1.Effects of zuoguiyin and its partial prescriptions on metabolism of free radical in senile mice

BACKGROUND: The method of replenishing kidney has becoming one of the therapies for anti-senility in gerontology in Chinese medicine.OBJECTIVE: To probe into the effects of the formula for nourishing kidney yin-zuoguiyin on serum malondialdehyde (MAD) content and superoxide dismutase (SOD) activity in metabolism of free radical.DESIGN: Complete randomized control experiment.SETTINGS: Microbiology and Immunology Department of Guangdong Medical College.PARTICIPANTS: The experiment was performed in Immunology Research Room of Wannan Medical College from September 2002 to July 2004. Forty male senile ICR mice were employed, provided by Institute of Experimental Animal of Medical Scientific Academy in Sichuan. They were randomized into senile control, zuoguiyin group, single shudi (Radix Rehmanniae Praeparata) group (single group) and shudi (Radix Rehmanniae Praeparata) removal group (removal group), 10 mice in each one. Zuoguiyin composes of shudi (Radix Rehmanniae Praeparata), shamyao (Rhizoma Dioscoreae), gouqi (Fructus Lycii), zhi gancao (Radix Glycyrrhizae Praeparata),fuling (Poria) and shangyurou (Fructus Corni) at the ratio of 9:6:6:3:4:5. After decocted routinely, the solution in zuoguiyin group was concentrated to 0.577 5 g/mL of raw herbs contained; that in single group was to 0.157 5 g/mL and that in removal group was to 0.42 g/mL. The mixed medical solutions of 100 g/L were prepared successively in each group.INTERVENTIONS: In zuoguiyin group, single group and removal group,solutions of zuoguiyin and shudi (Radix Rehmanniae Praeparata) and solution with shudi (Radix Rehmanniae Praeparata) removed were administrated successively, 4 times/day, 1.5 mL/time, totally for 75 days. No any drink was provided during the medication. The mice in senile control drank pipe water. After the eyeball extracted, blood of 2 mL collected and serum preserved, the thiobarbituric acid method was used to determine MAD content, the xanthine oxidase method was to determine SOD activity and the colorimetric method was to determine general capacity of anti-oxidation in serum.MAIN OUTCOME MEASURES: ① serum MAD content; ② serum SOD activity; ③ general capacity of anti-oxidation in serum.RESULTS: One mouse was dead in senile control and 3 mice in single group. Terminally, 36 mice entered result analysis. ① MAD content:Serum MAD content in zuoguiyin group and single group were lower than that in senile control [(5.280±1.204) μmol/L and (5.886 2.717) μmol/L,(9.533 3.494) μmol/L, (q=3.641-4.657, P < 0.05)]. Serum MAD content in zuoguiyin group and single group were basically same. ② SOD activity: SOD activity in zuoguiyin group and removal group were higher than senile group [(7.008±0.782) mkat/L and (6.989±0.809) mkat/L, (5.908±0.392) mkat/L,(q=3.641-4.657, P <0.05)]. SOD activity in zuoguiyin group and removal group were basically same and that in single group and senile control were basically same. ③ General anti-oxidation capacity: The general capacity of anti-oxidation in serum in zuoguiyin group and single group were stronger than that in senile control, that in removal group was weaker than senile control, but the differences were not significant (P > 0.05).CONCLUSION: Zuoguiyin remarkably reduces MAD content and improves SOD activity in serum of senile mice. It is indicated that zuoguiyin acts remarkably on improving metabolism of free radical in senile body,strengthening SOD activity and reducing organic injuries caused by MAD accumulation.
2.Effects of zuoguiyin and its partial prescriptions on cellular immune function in senile mice

Objective:To explore the effect of zuoguiyin and its partial prescriptions on cellular immune function in senile mice.Methods:Healthy male ICR senile mice were divided into 4 groups at random: old contrast group, ZGY group, prepared rhizome of rehmannia(PROR) group and none prepared rhizome of rehmannia(NPROR) group. The senile mice had been drunk zuoguiyin and its partial prescriptions for 75 days, then observing the differences through thymic index, spleen index, activity of T lymphocyte and the level of IL-2 after using different drugs.Results:Acted by ZGY, ZGY had enhanced cellular immune function of senile mice, appearing that thymic index, spleen index, activity of T lymphocyte and the level of IL-2 were distinctly higher than those of the old contrast group. Compared to ZGY,prepared rhizome of rehmannia also enhanced the activity of T lymphocyte. However, it had no effect on thymic index, spleen index and the level of IL-2.So its effect on anti-senility was not better than ZGY. None prepared rhizome of rehmannia group had no distinct effect on those indexes.Conclusion:Zuoguiyin had an action of antisenility by improving cellular immune function.
3.Investigation on the Prevalence of Human Demodex Among 2 248 Medical Students in Inner Mongolia

