1.Efficacy of paravertebral block for postoperative rehabilitation pain after unilateral ureterolithotomy

Objective To compare the efficacy and safety in patients received paravertebral block and epi-dural block for postoperative rehabilitation pain after unilateral ureterolithotomy.Methods 120 ASA Ⅰor Ⅱ patients were randomly allocated into research group (n =60)and control group (n =60).After operation,the research group received paravertebral block performed with peripheral nerve stimulator,the control group received epidural block.All patients received patient -controlled intravenous analgesia (PCIA)in the same time.The operation time,amount of bleeding,postoperative hospitalization time,operation completion,number of press PCIA,occurrence rate of postopera-tive adverse reaction in 48 h,and visual analog scores (VAS)at 4,8,16 and 24 h after surgery were recorded. Results The patients of the two groups were completed the stone surgery,there was no cases of conversion to open surgery.There was no significant difference in operation time,bleeding volume,postoperative hospital stay between the two groups (all P >0.05).But the PCIA times of the research group was (2.14 ±0.45)times,which of the control group was (4.20 ±0.68)times,the difference was statistically significant (t =4.27,P =0.029).The postoperative VAS scores of the research group[4 h (2.68 ±0.480)points,8 h (2.68 ±0.48)points,16 h (2.81 ±0.43) points,24 h (2.88 ±0.49)points]were significantly lower than those of the control group[(3.12 ±0.53)points, (3.37 ±0.58)points,(3.42 ±0.52)points,(3.39 ±0.55)points],the differences were statistically significant (t =2.68,3.13,306,311,all P <0.05).Of the research group,the nausea and vomiting occurred rate (23.33%), hypotension rate (0.00%),skin itching (5.00%),the incidence of decreased muscle strength (0.00%)were significantly lower than the control group(43.3%,31.6%,28.33% and 8.33%),the differences were statistically significant(χ2 =5.40,22.57,11.76,522,all P <0.05 ).Conclusion Nerve stimulator guidance for thoracic paravertebral nerve block for upper ureter incision to take stone surgery patients can significantly reduce postoperative pain and the PCIA analgesia drug dosage and the incidence of adverse reactions.
2.Influence of dexamethasone on foxml gene expression in pulmonary tissue following acute lung injury

Objective To study the foxml gene and its protective effect on the lung tissue of rats with acute lung injury (ALI) induced by lipopolysaccharide (LPS), and to observe the dexamethason' s (DEX) impacts on foxml gene and the prognosis of ALI. Method Seventy-two healthy mice were randomly(random number) divid-ed into three groups: control group (A group, n = 24), model group (B group, n = 24) and DEX treatment group (C group, n = 24). The observing intervals were respectively set in 24 h, 48 h and 72 hours. At each ob-serving interval, the foxml protein in lung tissue of mice was detected by using immunohistochemistry (IHC), and the expression of foxml gene in lung tissue was detected by using RT-PCR, as well as to observe the pathological changes in lung tissue. Results Comparisons were made between paired groups at 24 h,48 h and 72 h intervals in which the expression of foxml mRNA and the level of foxml protein in lung tissue of mice in C group were signifi-cantly higher than those in B group (P < 0.05), and those in B group were significantly higher than those in A group (P < 0.05). The expression of foxml mRNA and the level of foxml protein in lung tissue of mice in B group at 48 h interval were significantly higher than those both at intervals of 72 h and 24 h (P < 0.05), and the those at 72 interval were significantly higher than those at 24 h interval (P < 0.05). Compared with B group, the pathologi-cal changes in lung tissue of mice in C group were lessened. Conclusions In both model group and dexamethasone treatment group, the expression of foxml mRNA and the level of foxml protein in lung tissue of mice are increased significantly. Dexamethasone lessens the injury of both vascular endothelial cells and alveolar epithelial ceils of lung tissue, and it also significantly increases the expression of foxml mRNA and the level of foxml protein.

Objective To prolong the working time of the crew under a high temperature and high humidity environment in armored vehicles , improve work efficiency , and to prevent excessive heat load from doing any harm to the human body . Methods The man-machine environment was analyzed , the corresponding environment and the conditions aboard were further quantitatively studied , and man-machine-environment interactions were analyzed .Also,a questionnaire survey was conducted at the same time .Results and Conclusion The thermal equilibrium and thermal comfort calculation program were compiled and validated through the questionnaire survey in order to provide support for environment assessment and protection design .

The pathogenesis of autoimmune diseases(AID)involves the inflammatory response of the human body, and inflammatory pathways are also shared by mechanisms of aging.Clinical observations of AID have revealed evidence of aging-related diseases in younger adults.This article reviews the literature related to research progress on AID combined with aging-related diseases, such as cardiovascular diseases, diabetes and osteoporosis, and briefly explains the underlying mechanisms, in order to promote interdisciplinary research between the two fields.
5.Relationship between psychological quality and performance capacity among armored crew

Objective To investigate the relationship between psychological quality and performance capacity of the crew in armored equipment so as to provide scientific evidence for the training of crew .Methods The psychological quality of 82 crew members of armored cavalry was tested and analyzed by neurobehavioral testers and questionnaires .Results and Conclusion The education level and age had no significant effect on the performance capacity among the armored crew . The test results of psychological quality parameters showed the same trend as the performance capacity of the crew , but their psychological health status had obvious effect on the performance capacity .The psychological quality of the crew had great influence on their performance capacity ,so we should pay attention to mental intervention ,enhance psychological protection ability of the crew , and improve their combat effectiveness continuously .
6.Diagnostic value of Matrix metalloproteinase-9 and Epidermal growth factor in uterine myoma

