3.Analysis of etiology and antibiotic resistance of clinical bacterial isolates in pediatric patients with otitis media

Objectives To analyze the main pathogen of otitis media and antibiotics resistance of clinical bacterial isolates in pediatric patients. Methods Secretion specimens from 164 cases of otitis media were cultured. Antimicrobial susceptibility was determined. Results were analyzed according to the guidelines of Clinical and Laboratory Standard In-stitute (2012). WHONET 5.6 software was used to analyse the data. Results Pathogens were cultivated in 121 ear secretion specimens from 164 cases of otitis media and the positive rate was 73.8%. There were 9 cases of mixed bacterial infection, accounting for 5.5%. 130 strains of pathogens were isolated from middle ear secretion and the main pathogens were bacteria (83.1%) and fungus (16.9%). Among bacteria, 50 (46.3%) were Streptococcus pneumoniae and 21 (19.4%) Staphylococcus aureus. Among fungus, 17 (77.3%) were Candida spp and 5 (22.7%) Aspergillus spp. By age, the rates of infection caused by Streptococcus pneumoniae in infancy and early childhood were markedly higher than those in preschool and school-age, respectively (P<0.05). The no-susceptibility rate of Streptococcus pneumoniae to penicillin was 76.0%. The prevalence of meticillin-resistant Staphylococcus aureus (MRSA) was 14.3%. Conclusions Proper treatment depends on the secretion culture and drug sensitive test due to various pathogens of otitis media.
4.Changing patterns and antibiotic resistance of Staphylococcus aureus in a pediatric clinic from 2003 to 2012 year

Objective To investigate the changing patterns of Staphylococcus aureus (SA) and its resistance from a paediatric clinic in Beijing Children's Hospital Affiliated to Capital University of Medical Sciences.Methods SA isolated from hospitalized patients in Beijing Children's Hospital Affiliated to Capital University of Medical Sciences from Jan.2003 to Dec.2012 were analyzed.Antimierobial susceptibility was determined by disk diffusion method and Phoenix 100 microbiological system.Results were analyzed according to the 2012 guidelines of Clinical and Laboratory Standard Institute.WHONET 5.6 software was used to analyze the data.Results The clinical isolates of SA from inpatient were 2 843.The positive rates of SA were 10.0%,8.7%,9.0%,11.6%,12.7%,11.7%,10.5%,10.8%,11.9% and 11.7% form 2003 to 2012,respectively.The positive rate of Methicillin-resistant Staphylococcus aureus (MRSA) from inpatients was 10.9%.The positive rates of MRSA were 3.1%,5.4%,6.4%,4.1%,6.0%,10.6%,11.2%,12.7%,14.1% aid 20.1% form 2003 to 2012,respectively.The resistant rate of Methicillin-susceptible Staphylococcus aueus and MRSA to Penicillin were 91.4% and 100.0%,Oxacillin were 0 and 100.0%,Cefuroxime were 0.5% and 87.7%,Ceftriaxone were 0.1% and 90.3%,Amoxicillin-clavulanic acid were 0.9% and 86.7%,Erythromycin were 72.4% and 92.2%,Clindamycin were 50.6% and 80.3%,Ciprofloxacin were 2.2% and 30.1%,Trimethoprim-sulfamthhoxazol were 12.2% and 13.9%.Vancomycin-intermediate and Vancormycin-resistant Staphylococcus aureus were not found.Conclusions The prevalence of MRSA is increasing in paediatric inpatients.And MRSA isolates exhibit multidrug resistance.The sequential surveillance about SA is very important for guiding rational antimicrobial therapy and effective control of infections in paediatric patients.
5.Management and Control of Hospital Infection in Operating Room

OBJECTIVE To explore the canonical prevention and management method to improve the control quality safety of hospital infection in operating room,and the medical care quality.METHODS The key points included infection institution,nosocomial control,operating room management,effective infection disinfection,isolation and other measures.RESULTS According to canonical scientific management available to quality safety of infection prevention,it could prevent infection in operating room.CONCLUSIONS In order to prevent nosocomial infection the accomplishment of effective disinfection and isolation measures during contaminated operation in hospital operating room is an important guarantee to the medical care quality.
7.Clinical study on depression of aged patients with cerebral stroke hemiplegia convalescence

