1.Antibacterial Performance of Fabric Treated with Siliconequaternary Ammonium Microemulsion

Objective To study the antibacterial performance of the fabric treated with siliconequaternary ammonium microemulsion.Methods Escherichia coli,Staphylococcus aureus and Candida albicans were used as the test bacteria and the method of bacteriostasis circle,vibration flask were used to test the effects of the antibacterial agent qualitatively and quantitatively.Results The lowest bacteriostasis concentration for siliconequaternary ammonium microemulsion was 2%,the bacteriostasis effect on staphylococcus was superior to that on E.coli and it had a good bacteriostasis effect on the yeast.Pure cotton bacteriostasis effect was most noticeable in its antibacterial reorganization and it reached 92.34% of the bacteriostasis rate after washed for 30 times,and the bacteriostasis rate for the treated fabric could still keep 99.96% after 6 months of preservation.Conclusion The fabric reorganized with the siliconequaternary ammonium microemulsion has a good capacity of bacteriostasis and washes well in antibacterial performance.
2.Clnical observation, of the effects of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine on the treatment of hyperplastic scar.

OBJECTIVETo explore the clinical effects of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine ( Fuchunsan ) on the treatment of postburn hyperplastic scar.
METHODSSixty-three patients with hyperplastic scar after burn injury hospitalized from February 2012 to June 2014 in our department were treated with lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine (Fuchunsan). Patients were divided into early stage group (E, n = 35), middle stage group (M, n = 25), and late stage group ( L, n = 3) according to the formation time of scar, which was respectively 3 weeks to 3 months, longer than 3 months and less than or equal to 6 months, and 3 to 15 years in groups E, M, and L. The number of times of laser treatment of patients in each group was recorded. The degree of scar pain in patients of the three groups was assessed by the Numerical Rating Scale (NRS) before treatment and after treatment for 1, 2, and 3 times. The scar condition of patients in groups E and M was assessed by the Vancouver Scar Scale (VSS) before treatment and after treatment for 1, 3, and 5 times. Patients in group L did not receive VSS assessment but were evaluated by clinical observation only. Photos of scar in treating area were taken before treatment and after treatment for 3 and 5 times to evaluate the clinical effect. Data were processed with t test.
RESULTSPatients in groups E and M were treated with laser for (4.8 ± 1.1) and (7.7 ± 2.1) times respectively. In group L, the treatment was stopped in 2 patients after laser treatment for 5 times, and 1 patient received laser treatment for 12 times. The degree of pain in patients of groups E and M was alleviated significantly after treatment for one time, and the number of patients scoring 1-4 point(s) in NRS increased from 5 cases to 38 cases. After treatment for 2 and 3 times, the increase in the number of patients scoring 1-4 point (s) in NRS was on a small scale. Before treatment and after treatment for 1 time, VSS scores of patients in groups E and M were similar (with values respectively 0.641 and 0. 082, P values above 0. 05). After treatment for 3 and 5 times, VSS scores of patients in group E were respectively (9.2 ± 0.8) and (7.0 ± 1.1) points, which were significantly lower than those in group M [ (9.7 ± 1.0) and (8.2 ± 1.0) points, with values respectively -1.993 and -4.433 , P < 0.05 or P < 0.01]. After treatment for 3 times, the rate of improvement in appearance was respectively 88.6% (31/35) and 72.0% (18/25) in groups E and M, and it was respectively 100.0% (35/35) and 96.0% (24/25) after treatment for 5 times. No significant effect in appearance was found in the 3 patients in group L.
CONCLUSIONSEarly application of lattice ultra pulse carbon dioxide laser combined with traditional Chinese medicine (Fuchunsan) for the treatment of postburn hyperplastic scar is effective.

4.The Application of Temporary Balloon Occlusion of the Abdominal Aorta in High-order Position Sacral Tumor Surgical Operation

ObjectiveTo evaluate temporary balloon occlusion of the abdominal aorta in high-order position sacral tumor surgical operation as a useful adjuvant technique.MethodsReviewed 79 cases of patients from 2005 to 2015 treated in our department and the diagnosis of high-order position sacral tumor. Temporary balloon occlusion of abdominal aorta was used in 50 patients(male 29,female 21)during the sacral tumors surgical operations. The other 29 patients(male 18,female 11)with sacral tumors who received the non-temporary balloon occlusion therapy were used as control group. The statistical differences of the whole surgery time,the blood loss during the surgery,the happening of the postoperative deep vein thrombosis,the time of the postoperative extubation were analyzed. ResultsThe differences were statistically significant(P<0.001)in the whole surgery time of balloon occlusion group(146.36±29.38)min vs non-balloon occlusion group(206.03±125.93)min,the blood loss of balloon occlusion group(1610.70±491.14)ml vs non-balloon occlusion group(2658.62±562.213)mL, and the time of the postoperative extubation of balloon occlusion group(6.60±2.76)d vs non-balloon occlusion group(12.52±2.86)d. However,there was not significant difference of the happening of the postoperative deep vein thrombosis between balloon occlusion group and non- balloon occlusion group. ConclusionTemporary balloon occlusion of abdominal aorta is effective and reliable. It significantly reduced the time of operations,the loss of blood,mean days in hospital,effusion of post-operation and recurrence rate. It makes the operation of sacral tumors much more safer than before and is a technique worthy of popularizing.
5.Change in antimicrobial resistance of Acinetobacter baumannii causing hospital-acquired pneumonia

