1.Clinical application of solution focused approach in the functional gastrointestinal disorders

Objective To application of solution focused approach in the functional gastrointestinal disorders.Methods 64 hospitalized clients with functional gastrointestinal disorders were randomly divided into the observation group and the control group with 32 cases in each group.We applied the normal nursing intervention to the control group,while applied the solution focused approach to the observation group.The anxiety and depression score,score of coping way and gastrointestinal symptoms were evaluated using t test.Results The scores of anxiety and depression,coping way and gastrointestinal symptoms were better in the observation group than those in the control group.Conclusions Adoption of solution focused approach can relieve FGID sufferers'negative emotion,such as anxiety and depression,increase sufferers' coping capacity and symptoms,which demonstrate pivotal significance for rehabilitation of functional gastrointestinal disorders.
2.Application of Microorganisms in the Degradation of Polluted Chemicals in Aquatic Environments

Every year, waters on earth receive large quantities of wastewater from industry, agriculture, fish and poultry raising, and municipal sewage treatment plants. Consequently, the aquatic environment on the earth is under a serious challenge from a very large quantity of pollutants such as antibiotics, insecticides, herbicides, hydrocarbons, etc., contained in the domestic wastewater, industrial and agricultural waste water and illegal effluents. In particular, with the development of intensive aquiculture and poultry, the effluent pollution has recently become more and more serious with more attentions. Furthermore more and more chemical pollutants discharged into aquatic environment have been detected with the advancement of analytical techniques. These chemicals can cause toxic effects on water habitats after discharged into aquatic environment. However, microorganisms have many key functions in pollution control. In this review, applications of microorganism in the degradation of chemicals in aquatic environments are reviewed. It was concluded that most applications of microorganisms degrading chemicals focused on aquaculture waters, whereas other aquatic systems (such as river, lake, sea, coastal waters) have been scarcely studied.
3.Analysis of the Results of Drug Sampling and Testing in Hubei Province from 2013 to 2015

OBJECTIVE:To evaluate the quality of sampling drugs of Hubei province,and to provide reference for drug enter-prises and supervision department. METHODS:The data obtained by Hubei Institute for Food and Drug Control about provincial drug sampling and testing from 2013 to 2015 were summarized,the overall quality and unqualified items were statistically analyzed according to current standard. RESULTS & CONCLUSIONS:The number of total sampling batches was 7721 and that of the un-qualified batches was 333 (4.3%) during 2013-2015. Most of the unqualified samples came from distribution units (2.8%-9.0%) and medical institutions (5.2% -11.7%). Among all sample types, the herbal medicine has the highest unqualified rate (12.1%-37.4%),with the largest number of unqualified items(content determination,supplement inspection item,property,resid-ual amount of sulfur dioxide,ash content). The quality of pharmaceutical preparations is relatively stable,and the unqualified items correlated with drug types and dosage forms. The current issues of provincial drug sampling and testing are the variety of standard levels,unreasonable sampling resource allocation and the low level of the industry. Supervision department is proposed to readjust drug standard,reasonably plan sampling and testing process and strengthen production source controlling.
4.Therapeutic effect of nursing exercise rehabilitation on home patients with chronic heart failure

Objective:To explore and observe therapeutic effect of nursing exercise rehabilitation on home patients with chronic heart disease (CHF) .Methods :A total of 122 CHF patients ,who performed home‐rehabilitation in community from Jan 2012 to Feb 2015 ,were selected .According to sealed envelope extraction method ,patients were randomly and equally divided into routine nursing group (received routine nursing intervention ) and exercise intervention group (received early exercise intervention based on routine nursing group ) ,the intervention period was eight weeks for both groups .The 6min walking distance (6MWD) ,left ventricular ejection fraction (LVEF) ,scores of Minnesota living with heart failure questionnaire (MLHFQ) and self‐behavior management scale were measured and compared between two groups before and after nursing .Results:Compared with before nursing ,after nursing , there were significant rise in 6MWD ,LVEF and score of self‐behavior management in both groups , P<0.01 all . Compared with routine nursing group after nursing ,there were significant rise in 6MWD [ (482.44 ± 65.29) m vs . (546.24 ± 76.13) m] ,LVEF [ (35.69 ± 7.22)% vs .(38.83 ± 6.92)% ] and score of self‐behavior management [(89.19 ± 25.14) scores vs .(103.49 ± 22.44) scores] ,significant reductions in physical [(62.12 ± 17.97) scores vs . (42.32 ± 17.67) scores] ,emotional [ (67.76 ± 17.17) scores vs .(51.32 ± 16.41) scores] and social dimension score [ (72.43 ± 10.31) scores vs .(62.44 ± 10.45) scores] of MLHFQ in exercise intervention group ,P<0.01 all .Con‐clusion:Nursing exercise can significantly enhance heart function and athletic ability ,improve self‐care capacity , help to improve quality of life in home patients with chronic heart failure .
5.Effect of nasal continuous positive airway pressure and Bi-phase positive airway pressure on oxygen exchange indexes in newborns with mild to moderate neonatal respiratory distress syndrome

