Immediate placement of a silicone ball intraorbitally after enucleation prevents contraction of the orbit and retraction of the eyelids. The author placed the implant in the muscular cone behind the scleral shell, which prevented the frequent extrusion of the ball, and the intact attachment of the rectal muscles to the sclera was conducive to satisfactory motility of the ocular prosthesis. 8 cases received this modality of implantation with good results during a followup of 6～39 months. The author started using hydroxyapatite balls recently with promising success.

Objective To study the effect of letrozole (LE) on ovulation in patients with polycystic ovary syndrome (PCOS).Methods One hundred and sixty-four patients with PCOS were divided into LE treatment group (n=68) and chloramiphene (CC) treatment group (n=96).Growth rate of follicles,serum hormone level,endometrial thickness,and cervix mucus were observed before and after treatment.Results The growth rate of follicles was 1.67 and 1.78 mm/d,respectively,in LE and CC treatment groups (P=0.14).The serum T level on the day of LH peak was 2.1?1.9 and 2.0?1.7ng/dL,respectively,in LE and CC treatment groups (P=0.93).The serum testosterone (T) level on day of LH peak and on day 3 of menses was 2.1?1.9 and 1.4?1.5 ng/d,respectively,in LE and CC treatment groups (P=0.09).The serum E2 level on day of LH peak was 89.0?59.5 and 108.1?46.0 pg/ml,respectively,in LE and CC treatment groups (P=0.13).The endometrial thickness was 11.3?1.6 and 9.7?1.4 mm,respectively,in LE and CC treatment groups (P=0.07).The score of cervix mucus was 11.0?1.1 and 8.3?3.1,respectively,in LE and CC treatment groups (P=0.003).Conclusion Letrozole can effectively induce ovulation in patients with polycystic ovary syndrome,and does not increase serum T level.

Objective To observe the effects of storage condition on the quality of Semen Armeniacae Amarum. Methods Orthogonal design was adopted. The storage condition was optimized with temperature,humidity and storage time as the observation factors,with the content of amygdalin,and the content,acid value and peroxide value of fatty oil as the indexes.The content of fatty oil,acid value and peroxide value were measured according to the methods provided by 2005 edition of Chinese Pharmacopoeia. The content of amygdalin was measured by HPLC. Results Temperature was the main influencing factor of amygdalin content,then came humidity and storage time. Temperature was also the main influencing factor of the concent and the acid value of fatty oil,and the humidity was the second. For peroxide value,humidity was the main factor,then came temperature and storage time. Conclusion The optimal conditions of storing Semen Armeniacae Amarum as follows:storage in dry place with the temperature of 2～ 8 ℃,and in short storage time.

Methods: 40 cases of severe brain injury were randomly divided into experimental group and control group.The experimental group was treated by homogenized diet and the control group was treated by nutrison fibre.Parenteral nutrition was adopted when enteral nutrition was insufficient.The levels of blood glucose、serum proteins、nitrogen balance and complications were measured during nutrition support(1 d,15 d,30 d). Objective To explore the relationship between β2-microglobulin and D-Dimer concentration in the pregnancy-induced hypertension patients with kidney injury.Methods The 120 cases of kidney injury pregnancy-induced hypertension patients were enrolled in our hospital from Nov 2010 to Nov 2015 (served as observation group).The levels of BUN,Scr,UA,β2-microglobulin and D-Dimer were detected.The 100 cases of healthy subjects and 120 cases of normal pregnancy women were taken as controls,respectively.Results Compared with healthy control and normal pregnancy women,the levels of BUN,Scr,UA were increased significantly in the observation group (P<0.05).The results also showed that the expression of NGAL,NLRP3,NADPH oxidase subunits,TGFβ,Smad2,Smad3,β2-microglobulin and D-Dimer were sharply increased as compared with the other two group (P<0.05).Conclusion β2-microglobulin and D-Dimer are significantly related with pregnancy-induced hypertension patients with kidney injury and they would be taken as clinical indicators for the diagnosis of this disease.

Objective To investigate the diagnosis and treatment of Hashimoto's diseaseconcomitant with thyroid carcinoma. Methods Clinical data of 74 cages of thyroid carcinoma occuring on the background of Hashimoto's disease were retrospectively analyzed. Results All cases were papillary carcinoma pathologically.The serum TGAb and McAb level elevated in 56 and 68 cases,respectively.While TPOAB level was all elevated.45 cages underwent unilateral thyroideetomy and contralateral subtotal thyroidectomy,1 case did bilateral near total thyroidectomy,26 cases did unilateral thyroidectomy.and 2 cases with bilateral thyroidectomy.All cases underwent combined central region lymph nodes dissection(positive rate was 27%,20/74),10 cases underwent modied lymph node resection.69 cases were followed up rangins from 1 month to 7 years,with median length of 29 months.4 cases were re-admired for modified lymph nodes resection. Conclusion TGAb、McAb、FNAB and ultrasound is the main procedure forscreening Hashimoto's disese accompanied with thyroid carcinoma preoperatively.Radical thyroideetomy iS the therapy of choice.

