Furthering of the health reform calls for better health care system,greater public financing,and deepened reform of public hospitals.Introduced in this paper are the actions taken and outcomes of the health reform in Changchun city,including the essential medicines system to minimize drug prices,improvement of the primary healthcare system to minimize people's financial burden,greater public spending to enhance public welfare nature of medical institutions,enhanced competencies of primary medical institutions to introduce rational classification of patient flow,and enhanced equity of public services to promote people's health.On such basis,further reform proposals were made for Changchun's health reform.

Objective To compare the efficacy of rcmifentanil combined anesthesia and fentanyl or sufentanil combined anesthesia in patients undergoing cardiac surgery.Methods We searched the Coehrane library,PubMed,EMBASE,OVID and Chinese Biomedical Database for prospective randomized controlled trials involving the comparison of the efficacy of remifentanil combined anesthesia and fentanyl or sufentanil combined anesthesia in patients undergoing cardiac surgery.The quality of the studies was evaluated by the method recommended by Cochrane Collaboration.Evaluation indexes included the mechanical ventilation time after operation,duration of stay in hospital,and level of cardiac troponin,mortality,requirement for positive inotropic drugs and incidence of hyperalgesia and myocardial infarction during perioperative period.Meta-analysis was conducted using the RevMan 5.0 software.Results Sixteen prospective randomized controlled trials involving 1473 patients were included in our Meta-analysis.The patients were divided into 2 groups:fentanyl or sufentanil group ( n =644) and remifentanil group ( n =573).Compared with fentanyl or sufentanil group,the mechanical ventilation time after operation and duration of stay in hospital were significantly shortened,the level of cardiac troponin during the perioperative period was significantly decreased and the requirement for positive inotropic drugs during the perioperative period was significantly reduced ( P ＜ 0.05),and no significant change was found in the incidence of hyperalgesia or mortality of myocardial infarction during the perioperative period in remifentanil group ( P ＞ 0.05 ).Conclusion The efficacy of remifentanil combined anesthesia is better than that of fentanyl or sufentanil combined anesthesia in patients undergoing cardiac surgery.

Objective To evaluate the effectiveness of intratracheal injection of pulmonary surfactant (LISA) combined with continuous positive airway pressure (CPAP) on the treatment of neonatal respiratory distress syndrome (RDS). Methods The databases of PubMed, EMBASE, Cochrane, China National Knowledge Infrastructure (CNKI), Wanfang Database, Weipu Database and China Biology Medicine disc (CBM) were searched, and all of the randomized controlled trial of LISA combined with CPAP for treatment of RDS were included. RevMan 5.2 was used to perform meta-analysis. The differences of mortality, bronchopulmonary dysplasia (BPD), retinopathy of prematurity (ROP), intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), and other complications between LISA group and control group were compared in <34 weeks gestational age preterm infants with RDS. Results Five randomized controlled trials were included with 759 infants in total. Compared with control group, the rate of mechanical ventilation (RR: 0.32,95% CI: 0.13-0.82) and the incidence of BPD (RR: 0.61,95%CI: 0.42-0.88) were significantly decreased in LISA group. There were no differences in mortality and the incidences of ROP, IVH, and PVL between two groups (P>0.05). Conclusion The mechanical ventilation and the incidence of BPD is significantly reduced in the treatment of neonatal respiratory distress syndrome by LISA, but it has no effect on the incidences of other complications and mortality.

Objective To compare selenium content in hair samples of children in Kashin-Beck Disease (KBD) areas and control areas,and to provide academic data for KBD control and prevention.Methods In 2011,Longnan,Qingyang,Dingxi City of KDB areas in Gansu Provines were selected as survey point,meanwhile,colleagues of Tianshui City and Lanzhou City were selected as control.Hair samples of children aged between 6 and 12 years were taken from each survey point to determine the hair selenium.After using the nitricperchloric mixed acid to digest the sample,the concentrations of selenium were determined by hydride generator atomic fluorescence spectrometry.Results Two hundred and fifty-four samples from KBD areas and 102 samples from control areas were collected.The hair selenium of KBD areas[(0.22 ± 0.07),(0.22 ± 0.00),(0.20 ± 0.07)mg/kg],were higher than the control areas[(0.32 ± 0.08),(0.42 ±0.11)mg/kg].After comparation of selenium content in every sampling point,we found the range of children's selenium content was relatively narrow in KBD areas (0.08-0.46 mg/kg).The minimum value and the maximum value in KBD areas were lower than that of control areas (0.15-0.51 mg/kg).A totle of 51.1％ (182/356) of children was seriously lack of selenium,the hair selenium content was ＜ 0.25 mg/kg.It was also found that Se content was not significantly different between boys and girls.Conclusions Children's selenium is deficient in KBD areas in Gansu.It is important to ensure salt supplement with selenium for a long period of time and through other ways to increase Selenium intake to control KBD.

