Objective:To study the chemical constituents of the methylene chloride of Rubus chingii Hu..Methods:Chromatography on silica gel column,Sephadex LH-20 column,and recrystallization technique were used to isolate and purify the compounds.Spectroscopy methods including EI-MS,ES-MS,1H-NMR,13CNMR,HMQC,and HMBC were used to elucidate the structures of compounds.Results:Ten compounds were obtained and 9 compounds were identified as:hexacosanol(Ⅰ),?-sitosterol(Ⅱ),4-hydro-3-methlbenzal acid(Ⅲ),4-hydrobenzal dehyde(Ⅳ),oleanolic acid(Ⅴ),stigmast-5-en-3-ol,oleate(Ⅵ),1H-2-indenone,2,4,5,6,7,7a-hexahydro-3-(1-methylethyl)-7a-methyl(Ⅷ),4-hydroxy-3-methoxybenzoic acide(Ⅸ),and liballinol(Ⅹ).Conclusion:Compounds Ⅰ,Ⅲ,Ⅳ,Ⅵ,Ⅷ,and Ⅹhave been obtained from Rubus chingii for the first time.

Objectives:To investigate effect of the methionine/valine(Met/Val) depleted enteral nutrition on tissue distribution,RNA,DNA and protein metabolism in tumor bearing(TB) rats. Methods:SD rats underwent catheterization of jejunostomy.A suspension of Walker 256 sarcocarcinoma cells was subcutaneously inoculated.The TB rats were randomly divided into 4 groups:A,B,C and D.The TB rats received jejunostomy feedings supplemented with balanced amino acid,methionine/valine depleted nutnition for 6 days before injection of 7.4?10 5 Bq 3H methionine/vatine.The tissue uptake and 3H Incorporation rate of radioactity into RNA,DNA and proteins in tumor tissues at 0.5,1,2,4 h after injection were assessed. Results:①Uptakes of 3H by tumor tissues in group B at 2,4 h after injection were (8.09?0.12)% and (7.81?0.17)% respectively and they were (6.59?0.24)% and (6.16?0.06)% in group A( P

Objective To summarize the experience of aortic root surgery in Stanford A aortic dissection operations.Methods From January 2005 to September 2010, the clinical data of 59 patients with Stanford A aortic dissection was analyzed. There were 43 men and 16 women , ranging in age from 21 to 74 years old, duration of disease varied from 16 hours to 27 days. Among the group, 31 complicated by aortic valve incompetence, 12 Marfan syndrome, 9 single lower limb blood supply dysfunction, 6 right coronary artery involvement. All cases received aortic root surgery under deep hypothermic circulatory arrest. Bentall procedure was performed in 31 patients, Wheat procedure in 15 patients and David procedure in 13 patients.Results The time of cardiopulmonary bypass in the group was 149 to 204 min with an average of ( 171 ± 19) min,and the cross clamp time was 81 to 122 min with an average of (104 ±13) min, and the arrest time was 30 to 47 min with an average of (39 ±7) min. There was 1 case of operative death, which was treated on an emergency basis. Postoperative complications occurred in 20 cases. 17 cases experienced temporary mental dysfunction, 2 cases were infected with MRS A, 1 of which died from MODS, 1 case of single lower limb blood supply dysfunction remained after the first operation and recovered by reoperation (bilateral femoral bypass operation) . All cases were reexamined before discharge, postoperative mean LVEF was (56. 3 ±3.4)% (ranged 51% -62%), aortic annular diameter varied from 22 mm to 27 mm, aortic sinus diameter range from 23 mm to 31mm. 51 patients were followed up, with a mean follow-up time of (24.9 ± 17.2) months (ranged 2-54 months). 54 patients of Stanford A aortic dissection survived well with normal lives and activities. Four non cardiac deaths, one was chronic renal failure, the others were brain hemorrhage. Conclusion Prognosis and operative security of Stanford A aortic dissection patients can be significantly improved by individualized aortic root surgery.