Objective To observe the effect of nasal continuous positive airway pressure (nCPAP) and Biphase positive airway pressure (BIPAP) on the oxygen exchange indexes in newborns with mild to moderate neonatal respiratory distress syndrome (NRDS).Methods After infusion of pulmonary suffactant into lung bilaterally,116 cases with mild to moderate NRDS were treated with nCPAP(nCPAP group,n =75) and BIPAP(BIPAP group,n =41) respectively.Oxygen exchange indexes including arterial partial pressure of oxygen [pa (O2)],modified oxygenation index (P/F),alveolar-arterial oxygen tension difference (A-aDO2),respiratory index (RI),artery/alveolar oxygen partial pressure ratio[pa (O2)/pA (O2)] and pulmonary shunt fraction (Qs/Qt) were measured or calculated at 0 (before treatment),2,8 and 24 h after being treated with nCPAP or BIPAP.Results Six different oxygen exchange indexes of Pa (O2),P/F,A-aDO2,RI,pa (O2)/pA(O2) and Qs/Qt in both groups didn't show statistical significance at 0 hour:pa(O2) (kPa) (6.1 ±0.6 vs 6.1 ±0.6,t =0.11,P >0.05),P/F(kPa) (15.59 ± 1.45 vs 15.71 ± 1.45,t =1.59,P > 0.05),A-aDO2(kPa)(6.04 ±0.64 vs 6.24 ±0.69,t =1.59,P >0.05),RI(6.0 ± 1.0 vs 6.2 ± 1.1,t =1.35,P > 0.05),p,(O2)/pA(O2) (0.24 ±0.02 vs 0.25 ±0.03,t =1.63,P >0.05)and Qs/Qt(%)(11.9±1.6 vs 11.6 ± 1.6,t =1.10,P > 0.05).A-aDO2,RI,pa (O2)/pA (O2) and Qs/Qt of BIPAP group had more improvement than those of nCPAP at 2 and 24 h,and oxygen exchange indexes at 2 h of BIPAP group and those of nCPAP group were:A-aDO2 (kPa) (3.83 ±0.49 vs 4.24 ± 0.67,t =18.26,P < 0.05),RI(2.7 ± 0.5 vs 3.3 ±0.7,t =20.59,P < 0.05),p,(O2)/pA(O2) (0.35 ±0.03 vs 0.32 ±0.04,t =15.35,P <0.05) and Qs/Qt(%) (8.8 ± 1.6 vs 9.8 ±2.0,t =7.68,P < 0.05) ; 24 h indexes in BIPAP group and the indexes of nCPAP group were:A-aDO2 (kPa) (2.29 ± 1.19 vs 3.07 ± 1.67,t=18.43,P <0.05),RI(1.4 ±1.4 vs 2.3 ± 1.6,t=25.02,P <0.05),pa(O2)/pA(O2) (0.49 ±0.10 vs 0.42 ±0.11,t =14.96,P <0.05) and Qs/Qt(%) (6.5 ±3.0 vs 8.5 ±4.4,t =9.59,P <0.05).pa(O2) and P/F of both groups didn't show statistical difference significantly [Pa (O2)(kPa) (12.6 ± 1.0 vs 12.7 ± 1.0,t =3.76,P > 0.05),P/F(28.49 ± 3.17 vs 31.85 ± 2.85,t =3.76.P > 0.05)].Nineteen cases in nCPAP group needed invasive mechanical ventilation,and only 3 cases in BIPAP group underwent the procedure;there was a significant difference (x2 =4.01,P < 0.05).Oxygen exchange indexes in both groups were gradually improved from 2 h,8 h to 24 h,but 22 deteriorated cases showed no improvement,who underwent invasive ventilation.Positive end-expiratory pressure (PEEP) positive airway pressure (EPAP) was (0.42 ± 0.19) kPa in BIPAP group,and (0.56 ± 0.23) kPa in nCPAP group (t =3.45,P < 0.01).Conclusions 1.BIPAP and nCPAP can improve the ability of oxygen exchange in newborns with mild to moderate NRDS,but BIPAP is more effective than nCPAP,which can shorten the possibility of invasive ventilation and reduce the PEEP level.2.p,(O2),P/F,pa (O2)/pA (O2) and RI may be more suitable for evaluating oxygen exchange for neonatal NRDS.

