This study is to investigate the effects of aqueous extract of Schisandra chinensis Baill (WWZ), kadsurin, schisandrin A, schisandrin B and schisandrol B on rat hepatic CYP3A. Rats received a daily gavage of aqueous extract of WWZ for different times. The livers were harvested after gavage and subjected to microsome preparation. Microsomal CYP3A activity was determined by measuring the amount of the metabolite of testosterone (6 beta-hydroxytestosterone) with HPLC. Aqueous extract of WWZ, kadsurin and schisandrin A were incubated with microsomes obtained from rat. Microsomal CYP3A activity was determined by HPLC. Primary hepatocytes were separated and extracted from rat, then were treated with aqueous extract of WWZ, schisandrin A, schisandrin B and schisandrol B. Then, the expression of CYP3A1 mRNA was analyzed by RT-PCR. As for the in vivo assay, aqueous extract of WWZ significantly inhibited the enzyme activity of CYP3A after 12 h gavage. The inhibitory effect was converted to inductive effect after 3-day gavage. Aqueous extract of WWZ could induce the enzyme activity of CYP3A after 6-day gavage. Aqueous extract of WWZ and kadsurin showed a dose-dependent inhibition of CYP3A (IC50 of 487.8 microg mL(-1) and 6.2 micromol L(-1), separately). In rat primary hepatocytes, aqueous extract of WWZ (2.5 mg mL(-1)), schisandrin A (0.1 micromol L(-1)), schisandrin B (0.1 micromol L(-1)) and schisandrol B (10 micromol L(-1)) increased significantly the expression of CYP3A1 mRNA by 23%, 55%, 42% and 27%, respectively. Aqueous extract of WWZ could show dual effect on the enzyme activity of CYP3A in rat in vivo. Meanwhile, kadsurin showed a dose-dependent inhibition of the enzyme activity of hepatic CYP3A in vitro. And schisandrin A, schisandrin B and schisandrol B showed significant inductive effect on the expression of rat CYP3A1 mRNA.
4.Effect of early fluid balance on the prognosis in severe acute pancreatitis.

OBJECTIVE:
To observe the correlation between early fluid resuscitation and prognosis in patients with severe acute pancreatitis (SAP).
METHODS:
SAP patients admitted to the department of critical care medicine of the People's Hospital of Chuxiong Yi Autonomous Prefecture of Yunnan Province from June 2018 to December 2020 were enrolled and analyzed retrospectively. All patients were given the routine treatment according to their condition and relevant diagnostic According to their different prognosis, enrolled patients were divided into death group and survival group. The differences in gender, age, acute physiology and chronic health evaluation II (APACHE II) and Ranson score on admission between the two groups were analyzed. Taking 24 hours as an observation day, the fluid inflow, outflow, and net balance at the first, second, and third 24 hours after admission were recorded, and the ratio of the fluid inflow at the first 24 hours to the total fluid inflow in 72 hours (FV24 h-1 st) was calculated as a study index. Using 33% as the standard, compare the proportion of patients in the two groups who achieved FV24 h-1 st < 33%. The differences of various indicators between the two groups were compared, and the effect of early fluid balance on the prognosis of SAP patients was analyzed.
RESULTS:
Eighty-nine patients were included in the study (41 in the death group, 48 in the survival group). There were no statistically significant differences on age (years old: 57.6±15.2 vs. 49.5±15.2), gender (male: 61.0% vs. 54.2%), APACHE II score (18.0±2.4 vs. 17.3±2.3), and Ranson score (6.3±1.4 vs. 5.9±1.2) between the death group and the survival group at the time of admission on the intensive care unit (ICU) (all P > 0.05). The fluid intake of the death group in the first 24 hours, the second 24 hours and the third 24 hours after admission to ICU was significantly higher than that of the survival group, and the difference was statistically significant (mL: 4 138±832 vs. 3 535±1 058, 3 883±729 vs. 3 324±516, 3 786±490 vs. 3 212±609, all P < 0.05), and the fluid inflow in the death group at the first 24 hours was greater than 4 100 mL. After treatment, the fluid outflow of the death group at the three 24-hour periods after admission on the ICU was an increasing trend, but it was still significantly less than that of the survival group at the three 24-hour periods (mL: 1 242±465 vs. 1 795±819, 1 536±579 vs. 2 080±524, 1 610±585 vs. 2 932±752, all P < 0.01). Due to the fact that the total fluid inflow and total fluid outflow in the three 24-hour periods in the death group were more than those in the survival group, the net fluid balances in the three 24-hour periods in the death group were still significantly more than those in the survival group finally (mL: 2 896±782 vs. 1 740±725, 2 347±459 vs. 1 243±795, 2 176±807 vs. 338±289, all P < 0.01). There was no difference in FV24 h-1 st between the death group and survival group [FV24 h-1 st > 33%: 56.1% (23/41) vs. 54.2% (26/48), P > 0.05].
CONCLUSIONS
Fluid resuscitation is an important method for early treatment of SAP, but it also has many adverse reactions. Fluid resuscitation indexes such as fluid inflow, outflow, net balance, and FV24 h-1 st within 24 to 72 hours after admission are related to the prognosis of patients with SAP, and can be used as indicators to evaluate the prognosis of SAP. The optimized fluid resuscitation strategy can improve the prognosis of patients with SAP.

