1.Application of individualized psychological intervention in gastroscopy

Objective To investigate the role of individualized psychological intervention in gastroscopy.Methods 286 patients first underwent gastroscopy were randomly divided into the intervention and control groups,143 patients in each group. Individualized psychological intervention were provided for intervention group,and the patients of control group were just given routine nursing care. All the patients were measured by self rating anxiety scale (SAS) ,which were as measuring standards before and after intervention. At the same time,the blood pressure and heart rate were compared before and after intervention. Visual analogue scale (VAS) was observed to investigate the satisfaction of patients after operation. Results Compared with control group,anxiety rate and degree,blood pressure and heart rate in intervention group were lower( P < 0. 05 ). The test of VAS revealed that the satisfaction of patients in intervention group was significantly higher than that in control group ( P < 0. 05). Conclusion Individualized psychological intervention could improve negative emotion and relieve anxiety and affliction of patients underwent gastroscopy, and the satisfaction degree for gastroscopy was improved.
2.Expression of Axin in Peutz-Jeghers syndrome

Objective To study Axin expression in colorectal adenocarcinoma, colorectal adenoma, Peutz-Jeghers syndrome (PJS) polyp, and normal intestinal mucosa at mRNA and protein levels, and to explore the role of Axin in the pathogenesis of PJS. Methods The experiment samples, which were from the exsected normal mucosa, PJS polyps, adenomatous polyps and colorectal adenocarcinoma between Jan, 2001 and Mar, 2008 in Nanfang Hospital, each includes 32 sapamples. Immunohistochemistry was adopted to detect the Axin protein expression in each sample distinctively. Results transcriptase -polymerse chain reaction (RT-PCR) was used to detected and compare the Axin mRNA expression in 16 samples of each group. Results Of the tissues in each group, the positive Axin protein expression was located in the endochylema. 3.Prevalence of dyslipidemia and its influential factors in 2 028 students from primary and middle schools in Changsha

Objective:To determine the prevalence of dyslipidemia in students from the primary and middle schools and the influential factors,and to provide evidence for the prevention and control of dyslipidemia and relevant chronic diseases in primary and middle schools.Methods:A total of 2 028 students aged 7 to 17 from the primary and middle schools in Changsha were selected by stratified random cluster sampling.The contents of the study included questionnaire survey,physical measurement and blood tests for fasting blood triglyceride (TG),total cholesterol (TC),low-density lipoprotein cholesterol (LDL-C),and high-density lipoprotein cholesterol (HDL-C) levels.Non-high-density lipoprotein cholesterol (non-HDL-C) level was calculated by TC level minus HDL-C level.Chi-square test and non-conditional logistic regression model were used to analyze the factors that contributed to dyslipidemia.Results:The dyslipidemia rate was 18.6%,and the abnormal rates of TG,TC,LDL-C,non-HDL-C and HDL-C were 6.8%,5.6%,2.3%,4.2% and 8.6%,respectively.By chi-square test,the dyslipidemia rate in students with different ages,home locations,BMI groups,central obesity,time on watching TV or playing computer per day,and daily sleep time was statistically significant.Non-conditional logistic regression analysis showed that home location for the city (OR=1.332),overweight (OR=1.548),obesity (OR=2.201),central obesity (OR=1.695),watching TV or playing computer for more than 2 hours per day (OR=1.357),daily sleep time longer than 11 hours (OR=2.518) were the risk factors for dyslipidemia in students from the primary and middle schools.Conclusion:Nearly 1/5 primary and middle school students show dyslipidemia,which is associated with obesity and other bad behaviors.
4.Retrospective analysis in 13 children with Kasabach-Merritt phenomenon and review of literature

Objective To improve understanding of the clinical manifestations, diagnosis and treatment of childhood Kasabach-Merritt phenomenon (KMP). Methods The clinical data of 13 patients admitted for KMP to XXX from January 2010 to January 2016 was retrospectively analyzed, with a review of relevant literature. Results The patients were 10 males and 3 females. The age of presentation varied from newborn to 5 months. 12 patients had cutaneous manifestations, like petechiae, ecchymosis, jaundice, skin masses, etc, 1 patient had pleural effusion. The location of lesions varied. The laboratory hallmark consists of profound thrombocytopenia and hypofibrinogenemia with elevated D-dimers. The median time from initial presentation to diagnosis was 60 days. After approaches like surgery, corticosteroids, propranolol, interferon, sirolimus, etc, 10 patients got remission while 3 patients died. 6 patients treated with sirolimushad complete response. Conclusions KMP is characterized with vascular tumor, severe thrombocytopenia and consumptive coagulopathy. Clinically, KMP often presents with early-onset and delay in diagnosis. Surgery is an effective approach for KMP. Sirolimus appears to be a promising treatment for KMP.
5.Clinical Study of Bencycloquidium Bromide Nasal Spray in the Treatment of Rhinitis after Cold

