1.Relationship between serum CRP CH50 C3 concentration in premature labor and subclinical chorioamnionitis

Objective To determine the early diagnostic value of serum C -reactive protein (CRP)、 total complement( CH50 ) activity and complement 3 (C3) concentration of premature women in the subclinical infection.MethodsSolid phase enzyme linked immunosorbent assay ( ELISA method ) was used for measurements of serum CRP levels. According to modified Mayer simple tubular colorimetric method and simple agar immunodiffusion method, we determined the CH50、C3 concentration respectively.Results Premature women with subclinical chorioamnionitis showed higher serum CRP concentration than that of normal pregnancy women (P<0.01). In the subclinical chorioamnionitis, the more serious the infection, the higher the CRP concentration, while CH50 and C3 concentrations were correspondently decreased. Serum CRP concentration in premature labor was the most sensitive index that suggested subclinical chorioamnionitis. ConclusionsSerum CRP、CH50 and C3 concentrations in premature labor are simple,reliable and accurate indexes for early diagnosis of subclinical infection.
2.Impact Factors of Hand Hygiene among Healthcare Workers:Investigation and Analysis

OBJECTIVE To know the compliance and it′s impact factors of hand hygiene of healthcare workers.METHODS The wards and healthcare workers were sampled randomly.Survey the compliance,qualified rate and impact factors of hand hygiene of healthcare workers when they didn′t know to be observed by special training staffs.RESULTS The compliance and the qualified rate of hand hygiene was 30.2% bothly,and be affected by the time before or after operation,occupations and work post.The factors causing low qualified rate were anenough time and improper technique for washing hand,and drying hand wrong.CONCLUSIONS The hand hygiene for healthcare workers in hospitals should be strengthened.
3.Peripheral blood monocyte subpopulations in patients with syphilis

Objective To analyze the changes in peripheral blood monocyte subpopulations in patients with primary, secondary and latent syphilis. Methods Flow cytometry was used to detect CD14highCD16- and CD14+CD16+ monocyte subpopulations in peripheral blood from 58 patients with untreated syphilis, including 36 cases of latent syphilis,8 cases of primary syphilis and 14 cases of secondary syphilis, as well as from 65 normal human controls. Restflts Compared with the normal controls, the proportion of CD14+CD16+ monocytes among total monocytes was significantly elevated (12.0% ± 5.0% vs 6.0% ± 3.3%, t = 7.25, P < 0.01), while that of CD14highCD16- monocytes was down-regulated (88.0% ± 5.1% vs 94.0% ± 3.5%, t = -7.20, P < 0.01). No statistical difference was observed in the proportion of CD14+CD16+ or CD14hhighCD16- monocytes among the patients with primary syphilis, secondary syphilis and those with latent syphilis (all P > 0.05). Conclusions The changes in peripheral blood monocyte subpopulation in patients with untreated syphilis may be associated with the permanent infection of Treponema pallidum, but have no obvious correlation with clinical stage of syphilis.
4.The study of clonidine on the patients in PACU

OBJECTIVE To study the influenceof clonidine on the hemadynamics and plasma catecholamine of the postoperative patients in PACU. METHODS Twenty eight patients suffered from severe OSAHS who underwent modified UPPP and monitored in the PACU after operation were divided randomly into two groups:clonidine group and control group. Both group received the same protocol of sedation and analgesia. Patients in clonidine group received clonidine 3ug/kg injected slowly when entered the PACU,and patients in control group received same volume of NS. Esmolol and Nitroglycerin were used to adjust the hemadynamics. The changing of hemadynamics was recorded,blood samples were taken before operation,2 hours after entering PACU,before discharge from PACU to determine the changing of plasma catecholamine. RESULTS The hemadynamics was more stable in clonidine group than that in control group and less vasoactive agents were needed. The plasma catecholamine was much lower in clonidine group 2 hours after entering PACU and before discharge from PACU than that in control group. CONCLUSION Clonidine could be administered to stabilize the hemadinamics,to lower the level of stress hormone and could improve the recovery of the patients.
5.Disposable Over-shoes Using for Nosocomial Infection Control in Intensive Care Unit

OBJECTIVE To analyze the effect of the disposable over-shoes for the control of nosocomial infection of the intensive care units(ICU). METHODS The effects of the disposable over-shoes for the environment contamination and nosocomial infection control of the surgical ICU were investigated. RESULTS The mean of air bacteria colony counts when disposable over-shoes were worn was lower than that when without their use by healthcare workers (P0.05). The rates of nosocomial infection beteen them were 21.5‰ and 17.1‰,respectively. CONCLUSIONS The use of disposable over-shoes can't improve the environment quality and is not benefit for the control of nosocomial infection of surgical ICU.
6.Predictive value of lipoprotein-associated phospholipase A2 on perioperative myocardial injury in patients with stable angina after percutaneous coronary intervention

