Background Proliferative vitreo-retinal diseases include all the overly proliferative diseases of eyes.Many kinds of cytokines play important roles in their pathogenesis.Hypoxia-inducible factor 1α(HIF-1α)participates in the pathogenesis of a lot of ischemic diseases.HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases. Objective Present study was to observe the expression of HIF-1α in vitreous body in the eyes with proliferative vitreo-retinal diseases.The vitreous samples were collected during the vitrectomy from 71 eyes of 69 patients(experimental group)including 39 eyes with proliferative diabetic retinopathy(PDR)and 32 eyes with proliferative Vitreoretinopathy(PVR).Sixteen eyes of 16 patients as the cases control group,including 14 eyes with macular hole(MH)and 2 eyes with epiretinal macular membrane(ERM).What is more,there were 8 vitreous fluid samples of the normal control group.The concentrations of HIF-1α in vitreous body were detected by enzyme-linked immunosorbent assay(ELISA).The written informed consent was obtained from each patient before any medical procedure. Results The concentrations of HIF-1α in vitreous body in the eyes of experimental group,the cases control group and the normal control group were(294.08±2.97)、(260.41±8.29)、(16.38±3.56)mg/L respectively,showing a significant difference among the three groups(F=248.77,P=0.00).The concentrations of HIF-1α in vitreous body were obviously increased in the eyes of experimental group in comparison with the normal control group and the cases control group respectively(t=22.25,P=0.00;t=2.70,P=0.00 respectively).The concentrations of HIF-1α in vitreous body were increased in the eyes of the cases control group in comparison with the normal control group eyes(t=14.21,P=0.00).There was inferior correlation between the concentrations of HIF-1α in vitreous body in the eyes of experimental group and the course of ophthalmopathy. Conclusion Intravitreous HIF-1α level regulates in the eyes with proliferative vitreo-retinal diseases. Expression of HIF-1α in vitreous had inferior correlation with the course of ophthalmopathy. HIF-1α may play an important role in the pathogenesis of proliferative vitreo-retinal diseases.

Objective To study the expression of BMI-l gene in children with acute leukemia and its clinical significance.Methods The clinical specimens of 46 children with acute leukemia who were diagnosed lately in the Third Affiliated Hospital of Zhengzhou University and other hospitals in Zhengzhou from Jul.1,2008 to Apr.30,2009 were collected,while peripheral blood specimens of 30 healthy children were collected as control group.With the guardians′ informed consent,the experiment was approved through the hospital ethics committee.The level of BMI-1 mRNA′s expression was tested using reverse transcription-polymerase chain reaction(RT-PCR),while data were analyzed through the application of SPSS 12.0 statistical software.Results 1.The level of BMI-l gene′s expression in children with acute leukemia was significantly higher than that in control group(P0.05),after complete remission,BMI-1 mRNA was not detected in the 2 groups;4.Compared with the complete remission group,expression of BMI-1 mRNA in the untreated group and the recurrence group was significantly higher(P0.05).Conclusions BMI-1 gene was highly expressed in children with acute leukemia,and the level of the gene expression in patients of complete remission normalized,which suggests that the gene may be involved in the occurrence and the development process of leukemia;therefore,it is possible to regard the gene as a molecular marker to evaluate the development,relapse and prognosis of the patients with acute leukemia.

Objective To observe the clinical efficacy of acupuncture in treating hemifacial spasm and its effect on the surface electromyogram signal of orbicularis oculi muscle under contraction state.Method Seventy-six patients with hemifacial spasm were randomized into a treatment group and a control group, 38 cases in each group. The control group was intervened by conventional medications, while the treatment group was by acupuncture in addition to the intervention given to the control group. The surface electromyogram signal of orbicularis oculi muscle under contraction before and after intervention, clinical efficacies, and side effects were compared between the two groups.Result The MF and MPF values were significantly changed after intervention in both groups (P<0.05). After intervention, the MPF value in the treatment group was significantly different from that in the control group (P<0.05). The total effective rate was 92.1% in the treatment group versus 63.2% in the control group, and the difference was statistically significant (P<0.05). The TESS scores after intervention were significantly different from that before intervention in both groups (P<0.05). After intervention, the TESS score in the treatment group was significantly different from that in the control group (P<0.05).Conclusion Acupuncture is an effective method in treating hemifacial spasm, and it can release muscle fatigue.