OBJECTIVE:To evaluate the effects and safety of Bencycloquidium bromide nasal spray in the treatment of acute rhinitis after a cold. METHODS:A multicenter,dose parallel controlled,randomized,double-blind,placebo-controlled clinical tri-al was conducted. Two hundred and thirty-eight patients with acute rhinitis after a cold were selected and divided into group A(24 cases),B(24 cases),C(24 cases),D(24 cases),E(23 cases),F(24 cases),G(23 cases),H(24 cases),I(24 cases),J (24 cases). Group A-C were given Bencycloquidium bromide nasal spray 22.5μg,45μg,90μg,respectively,bid,spraying it once for each nostril. Group D-F were given Bencycloquidium bromide nasal spray 22.5 μg,45 μg,90 μg,respectively,tid,spraying it once for each nostril. Group G-I were given Bencycloquidium bromide nasal spray 22.5 μg,45 μg,90 μg,respectively,qid,spray-ing it once for each nostril. Group J was given placebo. All groups were treated for(4±1)d. Rhinorrhea score and continuous rhi-norrhea duration were compared among 10 groups,and the safety was evaluated. RESULTS:The rhinorrhea score and continuous rhinorrhea duration of 10 groups were improved significantly,with statistical significance (P<0.05). The improvement and trend of group I was slightly better than other 9 groups,without statistical significance(P>0.05). There was no statistical significance in the incidence of ADR among 10 groups(P>0.05). CONCLUSIONS:Bencycloquidium bromide nasal spray with 90 μg,qid times significantly improves rhinorrhea score and continuous rhinorrhea duration with good safety.
6.Investigation on the Occupational Ability and Humanistic Care Strategy among Undergraduate Nursing Students

Objective:To investigate the current status of occupational ability and explore the influencing factors among undergraduate nursing students,and thus to provide reference for improving their occupational ability.Methods:A sample of 1314 undergraduate nursing students from Shaanxi Province was investigated using self-designed Undergraduate Nursing Students Occupational Ability Evaluation Scale.Data were analyzed by SPSS21.0 software.Results:The undergraduate nursing students' occupational ability was at a moderate level (33.6 ± 5.0).The influencing factors of undergraduate nursing students' occupational ability included gender (P <0.001),grade (P =0.001),extracurricular activities (P =0.040) and extracurricular humanistic quality education (P < 0.001).Conclusion:The occupational ability of undergraduate nursing students is not high,and it is affected by many factors.And it can be improved by increasing extracurricular activities and extracurricular humanistic quality education.
7.Incidence and risk factors of postoperative cognitive dysfunction in patients underwent coronary artery bypass grafting surgery.

OBJECTIVE:
To investigate the incidence rate and the risk factors for postoperative cognitive dysfunction (POCD) in patients underwent coronary artery bypass grafting surgery.
METHODS:
A total of 147 patients underwent elective coronary artery bypass grafting (CABG) surgery between January to July 2013 were included in this study. POCD was diagnosed using a neuropsychological test battery. All enrolled patients were interviewed on the day before surgery, the seventh day and 3 months after surgery, respectively, by the same researcher, and were divided into two groups based on the results: the POCD group and the non-POCD group. The information, including age, sex, body mass index, educational status, comorbidities, history of smoking and drinking, ASA grade, left ventricular ejection fraction, operation method, duration of operations, regional cerebral oxygen saturation, the lowest haemoglobin concentrations and the haemoglobin concentration decline rate during the operation, tracheal catheter retention time, postoperative pain on visual analogue scales (VAS) and systemic inflammatory response syndrome score (SIRS score), were recorded based on a schedule of survey. Multivariate logistic regression was used to analyze the risk factors for POCD.
RESULTS:
A total of 101 patients finished this study. On 7 days and 3 months after surgery, 38 and 21 cases showed POCD, with an incidence rate at 37.6% and 20.8%, respectively. Interestingly, there was no significant difference in incidence of POCD between CABG and OPCABG group on both 7 days and 3 months after surgery (P>0.05). The logistic stepwise regression analysis indicated that the risk factors for POCD included advanced age (OR=1.177, 95%CI 1.071-1.292, P=0.001), the haemoglobin concentration decline rate (OR=1.334, 95%CI 1.152-1.545, P<0.05) and SIRS score (OR=2.815, 95%CI 1.014-7.818, P=0.047).
CONCLUSION
The incidence rate of POCD was 37.6% and 20.8% on 7 days and 3 months after surgery respectively. Advanced age, the haemoglobin concentration decline rate and SIRS score are independent risk factors for POCD in patients underwent coronary artery bypass grafting surgery.

