1.Suppressor of cytokine signaling-3, interleukin-6 and insulin resistance

Suppressor of cytokine signaling (SOCS)-3 is a negative regulator of interleukin (IL)-6 signal transduction. The results showed that IL-6 was not only correlated with insulin resistance but also capable to induce the expression of SOCS-3.Thus the high expression of SOCS-3 mRNA may contribute to one of the mechanisms of IL-6 dependent insulin resistance.
2.Operative treatment of posttraumatic instability of the elbow: evaluation and outcome

[Objective]To retrospectively analyze the therapy comparison between the unilateral hinged external fixator assisting the internal fixation, soft tissue repair and reconstruction and routine surgery for elbow instability of fracture dislocation.[Method]A total of 117 cases of elbow fracture dislocation were collected from Jan. 2001 to Jun. 2005. Some patients were treated by unilateral hinged external fixator, and others were treated by routine surgery and casting external fixation.The JOA assessment was applied for the pain, function, arc of motion, articular instability and deformity of elbow joint before surgery and after operational follow-up. [Result]Eighty-six cases were followed up by telephone and mail appointment.The average follow-up time was 18.6 months (from the range of 8 months to 30 months).The function and arc of motion of two group patients were assessed by JOA in post-operational follow-up, showing statistical significant difference (P0.05). The total assessment score between two groups indicates statistical significant difference.[Conclusion]The operation in combination with unilateral hinged external fixator for treating elbow instability of fracture dislocation will enhance lateral stability of elbows, guaranteen early safe elbow function exercises, facilitate stable environment for ligaments healing,suggesting a good subsidiary operational method.
4.Analysis of distribution and drug resistance of bacteria in 1 -6 months infants with lower respiratory infection

Objective To investigate the distribution and drug resistance of bacteria in 1 -6 months infants with lower respiratory infection(LRI).Methods Induced sputum was extracted from 326 infants with LRI who were 1 -6 months.Antibiotic susceptibility tests were performed after bacteria had been identified.Results 61 cases were detected pathogenic bacteria and the detection rate of bacteria was 18.71%.5 cases were detected two kinds of bacte-ria.66 bacterial strains were isolated among which gram -positive bacteria(33 strains)accounted for 50.00% and gram -negative bacteria(33 strains)accounted for 50.00%.Staphylococcus aureus was the most common gram -pos-itive bacteria and Streptococcus pneumoniae was the second.13 strains were methicillin -resistant Staphylococcus au-reus(MRSA).Hemophilus influenzae was the most common gram -negative bacteria,followed by Klebsiella pneumo-nia and Escherichia coli among which ESBL positive Klebsiella pneumonia were 5 cases and ESBL positive Escherich-ia coli were 4 cases.The common gram -positive bacteria had higher rate of penicillin resistance.MRSA had higher rate of penicillin,oxacillin,erythomycin and clindamycin resistance.Resistant strains to vancomycin and rina thiazole amine were not found.The common gram -negative bacteria had higher rate of ampicillin,ampicillin/shu tan,cefazo-lin and ceftriaxone resistance and had lower rate of cefepime,ceftazidime,piperacillin/he azole temple and imipenem resistance.Conclusion The common pathogenic bacteria in 1 -6 months infants with lower respiratory infection were Staphylococcus aureus,Hemophilus influenzae,Streptococcus pneumoniae,Klebsiella pneumonia and Escherichia coli. We should pay attention to the common antibiotic resistance.MRSA and ESBL positive bacteria were the common mul-tiple drug resistant bacterias.Reasonable selection of antibiotics should be based on susceptibility results earlier.
5.Comparison of the efficacy and safety between the drug winning the bidding for centralized procurement and the original drug of ticagrelor in acute coronary syndrome patients after percutaneous coronary intervention

