1.Analysis and Evaluation of Sickbed Utilization in the General Rehabilitation Center

ObjectiveTo analyze the sickbed setup and utilization in the general rehabilitation center and offer gists for strengthening hospital optimize project of sickbed setup.MethodsThe factual sickbed demand, standard sickbed demand and the ratio of confessing and requiring were counted and analyzed.ResultsThe sickbed turnover times of the internal medicine and ICU were accord with request, but the rate of sickbed utilization was higher, the sickbed utilization rate of the other section offices were higher. The sickbed turnover times were lower compared with control lever.ConclusionThe hospital manager should take some measures for optimize sickbed setup, and strengthen manage to improve the medical quality.
2.Effects of TCM Service Utilization on Health Behavior of Community Residents

Objective To understand the correlation between TCM service utilization and health behavior of community residents.MethodsAself-designed questionnaire about TCM service utilization and health behavior status was used to investigate 200 people from 4 communities on random sampling. The items in the questionnaire were evaluated and analyzed to test the correlation of TCM service utilization and health behavior of community residents.Results The two indexes significantly correlated in 0.01 level (bilateral),r=0.674, bilateralP=0.000.Conclusion The correlation between TCM community service utilization and health behavior status is positive. The higher the level of TCM community services utilization is, the higher the index value of health behavior status will be.
3.Effects of erigeron on hippocampus adenosine triphosphate of gerbils with cerebral ischemia reperfusion injury

BACKGROUND: Erigeron, the inhibitor of protein kinase C, functions to decrease the death of neurocytes caused by cerebral ischemia reperfusion.OBJECTIVE: To study the effects of erigeron on hippocampus adenosine triphosphate (ATP) and adenosine triphosphatase (ATPase) of gerbils with cerebral ischemia reperfusion injury.DESIGN: A random controlled experiment.SETTING: Anesthesia Department of the Fourth Hospital of Suzhou University, Anesthesia Department of the Fourth People's Hospital of Wuxi City, and Anesthesia Department of the Affiliated Hospital of Xuzhou Medical College.MATERIALS: The experiment was done in the key laboratory of anesthesiology of Jiangsu Province from March 2002 to May 2003. The 90 Mongolian gerbils were divided into 9 groups: sham operated group, cerebral ischemia group, cerebral ischemia reperfusion 1 hour group, cerebral ischemia reperfusion 12 hours group, cerebral ischemia reperfusion 24 hours group, erigeron + cerebral ischemia group, erigeron + cerebral ischemia reperfusion 1 hour group, erigeron + cerebral ischemia reperfusion 12 hours group, and erigeron + cerebral ischemia reperfusion 24 hours group,with 10 gerbils in each. The erigeron injection with flavone 4.5 g/mL was made by the Biomedicine Factory of Yunnan Province and numbered 990103.METHODS: Apart from the sham operated group, all groups were established the ischemia reperfusion models. The erigeron groups were abdominally injected erigeron 10 mL/kg 30 minutes before the ischemia. Meanwhile, the other groups were injected the normal saline 10 mL/kg. The 6 gerbils of each group were taken blood for testing ATP content and ATPase activity and the rest 4 were only examined the pathological changes of neurocytes of hippocampus CA1 region. During the ischemia, the temperature of drum membrane (reflecting the brain temperature) was monitored and kept at (37±0.2)℃ in order to avoid the influence of temperature on the result. The comparison among groups was shown with the single factor analysis of variance.CA1 region.of ATP content and ATPase activity: Compared with the cerebral ischemia reperfusion 24 hours group, the ATP content of the erigeron 1, 12, 24 hours groups was obviously increased [(0.25 ±0.08), (0.81 ±0.12), (0.58 ±0.07),(0.43±0.09) mmol/kg, P < 0.01], the Na+-K+-ATPase activity was obviously P < 0.01], and the Ca2+-ATPase activity was also obviously promoted days after ischemia, the findings under the low power lens were: the pyramidal belt of the cerebral ischemia reperfusion groups became thinner and loose and even disappeared, but that of the erigeron groups became obviously wider and thicker; the findings under the high power lens were: most of the pyramidal cells of the cerebral ischemia reperfusion groups were in the state of coagulation necrosis, and those survived pyramidal cells with complete nuclear membrane and clear nucleole were very few, but the survived pyramidal cells of the erigeron groups were obviously more than those of the cerebral ischemia reperfusion groups.CONCLUSION: Erigeron increases obviously the ATP content after ischemia reperfusion, especially at the early stage of reperfusion, and at the same time, the ATPase activity is also in an improved tendency, relieving the ischemia reperfusion injury and protecting the neurocytes.

5.Clinicopathologic analysis of aggressive angiomyxoma in the pelvis and vulva of 8 cases

Objective To study the clinical and pathologic features of aggressive angiomyxoma (AAM) and its occurrence, treatment and prognosis Methods The 8 cases of AAM were analyzed retrospectively, among which 6 cases in vulva, 2 cases in perineum, vagina and pelvis As well we performad immunohistochemical studies Results The neoplastic cells were at least focally immunoreactive for vimentin (7/7), smooth muscle actin (7/7), factor 8 (7/7), proliferating cell nuclear antigen (4/7), estrogen (3/7) and pregesterone (4/7) receptors All of the examined tumors were negative for S100 protein and cluster designation 68 Conclusions AAM is an uncommon mesenchymal tumor that preferentially involves the pelvic and perineal regions of females In immunohistochemical investigation the neoplastic cells of AAM exhibit fibroblastic and myofibroblastic features They are characterized by slow growth, infiltration of adjacent structures, absence of distant metastases, a tendency to recur locally and a fairly good prognosis Treatment should consist of wide surgical excision, as complete as technically possible
6.Target Monitoring of Nosocomial Infection in ICU

