Objective To establish the quality standard for Xiangshuyin Dispersible tablets. Methods Fructus citrus sarcodactylis and Borneolum syntheticum in the prescription were identified by TLC. The content of the total Naringin was determined by HPLC. The column of AichromBond-AQ-C18 (4.6 mm?150 mm,5 ?m) was used. The mobile phase was mobile phase A-mobile phase B (84∶16),mobile phase A was Water-Acetic Acid (100∶1),mobile phase B was Acetonitrile,at the flow rate of 1.0 mL/min,the column temperature of 30 ℃,and peaks were detected at 283 nm. Results The study on the quality control showed that the characteristic of identification by TLC was distinct and highly specific. The linear range of Naringin was 0.120 6～0.603 ?g,r =0.999 9,the average recovery was 97.90%,RSD=1.76% (n=5). Conclusion The method was easy to operate with accurate result and good reproducibility. It can be used effectively for the quality control of the preparation.

OBJECTIVE To enhance the control and management of nosocomial infection in operating room in order to reduce nosocomial infection rate.METHODS We estabilished some effective sterilization and isolation measures and infection monitoring systems,strictly carried out sterile operation rule and created the qualified environment of operating room.RESULTS The nosocomial infection rate of cleaning wound was 0.25%,there was no specific nosocomial infection.CONCLUSIONS Strengthening management of nosocimial infection in operating room can effectively prevent it and reduce the infection rate of cleaning wound.

Type 2 diabetes mellitus is a kind of chronic metabolic disease,accompanied by multiple organ damages and involving the skeletal system as well.Osteoporosis is the most likely bone lesions suffered by patients with type 2 diabetes.The study found that,type 2 diabetic patients,compared with the normal people,will suffer from fracture at high risk,due to the decreased of bone quality and the increased risk of falling.The decrease of bone quality are the result of the deterioration of bone architecture and the loss of bone mass.The abnormal gait as well as the impairment of cognitive and the muscle weakness of type 2 diabetes,especially for elderly patients,will reduce their balance ability and increase their risk of falling,lead to an increased prevalence of fractures.

Objective To analyze the distribution of class 1,2 and 3 integrons and their gene cassettes,and to explore its relationship with drug resistance in Shigella.Methods Antimicrobial susceptibility was detected by minimal inhibitory concentration (MIC) method.All the genes of integrons and gene cassettes were amplified by polymerase chain reaction (PCR).The amplicons were identified by amplified fragment length polymorphism (AFLP) and sequencing.Results Fifty seven multi-drug resistant strains were identified from a total of 62 Shigella strains (91.9％).Among multi drug resistant strains,52 strains carried integrons of class 1 (91.2 ％) and 55 strains carried integrons of class 2 (96.5％).Only 2 strains carried class 1 integrons alone,5 strains carried class 2 integrons alone and 50 strains had both class 1 and class 2 integrons.Class 3 integrons were not detected.The gene cassettes of typical class 1 integrons,dfrV and dfrA17-aadA5,were detected in 6 strains and 2 strains,respectively.Atypical class 1 integrons with gene cassettes blaOxA30 aadA1 were detected in 44 strains.The typical and atypical class 1 integrons coexisted in 6 Shigella flexneri strains.Gene cassettes for class 2 integrons were dfrA1 sat1-aadA1.Conclusions The multi-drug resistant Shigella strains are widely distributed in Ji'nan,and the atypical class 1 integrons and class 2 integrons are common in these strains.Coexistence of the two integrons is observed in some of the strains.

The concentration of serum adiponectin [(2.51±1.42)mg/L] was lower in the group of patients with type 2 diabetes mellitus as compared with that in normal controlgroup [(5.26±0.78)mg/L ,P＜0.01]. The concentration of serum adiponectin was lower in the diabetics with macrovascular complications (MVC) [(1.38±0.77)mg/L] as compared with those without macrovascular complication [(3.66±0.91)mg/L]. The concentration of serum resistin was higher in the diabetic group as compared with that in control group[(7.07±1.11 vs 6.09±0.47)μg/L, P＜0.01]. It was also higher in patients with MVC [(7.96±0.65)μg/L] compared with those without MVC [(6.10±0.43)μgL, P＜0.01].

