1.THE MORPHOLOGICAL OBSERVATIONS AND BONE HISTOMOPHOMETRIC STUDY OF THE CHICKEN EMBRYO FEMUR IN EXPERIMENTAL FLUOROSIS

The femur of chicken embryo in experimental fluorosis was observed under light and electron microscope, and studied by bone histomorphometric method. The results showed that most of osteoblasts were cuboid and crowded together on the surface of bone trabecula in experimental group. The osteoid was increased and mineralized bone was diminished. The bone reformation indexes were higher significantly and the bone resorption indexes did not show differences between the two groups. The cross striations of type Ⅰ collagen fibrils in osteoid layers of experimental group were indistinct and bone lamella were jumble array. Our results suggest that though fluoride has a stimulating action on the embryonic osteoblasts and enhanced the bone formation, the osteoblasts produced abnormal collagen fibrils which caused disorder of osteoid mineralization and led to osteomalacia.
2.Effect of Folic Acid on the Homocysteine level and Flow-mediated Vasodilation and Cognitive Function in the Treatment of Elderly Patients with Cerebral Infarction

OBJECTIVE:To discuss the effect of folic acid on the homocysteine(Hcy)and flow-mediated vasodilation and cog-nitove function in the treatment of elderly patients with cerebral infarction. METHODS:The data of 198 elderly patients with cere-bral infarction was randomly divided into the observation group and the control group by different medication. Control group was treated with conventional treatment;based on the treatment of control group,observation group was orally treated with Folic acid tabelts 5 mg,once a day. The treatment course of 2 groups was 8 weeks. The Hcy level and flow-mediated vasodilation(FMD)be-fore and after treatment,mini-mental state examination (MMSE) score and incidence of adverse reactions in 2 groups were ob-served. RESULTS:After treatment,Hcy level in observation group was significantly lower than before and control group,FMD was significantly higher than before and control group(P<0.01). MMSE score in observation group was significantly higher than control group(P<0.01). There were no obvious adverse reactions in 2 groups during treatment. CONCLUSIONS:Based on the con-ventional treatment,folic acid can effectively reduce the Hcy level of elderly patients with cerebral infarction,improve the cogni-tive function and reduce the brain tissue damage,with good safety.
3.Analysis of imaging and clinical feature of leukoencephalopathy with hearing loss first complaint

Objective:To investigate the imaging and clinical feature of leukoencephalopathy with hearing loss first complaint.Method:The head MRI and clinical feature of 7 cases leukoencephalopathy with hearing loss first complaint were retrospectively analysed,including their place,shape,range and blood plasma very-long-chain fatty acid(VLCFA).Result:The head MRI of 2 cases shows symmetry distributed long T_1 and T_2 signal like butterfly aliform in white matter beside both cornu posterious ventriculi lateralis.The head MRI of 5 cases shows abnormal long T_1 and T_2 signal in different part in Alba.The pathological changes often located in white matter of frontal lobe and secondly beside both eornu posterious ventriculi lateralis.The shape shows little patching in 7 cases.The range only shows beside cornu posterious ventriculi lateralis in 2 cases.The range only shows in frontal lobe in 1 cases.Two or more than two parts shows in the rest 5 cases.The result of VLCFA is normal in 3 cases.The rest 4 cases refused to have examination of VLCFA. Conclusion:The patient who firstly complained of hearing loss should be routinely done head MRI.The head MRI imaging could clearly show abnormal white matter in order to avoid omitting existent pathological changes.
4.Management of advanced head and neck cancer with intertissue injection of 32P radioisotope

OBJECTIVE To observe the effect of 32P intertissue injection on advanced head and neck cancer. METHODS Eight patients with advanced head and neck cancer were treated with 32P injection into the tumors according to the size of the cancer as determined by CT scan. RESULTS Local tumors were controlled evidently and the pain of the patients were alleviated. The survival periods of the 8 cases ranged from 3 to 18 months (average 14.2 months). CONCLUSION 32P intertissue injection offer an useful method for patients with advanced head and neck cancer .
5.Expression of fascin and correlation with MVD in sinonasal inverted papilloma.

