Objective To evaluate the efficacy and toxicity of gimeracil and oteracil porassium capsules combined with nedaplatin in the treatment of the patients with distant metastatic nasopharyngeal carcinoma.Methods Fourty cases were randomly divided into the observation group and control group by random digital table.19 cases in the observation group were treated with oteracil potassium capsules combined with nedaplatin.21cases in the observation group were treated with gemcitabine combined with nedaplatin.The response rate,response rate(RR) and progression free survival(PFS),toxicity were evaluated according to response evaluation criteria in solid tumors (RECIST).Results The response rate were 52.6％,52.3％ respectively,the median progression-free survival were 5.5 and 5.2 months,respectively.Grade Ⅲ an Ⅳ platelet supression were 10.5％,38.0％ between the observation group and control group.It was statistically significant between the two group(x2 =4.89,P ＜ 0.05).Conclusion Oteracil porassium capsules combined with nedaplatin is similar with gemcitabine combined with nedaplatin in the distant metastatic nasopharyngeal carcinoma,the adverse reaction was lighter than that of the control group.

Recently these tools primarily were consisted in the comprehensive diabetes mellitus management instruments and a fraction of the ones aim at the evaluation of the dietary management for diabetes. Timely and effective assessment of the effect on diabetes diet management depends on these tools. Therefore, the feasibilities, usefulness and deficit about these instruments were analyzed to prompt the application and investigation of the instruments in the future.

OBJECTIVE:To establish an HPLC method for the determination of the content of Indirubin in Banlangen granules. METHODS: The choromatographic separation was performed on C18 column (250 mm?4.6 mm,5 ?m) with column temperature set at 30 ℃. The mobile phase consisted of methanol-water(76∶24) at a flow rate of 1.0 mL?min-1.The dete-ction wavelength was set at 289 nm.RESULTS:The calibration curve of Indirubin was linear in the range of 0.073 5～0.612 5 ?g (r=0.999 9) and the average recovery was 98.78%(RSD=0.74%,n=9).CONCLUISON: The method is rapid and accurate, and it can be used for quality control of Banlangen granules.

Objective: To investigate the efficacy of progesterone tablets in the patients with unexplained recurrent spontaneous abortion and the influence on cellular immunity factor.Methods: Totally 90 cases of unexplained recurrent spontaneous abortion patients were randomly divided into the observation group and the control group (45 ones in each).After pregnancy, the control group was given intramuscular injection of human chorionic gonadotropin, and the observation group started to be treated with progesterone tablets on the 35th day after menolipsis.The levels of interleukin-4 (IL-4), interleukin-10 (IL-10), interleukin-17 (IL-17) and IFN-γ were determined, and the expression of T helper lymphocytes (Th17/CD4+), regulatory T lymphocytes (Treg/CD4+) and T helper lymphocytes regulatory T lymphocytes (Th17/Treg) was compared before and after the treatment.The pregnancy success rate and abortion rate were compared between the groups after the treatment, and adverse reactions were also observed.Results: After the treatment, the expression levels of IL-4 and IL-10 in the observation group were higher than those in the control group, and the expression levels of IL-17 in the observation group was lower than that in the control group, and the differences between the groups were significant (P＜0.05);after the treatment, the expression of Th17/CD4+ and Th17/Treg in the two groups decreased, and that in the observation group was lower than that in the control group;after the treatment, the expression of Treg/CD4+ in the two groups increased, and that in the observation group was higher than that in the control group, and the difference was significant (P＜0.05);after the treatment, the expression of IFN-γ in the two groups decreased, and that in the observation group was lower than that in the control group;after the treatment, the pregnancy rate in the observation group was higher than that in the control group with lower abortion rate, and the differences were statistically significant (P＜0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P＜0.05).Conclusion: Progesterone tablets in the treatment of unexplained recurrent abortion patients show significant effects, and can effectively improve the cellular immune factor in the patients, which is worth promoted application in clinics.

OBJECTIVE:To study the HPLC fingerprint profiles of crude and processed Semen Cassiae so as to provide basis for the establishment of chromatographic fingerprint of Semen Cassiae.METHODS: The samples of Semen Cassiae were hydrolyzed in the 1.5 mol?L-1 hydrochloride acid and then reflux extracted with chloroform.The HPLC analysis was run in gradient elution.The whole information of single-wavelength chromatographic fingerprint,two-dimension information data at full wavelength 220 nm～550 nm and the data of 18 fingerprint peaks compared with chrysophanol were treated by cluster analysis.RESULTS:It was observed that the contents of several chemical components were higher in the crude or processed Semen Cassiae than in chrysophanol.There were differences between the crude and the processed Semen Cassiae in HPLC fingerprint profiles though they were of good similarity.CONCLUSION: The HPLC fingerprints of liposoluble constituents were obtained with good resolution under the experimental chromatographic system.Three pure chromatographic peaks of chrysophanol,emodin and physcion have been identified.More reliable cluster result can be obtained from two-dimension information data at full wavelength 220 nm～550 nm than from the whole information of single-wavelength chromatographic fingerprint or from the information of chromatographic fingerprint of characteristic peaks.