Objective To assess the feasibility of quantitative measurement of subcutaneous and supraclavicular adipose tissue with iterative decomposition of water and fat with echo asymmetry and least square estimation-iron quantification sequence (IDEAL-IQ).Methods Totally 87 normal young female volunteers (20-35 years old) were recruited and divided into body mass index (BMI)＜24 kg/m2 group (n=72) and BMI≥24 kg/m2 group (n=15).Fat fraction (FF) and T2* relaxation rate (R2*) of supraclavicular adipose tissue,chest wall adipose tissue,abdominal wall adipose tissue and liver were measured,respectively.The differences of FF and R2* value of chest wall,subcutaneous and supraclavicular adipose tissue were compared between two groups,and the correlation between FF,R2* value of supraclavicular adipose tissue and BMI was respectively analyzed.Results FF of supraclavicular adipose tissue ([80.99 ± 7.73]％) was lower than that of chest wall subcutaneous adipose tissue ([93.04 ± 1.55] ％,P＜0.001).R2* of supraclavicular adipose tissue ([65.52±23.59]Hz) was higher than that of chest wall subcutaneous adipose tissue ([38.82±7.11]Hz,P＜0.001).The differences of FF and R2* values of supraclavicular adipose tissue,chest wall,abdominal wall subcutaneous adipose tissue and liver were significant between BMI＜24 kg/m2 group and BMI≥24 kg/m2 group (all P＜0.05).There was positive correlation (r=0.601,P＜0.001) between FF of supraclavicular adipose tissue and BMI and negative correlation (r=-0.409,P =0.001) between R2* of supraclavicular adipose tissue and BMI.Conclusion IDEAL-IQ technique can quantitatively assess the difference between subcutaneous and supraclavicular adipose tissue in healthy young women.FF and R2 * value has significant difference between the above mentioned positions.

Objective:To analyze the risk factors affecting regional lymph node metastasis in salivary gland mucoepidermoid carcinoma (MEC) and to establish a nomogram model for individually predicting lymph node metastasis in salivary gland MEC.Methods:The clinical data of 2 152 patients with salivary gland MEC from 1975 to 2020 were collected from the Surveillance, Epidemiology, and End Results (SEER) database of the National Cancer Institute. The collected data were divided into training cohort (1 506 cases) and validation cohort (646 cases) according to the ratio of 7∶3. Single-factor regression and multi-factor logistic regression were used to screen factors related to local lymph node metastasis in salivary gland MEC, with constructing of a nomogram. Calibration curve, receiver operating characteristic (ROC) curve, area under the ROC curve (AUC) and decision curve analysis were used to evaluate model performance in the validation cohort and the total cohort. Statistical tests were performed using SPSS (26.0) and R (4.3.0) software.Results:Multivariate logistic regression results showed that M stage [ OR(95% CI):12.360(3.295-46.365), P=0.014], pathological grade Ⅱ、Ⅲ、Ⅳ[ OR(95% CI): 1.956(1.329-2.879), 9.654(6.309-14.772), 9.298(6.072-14.238), P<0.001], T staging T2, T3, T4[ OR(95% CI): 1.706(0.932-3.124), 3.021(1.790-5.096), 3.311(1.925-5.695), P<0.001], and gender [ OR(95% CI):0.759(0.593-0.972), P=0.029] were independent factors affecting local lymph node metastasis in salivary gland MEC. Through verification in the validation cohort and the total cohort, the AUC values were greater than 0.8, and the calibration curve was close to the perfect reference line, proving that the constructed nomogram model had good specificity and sensitivity for predicting local lymph node metastasis in salivary gland MEC. Conclusion:M stage, pathological grade, T stage, and gender are risk factors for predicting regional lymph node metastasis and the established-nomogram has good predictive performance for local lymph node metastasis in salivary gland MEC.

Objective:To establish and validate a radiomics nomogram based on MR for predicting cervical lymph node metastasis in laryngeal cancer.Methods:One hundred and seventeen patients with laryngeal cancer who underwent MR examinations and received open surgery and neck dissection between January 2016 and December 2019 were included in this study. All patients were randomly divided into a training cohort ( n=89) and test cohort ( n=28) using computer-generated random numbers. Clinical characteristics and MR were collected. Radiological features were extracted from the MR images. Enhanced T1 and T2WI were selected for radiomics analysis, and the volume of interest was manually segmented from the Huiyihuiying radiomics cloud platform. The variance analysis (ANOVA) and the least absolute shrinkage and selection operator (LASSO) algorithm were used to reduce the dimensionality of the radiomics features in the training cohort. Then, a radiomic signature was established. The clinical risk factors were screened by using ANOVA and multivariate logistic regression. A nomogram was generated using clinical risk factors and the radiomic signature. The calibration curve and receiver operator characteristic (ROC) curve were used to confirm the nomogram′s performance in the training and test sets. The clinical usefulness of the nomogram was evaluated by decision curve analysis (DCA). Furthermore, a testing cohort was used to validate the model. Results:The radiomics signature consisted of 21 features, and the nomogram model included the radiomics signature and the MR-reported lymph node status. The model showed good calibration and discrimination. The model yielded areas under the ROC curve (AUC) in the training cohort, specificity, and sensitivity of 0.930, 0.930 and 0.875. In the test cohort, the model yielded AUC, specificity and sensitivity of 0.883, 0.889 and 0.800. DCA indicated that the nomogram model was clinically useful.Conclusion:The MR-based radiomics nomogram model may be used to predict cervical lymph node metastasis of laryngeal cancer preoperatively. MR-based radiomics could serve as a potential tool to help clinicians make an optimal clinical decision.