Objective To discuss the characteristics of extended-spectrum beta-lactamases(ESBLs)-producing Shigella and the relation between them and drug-resistance plasmid. Methods The suspicious ESBLs-producing isolates were screened by K-B disc diffusion method, and the ESBLs-producing strains were confirmed by confirmatory test recommended by the National Committee for Clinical Laboratory Standards. Furthermore, the partial blageneof these isolates were detected by PCR using universal primers for TEM, SHV, CTX-M-1 group, CTX-M-2 group and CTX-M-9 group, respectively. The entire blaCTX-M-9 and blaTEM were amplified by PCR using the primers outside the open reading frame (ORF) of these β-1actamases and products were directly sequenced. The conjugation experiment was performed to determine whether the resistance was transferable. Minimal inhibitory concentration (MIC) was detected with double agar dilution method. Results Of the 275 isolates, 12 strains were identified as ESBLs producers. Among them, 8 strains were CTX-M-14 carriers and 4 strains were CTX-M-3 carriers. All ESBLs-producing isolates are positive for plasmid conjugative transfer test. The transconjugants are only resistance to betalactams. Conclusions High resistance to beta-laetams in Shigella is caused by production of ESBLs in the local area. The ESBLs-produeing isolates can transfer the drug resistance through lateral transfer of plasmid.

Objective To explore the relationship between visfatin and mild cognitive impairment(MCI)in patients with type 2 diabetes mellitus(T2DM).Methods A perspective study involving 75 hospitalized T2DM patients were divided into groups with(MCI,n=35)and without (NMCI,n =40)mild cognitive impairment.Another 30 non-diabetic patients were chosen as normal control(NC).Fasting plasma levels of glucose (FPG),insulin (FINS),lipid,glycosylated hemoglobin (HbAlc),HOMA-IR and visfatin were measured and calculated.Results The serum visfatin level was higher in MCI(28.81±3.32)μg/L than in NMCI(20.69±3.40)μg/L and NC(19.06±2.35)μg/L (F=96.491,P＜ 0.01).Visfatin was negatively correlated with Montreal Cognitive Assessment (MoCA) total score (MoCA-TS) (r =-0.646,P ＜ 0.01),but positively correlated with course of disease,waist hip ratio,FPG,HbAlc,FINS,HOMA-IR and triglyceride (r=0.282,0.276,0.318,0.496,0.339,0.433,0.309,P＜0.05 or P＜0.01).MoCA-TS was negatively correlated with course of disease,HbAlc,HOMA-IR,triglyceride,total cholesterol,low density lipoprotein cholesterol (r =-0.582,-0.365,-0.234,-0.330,0.277,-0.238,P＜0.05 or P＜0.01),but positively correlated with high density lipoprotein cholesterol(r=0.290,P＜0.05).Higher values of visfatin(OR =3.246,P＜0.01),HbAlc(OR=2.308,P＜0.01)and course of disease(OR=1.634,P＜0.05)were the risk factors for MCI.Conclusions The elevated visfatin level might be a risk factor for MCI in T2DM patients.

Objective To investigate the role of interleukin-17 (IL-17) in spinal dorsal horns in neuropathic pain (NP) in rats and its effect on activation of astrocytes.Methods In vivo experiment Sixty-four male SPF Sprague-Dawley rats,aged 6-8 weeks,weighing 180-200 g,were randomly divided into 3 groups using a random number table:control group (group C,n =16),sham operation group (group S,n =24) and group NP (n =24).The animals were anesthetized with intraperitoneal pentobarbital sodium,the L5,6 spinal nerves of the left side of the rat were gently separated and exposed,tightly ligated with 5-0 silk suture and transected.In group S,the L5,6 spinal nerves of the left side of the rat were only exposed.In group C,no operation was performed.Mechanical pain threshold was measured at day 1 before operation and days 1,3,5,7,10 and 14 after operation.The expression of IL-17,IL-6,IL-1β and tumor necrosis factor-alpha (TNF-α) mRNA in the spinal dorsal horn was determined using quantitative real-time PCR at day 7 and day 14 after operation.At day 7 after operation,the activation of astrocytes in the spinal dorsal horn was detected.In vitro experiment Primarily cultured astrocytes of neonatal rats were randomly divided into 4 groups using a random number table:control group (group C,n=22),10 ng/ml IL-17 group (I10 group,n=18),50 ng/ml IL-17 group (I50 group,n-18) and 100 ng/ml IL-17 group (I100 group,n=22).In I10,I50 and I100 groups,the astrocytes were incubated with the culture medium containing 10,50 and 100 ng/ml IL-17,respectively.The proliferation of astrocytes was detected by MTT at 24,48 and 72 h of incutation or culture.The expression of IL-6,IL-1β and TNF-α mRNA was determined using quantitative real-time PCR.Results In vivo experiment Compared with group C,the mechanical pain threshold was significantly decreased at 3-14 days after operation,the expression of IL-17,IL-6 and IL-1β mRNA in the spinal dorsal horn was up-regualted at 7 days after operation,and the activation of astrocytes was increased in group NP,and no significant change was detected in the mechanical pain threshold at each time point after operation in group S.In vitro experiment Compared with group C,the proliferation of astrocytes was significantly increased at 48 h of incubation in I10 and I50 groups,the proliferation of astrocytes was significantly increased at 48 and 72 h of incubation,and the expression of IL-6 and IL-1β mRNA was up-regulated in I100 group,and no significant change was found in the proliferation of astrocytes in group S.Conclusion Up-regulated expression of IL-17 in spinal dorsal horns may be involved in the maintenance of NP,and the mechanism is related to promoted activation of astrocytes and induced inflammatory responses in rats.