Objective To explore the correlation between lipoprotein-associated phospholipase A2 (Lp-PLA2) and perioperative myocardial injury (PMI),and to discuss the predictive value of Lp-PLA2 in patients with stable angina after percutaneous coronary intervention (PCI).Methods A total of 222 consecutive patients with stable angina,who were admitted to Yixing Municipal People's Hospital,Jiangsu Province,China to receive PCI during the period from June 2015 to March 2017,were enrolled in this study.The patients' baseline data as well as the distribution pattern of coronary lesions,were recorded.According to the paclitaxel-PCI and the surgical cooperative study (SYNTAX) score,the severity of target vascular lesions was assessed,which was classified into low score group (0 to 22 points),middle score group (23 to 32 points) and high score group (≥33 points).The preoperative blood lipid level and renal function,both preoperative and postoperative Troponin T (cTnT),high sensitive C reactive protein(hs-CRP),as well as the postoperative Lp-PLA2 were tested.Results After the procedure,the Lp-PLA2 levels in patients with normal cTnT value (n=155) and in patients with elevated cTnT value (n=67) were(122.21±43.80) ng/ml and (224.53±65.00) ng/ml respectively (P<0.05).SYNTAX score analysis showed that low score group had 120 patients,middle score group had 78 patients and high score group had 24 patients,the Lp-PLA2 levels of the above three groups were (119.51±51.96) ng/ml,(178.67±61.49) ng/ml and (233.16±61.32) ng/ml respectively,the differences were statistically significant between each other among the three groups (P<0.05).Pearson correlation analysis indicated that a parallel correlation existed between Lp-PLA2 levels and postoperative cTnT values (R=0.492,P<0.05).Logistic regression analysis revealed that Lp-PLA2 was the independent risk factor for elevated cTnT value during the perioperative period of PCI (OR=7.377,95%CI=3.368-16.156,P<0.05).The area under ROC curve of Lp-PLA2 was 0.896 (95%CI=0.874-0.945,P<0.001),the best cut-off point was 179 ng/ml,and the sensitivity and specificity for the diagnosis of PMI were 92.2% and 66.7%,respectively.Conclusion Lp-PLA2 levels are closely correlated with the increased cTnT values after PCI,and the preoperative high level of Lp-PLA2 is the independent risk factor for PMI after PCI.

Objective To investigate the laparoscopic feature and its relationship with clinical manifesta-tions in infertile patients with endometriosis. Methods From Jan.2013 to Jan.2017,infertile patients who received hysteroscopy and hysteroscopy combination surgery in Ganzhou People′s Hospital were enrolled in this ret-rospective study.The basic clinical data and the distribution of endometriotic lesions were recorded and analyzed in patients with endometriosis simultaneously. Analysis was performed about the clinical manifestations and the rAFS stage of the patients. Results 204 patients are totally enrolled in the study. Among them,120 patients(58.8%) had endometriosis. The presence of menorrhea,dyspareunia,tenesmus,chronic pelvic pain,hypermenorrhea, adnexal mass,restricted uterine mobility,tenderness of adnexa,and tenderness nodules of posterior vaginal fornix are clinically statistically significant.In addition,the presence of ovarian endometrioma is also a significant clinical factor that is close related with the rAFS stage of infertile patients with endometriosis. Moreover,uterosacral liga-ment,rectovaginal septum,vagina and bowel were more common infiltrated in stage Ⅲ and stage Ⅳ endometrio-sis. Conclusions For infertile patients,detailed case history,overall bimanual examination and trans-vaginal ultrasound examination are very important to help diagnosis endometriosis preoperatively and provide evidence for the selection of individualized treatment strategies.
8. Effect of menopausal status on onset of impaired fasting glucose and type 2 diabetes mellitus