Objective To understand the difference of Demodex prevalence among medical students living and studying together with different classes, sexes and nationalities. \ Methods\ Demodex folliculorum and Demodex brevis were detected by using cellophane tape on the nasolabial grooves. \ Results\ The overall prevalence was 51\^5% in 2 248 medical students. Prevalence in different classes and sexes was: freshman 42\^6%, sophomore 49\^6% and third year students 66\^8%; 55\^7% in males and 48\^7% in females. 4.Effect of Ultrafiltrated Components of Three Kinds of Serum Containing Phlegm-resolving Prescriptions on Viability of THP-1 Cell-derived Macrophages and Macrophage Differentiation to Foam Cells

Objective To observe the effect of ultrafiltrated components of three kinds of serum containing phlegm-resolving prescriptions (Huanglian Wendan Decoction, Gualou Xiebai Banxia Decoction, and Erchen Decoction, respectively) on viability of human leukemic monocytic cell line THP-1 derived macrophages and macrophage differentiation to foam cells. Methods THP-1 cells were induced to differentiate into macrophages with phorbol-12-myristate-13-acetate ( PMA) . And then the achieved macrophages were used for the establishment of the atherosclerotic foam cell model after co-incubation with oxidized low density lipoprotein ( ox-LDL ) . Three kinds of serum containing phlegm-resolving prescriptions and blank serum were prepared and ultrafiltered by 10 KD ultrafiltrator, and then were co-cultured with the foam cell model, respectively. The viability of macrophages and the macrophage differentiation to foam cells were tested by methyl thiazolyl tetrazolium ( MTT) assay and oil red staining. Apoptotic rate of macrophages was determined by flow cytometry. Results After THP-1 cells were induced by PMA (100 nmol/L) for 20 hours and then were co-incubated with ox-LDL (50 μg/mL) for 24 hours, a typical atherosclerotic foam cell model was established successfully. The ultrafiltrated components of serum containing Huanglian Wendan Decoction had obvious effects on increasing the macrophage survival rate, decreasing macrophage apoptotic rate, and inhibiting macrophage differentiation to foam cells. The ultrafiltrated components of serum containing Gualou Xiebai Banxia Decoction or Erchen Decoction had an effect on enhancing the macrophage differentiation to foam cells but had no anti-apoptosis action. Conclusion Huanglian Wendan Decoction has anti-atherosclerosis action, which may be related to resisting macrophage apoptosis and decreasing macrophagic lipid-endocytosing function. Erchen Decoction and Gualou Xiebai Banxia Decoction are effective on increasing macrophagic lipid-endocytosing function.
5.Study on the application value of leukocyte,procalcitonin and C-reactive protein in early diagnosis of bloodstream infection

Objective To explore the clinical significances of leukocyte (WBC ) ,C‐reactive protein (CRP ) and procalcitonin (PCT) in early diagnosis of bacterial bloodstream infections (BSI) ,in order to provide references for early diagnosis of BSI . Methods The detection results of WBC ,PCT and CRP from 48 cases of patients with positive blood culture(positive blood culture group) and 50 cases of patients with negative blood culture(negative blood culture group) were retrospectively analysed .Then com‐pared the detection results between the two groups ,and between patients with gram‐positive bacteria infection and those with gram‐negative bacteria infection in the positive blood culture group .The diagnostic performance of WBC ,PCT and CRP were determined by drawing the receiver operator characteristic(ROC) curves ,and areas under ROC curves(ACUs) were calculated .The perform‐ance of the three indicators in predicting BSI were evaluated by using the binary logistic regression analysis .Results The levels of PCT and CRP in positive blood culture group was significantly higher than those in negative blood culture group ,had statistically significant differences(P<0 .05) ,while no statistically significant difference was found in WBC count between the two groups(P>0 .05) .In the positive blood culture group ,statistically significant difference was only found in PCT level between patients with gram‐positive bacteria infection and patients with gram‐negative bacteria infection(P<0 .05) .The AUCs of WBC ,PCT and CRP were 0 .579 ,0 .746 and 0 .624 ,respectively .The result of the binary logistic regression analysis showed that only PCT made a unique statistically significant contribution to the model(P=0 .013) for predictive diagnosis of BSI .And the positive predictive rate of joint detection of these three indicators was 71 .4% .Conclusion WBC ,PCT and CRP could be indicators for early diagnosis of BSI ,while PCT and CRP might have much more important significance ,moreover ,PCT could distinguish between gram‐positive bacterial in‐fection and gram‐negative bacteria infection .
6.Relationship between the Demodex and Bacteria Infection in Human Rosacea

Objective To understand the relationship between Demodex and bacteria infection in rosacea (brandy nose), and to find effective means for the treatment. MethodsCellophane tape was used to detect Demodex on the nasolabial grooves and the face; sebum and tissue on face was scraped and cultured to examine bacteria under microscope. The hospital_made anti_rosacea lotion was used on the affected part two times a day for 7 days. Results It was found that 193 (74^2%) of 260 cases with rosacea were infected by Demodex and 209 (80^4%) of the patients were infected by bacteria. The overall effective rate of the treatment for rosacea was 73^5%. Conclusion Bacteria infection in rosacea is an important factor inducing rosacea. The curative effect of the anti_rosacea lotion is good.
7.Analysis of clinical knowledge and clinical skills scores in obstetrics and gynecology training

