1.Breast approach endoscopic thyroidectomy versus conventional thyroidectomy:A comparison study

Objective To compare the clinical efficacy between endoscopic thyroidectomy via breast approach and conventional open thyroidectomy in patients with nodular goiter.Methods A series of 75 patients with unilateral nodular goiter was divided into two groups,either receiving endoscopic thyroidectomy through breast approach(Endoscopic Group,24 cases)or conventional open thyroidectomy(Open Group,51 cases).The therapeutic efficacy was compared between the two groups.Results In the Endoscopic Group,the endoscopic operation was successfully performed in 23 cases,whereas a conversion to open surgery was needed in 1 case because of large nodule(6 cm in diameter)and extensive adhesion of infrahyoid muscles(resulted from a previous injection therapy).The operating time was significantly longer in the Endoscopic Group(100.4?26.6 min)than in the Open Group(73.5?14.5 min)(t=5.627;P=0.000).The postoperative drainage volume in the Endoscopic Group(66.7?24.9 ml)was significantly more than that in the Open Group(13.3?6.4 ml)(t=14.403,P=0.000).The postoperative hospital stay was significantly longer in the Endoscopic Group(4.3?1.1 d)than in the Open Group(3.6?0.9 d)(t=2.886;P=0.005).The Endoscopic Group had significant higher hospital costs(11 572?1675.1 yuan)than the Open Group(5015.9?1211.0 yuan)(t=19.058;P=0.000).And a satisfactory cosmetic result was achieved in significantly more patients in the Endoscopic Group(22/24 cases)than in the Open Group(34/51 cases)(?2=7.235;P=0.007).There was no significant difference in intraoperative blood loss between the Endoscopic Group(22.5?15.3 ml)and the Open Group(18.3?7.5 ml)(t=1.273;P=0.207),and in consumption of analgesics between the Endoscopic Group(9/24 cases)and the Open Group(21/51 cases)(?2=0.028;P=0.867),respectively.No severe complications were encountered,such as massive hemorrhage,injuries of the recurrent or superior laryngeal nerve,or parathyroid gland injury.During a follow-up for 6 months,none of the patients was found recurrence after surgery.Conclusions Both breast approach endoscopic thyroidectomy and conventional open thyroidectomy are safe and effective.The former gives better cosmetic outcomes but slower recovery and high hospital costs compared to the latter.
2.Simultaneous Determination of Chloromycetin and Dexamethasone Acetate in Jiefu Cream by HPLC

OBJECTIVE:To determine the contents of chloromycetin and dexamethasone acetate in Jiefu cream.METHODS:A reversed-phase HPLC was established for the determination with C18 column as stationary phase.The mobile phase consisted of acetonitrile and 0.05%mol/L phosphate buffer solution(40∶60),the detection wavelength was 240nm.RESULTS:Chlorom-ycetin had a good linearity (r=0.9 998) in the range of 0.1~0.3mg/ml,the mean recovery was 100.6% with a RSD of 1.3%.Dexamethasone acetate had a good linearity(r=0.9 998) in the range of 0.02~0.08mg/ml,the mean recovery was 101.1% with a RSD of 1.7%.CONCLUSION:The method is simple,sensitive and accurate.

4.Effect of ambroxol hydrochloride combined with budesonide in treatment of children bronchial pneumonia on C-reactive protein level in patients with C-, WBC

Objective To explore and analyze the effect of ambroxol hydrochloride combined with budesonide in treatment of children bronchial pneumonia on C-reactive protein level and WBC levels.Methods 100 cases of patients with bronchial pneumonia treated in our hospital from May 2015 to December 2016, were randomly divided into observation group and control group, 50 cases in each group.The control group were given routine treatment, while the observation group were given ambroxol hydrochloride combined with budesonide treatment on the basis of the control group,the course of treatment was 14 days.C-reactive protein and WBC levels were measured before treatment and 14 days after treatment, the Resultswere observed and statistical.ResultsThere were significant differences in the content of CRP between groups and multiple time points(F=11.50, P=0.003;F=4.78, P=0.03;F=5.45, P=0.02).The content of CRP 14 days after treatment in the observation group were significantly lower than the control group(t=14.12,P<0.05), the difference was statistically significant.There were significant differences in the WBC counts between groups and multiple time points(F=11.52, P=0.003;F=4.73, P=0.03;F=5.44, P=0.02),the difference was statistically significant.The count of WBC 14 days after treatment in the observation group after treatment were significantly lower than the control group(t=8.12, P<0.05), the difference was statistically significant.The number of cured, effective and ineffective cases in the control group was 20, 13 and 15, respectively.The observation group was 30, 15, and 5 cases, respectively.By rank sum test, the difference of the effective rate between the 2 groups was statistically significant(u=2.33, P<0.05).And the total effective rate in observation group was 90.0%, which was significantly higher than the control group(70.0%)(x2=6.25, P<0.05), the difference was statistically significant.Conclusion Ambroxol hydrochloride combined with budesonide in the treatment of bronchial pneumonia in children is effective and can effectively reduce the CRP and WBC count, so it is worth popularizing widely in clinic.