8.The safety and efficacy of golimumab in treatment of 25 patients with active ankylosingspondylitis

Objective To evaluate the efficacy and safety of golimumab in patients with active ankylosing spondylitis (AS).Methods This was a randomized, double-blind, placebo-controlled trial. The subjects were randomized to receive either golimumab 50mg subcutaneously or placebo every 4 weeks. Patients in both groups received golimumab 50mg from week 24 to week 48. The primary endpoint was the proportion of at least 20% improvement in the Assessment of Spondyloarthritis International Society (ASAS20) at week 14. The secondary endpoints included at least 40% improvement in the Assessment of Spondyloarthritis International Society (ASAS40), ASAS partial-remission, Bath AS functional index, Bath AS disease activity index, Bath AS metrology index, enthesitis index and Jenkins sleep evaluation questionnaire. Results A total of 25 subjects were included in this study, 13 with golimumab and 12 with placebo. At Week 14, 6(46.2%) subjects achieved ASAS20 in golimumab group and 2(16.7%) in placebo group. Significant improvements of other efficacy endpoints were also found in golimumab group. Golimumab was safe and well to lerated. Most of the adverse events were slightly impaired liver function, where as elevated aspartate aminotransferase and/or alanine aminotransferase returned to normal without drug with drawal.Conclusion Golimumab improves AS activity, clinical symptoms and sleep disturbance in patients with active AS with good safety and tolerability.
9.Study on Protective Effects of Polygonum hydropiper Extract on Acute Gastric Mucosal Injury in Rats

OBJECTIVE:To study the protective effects of Polygonum hydropiper extract on acute gastric mucosal lesion (AGML)in rats. METHODS:48 rats were randomly divided into normal group(normal saline),model group(normal saline), positive group(ranitidine hydrochloride,0.05 g/kg),P. hydropiper extract low-dose,medium-dose and high-dose groups(2.7, 8.1,24.3 g/kg by crude drug),i.g. for consecutive 7 d,once a day. Except for normal group,other groups were given absolute ethyl alcohol to induce AGMI model after 1 h of last administration. 1.5 h after modeling,gastric mucosal lesion index of rats was calculated;the pathological changes of gastric tissue in rats were observed;nuclear factor E2 related factor 2(Nrf2)content and SOD activity in gastric tissue of rats were determined by ELISA. RESULTS:Compared with normal group,the gastric mucosa of model group was damaged obviously,there was blood capillary rupture in submucosa,gastric mucosal lesion index was increased significantly(P<0.01);Nrf2 content and SOD activity were significantly decreased in gastric tissue of rats(P<0.05 or P<0.01). Compared with model group,gastric mucosal lesion of rats was relieved to different extent;in positive group,P. hydropiper extract medium-dose and high-dose groups,gastric mucosal lesion index was decreased significantly(P<0.05),and Nrf2 content and SOD activity were increased significantly(P<0.05 or P<0.01). CONCLUSIONS:P. hydropiper extract has good protective effect on absolute ethyl alcohol-induce AGMI,the mechanism of which may be associated with raising Nrf2 content and enhancing SOD activity in gastric mucosal tissue.
10.Construction of key nursing technology system for hospital treatment of patients with nuclear radiation exposure

Objective To construct a key nursing technology system for the treatment of patients exposed to nuclear radiation in hospitals,and provide technical guidance and support for emergency nursing rescue in hospitals of nuclear radiation accidents.Methods A research group was composed of a team with rich experience in nuclear radiation accidents.Based on 4 scenarios of nuclear radiation accidents(including external irradiation,internal irradiation,external contamination,internal contamination),the literature search was conducted to form the first draft of the system.Delphi method was used to complete 2 rounds of expert letter consultation,and the final draft of the key nursing technology system for hospital treatment of patients with nuclear radiation exposure was constructed according to the revised opinions of experts.Results A total of 16 experts completed 2 rounds of correspondence.The effective recovery rates were 100%and 80%;the recommendation rates were 65%and 50%;the authority coefficients(Cr)were 0.778 and 0.797;the coefficient of variation(CV)of the 2 rounds of expert letter consultation was ≤0.25.Finally,a key nursing technology system for in-hospital treatment of patients with nuclear radiation exposure was formed,including 5 first-level indicators,26 second-level indicators and 74 third-level indicators.Conclusion The constructed key nursing technology system for hospital treatment of patients with nuclear radiation exposure is highly practical and scientific,and it is conducive to the formation of standardized nuclear radiation exposure treatment procedures,and provides a theoretical basis for the training and evaluation of nursing staff related to nuclear radiation exposure.