Objective To evaluate obstetrics and gynecology training quality by analysis of clinical knowledge and clinical skills scores for long-term graduates.Methods A retrospective analysis of clinical knowledge and clinical skills scores was performed in obstetrics and gynecology training for 134 long-term graduates.Results The clinical knowledge and clinical skills scores of obstetrics and gynecology training were 82.27?7.68,87.69?7.24.There was no,P﹥0.05.Only the scores of clinical skills judged by one teacher in interview was correlated with clinical knowledge scores,while there were no correlation between clinical knowledge judged by other five teachers and scores of clinical skills.Conclusion There was no correlation between clinical knowledge and the scores of clinical skills scores.Culturing students’practical ability and communication ability is worth strengthening,while the consistency of teachers’evaluation standard should be improved.
8.Range of motion of lumbar pedicle screw entrance point under physiological weight bearing

Objective To measure the range of motion (ROM) of the lumbar pedicle screw entrance point (LPSEP) in vivo during unrestricted motion under physiological weight bearing.Methods Eleven healthy volunteers aged 45-60 years underwent MRI scans in a supine position.Three-dimensional (3D) models of L2-5 were constructed.Next,each volunteer was asked to stand and was positioned in the following sequence:standing,45° flexion,maximal extension,maximal left-right twisting,while two orthogonal fluoroscopic images were taken simultaneously at each position.The MRI models were matched to the osseous outlines of the images from the two orthogonal views to determine the position of the vertebrae in 3D at each position.Coordinate systems were established to study the ROM of the LPSEP.Results The predominant translations were along anteroposterior and craniocaudal axis from supine to standing position (average,2.44mm and 2.35 mm).Rotation of the LPSEP occurred mainly around the mediolateral axis(average,3.91 °).During flexion-extension movements of the trunk,the predominant translation were along anteroposterior and craniocaudal axis and rotation were around mediolateral axis.During lateral bending and twisting,ROM of LPSEP did not rotate or translate in one dominant direction.Instead,the resulting motion represented a combination of rotation and translation in different directions.There was no significantly difference in translation and rotation between the three axis in L4,5segment (P>0.05).Conclusion The kinematic behaviors of the LPSEP of the upper lumbar spine (L2,3 and L3,4) are similar which are different from that of the lower lumbar spine (L4,5).
9.Effect of Probiotics on Intestinal Flora Disturbance and Bacteria Translocation of Rats

Objective To investigate the effect of probiotic supplement on the microbiology,bacterial translocation,and gut barrier function of the rats with abdominal infection. Methods After making the models of cecal ligation and perforation,the SD rats were divided into two groups.The rats in the control group were administrated with parenteral nutrition.The rats in the experimental group were administrated with parenteral nutrition and probiotics via the needle jejunostomy and neck vein for 5 d.The feces in the cecum were cultured by anaerobic bacterial growth.The vena cava blood and the homogenated tissues of the lives,hungs and mesenteric lymph nodes were cultured to determine bacterial translocation. Results The quantity of normal intestinal bacteria in the experimental group and the control group had significant difference except Enterobacteriaceae.The quantity of L.acidophilus and Bifidobacteria in the experimental group were higher than those in the control group.The quantity of C.perfrigens,the potential pathogenic germ in the experimental group were lower than those in the control group(P
10.Clinical observation of double incision combined surgery of single - stab trabeculectomy and phacoemulsification

AlM:To observe the effects of double incision combined surgery of single - stab trabeculectomy and phacoemulsification.
METHODS: Totally 28 cases ( 30 eyes ) with glaucoma and cataract undertook the modified combined surgery of single - stab trabeculectomy and phacoemulsification. After traditional phacoemulsification, cut the bulbar conjunctiva and Tenons capsule from the 11 o'clock to 1 o'clock, then puncture into the anterior chamber in 2mm behind the corneal limbus with 3mm tunnel knife, shaping a 3mm wide, 1/3-1/4 thickness scleral tunnel. Getting into the trabecular tunnel, bite off 3 pieces of trabecular tissue about 1mm × 1mm size. The changes in the imtraocular pressure ( lOP ) and the visual acuity before and after the surgery as well as filtering bleb ( OCT confirmed) and complications were carefully observed in 3-6mo postoperatively.
RESULTS: The postoperative visual acuity in 1wk postoperatively less than 0. 1 was found in 3 eyes, from 0. 1 to 0. 3 was found in 6 eyes,from 0. 3 to 0. 6 in 13 eyes, from 0. 6 to 0. 8 in 8 eyes. One eye had malignant glaucoma, and 8 eyes had cornea edema and slightly fibrin exudation in the pupil area; ln all cases maintained function conjunctival blebs of filtering, OCT confirmed this. lOP remained normal in 28 eyes in 3-6mo follow up, lOP of 2 other eyes could be controlled by anti-glaucoma eye drops.
CONCLUSlON:Double incision combined surgery of single- stab trabeculectomy and phacoemulsification is effective and safe, reduces the postoperative complications and is worthy of promotion.
