3.Study on the mRNA expression of potassium channel gene Kv1-5 in the developing rat brain

AIM To investigate the patterns of potassium channel gene Kv 1 5 mRNA expression in the developing rat brain by using RNase protection assay. METHODS (1) RNA was extracted from brain tissues by one step method, (2) The preparation of DNA template, (3) Synthesis of RNA probe in vitro transcription, (4) RNA:RNA. RESULTS Kv 1 5 was absent in embryo tissue, but significantly increased at postnatal day 25 with adult rat, the mRNA expression did not show any change. CONCLUSION there is close relationship between Kv 1 5 mRNA expression is pertinent to the development and function differentiation of rat brain.
4.ISO9000 and the quality management system in the digital hospital.

ISO9000 quality management system (ISO9000QMS) emphasize on the customer-oriented, managers' leadership and all staff's joining, adopt the process method and system management, spread the taking facts as a basis to make decision and improve consistently, and establish win-win relation with the suppliers. So, the digital hospital can adopt the ISO9000QMS. In order to establish the ISO9000QMS, the digital hospital should: (1) Design integrally, including analyzing the operation procedure, clarifying the job duties, setting up the spreading team and setting the quality policy and objectives: (2) Learning the ISO9000 quality standards; (3) Drawing up the documents, including the quality manual, program files and operation guiding files; (4) Training according the documents; (5) Executing the quality standard, including the service quality auditing, quality record auditing and quality system auditing; (6) Improving continually. With the establishment of ISO900QMS, the digital hospital can appraise more accurately, analyze quality matters statistically and avoid the interference of artificial factors.

The health check up flow of digital hospital can be consulted with the assembly line of industry factory. Because they have the following same features: highly specialized workstation, closeness and continuance, rhythm, balanced production, continuous production. The essential prerequisites are as the follows: The inspecting items and methods should be stable; advanced product mix and stable production design; standardized raw material, consumption, procedure, inspection method; there are lots of request for health inspection; the customers move at the least unit; the space arrangement should be reasonable; the time arrangement should be proportion. With the computer net, the digital inspection can achieves the raw material controlling accurately. The basis of check up line concerns about equipment, net and software, data collection, and personnel. The group technology is used in the health inspection flow design of the digital hospital in the field of items customers and zone redivided. The digital assembly linemic health inspect has the following stages: member registering, notice, check in, arrange order, time control, report, feedback and analysis. The assembly linemic has following advantages: increasing the productivity, the space utility, satisfaction of customer, fund returning, lowering the cost and ensuring the quality.

6.Expression and clinical significance of Wnt5a in gastric carcinoma

Objective To study the significance of Wnt5a gene expression in gastric carcinoma (GC). Methods The Wnt5a mRNA expressions in SGC7901 cell line and normal gastric mucosa GES-1 cell line were detected by RT-PCR. The Wnt5a protein expressions in 60 GC and 30 matched tumor-adjacent normal tissue samples were examined by immunohistochemistry using the streptavidin-peroxidase method.χ2 test was used to analyze Wnt5a expression on the clinical pathologic features of gastric carcinoma. Results Both Wnt5a mRNA and Wnt5a protein were significantly higher in SGC7901 cells than those in GES-1 cells. The positive rate of Wnt5a protein expression was also significantly higher in GC than that in tumor-adjacent normal tissue (P < 0.05). Positive expression of Wnt5a was associated with higher degree of regional lymph node metastasis and the later TNM stage (P < 0.05). Conclusion Higher expression of Wnt5a seems to promote invasion and metastasis of gastric cancer, and Wnt5a might play an oncogene-like role in the development of gastric carcinoma.
7.Relationship between the prevalence rate of hypertension and sleeping time in Chongqing area

Objective To investigate the prevalence rate of hypertension ,overweight ,obesity and obesity/overweight ,thus to explore the relationship between hypertension and sleeping time .Methods Healthy individuals having physical examinations in Health Management Center of Southwest Hospital were randomly selected to take questionnaire and physical examination ,and then we analyzed the correction relationship of hypertension and sleeping time .Results The prevalence of overweigh ,obesity ,hyperten‐sion and perhypertension was 36 .68% ,11 .36% ,22 .14% and 33 .43% respectively .The prevalence rate of hypertension for men was 2 fold as that of women .There was a negative correlation between hypertension and sleeping time .The prevalence of stage 2 hypertension tended to U.People who sleep time less than 5 h have more risk for Isolated systolic hypertension than who sleeping 6 h .Conclusion There is a high risk of hypertension and stage 2 and stage 3 hypertention for people whose sleep time less than 5 h or more than 10 h .
8.Analysis of clinical characteristics of decompensated cirrhosis patients with intestinal obstruction and related risk factors

