1.Effect of the blood lipid level in the prognosis of patients with sepsis

ObjectiveTo observe the changes of the blood lipid levels in patients with sepsis,and reveal the clinical significance of the blood lipid level in the prognosis of patients with sepsis.MethodsThe blood lipid levels and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ ) of 40 patients with sepsis(sepsis group) were recorded.The blood lipid levels were compared with those of 25 healthy people (control group).The correlation of the blood lipid level with APACHE Ⅱ score and prognosis of sepsis was analyzed.ResultsIn sepsis group,plasma total cholesterol(TC),high density lipoprotein (HDL),low density lipoprotein (LDL),apolipoprotein B (ApoB) was (3.92 ±0.96) mmol/L,(1.10 ±0.39) mmol/L,(2.44 ± 0.81 ) mmol/L and ( 1.03 ± 0.27) g/L respectively,which were significantly decreased compared with those in control group [ ( 4.40 ± 0.55 ) mmol/L,( 1.61 ± 0.42) mmol/L,(2.79 ± 0.47 ) mmol/L,( 1.13 ± 0.12 ) g/L] (P <0.05).In sepsis group,26 cases survived and 14 cases died.TC,triacylglycerol (TG),HDL,LDL,apolipoprotein A Ⅰ (ApoA Ⅰ ),ApoB,platelet count and albumin level of death patients was (3.33 ±0.92) mmol/L,(0.81 ±0.39) mmol/L,(1.03 ±0.27) mmol/L,(1.83 ±0.68) g/L,(1.03 ±0.27) g/L,(0.86 ±0.27) g/L,(140.0 ±82.3) × 109/L,and (32.00 ±5.52) g/L,respectively,which were significantly decreased compared with those of survival patients [ (4.24 ± 0.84) mmol/L,( 1.21 ± 0.44) mmol/L,( 1.25 ±0.30) mmol/L,(2.77 ±0.68) mmol/L,(1.25 ±0.13) g/L,(1.13 ±0.23) g/L,(215.9 ± 101.0) × 109/L,(36.12 ±6.30) g/L](P<0.05).APACHEM Ⅱ score of death patients was (20.5 ±4.2) scores,which was increased compared with that of survival patients [ ( 13.8 ± 4.8) scores ] ( P < 0.05 ).Multivarisble Logistic regression analysis showed plasma HDL and APACHE Ⅱ score was independent risk factor (standard regression coefficient,HDL =-6.222,APACHE Ⅱ score =0.337).Conclusions Disorders of lipid metabolism exist in patients with sepsis.Plasma HDL level is a good indicator in assessment of prognosis in patients with sepsis and it shows better results combined with APACHE Ⅱ score.
2.Determination of ginsenoside Rg_1 and Rb_1,notoginsecoside R_1 in XuesaitongEnteric Dripping Pill by HPLC

AIM: To study a method of determing ginsenoside Rg_1 and Rb_1,notoginoside R_1 contents in Xuesaitong Enteric Dripping Pill. METHODS: HPLC was used to determine gingsenoside Rg_1 and Rb_1 notoginse-(noside) R_1 contents. RESULTS: The average recoveries were 100.35% for ginsenoside Rg_1(RSD=1.34%),(102.01%) for ginsenoside Rb_1(RSD=1.48%),100.13% for notoginsenoside R_1(RSD=1.78%),n=6),respectively. CONCLUSION: The method is simple,senstive and precise.It could be used for the determination of ginsenoside Rg_1 and Rb_1,notoginsenoside R_1 contents in Xuesaitong Enteric Dripping Pill.
3.Determination of ginsenoside Rg1 and Rb1 notoginsenoside R1 in Tiaojing Yangyan Capsule by HPLC

AIM: To study a method of determining ginsenoside Rg1 and Rb1,notoginsenoside R1 contents in Tiaojing Yangyan Capsule(Radix Astragali,Fructus Ligustri Lucidi,etc.). METHODS: HPLC was used to determine ginsenoside Rg1 and Rb1,notoginsenoside R1 contents. RESULTS: The average recoveries were 99.03% for ginsenoside Rg1(RSD=1.59%),98.95% for ginsenoside Rb1(RSD=1.40%),99.18% for notoginsenoside R1(RSD=1.97%) respectively n=6. CONCLUSION: The method is simple,sensitive and precise.It can be used for the determination of ginsenoside Rg1 and Rb1,notoginsenoside R1 in Tiaojing Yangyan Capsule.
4.Inhibitory effect of SM-1 on human liver microsomal cytochrome P450 enzyme