5.Correlation between symptom clusters and health behavior compliance in patients receiving chemotherapy for gynecological malignant tumors

Objective:To explore the types of symptoms in patients receiving chemotherapy for gynecological malignant tumors and their correlation with health behavior compliance.Methods:In this cross-sectional study, totally 422 inpatients receiving chemotherapy for gynecological malignant tumors at a Class Ⅲ Grade A specialized maternal and children's hospital in Chengdu between January and June 2019 were selected by convenient sampling, and investigated with the Chinese version of the M. D. Anderson Symptom Inventory (MDASI) , the Revised Gynecological Malignant Tumor Symptom Module, and the Health Behavior Compliance Questionnaire. A total of 422 questionnaires were distributed, and 398 valid questionnaires were collected, accounting for an effective recovery rate of 94.31%.Results:The 398 patients receiving chemotherapy for gynecological malignant tumors had multiple symptoms. The top 3 symptoms were nausea (96.23%, 383/398) , distress (93.22%, 371/398) , and vomiting (92.46%, 368/398) , and the median symptom severity score was 1.00-7.00. The top 3 incidences of daily life disturbances were interpersonal relationship (76.63%, 305/398) , walking (76.38%, 304/398) , and work (72.61%, 289/398) , and the median score of daily life disturbances was 3.50-5.00. Exploratory factor analysis revealed 4 symptom clusters, namely, emotional symptom cluster, fatigue symptom cluster, sickness symptom cluster, and gastrointestinal symptom cluster. The results of Pearson correlation analysis showed that the total score of health behavior compliance was positively correlated with the scores of emotional, fatigue, sickness and gastrointestinal symptom clusters ( P<0.05) . Multiple linear regression analysis demonstrated that the sickness and gastrointestinal symptom clusters were the influencing factors of health behavior compliance ( P<0.001) . Conclusions:Patients receiving chemotherapy for gynecological malignant tumors are troubled by multiple symptoms. Nursing staff should establish an efficient symptom management model to improve the patient's symptom experience and health behavior compliance.
6.Association of anterior chamber angle open parameters with postoperative intraocular pressure following laser peripheral iridotomy