Objective:
To investigate the relationship between menopausal status at different FPG levels and the risk of new onset of impaired fasting glucose (IFG) and type 2 diabetes mellitus (T2DM).
Methods:
Female subjects were selected from the females who joined in the epidemiological survey between May and September in 2009 in six counties of Guanlin Village and Xushe Village in Yixing City in Jiangsu Province by cluster sampling method. Subjects with diabetes at baseline or pre-menopause with age above 65 years old or surgical menopause were excluded. Finally there were 2 084 eligible subjects included in this study. According to FPG at baseline, subjects were categorized into two groups as normal FPG group (FPG<5.6 mmol/L) and IFG group (5.6≤FPG≤6.9 mmol/L). Follow-up study was conducted from May in 2014 to January in 2016. Cox regression model was used to investigate the association between menopausal factors and IFG and T2DM among groups based on FPG.
Results:
The average age of subjects at baseline was 58.08 (51.74-65.82) years old, and the number of post-menopausal women was 1 631 (78.3%). The number of subjects in normal FPG group was 1 569 (75.3%), and in IFG group was 515 (24.7%). There were 104 subjects with new onset T2DM among which 34 subjects from normal FPG group and 70 subjects from IFG group. And there were 199 subjects with new onset IFG in normal FPG group. Among subjects with normal FPG, the incidence density of IFG in post-menopausal women (2 752/100 000 person-years) was statistically higher than that in premenopausal women (1 670/100 000 person-years) (
9.Effect of calcium carbonate combined with vitamin K on bone metabolism indexes, Th1 cytokines and safety in children with glucocorticoid-induced osteoporosis

Objective:To investigate the effect of calcium carbonate combined with vitamin K on bone metabolism indexes, Th1 cytokines and safety in children with glucocorticoid-induced osteoporosis.Methods:A total of 96 children with glucocorticoid-induced osteoporosis were enrolled in this study. They were randomly divided into control group and study group, 48 cases in each group. The control group was treated with calcium carbonate, and the study group was treated with calcium carbonate combined with vitamin K. The bone metabolism index serum total bone type Ⅰ collagen N-terminal peptide was compared between the two groups before and after treatment. PINP), serum β-Carboxy I terminal peptide ( β-CTX), osteocalcin (osteocalcin, P<0.05), and serum β-CTX. BGP), serum bone alkaline phosphatase (BALP), Th1 cytokine interferon- γ (IFN- γ), interleukin-2 (IL-2) levels and adverse reactions were also observed. Results:There was no statistical significance in the general data of the two groups (all P>0.05). Compared with that before treatment, bone mineral density of femoral neck increased in both groups after treatment, and the improvement in the study group (-1.02±0.49) was more significant than that in the control group (-1.52±0.65) ( t=4.26, P<0.001). Compared with those before treatment, the levels of PINP, β-CTX, BGP and BALP in 2 groups after treatment were decreased, and compared with those of the those of the control group (PINP: 31.65±6.58; β-CTX: 0.34±0.05; BGP: 4.95±1.28; BALP: 40.54±7.84), all indexes of the study group after treatment (PINP: 26.54±7.06; β-CTX: 0.24±0.03; BGP: 3.05±1.09; BALP: 35.96±7.02) improved significantly ( t=3.67, P<0.001; t=11.88, P<0.001; t=7.83, P<0.001; t=3.02, P<0.003). Compared with that before treatment, IL-2 level was increased while IFN- γ level was decreased in both treatment groups. Additionally, in comparison to the control group (IL-2: 163.89±30.85; IFN- γ: 196.61±21.05), IL-2 level (198.32±32.14) was higher and the IFN- γ level (163.25±18.43) was lower in the study group after treatment ( t=5.35, P<0.001; t=8.26, P<0.001). The incidence of adverse reactions between the two groups was not statistically significant ( χ2=0.15, P=0.695) . Conclusion:Calcium carbonate combined with vitamin K in the treatment of children with glucocorticoid-induced osteoporosis can improve bone metabolism indexes and Th1 cytokine levels in children, and the clinical therapeutic effect is good.
10.The questionnaire survey about 0-24 months infantile breastfeeding and service requirement of child care clinic in Tianjin Binhai district

Objective To investigate 0-24 months infantile breastfeeding and service requirement of child care clinic in Tianjin Binhai district. Methods A total of 350 cases from June 1,2014 to June 1, 2016 were adopted by questionnaire method about breastfeeding and service requirement. Results In the survey, all of them were breastfeeding, and the 0-6 months breastfeeding rate was 84.4%(295/350). They were divided into three groups: urban, rural and external population, the main of population was urban(64.0%, 224/350). The breastfeeding rate was closely relate to the education of mother, graduate and above almost above 63.7%(223/350);exclusive breastfeeding of three groups were 25.4%(57/224), 25.8%(16/62), 34.4%(22/64) in urban, rural and external population respectively. Generally lower the national level because of the food supplement,4-5 months was 31.8%(21/66), 6-9 months was 65.4%(34/52),10-12 months was 85.0%(51/60). Insufficient breastmilk, mother was ill, working mother, think should be the four main reasons in the weaned process. Conclusions There are still some problems for the ideas and methods of breastfeeding in child care clinic, still need to strengthen the nursing management of breastfeeding.
