Objective To analyze the significance of global end-diastolic volume index (GEDVI) in acute kidney injury (AKI) after septic shock. Methods A retrospective analysis of 61 patients was performed. The patients were diagnosed of septic shock in emergency ward of Shenyang Military Hospital from 2012 March to 2013 May and were monitored by pulse indicator continuous cardiac output (PiCCO).The patients were divided into two groups:low GEDVI group (GEDVI < 700 ml/m2,29 cases) and high GEDVI group (GEDVI≥700 ml/m2,32 cases) by evaluating GEDVI of 24 hour after PiCCO.Several physiologic and biochemical indexes were recorded, including the hemodynamic parameters at the beginning and the 24 h of PiCCO monitoring,Scr,BUN,lactic acid, incidence and mortality of AKI,baseline glomerular filtration rate, baseline Scr,APACHE Ⅱ scores, mortality during the period of emergency ward or within 28 d after the diagnosis. Results A total of 26 cases in high GEDVI group (81.3%) were attacked with AKI,while 16 cases in low GEDVI group (55.2%) were attacked with AKI,the incidence of AKI in high GEDVI group was significantly higher than that in the low GEDVI group.A COX regression analysis of mortality was performed between the patients staying at emergency ward and during 28 d after diagnosis. The results indicated that AKI and GEDVI had no relation with patients' death. Therefore,AKI and GEDVI could not be considered as the risk factors for the prognosis. Conclusions High GEDVI can significantly increase the incidence of AKI after septic shock, therefore high GEDVI should be avoided as much as possible in the course of clinical treatment.

Objective To study the radiographic findings in the diagnosis of pulmonary lymphoma and pseudolymphoma. Methods Eight patients with pulmonary lymphoma and 2 with pseudolymphoma were examined by X-ray film, tomography, and CT. Results Single or multiple nodules or masses were observed in 8 patients with pulmonary lymphoma, shaggy borders or halo of ground-glass attenuation in 7 patients, 2 patients had multiple patchy infiltrates bilaterally, 2 had diffuse interstitial infiltrates and 1 had miliary nodules. Multiple consolidations with air bronchogram and without hilar and mediastinal lymphadenopathy were observed in 2 patients with pulmonary pseudolymphoma. Conclusion Radiographic findings of pulmonary lymphoma were varied, the most common findings were the nodules or masses with shaggy borders or halo of ground-glass attenuation. The specific findings of pulmonary pseudolymphoma were multiple consolidations with air bronchogram and without hilar and mediastinal lymphadenopathy. The final diagnosis relied on pathology.

Objectives To analyze the main pathogen of otitis media and antibiotics resistance of clinical bacterial isolates in pediatric patients. Methods Secretion specimens from 164 cases of otitis media were cultured. Antimicrobial susceptibility was determined. Results were analyzed according to the guidelines of Clinical and Laboratory Standard In-stitute (2012). WHONET 5.6 software was used to analyse the data. Results Pathogens were cultivated in 121 ear secretion specimens from 164 cases of otitis media and the positive rate was 73.8%. There were 9 cases of mixed bacterial infection, accounting for 5.5%. 130 strains of pathogens were isolated from middle ear secretion and the main pathogens were bacteria (83.1%) and fungus (16.9%). Among bacteria, 50 (46.3%) were Streptococcus pneumoniae and 21 (19.4%) Staphylococcus aureus. Among fungus, 17 (77.3%) were Candida spp and 5 (22.7%) Aspergillus spp. By age, the rates of infection caused by Streptococcus pneumoniae in infancy and early childhood were markedly higher than those in preschool and school-age, respectively (P<0.05). The no-susceptibility rate of Streptococcus pneumoniae to penicillin was 76.0%. The prevalence of meticillin-resistant Staphylococcus aureus (MRSA) was 14.3%. Conclusions Proper treatment depends on the secretion culture and drug sensitive test due to various pathogens of otitis media.