OBJECTIVE To compare the efficacy and safety between the drug winning the bidding for centralized procurement and the original drug of ticagrelor in acute coronary syndrome (ACS) patients one year after percutaneous coronary intervention (PCI). METHODS Overall 420 ACS patients treated with PCI in our hospital from July 2021 to June 2023 were divided into centralized procurement group (156 cases) and original drug group (264 cases) according to the different varieties of ticagrelor. All patients were given Aspirin enteric-coated tablets 100 mg regularly, once a day; on this basis, patients in the centralized procurement group were given Ticagrelor tablets winning the bidding, while patients in the original drug group were given the original drug of Ticagrelor tablets. The dosage of Ticagrelor tablets taken by both groups of patients was 90 mg, twice a day, and the course of treatment was 12 months. The blood routine indicators were compared before treatment and one week after treatment, while the incidence of net adverse clinical events (NACE), major adverse cardiovascular and cerebrovascular events (MACCE) and bleeding events, and the survival rate without MACCE and bleeding events during a one-year follow-up after PCI were also compared between two groups. RESULTS There were no statistically significant differences in blood routine indicators before treatment and one week after treatment, as well as the incidence of NACE, MACCE and bleeding events, survival rate without MACCE and bleeding events during a one-year follow-up after PCI between the two groups (P>0.05). CONCLUSIONS During a one-year follow-up after PCI, the efficacy and safety of the drug winning the bidding for centralized procurement of ticagrelor are comparable to the original drug.
6.The effect of 1,25(OH)D3 deficiency in the secondary dentin formation and mineralization and caries of the mice.

OBJECTIVETo determine the role of 1,25(OH)2D3 on the secondary dentin formation and mineralization of the mice.
METHODSThe differences of the mandible mineralization between the wild-type and 1-alpha-hydroxylase gene knockout mice at 6 weeks old were assessed by hematoxylin-eosin (HE) staining, immunohistochemistry, alkaline phosphatase (ALP) histochemistry staining.
RESULTSThe ratio of caries were increased significantly, while the secondary dentin was reduced significantly, the deposition of type I collagen and osteocalcin on the secondary dentin of occlusion surface was decreased significantly, but the deposition of the Biglycan on the dentin was increased significantly, the active of ALP on the odontoblasts were reduced significantly in 1-alpha-hydroxylase gene knockout mice compared to that in the wild-type littermates.
CONCLUSION1,25(OH)2D3 deficiency lead to a defect in the secondary dentin formation and mineralization and caries of the mice.

9.Clinical effect of the relative poking reduction for the treatment of the femoral neck fractures with insertion and forward angle.

OBJECTIVETo explore the reduction and clinical effect in the near future of percutaneous K-wire relative poking reduction techniques for difficult to reduction of complex insert and forward angulation femoral neck fracture.
METHODSThe clinical data of traumatic femoral neck fractures treated in our hospital from January 2012 to September 2015 were retrospective analysis, 15 patients with skeletal traction and distal elevation still can not eliminate the fracture angulation deformity were treated by percutaneous K-wire relative poking reduction technique to correct the angular and using cannulated screw fixation, including 5 males and 10 females, aged from 44 to 72 years with an average of 60.06 years old. The fractures were Garden type III. The reduction effect was evaluated according to Garden alignment index and clinical evaluation in the near future outcome was assessed according to Harris Hip Score.
RESULTSAll patients were healed by first intention without pulmonary infection and deep vein thrombosis. The patients were followed up for 6 months to 3 years, all the fractures healed, and 1 case had limited necrosis of the femoral head. According to Garden alignment index, 14 cases were level I reducation, and 1 case was level II. Limb shortening was 1 to 5 mm with an average of (2.73±1.37) mm. Functional evaluation according to Harris score standard, at the latest follow-up the average value of pain, function, deformity and joint activity were 42.1±2.5, 37.2±4.6, 3.2±0.5, 4.1±0.3, the total average value was 86.6±9.5;13 cases were excellent, 1 case was good, 1 case was poor.
CONCLUSIONSPercutaneous K-wire relative poking reduction technique to correct the difficult to reduction of complex insert and forward angulation of the femoral neck fracture is simple, repeatable, and achieve the precision of the reset;the effect of blood supply of the hip joint is small, which provides favorable conditions for fracture healing.
10.Design and preclinical study of a fracture reduction and fixation system

Objective To investigate the safety, efficacy, and clinical value of a fracture reduction and fixation system. Methods The design and application of components [Kirschner wire (K-wire) limiter and K-wire guiding locking device] of the fracture reduction and fixation system were described. Six surgeons implanted K-wires into synthetic bones using the K-wire limiter or by perception respectively, and the length of K-wire protrusion was assessed. K-wire guiding locking device combined with locking plates and two 1.5 mm K-wires, two 1.5 mm K-wires, as well as two 2.0 mm K-wires were used to fix a fracture model constructed by synthetic bones, and the torque at failure was measured.