Aim To investigate the effect of SM-1 on seven main cytochrome P450(CYP450)in human liver microsomes.Methods Substrate or SM-1 was incubated with human liver microsomes for 30 min in vitro,and divided into control group and experimental group.The effects of SM-1 on the main phase I metabolic enzymes in human liver microsomes was detected by HPLC.Phenacetin,bupropion,paclitaxel,tolbutamide,omeprazole,dextromethorphan,testosterone were investigated as probe drugs.Results Inhibition rate of SM-1 on the classical substrate of human liver microsomal CYP was 0.05%,3.37%,0.08%,2.07%,4.20%,-0.15%and 10.84%,respectively.Conclusions SM-1 may have inhibitory effect on CYP3A4.Attention should be paid to the interaction of clinical drug induced by CYP enzyme inhibition.
5. Expression and significance of STAT6 in solitary fibrous tumor

Objective:
To study the clinicopathologic features, the differential diagnosis and the expression of STAT6 in solitary fibrous tumor (SFT).
Methods:
Eighty cases of SFT were evaluated. The expression of STAT6, CD34, CD99 and bcl-2 protein was studied in these cases and in other groups of soft tissue tumors by immunohistochemical EnVision method. The results were analyzed and relevant literature were reviewed.
Results:
The expression rate of STAT6 in SFT was 97.5% (78/80) and that in other soft tissue tumors was 3.3% (3/90). The difference was significant (

Objective To Pulse oximetry saturation has been wildly used clinically.It has been reported that pulse oximetry plethysmographic waveform (POP) reflected the peripheral tissue perfusion.In this study,we parameterized POP,observed the value of POP parameters in normal adults,and established the normal reference value range.Methods A multi-center prospective descriptive study.Total of 1 019 adult volunteers with normovolemia from 7 cities were enrolled in this study.Sex,age,height,weight and pulse oximetry data in awake and spontaneous breathing under in quiet conditions in the room temperature were collected.POP parameters and perfusion index were analyzed using MATLAB 2012a software.The normal reference value ranges of POP parameters,including the amplitude of POP (Amp) and the area under the curve of POP (AUC),were formulated.Results Statistical differences of POP parameters were detected between men and women in the normal adult.The 95% confidence reference value of POP parameters in normal population was as follows:Amp (104.8-2298.7) PVA and AUC (3265.8-6028.5) PVPGin total,Amp (129.4-2433.6) PVA and AUC (3319.0-5862.2) PVPG in male;Amp (89.5-2138.2) PVA and AUC (3163.9-5929.9) PVPG in female.Conclusions POP,including the amplitude of POP (Amp) and the area under the curve of POP (AUC),had normal reference value ranges in normal adults.

Endo-periodontal lesions(EPLs)involve both the periodontium and pulp tissue and have complicated etiologies and pathogenic mechanisms,including unique anatomical and microbiological characteristics and multiple contributing factors.This etiological complexity leads to difficulties in determining patient prognosis,posing great challenges in clinical practice.Furthermore,EPL-affected teeth require multidisciplinary therapy,including periodontal therapy,endodontic therapy and others,but there is still much debate about the appropriate timing of periodontal therapy and root canal therapy.By compiling the most recent findings on the etiology,pathogenesis,clinical characteristics,diagnosis,therapy,and prognosis of EPL-affected teeth,this consensus sought to support clinicians in making the best possible treatment decisions based on both biological and clinical evidence.

OBJECTIVE:To study the effect of isofla vaspidicacid PB (called PB for short )on the biofilm adhesion and the gene expression of ergosterol metabolism related enzymes in Trichophyton rubrum . METHODS :M38-A2 method was adopted to determine MIC of PB to T. rubrum . MTT assay was used to screen the biolfilm condition and initial adhesion period of T. rubrum . The effects of different concentrations of PB (40,80,160 µg/mL)on the adhesion duration of T. rubrum (growth control group without PB was set up ,similarly hereinafter )were evaluated and the adhesion rate was calculated by using XTT assay ;the effects of different concentrations of PB (20,40,80 µg/mL)on the biofilm formation of T. rubrum at different initial adhesion periods (3,5,9 h)were observed and the adhesion rate was calculated by using XTT assay combined with inverted microscope ;qRT-PCR method was used to detect the effects of PB (320 µg/mL)on the mRNA expression of ergosterol metabolism related enzyme gene ERG6 and ERG11 in biofilm of T. rubrum . RESULTS :MIC of PB to T. rubrum was 20 µg/mL. The biofilm of T. rubrum in RPMI-1640 medium containing 10% FBS was the most metabolism activity at 6 h of initial adhesion. Compared with growth control group ,after treated with different concentrations of PB ,adhesion rate and mRNA expression of ERG6 and ERG11 in biofilm were decreased significantly (P<0.01). Hyphae decreased or even disappeared ,and the adhesion inhibition rate (at 5 and 9 h of initial adhesion )increased significantly (P<0.05 or P<0.01). CONCLUSIONS :PB can inhibit the adhesion of T. rubrum and reduce the hyphae ;the mechanism may be associated with the inhibition of the biofilm adhesion and mRNA expression of ergosterol metabolism related enzyme gene ERG6 and ERG11.
9.Clinical implications of first CD4+T cell counts among newly reported HIV-infected Burmese in Dehong Prefecture of Yunnan Province in 2019 and 2020