Objective To discuss the diagnostic value of Matrix metalloproteinase-9 (MMP-9) and Epidermal growth factor (EGF) in uterine myoma.Methods Seventy-three patients with uterine leiomyoma treated by surgery as treatment group,and 65 healthy women were selected as control group.The levels of MMP9 and EGF in both groups were detected by enzyme-linked immunosorbent assay method.Results The levels of MMP-9and EGF in treatment group were significantly higher than those of control group((1.72±0.36) mg/L vs.(1.28±0.25) mg/L, (342.16± 128.65) ng/L vs.(209.84± 100.34) ng/L;t =8.241,6.678;P <0.01).There were no relationships between the levels of MMP-9 and EGF with maximum diameter of uterine myoma (P >0.05).Conclusion Serum levels of MMP-9 and EGF have some clinical value in diagnosing uterine leiomyoma.
7.Risk screening of osteoporosis in community residents and its influencing factors

Objective To explore the influencing factors of high risk osteoporosis (OP) population. Methods Using multi-stage cluster sampling method, 1 district was selected from each of the 6 cities randomly, then 3 communities were selected from each district randomly. Questionnaire investigation and physical examination were applied to community residents aged 40-69 years old. The questionnaire mainly included basic information, OP risk assessment (using the 1 minute test method developed by the International OP Foundation), health status and lifestyle information (recent 12 months). The relationship between high-risk groups and influencing factors was analyzed by unconditional logistic regression. Results Totally 6577 valid questionnaires were collected, 2069 were male (31.46%) and 4508 were female (68.54%). A total of 166 cases (2.52%) were found as self-reported OP patients at the age of 40-69, self-reported rate of male (1.55%) was lower than that of female (2.97%) (χ2=11.719, P<0.01), the rate was higher among people aged 60-69 years than people aged 40-49 (χ2=44.766, P<0.01);3400 cases (52.28%) were found as high-risk individuals,there were more male at higher risk than female (OR=1.845). Among different age groups, the 50-59 and 60-69 years age groups had more high risk people than that of 40-49 years group (with OR=2.171 and OR=2.854 respectively). Comparative analysis was also conducted for the following factors: degree of education [college or above (OR=0.517) compared with primary school or junior middle school], occupation [compared with civil servants, technical management personnel (OR=2.289), business services (OR=2.224), farming/forestry/fishing (OR=2.258) and production/transportation staff (OR=2.552)], self-reported history of chronic disease (OR=1.596), fracture (OR=4.061), body pain (OR=2.286) and foot cramps (OR=1.923), as well as calcium/vitamin D intake (OR=1.357), increased sunlight (OR=1.256), daily walking steps>5000 (OR=1.336) and BMI>24.00 (OR=1.322), they were all related to a increased proportion of high risk population. Conclusion The proportion of OP high-risk groups is higher among people aged 40-69. Gender, age, cultural level, occupation, bad health status and lifestyle changes are closely related to higher OP risk. Community residents OP screening should be continued to further study the risk factors of OP.

Objective:
To compare the value of right ventricular (RV) free wall longitudinal strain (FWLS) by speckle tracking echocardiography (STE) and conventional parameters in evaluation of RV dysfunction in chronic thromboembolic pulmonary hypertension (CTEPH).
Methods:
Sixty CTEPH patients were enrolled as group A and 45 pulmonary embolism (PE) patients with normal pulmonary pressure were enrolled as group B in this study. CTEPH patients were divided into 2 subgroups using the World Health Organization (WHO) function classification: patients with WHO Ⅰ-Ⅱ were designated as group A1 and those with WHO Ⅲ-Ⅳ were designated as group A2. Conventional RV functional parameters including tricuspid annular plane systolic excursion (TAPSE), tissue Doppler-derived tricuspid annular systolic velocity (S′), fractional area change (FAC), RV index of myocardial performance (RVIMP), and STE-derived RV FWLS were measured and compared. Clinical right heart failure (RHF) was defined as the presence of symptoms of heart failure and signs of systemic circulation congestion during hospitalization.

10.Factors affecting the consumption of sugar-sweetened beverages among middle school students in eight counties of four provinces (municipality) in China

Objective To explore the factors related to the consumption of sugar-sweetened beverages (SSB) among middle school students in four provinces (municipality) in China.Methods A total of 3 469 middle school students were recruited from the Children health cohort in eight counties within the four provinces (municipality).We collected information regarding demography,lifestyle,and family and school situation as well as the consumption frequency of SSB in the previous week.High SSB consumption was defined as "positive" if the frequency was once a day or more.Mean consumption time was calculated,and then logistic regressions were performed to explore the factors influencing SSB consumption.Results Prevalence of high SSB consumption was 11.18% among the subjects of the eight counties,and the consumption frequency of SSB was 0.21 times per day.Multivariate logistic regression analysis revealed that the following factors were associated with high consumption of SSB:higher grade,rural region,male gender,physical exercise,habit of eating snacks,eating out,and fast-food consumption (P<0.01).Factors influencing high SSB consumption were not completely equal between genders:physical exercise at school was associated with high SSB consumption only in female students (P<0.01),while higher grade and eating out in the past week were associated with high SSB consumption only in male students (P<0.05).Conclusion Factors associated with high SSB consumption among middle school students included grade,region,physical exercise at school,snack consumption,eating out,and fast-food consumption.These factors differed between genders.Intervention programs addressing these factors may be beneficial in reducing SSB consumption in middle school students.