Objective To explore the relation ship between expression of survivin gene in leukemia cells and its clinical effects, and to study the mechanism of survivin resist-apoptosis function in leukemia.Methods Survivin expression was detected by Western blots analysis and expressions of Fas and Caspase were examined by immunohistochemistry in 18 leukemia patients.Results Thirteen cases in peripheral blood mononuclear cell survivin positive expression was detected in 18 leukemia patients(72.2%), but no survivin expression in 10 normal persons. OBJECTIVETo develop a solvent desorption gas chromatographic method for determination of n-pentanol in the workplace air.

METHODSn-Pentanol in the workplace air was collected with activated carbon tubes, desorbed with 2% 2-propanol in carbon disulfide, separated with a nitroterephthalic acid-modified FFAP capillary column, and detected with flame ionization detector.

RESULTSThe limit of detection was 0.2 mg/L; the lower limit of quantification was 0.6 mg/L; the linear range was 0.6-4072.0 mg/L. The minimum detectable mass concentration was 0.2 mg/m3 for 1.5 L of air sample. This method was highly repeatable. The relative standard deviations were 2.3%-5.4%. The average desorption efficiencies were 86.9%-94.2%. The absorption efficiencies were 100%. The breakthrough volume was above 8.0 mg in 100-mg activated carbon. The samples in activated carbon tubes could be stored for at least 14 days at room temperature.

CONCLUSIONThe method is feasible for determination of n-pentanol in the workplace air.

Objective To explore the relationship between bronchial asthma severity and the levels of nerve growth factor(NGF) in induced sputum.Methods One hundred and seventy children with asthma were divided into two groups, including 87 cases of chronic duration group and 83 cases of acute phase group.Before treatment lung function test was conducted, induced sputum was taken immediaitely, the levels of NGF were measured by enzyme-linked immunosorbent assay and Eosinophils (EOS)in induced sputum were count.Comparative analysis was conducted.Results The levels of NGF,FEV1 ,FVC,PEF and EOS in duration group were significantly elevated compared with acute phase group (all P ＜ 0.05).Significant differences in NGF, FEV1, FVC, PEF and Eos were found by comparison of different extent in chronic duration group and acute phase group (all P ＜ 0.05), the levels of NGF were positive correlated with illness severity (all P ＜ 0.05) in children with afsthma in chronic duration and acute phase, the levels of NGF were positive correlation with FEV1, FVC, PE-Fand EOS (all P ＜ 0.05) in children with asthma in chronic duration, the levels of NGF were positive correlation with FEV1 and PEF in children with Asthma in acute phase,but the levels of NGF has no correlation with FVC and EOS (all P ＞ 0.05) in children with Asthma in acute phase.Conclusion The levels of NGF in induced sputum of children with asthma in acute phase are markedly elevated compared with children with asthma in chronic duration, the levels of NGF are positive correlated with illness severity in children with asthma in chronic duration and acute phase.

Objective To investigate the potential effects of simvastatin on angiotensin Ⅱ-stimulated secretion and proliferation of adrenocortical carcinoma H295R cells.Methods The H295R cells were divided into control group, Angiotensin Ⅱgroup, simvastatin group and Angiotensin Ⅱ plus simvastatin group.Cortisol in medium was determined by chemiluminescent method , and aldosterone was determined by radioimmunoassay .The mRNA expression of 11 beta-hydroxylase ( CYP11B1 ) and aldosterone synthase ( CYP11B2 ) were examined by RT-qPCR.Cell proliferation was detected by MTS method.Results Compared with control group, angiotensin Ⅱincreased the secretion of cortisol and aldosterone, and the expression of CYP11B1 and CYP11B2.Simvastatin decreased cortisol secretion and CYP11B1 mRNA expression ( P<0.05 ) .Simvastatin also inhibited angiotensinⅡ-induced the secretion of cortisol and aldosterone , and the expression of CYP 11 B1 and CYP11 B2 compared with Angiotensin Ⅱgroup ( P<0.05 ) .Angiotensin Ⅱhad no effect on cell proliferation , while simvastatin significantly inhibited cell proliferation .The inhibitory effect of simvastatin on proliferation was enhanced when simvastatin was prescribed with angiotensin Ⅱ( P<0.05 ) .Conclusions Simvastatin inhibits angiotensin Ⅱ-induced secretion of cortisol and aldosterone in H295R cells.Simvastatin inhibits cell proliferation, which could be enhanced by angio-tensin Ⅱ.