6.Preparation and Quality Control of Dianozol Effervescent Suppository

OBJECTIVE:To prepare the Dianozol effervescent suppository and establish the standard of quality con?trol.METHODS:Semi-synthetic glyceride was used as the base material,Dianozol as main ingredient,sodium bicarbonate and citric acid as foaming agents.The UV-spectrophotometry was used for quantitative analysis.RESULTS:The calibration curve of Dianozol was linear in the range of7~24?g/ml(r=0.9999).The recovery rate of Dianozol was100.26%,RSD=0.18%(n= 5).CONCLUSION:The Dianozol effervescent suppository was feasible in technological process.The method of content deter?mination is simple and accurate.
7.Laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection through retropancreatic approach in radical gastrectomy for advanced gastric cancer

Laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection is still difficult to accomplish,which restrains its application in total gastrectomy for advanced proximal gastric cancer.Based on our anatomical understanding of pre- and retropancreatic spaces,features of splenic vessels and distribution of perigastric lymph nodes,we combined the characteristics of laparoscopic surgery and developed a novel technique for laparoscopic pancreas- and spleen-preserving splenic hilar lymph nodes dissection through retropancreatic space.The key lies in mobilization of the splenic pedicle through retropancreatic space,dissection of peri-vascular lymph nodes in sequence of trunk-to-branch,in-sheath vascularization of the splenic vessels.From August 2009 to December 2010,this technique was performed on 6 patients in Nanfang hospital.The initial results suggested that this technique could be safe and feasible for skillful surgeons.Further studies on the application of this technique in total gastrectomy for advanced proximal gastric cancer would be needed.

Correct implementation of laparoscopic total gastrectomy with D2 lymph node dissection depends on the understanding of anatomical features of peripancreatic space, the landmark of pancreas and blood vessels, and the diverse perigastric vascular anatomy and standardized surgical procedures designed according to the regional distribution of lymph nodes.From September 2006 to November 2009, laparoscopic total gastrectomy with D2 lymph node dissection surgery were completed in 12 cases in the Nanfang Hospital. A reasonable anatomy method and a simple, effective surgical procedure of laparoscopic D2 lymph node dissection were introduced.

10.ATP-tumor chemosensitivity assay directed chemotherapy in patients with cervical cancer

Objective:To select suitable chemotherapy for cervical cancer patients by ATP-tumor chemosensitivity assay.
Methods:Seventy-two hospitalized patients with cervical cancer between July 2007 and October 2009 were enrolled. The patients were randomly divided into a trial group (n=35) and a control group(n=37). ATP-TCA was used to detect the sensitivity of 35 samples of cervical cancer in the trial group to 6 combined chemotherapy regimens. The chemotherapy regimen in the trial group was confirmed by the results of susceptibility testing and that in the control group was confirmed by clinical experience. One-year recurrence rate and 3-year survival rate of two groups were compared after 3 year follow-up.
Results:ATP-TCA was measured in 32 of the 35 patients in the trial group. The sensitive patients for paclitaxel+carboplatin, paclitaxel+oxaliplatin, bleomycin+ifosfamide+cisplatin, bleomycin+vincristine+cisplatin, fluorouracil+cisplatin, and gemcitabine+cisplatin were 20, 18, 17, 18, 17, and 21, respectively. There was no significant difference in the 1-year recurrence between the two groups (P>0.05), while the 3-year survivors in the trial group were more than those in the control group (P<0.05).
Conclusion:ATP-TCA method is good for patients with cervical cancer because it is sensitive, effective, and individualized.