OBJECTIVE:
To investigate the expression of Fascin in sinonasal inverted papilloma (SNIP) and the relationship between Fascin expression and microvessel density (MVD).
METHOD:
The expression of Fascin and CD34 were determined by immunohistochemistry in 30 cases of SNIP, 12 cases of nasal squamous cell carcinoma (NSCC) and 10 cases of normal inferior turbinate (IT) tissues. Meanwhile, CD34, measure of MVD, was counted at the same time.
RESULT:
Fascin was expressed in 17 of 30 (56.6%) cases of SNIP, 10 of 12 (83.3%) cases of NSCC and 2 of 10 (20.0%) cases of normal IT tissues. Expression of Fascin was significantly increased in SNIP and NSCC compared to normal tissues (P < 0.01). MVD counts was 23.64 +/- 2.74, 45.10 +/- 5.45, 74.76 +/- 7.32, respectively (P < 0.01). There was a positive correlation between the expression of Fascin and MVD counts.
CONCLUSION
The high expression of Fascin and angiogenesis are significantly associated with biological behaviour of SNIP. Fascin may play an important role in the pathogenesis of SNIP and NSCC and synergic role with angiogenesis in progression of SNIP.

6.Vitamin D receptor gene polymorphisms of sporadic myasthenia gravis in 267 Chinese Han patients

Objective To explore the associations between vitamin D receptor ( VDR) Fok- Ⅰ and Apa- Ⅰ polymorphisms and myasthenia gravis (MG) in Chinese Han population.Methods Polymorphisms of VDR Fok- Ⅰ and Apa-Ⅰ were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method.The frequencies of genotypes and hyplotypes were compared among 286 normal controls and 267 MG patients in different subgroups classified by gender,age of onset,presence of thymoma,and Osserman classification sat maximal severity in the follow-up.The association between the genotypes and maximal severity of MG and short-term glucocorticoid treatment were also investigated.Results There were no significant differences in frequencies of genotypes and hyplotypes of both Fok-Ⅰ and Apa-Ⅰ between MG group and control group,and among subgroups of MG.The Fok- Ⅰ showed no statistical difference between the patients with better and less improvement after short-term treatment of glucocorticoid.The frequency of Apa-Ⅰ alleles in the patients with better improvement (55/186,29.6% ) significantly differed from the less improved group ( 7/48,14.6%,OR = 2.46,95% CI 1.04-10.43,x2 = 4.400,P = 0.036).The patients with the genotype A A/Aa were more likely to improve better after the treatment(48/93,51.6%) than in the worse group(7/24,29.2%,OR =2.59,95% CI 0.98-14.60,x2 =3.858,P= 0.049).Conclusions Alleles and genotypes of VDR-Fok- Ⅰ and Apa-Ⅰ were not found to be related with MG onset and severity.MG patients with VDR-Apa-A allele may have better improvement short-term treatment of glucocorticoid.

8.Development and evaluation of reliability and validity of new scales measuring severity of myasthenia gravis

Objective To develop new scales on the severity of myasthenia gravis ( MG) which are suitable for clinical practice in China .Methods A final version of new comprehensive scale was developed by item optimization from a preliminary scale established by combination of Delphi method and previous evaluation of items from source scales .With the original quantitative data from 60 MG patients, items were selected on the basis of interobserver and test-retest reliability of items , the contributions of items on internal consistency and construct validity of the preliminary scale .Evaluation of this final scale includes internal consistency, interobserver and test-retest reliability, construct validity and correlation with currently used scales.A self-rating version was simplified from the final scale by including items that can be measured by patients, which was then evaluated in 57 generalized MG patients.Results One item ( palpebral fissure ) was deleted from the preliminary scale due to poor interobserver and test-retest reliability.The remaining nine items constituted the final comprehensive scale which covered six commonly involved muscle groups in MG.All items in the comprehensive scale had fair to excellent interobserver reliability ( K=0.43-0.80, P<0.01) and fair to excellent test-retest reliability ( K =0.41 -1.00, P<0.01) .The comprehensive scale had acceptable internal consistency (Cronbach ɑ=0.63), excellent interobserver reliability (intraclass correlation coefficient (ICC) =0.88, P<0.01) and test-retest reliability (ICC=0.86, P<0.01).The comprehensive scale had four common factors and good construct validity ( cumulative dedication rate 68.46%).There was high correlation between the comprehensive scale and Myasthenia Gravis Composite ( MGC ) , Absolute and Relative Score of MG ( ARS-MG ) and Quantitative Myasthenia Gravis score (QMGs) ( r=0.87 -0.90, P<0.01), and moderate correlation between the comprehensive scale and Myasthenia Gravis Activities of Daily Living (MG-ADL) (r=0.68, P<0.01).A self-rating scale was developed by deleting ocular and facial items and keeping the remaining six items , which covers the bulbar , limb, neck and respiratory muscles.Internal consistency (Cronbach ɑ=0.69), interobserver reliability and test-retest reliability (ICC=0.90 for both, P<0.01) of the self-rating scale were better than those of the comprehensive scale.The self-rating scale had two common factors and good construct validity ( cumulative dedication rate 64.24%) .There was high correlation between the self-rating scale and the comprehensive scale, MGC and ARS-MG (r=0.74-0.86, P<0.01), and moderate correlation between the self-rating scale and QMGs and MG-ADL ( r=0.55 -0.68, P<0.01 ) .Conclusions Item optimization was performed from a preliminary scale established by Delphi method .A new comprehensive scale and a self-rating scale were developed .The comprehensive scale and self-rating scale had good reliability and validity , which can be used to assess the severity of MG reliably and effectively .
9.Progress of diagnosis and treatment of Osgood-Schlatter disease

