1.The correlation study between heart rate variability and atherosclerosis in primary hypertension patients

Objective To study the correlation between heart rate variability and atherosclerosis in primary hypertension patients.Methods 282 primary hypertension patients with carotid ultrasound were selected and divided into the arteriosclerosis group (n =201 )and the non -arteriosclerosis group (n =81 )according to the carotid ultrasound examination results.The ambulatory blood pressure and electrocardiogram (ECG)in the two groups were recorded and compared,as well as heart rate variability index.Results Age[(60.2 ±5.2)years]and body mass index [(27.8 ±1.7)kg/m2 ]in the atherosclerosis group were significantly higher than those in the non -arteriosclerosis group[(56.4 ±7.1)years,(26.2 ±2.3)kg/m2 ,t =4.98,6.43,all P <0.05].The course of hypertension in the atherosclerosis group[(11.3 ±1.0)years]was significantly longer than that in the non -arteriosclerosis group[(8.6 ± 1.1)years,t =19.93,P <0.05].In the atherosclerosis group,24h SBP,24h DBP,dSBP,dDBP,nSBP and nDBP were significantly longer than those in the non -atherosclerosis group(t≥6.10,all P <0.05).In the atherosclerosis group, heart rate variability time domain analysis indicators including SDNN,SDANN,SDNN index,RMSSD and PNN50 were (105.7 ±5.1)ms,(96.8 ±12.5)ms,(41.4 ±8.5)ms,(28.5 ±4.7)ms,(10.2 ±3.1)% respectively,which were all significantly lower than those in the non -atherosclerosis group[(127.3 ±4.8)ms,(139.0 ±13.4)ms,(48.2 ± 7.3)ms,(34.9 ±5.0)ms,(13.7 ±4.3)%],the differences were statistically significant(t≥5.76,all P <0.05). Conclusion In primary hypertension patients,heart rate variability has obvious correlation with arteriosclerosis, which heart rate variability is reduced and have low blood pressure regulation,so that the heart rate variability can be used as an important indicator for primary hypertension disease and prognosis judgement.

3.Protective effects of piperine on alveolar bone and collagen in a periodontitis model

BACKGROUND:Piperine in models of pancreatitis, gout, middle cerebral artery infarction has anti-inflammatory, antioxidant and immune regulatory effects, but its effects on periodontitis model are not clear.
OBJECTIVE:To observe the protective effect of piperine on bone absorption and degradation of col agen in experimental rat models of periodontitis.
METHODS:Rat models of periodontitis were established by ligaturing the dental cervix of rat mandibular first molar with 3-0 silk. On day 1 before model establishment, rats were intragastrical y administered piperine 50 and 100 mg/kg. There were healthy control group and model group. Related detection was performed 8 weeks after model establishment.
RESULTS AND CONCLUSION:(1) Results of quantitative CT analysis:compared with the healthy control group, the distance from the first molar enamelo-cemental junction to the alveolar ridge crest was significantly lower in the model group (P<0.05). The degree of alveolar damage was significantly improved in the 50 and 100 mg/kg piperine groups compared with the model group (P<0.05). (2) Related factor protein expression:compared with the model group, matrix metal oproteinase-8,-13 and interleukin-1βprotein expression was significantly decreased in the 100 mg/kg piperine group (P<0.05);matrix metal oproteinase-8 protein expression was significantly decreased in the 50 mg/kg piperine group (P<0.05). (3) Col agen fiber morphology:compared with the model group, col agen fibers arranged orderly and col agen fiber area significantly increased in the 50 and 100 mg/kg piperine groups (P<0.05). (4) Results confirmed that piperine could reduce the alveolar bone resorption, reduce the degradation of col agen fibers and protect the periodontal tissues in models of periodontitis. Its mechanism is associated with the inhibition of matrix metal oproteinase-8,-13 and interleukin-1βprotein expression.
4.Study on MRI features and its clinical histopathological characteristics of triple-negative breast cancer

