2.Comparison between Liposuction and Open Surgery for Accessory Breast

Objective To compare the efficacy of liposuction and open surgery for patients with accessory breast.MethodsA total of 73 patients with accessory breast,who were treated in our hospital from June 2000 to December 2007,were enrolled in this study and divided into open surgery(June 2000 to June 2004,43 patients with accessory breast at 80 sides)and liposuction(April 2004 to December 2007,30 cases with accessory breast at 58 sides)groups.The outcomes of the patients,operation time,and intraoperative blood loss in the two groups were compared.Results The curative rate of the both groups were 100%.However,the open surgery group had more blood loss [(49.6?13.3)ml vs(28.9?6.9)ml,t=10.836,P=0.000] and longer operation time[(57.4?11.1)min vs(27.3?4.3)min,t=19.597,P=0.000].Conclusions Compared with open surgery,liposuction leads to quicker recovery and less trauma without leaving scars,and thus is more acceptable to the patients.
3.Increased carotid intima-media thickness in type 2 diabetes mellitus and risk factors.

Objective To investigate the carotid intima-media thickness(C-IMT) in type 2 diabetes mellitus(T2DM) and the relationship with risk factors. Methods From 2002-01 to 2003-03, we measured C-IMT by B-mode ultra-sonography in 67 patients with different durations of T2DM. Blood was drawn from patients for the analysis of serum creatinine and C-reactive protein(CRP). 24-hour urine collection was performed to determine the level of urine creatinine clearance. Results C-IMT in patients increased with durations and with decreasing creatinine clearance. Multiple regression analysis revealed that C-reactive protein significantly and independently affected the C-IMT. Conclusion C-IMT increases with durations of T2DM and with BMI of the patients. CRP is the risk factor for carotid atherosclerosis in T2DM.
4.Clinical analysis of 12 cases of primary synovial chondromatosis of hip joint

Objective To study the clinical, imageological and pathologic characters of primary synovial chondromatosis of hip joint, and effect of open surgery. Method 12 cases of primary synovial chondromatosis of the hip joints admitted from May 1973 to May 2004 were retrospectively studied in respect to clinical manifestations, X-ray picture, and pathological findings. The effects of operation were followed up. Results Intermittent pain, swelling, limitation of hip joints were the main complaints. Mass and myatrophy near involved hip joint were discernible in a few patients. Although there was no obvious image of calcification, swelling of articular capsule was seen in X-ray film in 4 patients. CT scan showed arthroedema, loose bodies and bone erosion of the femoral head in the hip joint. The X-ray films of the other 8 patients showed obvious calcified loose bodies. All cases were diagnosed by pathology after operations. 8 cases were categorized Milgram Ⅱstage, and 4 cases Milgram Ⅲ stage. Eight patients were followed-up for a period of 1 to 7 years (mean 5 years). Recurrence occurred in 2 cases. Conclusion The diagnosis should be based on clinical characters, X-ray films and pathologic findings, and the pathology is most reliable. In order to decrease recurrence, CT or MRI examinations should be taken before the operation. X-ray monitoring should be carried out during the operation of the patient in whom loose bodies were found in X-ray film before the operation.
5.Thyroid cancer stem cells and thyroid cancer treatment:theory and applications

BACKGROUND:Thyroid cancer stem cells are one of the reasons for tumor resistance that promotes tumor development. The research of thyroid cancer stem cells has provides a new clinic means for the diagnosis and treatment of thyroid cancer. OBJECTIVE:To overview the discovery, identification, and correlation of thyroid cancer stem cells with thyroid cancer. METHODS:A computer-based online search of PubMed database and Wan Fang database between 1995-01/2014-01 was performed to search related articles with the key words of“thyroid cancer, cancer stem cell, stem cell, cancer suppressor gene”in English and Chinese, respectively. Literatures related to thyroid cancer stem cells were selected;in the same field, the articles published lately in authoritative journals were preferred. RESULTS AND CONCLUSION:A total of 561 literatures were primarily selected, and 57 documents were involved for result analysis according to inclusion criteria. Cancer stem cells have become a focus in the study of carcinogenesis. Stem cells also exist in the thyroid gland and its tumor. At present, there are several ways to isolate and identify thyroid cancer stem cells. The tumor thyroid stem cells are closely related to the occurrence, transfer and treatment of tumors. But whether we can cure thyroid cancer through restraining or eliminating thyroid cancer stem cells is stil unknown that needs further studies.
6.EARLY TREATMENT FOR THE FRACTURE OF MAXILLOFACIAL BONE COMBINED WITH CEREBRAL INJURY

To assess early treatment for fracture of the maxillofacial bone complicated by cerebral injury, 120 patients were retrospectively reviewed. The change in GCS, the incidence of stress ulceration, nosocomial pneumonia and the injury of liver or kidney were determined 1 week after fixation of the fracture of the maxillofacial bone at different stages. There was no significant influence of the time of the operation on the severity of brain injury and the incidence of the complications. It is our assertion that the bone fracture could be fixed at an earlier stage, and RIMF is a kind of satisfactory fixation method.
7.Effect of puerarin injection on the expression of protein kinase B in experimental insulin resistance rats

