the control group was 70.45%,the difference was statistically significant (x2 =8.87,P < 0.05).The ALT,AST,r-GGT after treatment of the observation group were (37.14 ± 3.56) U/L,(30.45 ±3.35) U/L,(51.65 ± 3.46) U/L,which were significantly reduced than before treatment(t =8.01,8.72,7.80,all P < 0.05),and the differences were significant compared with the control group (t =7.57,8.34,7.29,all P < 0.05).The TG,TC of blood lipid indexes indices after treatment of the observation group were (1.48 ±0.26) mmol/L,(5.18 ± 0.86)mmol/L,which were significantly lower than those before treatment (t =7.62,8.14,all P < 0.05),and the differences were significant compared with the control group (t =7.10,7.55,all P < 0.05).Conclusion The combination of silybin and nucleotides in the treatment of viral hepatitis with NAFLD has significant effect,can significantly improve the clinical symptoms and liver function indicators,and gradually restore the level of blood lipids,it is worthy of promoting the application.

[Summary] Cardiovascular disease ,diabetic nephropathy and diabetic retinopathy are common complications of diabetes ,and are the significant reason of cardiovascular mortality ,end-stage renal disease ,and blindness in adults patients with type 2 diabetes mellitus. At present, the researche on the pathogenesis of diabetic complications mainly focus on oxidative stress and chronic inflammation. In addition, a large number of studies have pointed out that bilirubin has antioxidant and anti-inflammatory effects. Therefore, bilirubin and diabetic complications are closely linked.

10.CONNECTION BETWEEN THE SUPERIORCOLLICULUS AND LATERAL GENICULATE BODY IN THE GOLDEN HAMSTER

Connection between the superior colliculus(SC)and the dorsal and ventral lateral geniculate nuclei (LGd and LGv) were studied in adult golden hamsters using anterograde and retrograde pathway tracing methods 1. In four hamsters, the cytoarchitecture of the SC, LGd and LGv was examined in brain sections stained with Nissl's or Loyez's method. 2. A single injection of a mixture of ~3H-leucine and ~3H-proline was made into various regions of the SC in each of 4 animals (1 day survival) for studying the pattern of terminal distribution of the projections from the SC to the ipsilateral LGd and LGv. In animals where the injection site was located in the superficial gray layers of the lateral portion of the SC, labelled terminals were found in the caudal lateral part of the LGd and LGv. In the case where the injection was restricted in more medial part of the SC, the terminal labelling was observed in the rostral lateral portion of the lateral geniculate nuclei. 3. One day after an injection of horseradish peroxidase (HRP) into the LGd (4 animals), LGv(2 animals), or LP(2 animals) labelled neurons were observed in the superficial gray layers of the ipsilateral superior colliculus. No HRP-labelled neuron was detected in deeper laminae in the colliculus. These results indicate that neurons in the superficial gray layers of the SC project to the ipsilateral LGd and LGv, and the projections are organized topogra- phically according to the retinotopic or visual field map