Objective To evaluate the effect of Diver CE aspiration thrombus catheter used in patients during the recovery of acute myocardial infarction (AMI) with percutaneous coronary intervention (PCI). Methods A total of 50 AMI patients were enrolled from January 2010 to December 2011. Twenty-ifve patients received PCI and used the Diver CE aspiration thrombus catheter (aspiration group);and the other 25 patients received routine PCI (routine PCI group). The basic characteristics, PCI characteristics, myocardial perfusion indexes and clinical data were compared and analyzed. Results There was no statistically difference between two groups in basic characteristics. Compared with the routine PCI group, higher rate of myocardial blush grade 3 but lower thrombus scores were found in the aspiration group. The incidence of no-lfow or slow-lfow were also lower in aspiration group (all P<0.05). The LVEF after 6 and 12 months were higher in the aspiration group (both P<0.05), no difference in MACE between the two groups. Conclusions Using the Diver CE thrombus aspiration in PCI is a simple and safe by method for treatment of acute myocardial infarction with high thrombosis burden. It can improve reperfusion decrease the rates of no-relfow on slow-relfow and can improve the long-term left ventricular function.

Objective: To evaluate the correlative risk factor of culprit lesion plaque composition in patients with acute coronary syndrome (ACS).
Methods: A total of 55 ACS patients with 55 culprit vessels were divided into 2 groups:Unstable angina group, n=40 and AMI group, n=15. Coronary angiography (CAG) was conducted in all patients, the minimal lumen area and plaque burden at the minimal lumen area and the largest necrotic core were examined. Intravascular ultrasound with virtual histology (IVUS-VH) were performed to calculate the percentages of necrotic core area (NC%), dense calcium (DC%), ifbro-fatty area (FF%), ifbrous area (FI%) and to calculate the remodeling index. Plasma levels of matrix metalloproteinase-9 (MMP-9), oxidative low density lipoprotein (Ox-LDL) and hypersensitive c-reactive protein (hs-CRP) were also compared between 2 groups.
Results: In AMI group and UAP group, NC%at minimal lumen area were (27.90 ± 9.72)%and (19.41 ± 11.53)%, t=-2.533, P=0.014;NC%at the largest necrotic core were (39.09 ± 8.82)%and (31.99 ±1 2.35)%, t=-2.032, P=0.047;plasma levels of MMP-9 were (14.63±2.71) μg/L and (12.65±2.72) μg/L, t=-2.411, P=0.019;Ox-LDL were (196.21±25.89) μg/L and (159.57±33.39) μg/L, t=-3.832, P=0.001;hs-CRP were (1.57±1.93) mg/L and (0.31±0.33) mg/L, t=-2.514, P=0.024 respectively. Plaque composition was not related to plasma levels of MMP-9, Ox-LDL and hs-CRP. In the largest necrotic core of cruprit lesion and the minimal lumen area, MLA were (5.25±2.15) mm2 and (3.45±1.07) mm2, t=-6.348, P=0.001;NC%were (33.93±11.85)%and (21.73±11.62)%, t=-8.752, P=0.001 respectively.
Conclusion: ①Among ACS patients, AMI patients had the larger necrotic core area at cruprit lesion and higher MMP-9, Ox-LDL and hs-CRP.②There was no largest necrotic core composition at the minimal lumen area in the same cruprit lesion.③Plaque composition was not related to MMP-9, Ox-LDL and hs-CRP.