The intestine microbiota and the host interacts with each other.Resently,the association and causality between gut microbiota and cardiovascular diseases have been well studied.Some underlying mechanisms,especially metabolic pathways are known, including the role of trimethylamine N-oxide in the pathogenesis of atherosclerotic heart disease and heart failure and that of short chain fatty acids and their receptors in blood pressure regulation.
7.Analysis of the Utilization of TCM Injection in Our Hospital during the Period of 2011-2014

OBJECTIVE:To provide reference for rational application of TCM injection. METHODS:The utilization of TCM injection in our hospital during 2011-2014 was analyzed in terms of species,consumption sum,DDDs and DDC,etc. RESULTS:The type and consumption sum of TCM injection kept stable in our hospital during 2011-2014. TCM injection for activating blood circulation to dissipate blood stasis as Shuxuetong(Heilougjiang),Danhong,Shuxuening took up the front of consumption sum in 4 years. Top 3 TCM injections in the list of DDDs were Shuxuetong,Shuxuening(Heilougjiang), Danhong/Xueshuangtong corre-sponded to consumption sum. DDC of TCM injection were kept around 100 yuan. DUI ratio of TCM injection were almost equal to 1 or close to 1. CONCLUSIONS:The amount and DDDs of TCM injection are synchronous in our hospital,and the utilization of TCM injection is stable and rational basically in our hospital. TCM injection with suitable price and definite efficacy take up a large share,and the amount of TCM injection for clearing away heat and toxic material with anti-inflammatory effect increase rapidly.
8.Cyclooxygenase-2 and choroidal neovascularization

Choroidal neovascularization(CNV)is one of common causes of vision loss.The pathogenesis and development of CNV are a comprehensive process which is regulated by multiple factors and cytokines.Cyclooxygenase-2(COX-2)is an inducible isoform of cyclooxygenase and rate limiting enzyme in the prostaglandin biosynthesis pathway.COX-2 plays an important role in neovascularization by modulating vascular endothelial growth factor(VEGF),migration and apoptosis of endothelial cell.Recently,some experimental studies demonstrated that COX-2 involves in the information of CNV and the inhibitor of COX-2 can suppress CNV.These results provide a new prospect for the prevention and treatment of the CNV.Biological characteristics of COX-2 and its relationship with CNV are reviewed in this article.
9.Effects of endothelial nitric oxide synthase gene transfection on cytokines and cAMP production in human phagocytic cells

AIM and METHOD: Human endothelial nitric oxide synthase (eNOS) gene was transfected into human phagocytic cell U937 and the effects of gene transfer on cytokines and cAMP production were observed. RESULTS: A functional eNOS was stably expressed in transfected U937 cells, but NO release was undetectable in intact transfectants. However, eNOS gene expression upregulated tumor necrosis factor - a release and downregulated interleukin - 10 and cAMP production in either presence or absence of NOS inhibitor N? - monomethyl - L - arginine. CONCLUSION: The function of tranfected eNOS gene product showed cellular speciality. The effector molecule that changed the produced pattern of cytokines and cAMP in phagocytic cells seems not likely the nitric oxide.
10.Discussion on Establishment of Distinctive Protective Law on Traditional Chinese Medicine

OBJECTIVE:To discuss the issue of how to take advantage of intellectual property law to promote the sus?tainable development of our traditional Chinese medicine,and to establish the related legal system to protect the intellectual property of traditional Chinese medicine.METHODS:On the basis of literature review,the necessity and foundation of establishing distinctive protective law on traditional Chinese medicine were discussed.RESULT:There were internationally precedents of legislation on biodiversity and cultural diversity;Neither“Trade-Related Aspects of Intellectual Property Rights”nor other WTO files forbad individual country to establish its own policy according to its specific situation,however,these policies should not be against the fundamental principle of WTO;Moreover,foreign countries is paying much more at?tention on legislation of traditional Chinese medicine and herb than they used to.DISCUSSION:It is of great necessity to establish the distinctive protective law on traditional Chinese medicine,which can best be enforced by promoting the self de?velopment of traditional Chinese medicine,especially the investment and innovation in this field.