Objective To explore the effect of case discussion and process evaluation on the teaching of obstetric and gynecological nursing. Methods In April 2014, case discussion and process evaluation were used in the teaching of hypertensive disorders during pregnancy for 50 undergraduate students recruited in 2011. Group tests were used to evaluate the learning outcomes. Results The score of the 50 students ranged from 50 to 100, with the mean score of 83.6 ± 11.2 for the individual test. By group test, the score ranged from 64 to 88,with the mean score of 78.5 ± 8.8, 87.8% to 95.9% of the students were satisfied with this teaching method. Conclusions Case discussion and process evaluation can improve the teachers′competence, enrich the contents of teaching. It is effective to cultivate the students′critical thinking and competence in using knowledge to solve the clinical problems.

Objective To analyze the significance of global end-diastolic volume index (GEDVI) in acute kidney injury (AKI) after septic shock.Methods A retrospective analysis of 61 patients was performed.The patients were diagnosed of septic shock in emergency ward of Shenyang Military Hospital from 2012 March to 2013 May and were monitored by pulse indicator continuous cardiac output (PiCCO).The patients were divided into two groups:low GEDVI group (GEDVI ＜ 700 ml/m2,29 cases) and high GEDVI group (GEDVI≥700 ml/m2,32 cases) by evaluating GEDVI of 24 hour after PiCCO.Several physiologic and biochemical indexes were recorded,including the hemodynamic parameters at the beginning and the 24 h of PiCCO monitoring,Scr,BUN,lactic acid,incidence and mortality of AKI,baseline glomerular filtration rate,baseline Scr,APACHE Ⅱ scores,mortality during the period of emergency ward or within 28 d after the diagnosis.Results A total of 26 cases in high GEDVI group (81.3％) were attacked with AKI,while 16 cases in low GEDVI group (55.2％) were attacked with AKI,the incidence of AKI in high GEDVI group was significantly higher than that in the low GEDVI group.A COX regression analysis of mortality was performed between the patients staying at emergency ward and during 28 d after diagnosis.The results indicated that AKI and GEDVI had no relation with patients' death.Therefore,AKI and GEDVI could not be considered as the risk factors for the prognosis.Conclusions High GEDVI can significantly increase the incidence of AKI after septic shock,therefore high GEDVI should be avoided as much as possible in the course of clinical treatment.

Objective To investigate the effects of regular insulin (RI)on duodenal smooth muscle in diabetic mice. Methods Diabetes mellitus (DM) model was established by intraperitoneal injection of 150 mg/kg streptozotocin (STZ) in male BALB/c mice. The model mice were divided into DM group and DM treated with RI group with 6 each. Meanwhile, 6 normal mice were served as controls. The mice in treatment group were intraperitoneally injected with 40 U/kg of RI daily.Whereas the mice in DM and control groups were intraperitoneally injected with phosphate buffer solution (pH = 7. 40). After 6 weeks, the small intestinal transit rate of mice was determined by lavage of Indian ink. Interstitial cells of cajal (ICC) in duodenal myenteric plexus were counted using immunohistochemical staining. Slow waves of duodenal smooth muscle cells were recorded with intracellular recordings. Data were analysed by SPSS 17.0 software, and comparisons among three groups were done using LSD test. Results After intervention for 6 months, the clinical presentations,such as more water and food intake and polyuria, were improved in treatment group. The body weight was increased in treatment group [(23.33±3.13) g] compared with DM group [(15.42±1.40) g,P＜0.01] ,but dereased compared with control group [(26.78 ± 2.09) g, P＜0.05]. The level of blood glucose in DM group was significantly higher than that in control and treatment groups(P＜0.01). Small intestine transmission rate was significantly reduced in DM group than that in control and treatment groups (P＜0.01), but it was slower in treatment group than that in control group (P＜ 0. 01 ). Immunohistochemical study showed that the number of c-kit positive cells reduced obviously in DM group than that in control group and treatment group (P＜0.05), whereas it was lower in treatment group than that in control group (P ＜ 0.05). The slow wave frequency and amplitude of duodenal smooth muscle cells in DM group were reduced when compared with control and treatment groups (P＜0.01) and both were lower in treatment group than that in control group (P＜0. 01 ). Conclusion The findings indicate that DM mice have gastrointestinal dysmotility and exogenous insulin may improve small intestinal dysmotility in DM mice.