Objective To evaluate the relationship between anterior chamber angle and intraocular pressure (IOP) after laser peripheral iridotomy (LPI) treatment.Methods A retrospective cases control study was adopted.Fifty-eight patients (58 eyes) who were diagnosed as primary angle closure (PAC) were included in this study.Ultrasound biomicroscopy (UBM) parameters in angle opening distance (AOD),trabecular iris area (TISA) and angle recess area (ARA) examination were performed before LPI.The changes of intraocular pressure (IOP) were compared between different time-points (before and 1 hour,2 hours,8 hours,24 hours,2 weeks,6 months and 12 months after LPI).The patients were divided into IOP≤21 mmHg group (41 eyes) and IOP>21 mmHg group (17 eyes) after LPI.Relationship between anterior chamber angle and IOP after LPI treatment was explored.This study was approved by Ethic Committee of the Henan Eye Institute and informed consent was obtained from each patient.Results The IOPs were increased in 1 hour,2 hours after LPI and lowered in 2 weeks,6 months,12 months after LPI compared with IOP before LPI,with significant differences between them (all at P<0.01).Twelve patients suffered transient elevated IOP and recovered by self-healing or treatment.IOP of 4 patients were elevated after 6 months to 1 year follow-up.The IOPs in 2 weeks,6 months and 12 months after LPI were lowered compared with IOP before LPI,with significant differences between them (all at P<0.01).The UBM parameters were significantly increased in 2 weeks,6 months,12 months after LPI in comparison with IOP before IPL (all at P<0.01).IOP and UBM parameters values were significantly different between IOP>21 mmHg group and ≤21 mmHg group after LPI.Regression analysis indicated that ARA750 (OR =0.75,P<0.05) was correlated to the IOP after LPI rather than IOP before operation,AOD and TISA (P>0.05).Conclusions ARA750 value is correlated with the IOP variations after LPI.UBM structured observation can improve the surgical successful rates and safty and prevent complications.
7.Study on the Synthesis of Shikimic Acid Derivatives and Their Reversal Effects on Paclitaxel-resistant Human Breast Cancer Cells

OBJECTIVE:To struc turally modify shikimic acid ,and to investigate the reversal effects of its derivatives on paclitaxel-resistant human breast cancer cells MCF- 7/PTX. METHODS :Using shikimic acid as the lead structure ,1-position carboxyl group was structurally modified to synthesize a series of shikimic acid derivatives through esterification ,amidation, hydrogenation and reduction ,etc. Using non-drug resistant cells MCF- 7 as reference ,MTT assay was used to screen derivatives with inhibitory activity as well as half-inhibitory concentration (IC50)and reversal index (RI)of derivatives to MCF- 7/PTX. With the drug resistance-related transgelin 2 as the target ,the molecular docking of the active derivatives with the drug resistance-related protein was carried out by using Glide 1.0 computer-aided design software. RESULTS :Totally 15 derivatives were obtained (T1-T15), of which T 4-T15 were obtained for the first time. MTT assay showed that (3R, 4S, 5R) -N-benzyl-3, 4, 5-trihydroxy-1-cyclohexene-1-formamide(T7),(3R,4S,5R)-N-(3,4,5-trihydroxy-1-cyclohexenylmethyl)-benzylamine(T14), (3R,4S,5R)-3,4-O-isopropyl-5-O-acetyl-1-cyclohexene-1-methyl formate (T15)inhibited MCF- 7 and MCF- 7/PTX cells to a certain extent ;IC50 values of T 7,T14 and T 15 combined with pacliaxel to MCF- 7/PTX cells were significantly lower than that in negative control (Paclitaxel alone )group(P<0.05). RIs of T 14 and T 15 were higher ,and RIs of the highest dose were 8.8 and 9.3, which were equivalent to positive control verapamil (10.8). Th e results of molecular docking showed that the hydroxyl groups at positions 3,4 of T 7 could form multiple hydrogen bonds with ; Arg625 and Asp 627 in the catalytic region of transgelin 2. In addition to the hydrogen bond mentioned above at T 7,the mail:batistuta28@126.com secondary amine side chain at position 1 of T 14 could also form hydrogen bond with Glu 657 of transgelin 2. When the hydroxyl group on the T 15 mother nucleus was derived from the donor group ,the binding of the hydroxyl group to transgelin 2 was closer and the inhibition was enhanced. CONCLUSIONS : The derivatives T 7,T14 and T 15 have certain reverse activity to paclitaxel-resistant human breast cancer cells. The polyhydroxy structure of the mother nucleus is the main structural region of the hydrogen bond between shikimic acid and its derivatives and transgelin 2. The derivation of its power supply group or the introduction of secondary amines and hydrophobic groups into the 1-carboxyl group of shikimic acid is benifit for enhancing the drug resistance reversal effect of derivative .