ObjectiveTo investigate the demographic characteristics, high-risk behavior and first CD4+T cell(CD4 cell) counts among newly reported HIV-infected Burmese in Dehong Dai Jingpo Autonomous Prefecture (Dehong Prefecture) from 2019 to 2020. MethodsA cross-sectional survey was conducted among newly reported HIV-infected Burmese cases in Dehong Prefecture from 2019 to 2020 to collect related information and measure their first CD4 cell counts. ResultsAmong 1 228 HIV-infected Burmese cases, 846 (68.9%) were males and 382 (31.1%) were females; 445 (36.2%) were from Kachin, and 210(17.1%) were from Shan. Of them, 960 cases (78.2%) had participated in the first CD4 cell counts testing, and 405 cases (42.2%) had a first CD4 cell counts <350 cells·μL-1. In the multivariable logistic regression analysis, age ≥30 years (adjusted OR=1.44, 95%CI: 1.07‒1.92) and history of illicit drug use in Myanmar (adjusted OR=0.41, 95%CI: 0.21‒0.79) were independently associated with first CD4 cell counts <350 cells·μL-1. ConclusionThe first CD4 cell counts are generally low among newly reported HIV-infected Burmese in Dehong Prefecture, especially those aged ≥ 30 years and with a history of intravenous drug use in Myanmar. Therefore, early detection of HIV infection should be strengthened for timely intervention and treatment management among Burmese.
10.Characteristics of HIV-infected persons and their spouses who became HIV-positive in HIV discordant couples in Dehong Prefecture, Yunnan Province

ObjectiveTo determine the characteristics, viral load and immunological status of HIV-infected persons and their spouses who became HIV-positive, and the reasons for HIV seroconversion in 55 HIV discordant couples in Dehong Dai and Jingpo Autonomous Prefecture (Dehong Prefecture), Yunan Province. MethodsData on the 55 couples meeting the criteria of having a previously positive spouse were retrieved from the AIDS Integrated Prevention and Control Data Information System of the China Disease Control and Prevention Information System during 2015-2021. General socio-demographic information, age at diagnosis, exposure history, CD4+T lymphocyte count, and antiviral treatment were collected. Descriptive analysis and chi-square test were used to compare the distribution of pre-HIV-positive spouses and their HIV seroconverted spouses. ResultsA total of 55 spouses from HIV discordant couples had HIV seroconversion. Of them, 72.7% (40/55) of pre-HIV-positive spouses were husbands. The most recent CD4+T lymphocyte count in the pre-HIV-positive spouses was (328.31±246.27) cells·μL-1 at the time of diagnosis of their seroconverted spouses, of which 36.3% (20/55) had a CD4+T lymphocyte count of less than 200 cells·μL-1. Furthermore, of those pre-HIV-positive spouses with low CD4+T lymphocyte count, 45.0% (9/20) had an undetectable viral load, 15.0% (3/20) <400 copies·mL-1, and 25.0%(5/20) ≥400 copies·mL-1. Additionally, 16.4% (9/55) of the pre-HIV-positive spouses did not have a viral load test. The main reasons for HIV seroconversion among HIV-negative spouses in the discordant couples were poor condom use, poor compliance with antiviral therapy, and treatment discontinuation. ConclusionThe follow-up management of HIV discordant couples should be strengthened in Dehong Prefecture, especially the monitoring of viral load levels and immunological status of pre-HIV-positive spouses, to improve their compliance with antiviral therapy and reduce treatment discontinuation, which would effectively prevent and control HIV transmission between spouses.