Objective To evaluate the effects of Taoist cognitive psychotherapy on depression of aged patients with cerebral stroke hemiplegia convalescence. Methods Seventy-seven hemiplegia convalescence patients with depression were divided into general treatment group (38 patients, received general back-up psychology therapy) and combined treatment group (39 patients, received general back-up psychology therapy and Taoist cognitive psychotherapy) by random digits table. All patients were treated for 8 weeks and followed up for 6 months. Two groups were evaluated with HAMD and SS-QOL before treatment and at the end of the 2 weeks,4 weeks, 8 weeks and 6 months after treatment. The results were analyzed with statistics. Results In general treatment group, the HAMD scores were gradually decreased, and the HAMD scores of patients after 8 weeks' treatment were significantly lower than those before treatment[(22.35 ± 4.69)scores vs. (29.62 ± 5.95 ) scores,t = 6.425 ,P < 0.01]. At the end of 6 months after treatment, the scores increased [(24.48 ± 4.12 ) scores vs. (22.35 ± 4.69 ) scores, t = 2.014, P < 0.05], but they were lower than those before treatment(t = 4.836, P < 0.01 ). At the end of 6 months after treatment, the SS-QOL scores were lower than those after 8 weeks' treatment (t =2.543,P <0.05),but they were higher than those before treatment (t = 2.790,P < 0.05 ). In combined treatment group, the HAMD scores decreased continuously,and the scores after 8 weeks' treatment [(20.08 ± 4.60) scores] and 6 months' treatment [( 15.21 ± 3.42)scores] were significantly lower than those before treatment [( 30.14 ± 4.92 ) scores] (t = 8.341,15.443, P <0.01). Meanwhile,the HAMD scores after 6 months'treatment were significantly Iower than those after 8 weeks' treatment (t =4.724,P < 0.01 ). The SS-QOL scores after 8 weeks' treatment [( 117.56 ± 26.22)scores] and 6 months' treatment [(126.57 ±21.82) scores] were significantly higher than those before treatment[(86.54 ± 23.90) scores] (t = 6.716,8.916,P < 0.01 ) ,and there was significantly difference(t=2.378,P < 0.05). The HAMD scores of combined treatment group after 8 weeks' and 6 months' treatment were significantly lower than those of general treatment group at the same time(t = 2.118, P < 0.05 ;t = 8.405,P< 0.01 ) ,and SS-QOL scores were significantly higher than those of general treatment group at the same time (t = 3.123,P < 0.05 ;t = 6.580,P < 0.01 ). Conclusions General back-up psychology therapy combined with Taoist cognitive psychotherapy can improve depression and life quality of cerebral stroke hemiplegia convalescence in aged patients. The effects of Taoist cognitive psychotherapy is slower, but it is more beneficial in the long time.
9.Strengthening Management of Hospital Infection Control in Outpatient Operating Room

10.Effect of dietary fat acids on incretin and islet function

Objective To explore the effect of dietary fat acids on incretin and islet function in healthy adults .Methods Before each test, healthy subjects received a 1-week pre-experiment eucaloric diet .Fifteen subjects consumed two meals containing different fat acids , including high saturated fat acid ( HSF) and high monounsaturated fat acid ( HMF) .On two separate occasions,they underwent a minimum of 1-week washout between meals .At 0,30,60,120,180 and 240 min following meal intake, the plasma concentrations of gastric inhibitory polypeptide (GIP), glucagon-like peptide-1(GLP-1) and serum concentrations of glucose, insulin, triglycerides ( TG) and free fatty acid ( FFA) were measured.Results Postprandial glucose did not increase significantly following HSF and HMF meals (P>0.05).Compared with HMF meal, significant increase in AUCins240min,AUCTG240min and AUCFFA240minwas observed following HSF meal (P<0.05).Compared with HMF meal,a significant decrease in AUCGIP30min, AUCGIP240min, AUCGLP-1 30min and AUCGLP-1 240min was found following HSF meal (P<0.05).No significant difference was observed inΔI30/ΔG30 following HSF and HMF meals (P>0.05). AUCI/AUCG was significantly lower following HMF meal as compared with HSF meal (P<0.05).Conclusion This study demonstrates that the function of GIP ,GLP-1 andβcell is affected by the dietary fat acids in healthy adults .The HMF meal may stimulate GIP and GLP-1 secretion to a greater extent than HSF meal .