Objective To assess the efficacy of high intensive narrow band blue light and red light combined with Cuochuangling granule,a traditional Chinese medicine (TCM),in acne vulgaris(AV).Methods Forty-two patients with mild to moderate AV were randomly divided into two groups.combination group(n=25)receiving photothempy with blue light and red light plus oral Cuochuangling granule,TCM group(n=17)treated with oral Cuochuangling granule alone.Cuochuangling granule was given thrice daily,and phototherapy was performed at a dose of 48 J/cm2 with a least interval of 48 hours.All patients were treated for 4 weeks.Before and after the treatment,sebum secretion level and water content in stratum corneum were measured by photometry and capacitance method,respectively,skin pH value was examined,and the number of lesions was counted.Results The total effective rate was 84%in the combination group,and 71%in TCM group.A significant increase was observed in the clearance rates of total lesions and inflammatory lesions in the combination group compared with the TCM group(both P＜0.01).The water content in stratum corneum increased from 210.47 to 294.70(P＜0.01)in the combination group,and the increase was greater than in the TCM group.No significant changes were noticed in the sebum secretion level or skin pH value alter the treatment in either group.Conclusions Phototherapy combined with Cuochuangling granule is highly effective in the treatment of mild to moderate acne,with an increase in the water content in stratum corneum after treatment.

Adiponectin is a kind of adipocyte-specific protein which is only inversely related with obesity by now. It is involved in enhancing insulin sensitivity,anti-atherogenic and antiinflammatory activities.Recent studies have reported that adiponectin is closely correlated with the genesis and development of a variety of obesity related malignant tumors,especially postmenopausal breast cancer.Obesity is an independent risk factor for the development of breast cancer.Recently researches about the correlation between adiponectin and breast cancer relations and the action mechanisms,signaling pathways have made a progress.

Objective To explore the relationship between chronic tonsillitis and allergic purpura.Methods Fifty-six allergic purpura children with chronic tonsillitis were selected,26 children were given tonsillectomy (operation group),and 30 children were not given tonsillectomy (no-operation group).The duration of abdominal pain,duration of erythra,proteinuria negative time,hematuria negative time,antistreptolycin O rite,complement C3 were compared between the 2 groups,and the 24 h urine protein quantitarion and urine red blood cell count in operation group were compared between preoperative and postoperative.Results The duration of erythra,duration of abdominal pain,proteinuria negative time,hematuria negative time in operation group were significantly better than those in no-operation group [(10.58 ±4.22) d vs.(16.70 ±9.68) d,(6.49 ±2.45) d vs.(9.20 ± 2.21) d,(12.3 ±4.1) d vs.(16.5 ± 5.6) d,(10.1 ± 3.3) d vs.(12.7 ± 4.9) d],there were statistical differences (P ＜ 0.01 or ＜ 0.05).There were no statistical differences in antistreptolycin O tite and complement C3 between the 2 groups (P＞ 0.05).In operation group,the 24 h urine protein quantitation and urine red blood cell count postoperative were significantly better than preoperative [(0.72 ± 0.28) g vs.(1.61 ± 1.22) g,(14.95 ± 4.76) / μ l vs.(56.61 ± 12.57)/μ 1],there were statistical differences (P＜ 0.01).Conclusion Chronic tonsillitis is one of the important factors which can make allergic purpura erythra relapses and induce purpura nephritis,and tonsillectomy is the optional means of treatment of purpura nephritis with chronic tonsillitis and erythra relapses.

With the intense development of neoadjuvant chemotherapy for breast cancer research in recent years, this approach has been used widely in clinical treatment. Neoadjuvant chemotherapy can increase the chance of operation for patients with local y advanced breast cancer. Moreover, it can improve breast-conserving rate in larger-tumor patients, thereby improving the quality of life of these patients. However, some controversies regarding the use of neoadjuvant chemotherapy for breast cancer patients remain to be resolved. This review focuses on clinical studies and re-recognition of neoadjuvant chemotherapy as a viable treatment for breast cancer.