OBJECTIVE To strengthen and explore the effect of target monitoring in ICU.METHODS To put out prospectively the target monitoring of the nosocomial infection in ICU from Jan 2006 to Dec 2007.RESULTS A total of 3917 patients were monitored and 217 of them suffered from infection.The day infection rate was 11.7‰,and the infection rater per thousand day were 13.4‰,10.0‰ in 2006 and 2007,respectively.The infection rate related to urinary catheter,central venous catheter and ventilator were 1.2‰,0.9‰,4.1‰,2.8‰,14.2‰,6.0‰ in the past two years,respectively.It decrease 0.3‰,1.3‰,8.2‰,respectively.Ventilator-associated infection was the main infection part of hospital infection,and the patients after liver transplantation ICU had the highest infectionrate.CONCLUSIONS Prospective target monitoring is a scientific monitoring method that plays an important role in controlling and preventing hospital-acquired infection.It deserves recommendation.
7.Long-term effects of meniscal allograft transplantation to repair rabbit knee cartilage by different methods

BACKGROUND: There are no ideal substitutes for meniscus. Meniscel allograft transplantation has become a feasible method to retain maniscal function following injury. Currently, the meniscus conservancy methods include fresh meniscus, low temperature preserved meniscus; freeze-dried meniscus.OBJECTIVE: To observe the long-term effects of meniscus allograft transplantation by different methods.METHODS: A total of 70 adult New Zealand rabbits were used, and 30 were selected as meniscus donors. The remaining 40 rabbits were divided into 4 groups (n=10): after meniscectomy, control group was not treated; fresh group underwent fresh meniscus allograft transplantation; low temperature group underwent low temperature preserved meniscus allograft transplantation; freeze-dried group underwent freeze-dried meniscus allograft transplantation. Of 30 donors, 10 received fresh meniscus transplantation, and the other received temperature preserved meniscus and freeze-dried meniscus transplantation 1 week after low temperature and freeze dried preservation. The rabbits were sacrificed 12 months after surgery for gross and histological observation.RESULTS AND CONCLUSION: The meniscus allografts of rabbits healed well, and no rabbits died. Gross observation showed that the appearance and elasticity of fresh meniscus group was similar to normal meniscus; tibial plateau was covered well, with no additional wearing in the cartilage of tibial plateau or condyles of femur. In low temperature and freeze-dried preservation groups,the meniscus grafts healed well, but their volume was small and the elasticity was poor; tibial plateau was only partially covered.Pathological observation suggested that fresh meniscus group had orderly arranged collage fiber and similar chondrocyte appearance, number and distribution to normal meniscus, while sparse collagen fiber end small number of meniscus grafts were observed in the other groups. Results show that meniscal allogrefting can survive, retain certain function and structure and prevent the degenerative changes of the knee joint. Moreover, fresh meniscal grafts exhibit best effects, as the grafts following low temperature or freeze-dried preservation degenerate and their volume diminish 1 year following meniscectomy.

9.Prevention and treatment of splenic injury during the urological surgery

Objective To discuss the cause,treatment and prevention of splenic injury during the urological surgery.Methods The clinical data of 16 cases with splenic injury in operation for renal and adrenal tumors in 496 cases were retrospectively analyzed.Nine cases were left radical nephrectomy,3 cases were left renal hamartoma enucleation,4 cases were left adrenal tumor resection.Damage located at outer edge of the spleen in 8 cases,the splenorenal ligament in 6 cases,and the splenic hilum in 2 cases.In these 16 cases,14 patients spared the spleen (Ⅰ Grade injury 8 cases,Ⅱ grade 6 cases).The injuryed spleen was directly pressed with hemostatic gauze in 3 cases; 5 patients used coagulation,bonding,hemostatic gauze to stop bleeding; 2 cases of grade Ⅱ injury used U-shaped suture and coated with fibrin glue,then compressed with hemostatic gauze to stop bleeding; 2 cases of grade Ⅱ injuries with the greater omentum stitched into the seam,sprayed biological glue,were compressed with gelatin sponge; 2 cases of grade Ⅱ injury underwent splenic artery branch ligation.The other 2 cases (1 Ⅱ grade and 1 Ⅲ grade) underwent splenectomy.Results All of the 16 patients were cured and followed up for 6 months to 5 years.There was no delayed bleeding of spleen and splenic dysfunction.One patient died of tumor recurrence 6 months after operation.Conclusions Splenic injury is a common complication during urological surgery,especially the tumor is large or adhered to spleen in the upper pole of left kidney.Once spenic injury occurs,doctors should choose the right treatment plan according to surgical injury,and try to save the spleen.
10.Berberine-induced apoptosis in leukemia Molt-4 cell line and expression of NF-κB/p65 and Caspase-3

[Objective] To study the inhibition effect of berberine on cell proliferation in acute T lymphocytic leukemia cell line Molt-4,and its mechanisms.[Methods] The logarithmic growing cell lines were incubated with 0,10,100 μg/ml berberine for various hours.The apoptosis induced by berberine was detected by flow cytometry,eletrophoresis and microscopy.The protein pro-Caspase-3 and p65 were determined by Western bolt assay.[Results]Treated with berberine,Molt-4 cells were found with apoptosis bodies by microscopy.By flow cytometry,it was found that number of G2/M cells was increased and number of S cells was reduced.Necrosis band was found in DNA ladder electrophoresis.The expression of pro Caspase-3 was activated partly and p65 was down-regulated in Molt-4 cells treated with berberine.[Conclusion]Berberine can induce Molt-4 cells apoptosis, and NF-κB、Caspase-3 have been shown to regulate the apoptosis programme.