· Age-related macular degeneration (AMD) is the leading cause of legal blindness in individuals aged over 65 in the United States and other industrialized nations. Till now, we have limited choices of treatment for this kind of disease. Treatment available can be grouped into two major categories: physical and pharmacological therapies. The former received extensive attention with little success whereas the latter attract new attention with great hope of success. The pharmacological therapies indude photodynamic therapy (PDT), steroids, vascular endothelial growth factor (VEGF) inhibitors, extracellular matrix (ECM) modifiers, gene therapy, nutrition supplements, choroidal blood flow facilitators and the like. PDT treatment is the only available effective treatment for certain forms of neovascular AMD. Anecortave acetate,as a synthetic derivative of cortisol, might stabilize vision in patients with predominantly classic subfoveal choroidal neovascularization (CNV) for up to 6mo through subtenon juxtascleral depot application. Intravitreous injection of VEGF aptamer stabilized or improved vision in 87.5% of patients with subfoveal CNV 3mo after treatment. Malfunction of choroidal blood flow is found in early stage of AMD. Elevation of intravascular pressure is the crucial hemodynamic factor in age-related macular degeneration, resulting in a decrease of the blood flow of choriocapillaries. Chain reactions are triggered which lead to retinal pigment epithelium (RPE) degeneration,Bruch's membrane breakdown, CNV formation, AMD and blindness in the end. Therefore, specific drugs that can increase the choroidal blood flow could be very useful to prevent the AMD from developing and worsening. Although most of them are still in the experimental stage,it is hopeful to find a way to treat AMD at the early stage and to prevent the disease to be triggered and developed.

Objective To observe the pathological regression of rabbit liver after the low-dose irradiation with high intensity focused ultrasound (HIFU), and to explore the role and mechanism of the irradiation in changing acoustic environment of rabbit liver. Methods Pathological study was performed in 10 New Zealand white rabbits, the livers were observed under light and electron microscope. The observation was done immediately after low-dose HIFU irradiation and in 2, 3, 5, 7 days after the low-dose irradiation with HIFU. Results Light microscope changes: Edema existed in hepatocytes after irradiation instantly, congestion and aggravated edema were found at 2-3 days, then the injured cells recovered gradually. Electron microscope changes: Detached endothelial cells of hepatic sinusoid and swelling organelles were observed after irradiation, while 2 or 3 days later, erythrocyte aggregation was found in hepatic sinusoid and cavitation was found in cytoplasm. Thereafter, organelles swelling reduced till resumed to normal. Conclusion Low-dose HIFU irradiation can make corresponding changes to the acoustic environment of rabbit liver.

Objective To assess the influence of pre-exposure to non-ablating ultrasound on the effect of high intensity focused ultrasound(HIFU)ablation.Methods Forty rabbits with transplanted VX2 tumors on their livers were divided into three pre-exposure groups(60 W,80 W and 100 W)and a control group(O W),with 10 rabbits in each group.Each group was pre-exposed to lower intensity focused ultrasound at the corresponding power.From each group,5 rabbits were randomly selected to be exposed to HIFU on the next day.Each group received 14scans.The rabbits were sacrificed 24 h later to take tissue samples for tfipheny tetrazolium chloride(TTC)staining to measure the amount of coagulative necrosis.Hematoxylin and eosin(HE)staining and succinate dehydrogenase (SDH)were also measured to observe the structure and activation of pre-exposed tumors.Results After HIFU exposure,the ablated volumes increased with pre-exposure acoustic intensity,and all volumes were larger than those in the control group.The ablating efficiency in the 100 W group was the highest.The pre-exposure did not itself ablate tumor tissue,but in the 80 W and 100 W groups the structure and activation of the tissues changed.Those in the 60 W group were not obviously altered.Conclusion Prior exposure to non-ablating ultrasound can highly enhance the efficacy of HIFU ablation.

ME-THODS:Through reviewing the transformation process of hospital pharmacy and pharmacist's duty,the present structure,con-tent,working model and achievements of pharmacy in our hospital were introduced based onlarge pharmacyof taking clinical pharmacy as core,paying attention to personnel training,technical innovation,physican,pharmacist and nurse cooperation,multi-ple discipline integration,etc.;the direction of hospital pharmacy was explored. RESULTS:Many adjustments were made to facili-tate the smooth and orderly development of hospital pharmacy under the new situation,as that establish the rational drug use and quality safety system,drug supply and operation management system working mode;that the development strategy of pharmacy was identified asfocusing on the rational drug use,combining the management functionandproviding professional pharmaceuti-cal care for the clinic and patients,providing professional technical support for the hospital decision and medical management;that the construction of clinical pharmacy specialty/key discipline and drug clinical trial technology platform in accordance with the international standard and so on were lined in the project of pharmacy;that quality control circle was cited,pharmaceutical supervi-sion and pharmaceutical care were expanded to the public and the fine;that a fine,professional pharmacist team was cultured. CONCLUSIONS:The construction of future hospital pharmacy needs the concept oflarge pharmacywith multi-disciplinary inte-gration to develop towards standardization,refinement,specialization and modernization.
