Objective To evaluate the feasibility, safety and clinical outcome of laparoscopic radical resection of rectal cancer in elderly patients with common chronic comorbidities. Methods The clinical data of 192 elderly patients with rectal cancer and common chronic comorbidities who were admitted to the Nanfang Hospital from January 2003 to December 2008 were retrospectively analyzed. All patients were divided into laparoscope group (n=91) and open group (n = 101). The operative procedures, clinicopathological data and outcomes of the two groups were collected and compared. All data were analyzed using the t test and chi-square test, and the survival of patients was analyzed using the life table and Kaplan-Meier curves. Results The sphincter preservation rate, operation time, postoperative length of hospital stay, morbidity rate were 85.7% (78/91), (194 ± 61) minutes,(14 ±8)days, 24.2% (22/91) in the laparoscope group, and they were 85.1% (86/101), (187 ±58) minutes,(14 ±8)days and 28.7% (29/101) in the open group, with no significant difference between the two groups (x2=0.012, t=0.874,-0.265, x2 =0. 505 , P > 0. 05) . The intraoperative blood loss was (108 ±78)ml in the laparoscope group, which was significantly less than (270 ± 600) ml in the open group (t =-2. 650, P <0.05). The time to first flatus, time to liquid diet, time to out-of-bed activity were (3 ± 1) days, (4 ± 2) days and (3 ± 1)days, which were significantly shorter than (4 ± 1)days, (5 ± 2)days and (5 ± 1)days in the open group, respectively (t =-4. 545,-4. 587,-13. 310, P < 0.05). The length of rectum resected and the number of lymph node dissected were (18 ± 5)cm and 9 ± 7 in the laparoscope group, and (18 ± 5)cm and 9 ± 8 in the open group, respectively, with no significant difference between the two groups (t = 1. 457, 0. 021, P > 0.05), while the distance of distal resection margin to the tumors was (3.8 ± 1.5) cm, which was significantly longer than (3.1 ± 1.5) cm of the open group (t = 0. 283, P < 0. 05). The 3-year cumulative survival rate, overall recurrence rate, local recurrence rate and distal metastasis rate in the laparoscope group were 76%, 12. 1% (11/91), 2.2% (2/91) and 9.9% (9/91), and they were 82%, 14.9% (15/101), 6.9% (7/101), 7.9% (8/101) in the open group, respectively, with no significant difference between the two groups (U=2. 600, x2 =0. 312, 2.400,0. 230, P > 0.05). There were no significant difference in the cumulative survival rate between patients in TNM stage Ⅰ, Ⅱ and Ⅲ in the two groups (P > 0.05). Conclusion Laparoscopic radical resection of rectal cancer is safe and feasible for elderly patients with common chronic comorbidities, and it has the advantages in quick recovery of patients after operaion.

Serum matrix metalloproteinase-9 (MMP-9)and advanced glycation end products (AGE)levels were measured in the diabetic patients with or without foot problems as well as in healthy retired people. In diabetic patients serum MMP-9 and AGE levels were significantly higher than those in healthy subjects. Median MMP-9 level in patients with diabetic foot was higher than that in diabetic patients without foot ulcers. Increased serum MMP-9 in diabetes might predict occurance of diabetic foot and poor wound healing of the foot ulcers.