Objective·To analyze the clinical characteristics of decompensated cirrhosis patients with intestinal obstruction and related risk factors.Methods·Clinical data of 1 783 decompensated cirrhosis patients treated between March 2010 and March 2016 were collected.Of them,128 (7.18%) patients with intestinal obstruction were screened as the observation group and 128 patients without intestinal obstruction were randomly selected as the control group.Clinical data of two groups were retrospectively investigated,clinical characteristics were compared and analyzed,and related risk factors were analyzed with the Logistic regression analysis.Results·The clinical symptoms of decompensated cirrhosis patients with intestinal obstruction were hidden and misdiagnoses or delayed diagnoses were common.The incidences of abdominal pain,abdominal distension,vomiting,stop exhaust defecate,ascites,electrolyte disorders,fever,and spontaneous peritonitis were significantly higher in the observation group than in the control group (P < 0.05).The results of univariate analysis showed that age,history of abdominal surgery,white blood cell count,serum sodium,serum potassium,neutrophil percentage,and serum albumin were risk factors for decompensated cirrhosis patients with intestinal obstruction.The results of multivariate analysis indicated that age,history of abdominal surgery,white blood cell count,serum sodium,serum potassium,neutrophil percentage,and serum albumin were independent risk factors for decompensated cirrhosis patients with intestinal obstruction.Conclusion·Decompensated cirrhosis patients with age ≥ 50 years old,a history of abdominal surgery,the abdominal cavity infection,low potassium,hyponatremia,and lower serum albumin are likely to develop the intestinal obstruction.
9.The risk factors of prehypertension and the relationship between blood pressure and atherosclerosis

Objective To explore the risk factors of prehypertension and the relationship between blood pressure and atherosclerosis.Methods The data of 456 cases in the Department of General Medicine were cross-sectional analyzed,including 174 subjects with normotension and 282 with prehypertension.The information was consisted of demographic characteristics,blood pressure,blood biochemical metabolism index,brachia-ankle pulse wave velocity (baPWV),ankle-brachial index (ABI) and carotid intima media thickness (CIMT).Results Compared to normotension group,the levels of systolic pressure,diastolic pressure,pulse pressure,body mass index,fasting blood glucose,glycosylated hemoglobin,triglyceride,uric acid,and C-reactive protein in prehypertension group were significantly increased (P < 0.05) ; and high-density lipoprotein cholesterol was significantly decreased (P <0.05) ; the body mass index(OR =1.185),triglyceride(OR =1.302),and fasting blood sugar (OR =1.690) were the independent risk factors of prehypertension ; baPWV and CIMT in prehypertension group were higher,but ABI and artery atheromatous plaque rate were not obvious changed.When baPWV or CIMT were set as the dependent variables,multiple linear regression analysis showed that systolic blood pressure(β =0.226,P =0.007),fasting blood glucose(β =0.209,P =0.018),and age(β =0.279,P =0.002) were risk factors of baPWV;systolic blood pressure(β =0.118,P =0.015),body mass index(β =0.109,P =0.001),and age(β =0.396,P =0.001) were risk factors of CIMT.Conclusions Body mass index,triglycerides,and fasting blood sugar were the independent risk factors of prehypertension.The early subclinical damage of hardening of the arteries was occurred in the prehypertension cases,and systolic blood pressure was closely related with baPWV or CIMT.
10.Clinical application of different recruitment maneuvers in children with congenital heart disease complicated by acute respiratory distrees syndrome after surgery

Objective To compare the individual effects of three recruitment maneuvers (RM) in children with congenital heart disease complicated by postoperation acute respiratory distress syndrome (ARDS).Methods A prospective single-blind randomized controlled trial was conducted.Thirty-two children with congenital heart disease complicated with ARDS after open-heart surgery undergoing mechanical ventilation were randomized into three groups,to whom three different RM was respectively performed,namely sustained inflation (SI),progressively increased positive end expiratory pressure (IP) and pressure control ventilation (PCV).Blood gas analysis was done every 6 hours,and the lung RM was performed if oxygenation index (OI) ≤ 300 mmHg (1 mmHg =0.133 kPa).The OI,the dynamic lung compliance (Cdyn),as well as the parameters of hemodynamics before,during and after RM for 15,30,and 60 minutes were recorded and analyzed before and after RM.Results During the treatment process,the OI was significantly increased during the process and 15,30 or 60 minutes after RM compared with that before RM,with no statistical difference among groups (F value was 1.027,0.403,0.264,0.172,0.159,and P value was 0.367,0.671,0.769,0.843,0.853).The Cdyn at all time points in each group was also significantly increased,but there was no statistical difference among groups (F value was 0.009,0.015,0.206,0.010,0.389,and P value was 0.991,0.985,0.814,0.990,0.683).In the process of RM,the heart rate (HR) and mean arterial pressure (MAP) of the children were lowered compared with those before RM [HR (bpm):131.67 ± 9.56 vs.138.93 ± 5.22 in SI group,133.27 ± 9.54 vs.140.33 ± 7.74 in IP group,137.13 ± 7.39 vs.142.40 ± 9.18 in PCV group,all P < 0.01;MAP (mmHg):55.07 ± 4.43 vs.65.87 ± 4.46 in SI group,58.82 ± 6.04 vs.64.02 ± 7.65 in IP group,57.89 ± 4.71 vs.65.36 ± 5.37 in PCV group,all P < 0.01],but it recovered immediately.CVP in all three groups was increased during RM [cmH2O (1 cmH2O =0.098 kPa):11.60±0.99 vs.5.53±0.74 in SI group,10.33± 1.35 vs.5.40±0.74 in IP group,10.20±0.94 vs.5.80±0.68 in PCV group,all P < 0.01].There was significant difference in CVP during RM among three groups (F =7.327,P =0.002),and CVP in SI group was higher than that of other two groups (both P < 0.05).CVP returned to the former level in 15 minutes after RM in IP and PCV groups,and recovered in 30 minutes in SI group.Conclusions All of the RM methods can effectively improve oxygenation and pulmonary compliance of the children with complication of ARDS,and they complement the inadequacy of lung protective ventilation.PCV and IP are more effective than SI in the uniform re-expansion of alveoli after RM and recovery of hemodynamics.