Objective To analyze triple receptor-negative breast cancer (TNBC) and non-triple receptor-negative breast cancer (non-TNBC),to determine the MR imaging characteristics of TNBC.Methods Data from two groups TNBC and non-TNBC patients were reviewed.Recorded the MRI features which involved the morphological features,kinetic lesion features and measured ADC values,As well as their clinical features and the pathological findings.Data were statistical analyzed in SPSS 17.0 software.Results There were significant difference between TNBC and non-TNBC in histological types,histologic grade,the situation of lymph metastases and mass shape,margin subgroup and lesion enhanced type,the ADC value (P < 0.05).The special histological types for TNBC were squamous carcinoma (metaplastic carcinomas),medullary carcinoma,basal-like carcinoma.The number of lymph node metastasis in the TNBC group was 78.9 % (15/19).Histologically,TNBC were mainly Ⅲ grade (89.4 %).In the group of TNBC,the percent of lobulated mass,margin smooth and rim enhancement was 52.6 %,36.8 %,42.1%,the higher ADC value of TNBC than non-TNBC.While there were no statistical significant differences in the age of onset,the maximum tumor diameter,the situation of menstruation,the situation of family history of breast cancer,the number of the mass,enhancement lesion type and the time-signal intensity curve type (P > 0.05).Conclusion Some MRI features combined clinical and histopathological characteristics would be helpful in the diagnosis of TNBC.
5.A survey of sub-health and TCM physical constitution of stroke patients' families

Objective To explore the sub-health and TCM physical constitution of stroke patients’families.Method Four hundred and twelve family members of stroke patients hospitalized during September 2001 to May 2013 were involved in the survey by filling out TCM physical constitution scale(TCMPC)and the sub-health self-rating scale(SSS)for the purpose of investigating the sub-health and conditions of TCM physical constitution.Results Of all the patients,189 were in sub-health,accounting for 45.87%. There were 7 main clinical symptoms related to sub-health,including poor sleep,less activeness,proneness to upsets and irritation, memory loss,depression,tension in interpersonal relations and appetite loss,with the incidences ranging from 64.55%to 48.15%. In TCM,51.85%of the family members were balanced in physical constitution,10.58%of them had Yang-deficiency constitution, 14.29%of them had Yin-deficiency,23.28%of them other kind of constitution.Conclusions The family members of stroke patients have a high rate of sub-health.The members in sub-health hold a balanced constitution,Yang-deficiency or Yin deficiency.In order to eliminate their sub-health,improving the quality of care for patients with stroke,we should guide the families of stroke patients to develop good living habits,strengthen the TCM mental nursing,help them to establish a good social support system.

7.Status of COPD patients with deep venous thrombosis in RICU

Objective To analyze the incidence and related risk factors of chronic obstructive pulmonary disease (COPD) patients with deep venous thrombosis (DVT) in the respiratory intensive care unit (RICU).Methods We recruited 204 patients from RICU of our hospital between January 2015 and December 2015.The basic clinical data of the patients were also collected.Bedside venous ultrasonography of double lower limbs was taken 24-48 hours after admission.The patients were divided into DVT group and non-DVT group.The clinical data of the two groups were analyzed and compared using Chi-square test or t test,Logistic regression analysis was used to analyze risk factors,and ROC curve analysis was used to identify the disease.Results Among the 204 patients,39 were identified with DVT,with the overall incidence rate of 19.1%.D-dimer in DVT group was significantly higher than that in non-DVT group (P<0.001).The two groups did not significantly differ in age,sex,smoking history,underlying diseases,or other laboratory test results.The area under the ROC curve of D-dimer was 0.787,and the cut off value was 1.985 mg/L (sensitivity of 0.641,specificity of 0.817).Conclusion D-dimer is closely associated with increased incidence of DVT in COPD patients.

OBJECTIVE:To discuss the related measures for the reduction of medical expenses.METHODS:The related data concerning hospital drug revenues before and after the operation of’pharmaceuticals purchase by public bidding’were compared,and reasons for the high undescended hospital drug expenses were analyzed.RESULTS:Although the drug revenues kept increasing year by year,yet the profits gained by hospitals from which decreased year by year,which may resulted from many factors.CONCLUSIONS:The operation of the’pharmaceuticals purchase by public bidding’failed to bring down the high medical expenses.