Background and purpose:Cytotoxic T lymphocyte (CTL) plays a vital role in the process of anti-tumor immunology. The aim of this study was to investigate whether changes in concentration of IL-2 (50, 200 and 1 000 U/mL) would affect the sub-population and cytotoxic function of cells cultivated by peptide-specific CTL induction systemin vitro and also observe whether using the concentration of IL-2 at a range of 50-1 000 U/mL isbeneifcial to regulatory cells (Tregs) enrichment. Methods:Peripheral blood from 10 healthy donors and 10 cancer patients that were HLA-A2 positive, were collected in the study. HLA-A2 restricted CTL epitope P321 (ILIGETIKI) derived from COX-2 pulsed with different concentrations of IL-2 were used to induce peptides-speciifc CTLin vitro. Flow cytometry was performed to analyze the proliferative capability, the proportion of different T-cell subsets, and secretion of perforin, granzyme B and IFN-γ. IFN-γ secretion was assessed by ELISpot assay. Results:High concentration of IL-2 increased the proliferative activity. The percentage of CD4+ T cells of cancer patient group was signiifcantly higher than that of healthy donor group, while the percentage of CD8+ T cells of cancer patient group was signiifcantly lower than that of healthy donor group. And there was no signiifcant difference in the percentages of CD4+ T cells, CD8+ T cells and Tregs among groups with different IL-2 concentrations. No difference was seen in cytokine (perforin, granzyme B, IFN-γ) secretion capacity of CD8+ T cells. ELISpot study revealed that high-dose IL-2 resulted in the increasing of IFN-γ secretion. Conclusion:The sub-population and the function of cells cultured by peptide-speciifc CTL induction systemin vitro are not affected by different concentrations of IL-2. Furthermore, high concentrations of IL-2 (50-1 000 U/mL) do not provide the enrichment for Tregs. Higher concentration of IL-2 is likely to cause high secretion of IFN-γ in ELISpot assay. In order to exclude the distraction of NK cells or NKT cells, the concentration of 50 U/mL is better choice.

Liver transplantation is the main treatment for end-stage liver diseases and liver tumor. To solve the problem of the insufficient donor liver, autologous liver transplantation (ALT) is becoming widely acknowledged, which could serve as a complementary technique for treating unresectable liver space-occupying lesions in routine operation. However, compared with allogeneic liver transplantation, the long anhepatic phase during ALT may influence the outcome of such patients.Veno-venous bypass (VVB) as a technology which can maintain the stability of hemodynamics and internal environment in the anhepatic phase is routinely used in ALT.In this paper, the application of VVB in the ALT was mainly discussed.

Objective To investigate the association of serum dehydroepiandrosterone sulfate( DHEA-S) with the cognition in male type 2 diabetes mellitus(T2DM)patients. Methods 99 male patients cared at Tangshan Gongren Hospital and another 97 male healthy controls without T2DM from the medical examination center were recruited. Data on demographic characteristics and clinical parameters were collected, DHEA-S was measured by radioimmunologic assay. Cognitive performance was assessed by the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS). Results (1) Serum DHEA-S levels were lower in male T2DM patients than that of controls[(2. 66 ± 0. 78 vs 4. 02 ± 1. 24) μmol/ L, P<0. 01];(2) Compared with controls, RBANS scores including immediate memory(79. 24 ± 17. 47 vs 86. 25 ± 15. 21, P<0. 01), visuospatial/ constructional(83. 98 ± 17. 98 vs 97. 24 ± 11. 51, P<0. 01), attention(96. 04 ± 14. 65 vs 101. 45 ± 13. 93, P<0. 01), delayed memory (89. 28 ± 11. 74 vs 97. 41 ± 9. 41, P<0. 01), and total scores(85. 85 ± 11. 46 vs 94. 60 ± 10. 91, P<0. 01)were all lower in male T2DM patients;(3) RBANS scores including delayed memory(84. 53 ± 12. 23 vs 93. 94 ± 9. 18, P<0. 01)and total scores(80. 33 ± 10. 91 vs 91. 26 ± 9. 25, P<0. 01)in T2DM patients with low-level DHEA-S(DHEA-S-L)were all lower than those of patients with high-level DHEA-S;(4)Male T2DM patients with cognitive impairment had lower levels of DHEA-S than patients without cognitive impairment(2. 31 ±0. 79 对 2. 90 ±0. 67, P<0. 01);(5) In male T2DM patients, DHEA-S was positively correlated with delayed memory(r = 0. 252, P = 0. 019) and total scores(r=0. 258, P= 0. 016). Conclusion Male T2DM patients are with lower serum DHEA-S levels and worse cognitive performance, and serum DHEA-S was positively correlated with their cognitive performance, suggesting that serum DHEA-S may be involved in the cognitive deficits of male T2DM patients.