Objective To investigate antimicrobial resistance of Acinetobacter baumannii (A.baumannii)causing hospital-acquired pneumonia (HAP),so as to guide rational use of antimicrobial agents and reduce antimicrobial re-sistant rate.Methods Clinical data of patients with A.baumannii HAP in a hospital between 2009 and 2012 were collected and analyzed retrospectively.Antimicrobial resistance data were summarized and analyzed statistically ac-cording to uniformed methods.Results A total of 284 patients developed A.baumannii HAP,patients mainly distributed in intensive care unit(45.07%),department of respiratory diseases(19.01%),neurosurgery (9.15%),and neurology (5.99%).The incidences of A.baumannii HAP,multidrug-resistant A.baumannii (MDR-AB)HAP,and extensively drug-resistant A.baumannii (XDR-AB)HAP increased year by year.The total resistant rates of A.baumannii to cef-operazone/sulbactam (9.51%)and minocycline(9.86%)were low,to levofloxacin(43.66%)and ciprofloxacin(45.42%) were relatively higher;resistant rates to imipenem and meropenem were 16.20%and 20.42% respectively.The resistant rates to meropenem,imipenem,cefoperazone/sulbactam,and ampicillin/sulbactam increased year by year (all P <0.05). Conclusion Antimicrobial resistance of A.baumannii is serious,it is essential to strengthen the monitoring of antimicrobial resistance of A.baumannii,and prevent the spread in hospital.
6.Experimental Study on Yinhuang Solution on Bladder Mucosal Injury in Rat

ObjectiveTo investigate the effects of Yinhuang solution on the bladder mucosal before and/or after mucosal injury.Methods32 SPF female SD rats were randomly divided into 4 groups with 8 rats in each group. Group A was used for the normal control group, group B for the model control group, group C for preventive group, group D for preventive and therapeutic group. Injured rats were created by hydrochloric acid applied on bladder mucosa. After the models were established or before and after the models were established, bladders were perused with Yinhuang solution. Severity of inflammatory infiltration were observed by HE staining, histological score and heat shock protein 70 (HSP70) expressed in the mucosa.ResultsEndoscopic pathological change showed that group B had a large number of inflammatory cell infiltration, significant expansion of capillary, epithelial cell necrosis. Group C had a small amount of expansion of capillary, inflammatory cell infiltration. Group D scattered in expansion of capillary, a small amount of inflammatory cell infiltration. Concerning the results of histological score and HSP70 expression, there was a significant difference between group A and groups B, C or D (P<0.01); there was a significant difference between group B and groups C or D (P<0.01); there was a significant difference between group C and group D (P<0.05).ConclusionYinhuang solution can evidently improve the repair capability of bladder mucosa after injury.

8.Effect of Third Sacral Nerve Root Electro-acupuncture Therapy on Acontractile Detrusor

ObjectiveTo observe the effect of third sacral nerve root electro-acupuncture on acontractile detrusor. Methods84 patients with acontractile detrusor were divided into 2 groups received urethral catheterization alone (control) or electro-acupuncture on third sacral nerve (treatment) respectively. The voiding diary (including urination times, nocturia times and urinary volume) and urodynamics test (including maximum bladder volume, maximum systolic pressure detrusor, the effective time of detrusor contraction, maximum urinary flow rate, average urinary flow rate and residual urine volume) were observed before and 8 week after treatment. ResultsCompared with those before treatment, the urination times, nocturia times and residual urine volume decreased (P<0.05) in treatment group, while maximum systolic pressure detrusor, the effective time of detrusor contraction, maximum urinary flow rate, average urinary flow rate and residual urine volume improved (P<0.05), but the maximum bladder capacity showed no significant difference (P>0.05). ConclusionElectro-acupuncture on third sacral nerve is an effective intervention for acontractile detrusor.
9.Application of case three-dimensional teaching method combined with standardized patient in the clinical clerkship teaching reform of gastrointestinal surgery

Objective:To explore the application value of case three-dimensional teaching method combined with standardized patient (CTTM+SP) in the clinical clerkship teaching reform of gastrointestinal surgery.Methods:A total of 120 students of clinical medicine having clerkship in Binzhou Medical University Hospital were recruited in this study, and they were randomized to the experimental group and the control group, with 60 students in each group. The experimental group took the CTTM+SP teaching method, and the control group adopted the lecture-based learning teaching method. The comprehensive ability of the two groups was evaluated by department graduation examination and satisfaction survey.Results:The examination performance and the satisfaction rate of students in the experimental group were significantly higher than those of the control group ( P < 0.05). Conclusion:CTTM+SP teaching method is helpful for the probationers to master the theoretical knowledge of gastrointestinal surgery systematically, improve the clinical thinking ability and the ability to solve practical problems, mobilize the students' learning motivation and interest, and improve the teaching effect of gastrointestinal surgery clinical clerkship.
10.Application of Bone Anchor and AO/ASIF Hollow Screw in Treatment of Trans-scaphoid Perilunar Dislocation

Objective To investigate the outcome of trans-scaphoid perilunar dislocation treated with bone anchor and AO/ASIF hollow screw.Methods Eight patients with trans-scaphoid perilunar dislocation were treated by Depuy Mitek bone anchor and AO/ASIF hollow screw.The postoperative follow-up period was 6~24 months(average 11.1±5.3 months).All patients were assessed by the Cooney's grading system.Results The outcomes of 8 patients were execellent in 5 cases,good in 2 cases,general in 1 case.6 cases got pain relief with an increase of range of motor and grip strength at certain drgree.Postoperative roentgenogram displayed congruity among wrist bones,with no fracture disunion,no loose internal fixation and no traumatic arthritis.Conclusion The bone anchor and hollow screw have advantages as simple-manipulated and reliable for treating trans-scaphoid perilunar dislocation.