AIMTo develop a rat model of insulin resistanc e,and to study the effect of puerarin injection on expression of protein kinase B in insulin resistanc e rats. METHODS30 SD rats were randomly divded into two groups. The model group were given the high fat diets(0 30 pig fat, 0 10 sucrose, 0 0 3 cholesterol) for 8 weeks. Then 9 rats of the group had been choosen as the pu erarin treatment group and were given abdominal injection (100 mg?kg -1 ?d -1 ) for 4 weeks. By the end of the experiment, Western blot and computer i mage pattern analyze system were used to analysis expression of PKB in skeletal muscles. RESULTS(1) The model group showed a hyperglycemia, hype rinsulinism and obviously visceral obesity by high fat-feeding, the insulin res istance index (ISI) decreased while the HOMA insulin resistance index (HOMA-IR) increased compared to the normal group, insulin resistance was induced in this way; (2) Compared with the pathology control group:fasting blood glucose (FBG), fasting insulin (FINS), visceral fat mass and the HOMA insulin resistance index (HOMA-IR) of the treating group were significantly decreased whereas the insuli n index (ISI) enhanced. Expression of PKB was markedly increased by puerarin tre atment. CONCLUSIONPuerarin injection can significantly elevate e xepression of PKB and ameliorate the state of insulin resistance. Improved biolo gic effect of insulin and prevention of the deleterious effect of fat may be in volved in the mechanism concerned.

Background C57BL/6andB10R Ⅲareroutinemurinespeciesusedinexperimental autoimmune uveitis (EAU).The inflammation is light for mouse after immunization whereas it is prominent for B10R Ⅲ.ObjectiveThis study was to observe the killing effect of interphotoreceptor retinoid binding protein (IRBP) 1-20-specific T cells on mouse retinal astrocyte.Th1 and Th17 cells effect in the EAU mechanism was discussed.MethodsB10RllⅢ mice and C57BL/6 mice were immunized with IRBP 161-180 and IRBP 1-20 in complete Freund adjuvant (CFA).The infiltrating cells of diseased B10R Ⅲ eyes were analyzed by flow cytometry.IRBP 1-20-specific T cells were isolated from the drainage lymph node and spleen and cultured in IL-2 or IL-23 for Th1 and Th17 cells polarization,respectively.Th1 and Th17 cells cultured for 5 days were seeded on the mouse retinal astrocyte monolayer pretreated with gamma interferon.Cell interaction was observed and the quantity of TNF-α was tested by ELISA.Every test was repeated 6 times and the mean was calculated.The maintenance of experimental animals complied with the Statement of ARVO.ResultsThere were lots of infiltrating cells in the eyes of B10Rm mice after immunization,including 9.5% IFNγ+ cells,5.1% IL-17+cells and 41.4% CD45+ cells.Six days after IRBP1-20 stimulation and cultured by IL-2 and IL-23,44.0% and 8.0% cells were IFNγ+,and 1.0% and 26.0% cells were IL17+.Twentyfour hours after the interaction between Th1 or Th17 and retinal astrocyte,retinal astrocyte died and detached.The killing effect of Th17 was stronger than Th1.48 hours after co-culture of Th1 or Th17T cells with astrocytes,the concentrations of TNF-α were ( 500± 10 ) and ( 801 ±24 μg/L) μg/L,respectively,with a significant statistical difference (t =-20.36,P =0.00).ConclusionsBoth Th1 and Th17 can kill retinal astrocyte,but Th17 plays a key role in the EAU pathogenesis process.The killing effect is caused by intercellular contact and interaction under the induction of cytokines.
10.T-SPOT .TB clinical value in latent tuberculosis infection by immunosuppressive therapy

Objective To explore the T‐SPOT .TB technology in latent tuberculosis infection (LTBI) who immunosuppres‐sive therapy results in screening for latent tuberculosis infection prevention and control to provide a new basis .Methods Applica‐tion of T‐SPOT .TB kit 162 immunosuppressed patients need to be applied to detect M .tuberculosis‐specific T cells ;while doing all cases tuberculin (TST ) skin test ;of which 28 cases of T‐SPOT .TB‐positive patients before screening technique using anti‐TNF‐αbiologics were given prophylactic treatment of anti‐TB drugs for 4 months and followed a year .Results The positive rates and ac‐curacy rate of T‐SPOT .TB assay were 36 .4% and 94 .9% ,while the positive rates and accuracy rate of TSTs were 28 .4% and 69 .6% .The difference between T‐SPOT .TB assay and TST were statistical significance(P < 0 .05) .Through our 28 cases of T‐SPOT .TB positive screening technology ,prophylactic anti‐TB drugs to treat patients for 4 months and 1 year of follow‐up ,no case of tuberculosis occurred .Conclusion These results demonstrate that the performance of T‐SPOT .TB is better than the classic TST for detection of LTBI in patients receiving immunosuppressive therapy for treatment of systemic autoimmune disorders .The T‐SPOT .TB assay will be a useful tool in early and rapid diagnosis of latent tuberculosis infection .T‐SPOT .TB for LTBI patients di‐agnosed with prophylactic anti‐TB drug treatment is necessary ,has important clinical significance .