Osgood-Schlatter disease (OSD) is a common strain disease in adolescents, which is more common in youth sports enthusiasts, athletes and soldiers. The clinical manifestations of OSD are typical, and the diagnosis can be made based on the medical history, physical examination and knee X-ray examination, but it needs to be differentiated from other diseases that may cause anterior knee pain. The risk factors of OSD development include males, 12-15 years of age in males and 8-12 years of age in females, high body mass index, skeletal-muscular anatomical variants, and large amount of exercise, etc. OSD is a self-limited disorder that gradually resolves as the skeleton maturates. Most patients do not need treatment or can also be treated with conservative methods such as exercise therapy, medications and physical therapy, but may be left with sequelae that affect motor function and quality of life. Clinical attention should be paid to it adequately. If the conservative treatment is ineffective, active operation should be performed. The principle of operation is to remove bone fragments and reconstruct tibial tuberosity. Common surgical procedures include traditional open surgery and arthroscopic surgery. Arthroscopic surgery, with its advantages of minimally invasive and rapid recovery, is currently advocated as the best surgical procedure, but the mid- and long-term efficacy is unclear.
10.Accuracy of infiltrating B ultrasound guided A-scan segment biometric measurement of axial length in dense cataract

AIM:To observe the accuracy of infiltrating B ultrasound guided A-scan segment biometric measurement of axial length in dense cataract.METHODS: Perspective study. A total of 86 patients(90 eyes)with dense cataract were selected from Chengdu Aier Eye Hospital from August 2020 to August 2022. There were 61 males(59 eyes)males and 25 females(31 eyes)females, with an average age of 66.49±14.55 years. The axial length(AL), anterior chamber depth(ACD)(including corneal thickness), corneal curvature(K), lens thickness(LT), central corneal thickness(CCT), and white-to-white(WTW)were measured preoperatively by contact A-scan, infiltrating B ultrasound guided segmented A-scan, and IOL Master 700, respectively. At 1 wk postoperatively, AL was retested by IOL Master 700 in aphakic mode. Furthermore, the agreements and correlations of AL obtained by the three kinds of devices were analyzed.RESULTS:The AL measured by contact A-scan and infiltrating B ultrasound guided segmented A-scan were 23.40(22.63, 23.89)mm and 23.70(23.04, 24.25)mm, respectively, and the AL measured by IOL Master 700 at 1 wk postoperatively was 23.72(23.01, 24.27)mm. There were statistical significant difference in AL measured by the three methods(P=0.018), while there were no statistical significant difference in AL measured by infiltrating B ultrasound guided segmented A-scan and IOL Master 700(P=0.991). Bland-Altman analysis showed that there was a good agreement in AL measured by infiltrating B ultrasound guided segmented A-scan and IOL Master 700(P=0.0809). The AL measured by infiltrating B ultrasound guided segmented A-scan and IOL Master 700 was positively correlated(rs=0.992, P<0.0001), and the AL was positively correlated between preoperative contact A-scan and postoperative IOL Master 700(rs=0.989, P<0.0001).CONCLUSION:For dense cataract, infiltrating B ultrasound guided A-scan segment biometric measurement, which has good correlations and agreement, is closer to the AL measured by IOL Master 700 postoperatively than that measured by contact A-scan.
