Objective To investigate the effect of adrenomedullin on epithelial barrier function of Caco-2 cells under hypoxia/reoxygen-ation injury and the molecular mechanism.Methods Build hypoxia/reoxygenation injury model,Caco-2 cells were randomly divided into three groups:normal control group( N) ,hypoxia/reoxygenation group( H/R) and hypoxia/reoxygenation plus 1 μmol/L adrenomedullin group(H/R+AM).Transepithelial electrical resistance(TEER) was determined,and the laser confocal microscope was used to detect the construction of tight junction.The protein expressions of tight junction proteins (Occludin and ZO-1) and nuclear factor kappa B(NF-κBp65) were examined by using western blotting.Results Pretreatment of AM significantly attenuated the disruption of morphological structure of tight junction caused by H/R.The TEER of N group,H/R group and H/R+AM group were(157.68 ±5.54),(96.06 ±3.42),(134.56 ± 4.72) Ohm/cm2 respectively.After AM pretreatment, the TEER were significantly raised compared with that in H/R group by 40.00%(P<0.05).The relative protein expressions of Occludin and ZO-1 in N group,H/R group and H/R+AM group were [(0.43 ±0.03), (0.27 ±0.04)],[(0.20 ±0.03),(0.15 ±0.07)],[(0.32 ±0.15),(0.21 ±0.03)]respectively.After AM pretreatment,the protein ex-pression of Occludin and ZO-1 were significantly raised compared with that in H/R group(by 60.00% and 40.00%).Simultaneously,the relative protein expressions of NF-κBp65 in N group,H/R group and H/R+AM group were (0.53 ±0.30),(2.89 ±0.16),(1.75 ±0.25) respectively.And H/R induced NF-κBp65 activation was significantly inhibited by AM treatment (decreased as compared with H/R group by 40.00%).Conclusion AM can attenuate intestinal epithelial barrier dysfunction by regulating the expression of intestinal tight junction pro-teins.The inhibition of NF-κB activation may be involved in this mechanism.

Objective To explore the clinical significance on power spectra assay of heart rate (HRPSA) to cardiovascular autonomic nervous system function in patients with interictal epilepsy.Methods HRPSA of 48 patients with epilepsy and 31 healthy controls were assessed in the study. their heart rate variability(HRV) was analysed.Results HRV was significantly reduced in patients with interictal epilepsy, especially in the patients received carbamazepine treatment.Conclusion The cardiovascular autonomic nervous system function exists disturbance in patients with interictal epilepsy,especially in the patients received carbamazepine treatment.

Objective To investigate effect of a new lumbar dynamic internal fixation system(DIFS)on the stability and intradiscal pressure of lumbar segment.Methods Eight fresh calf lumbar specimens were collected to build test models,which were loaded with pure moments of 8 N·m in flexion/extension,lateral bending,and rotation planes.The test states included intactness,destabilization,dynamic fixation,semi-rigid fixation,and solid fixation.The data were collected from the third loading cycle to determine range of motion and neutral zone of corresponding segments,and the intradiscal pressure.Results Destabilization significantly increased segmental range of motion and neutral zone in three motion planes without obvious influence on intradiscal pressure.The three fixation systems were all able to stabilize and load destabilizing segments.DIFS restored the range of motion of unstable segment to 77％,60％,61％ and 70％ of the intact level in anterior flexion,posterior extension,lateral bending and rotation planes.Meanwhile,DIFS bore 45％,29％ and 40％ of tbe intradiscal load in lateral bending,rotation and flexion directions,but 100％ of the intradiscal load in back extension.The stability and intradiscal pressure of adjacent segments were not affected by the test states.Conclusion DIFS can effectively restore range of motion of unstable segments and share appropriate intradiscal load in most directions.

Objective To create a new animal model which can preferably simulate the human's scoliosis. Methods Forty-five 4-week-old female Wistar rats were randomly divided into 3 groups. Group 1 underwent subcutaneous left scapula-to-contralateral ilium tethering procedure with a non-absorbable suture,which made the spine convex towards right side, and then removed forelimbs and tails of rats to create the bipedal rats. Tethering sutures were cut at postoperative 8th week, and the spines of rats were then observed during 2 weeks. Group 2 were the same as group 1 but in which scapula-to-ipsilateral ilium tethering procedure was performed. Group 3 were the same as group 1 except that the bipedal rats were not created. All postoperative rats were fed separately in special high cages for group 1 and group 2 or in standard cages for group 3. Operative duration, postoperative first food-taking duration, operative mortality, body weight, relative length of the spinal cord, Cobb angles in the coronal and sagittal planes, modeling mortality, the incidences of re-operation, scoliosis and vertebral rotation among the 3 groups were compared. Results There were no significant differences in the degree of scoliosis or kyphosis among the 3 groups at the time of initial tethering. At 2 weeks after tether release, the incidence of vertebral rotation was significantly higher in group 1 than group 2 (86.7% vs. 33.3%), and the degree of scoliosis or kyphosis was significantly larger in group 1 than group 3 (coronal plane: 30.2°±10.4° vs. 20.5°±7.7°; sagittal plane: 55.6°±10.0° vs. 43.8°±6.3°). There was no significant difference in postoperative first food-taking duration among the 3 groups, and no death of rats occurred in all groups during intraoperative and postoperative 6 hours, although operative duration was significantly longer in group 1 or group 2 than group 3. There were no significant differences in body weight,relative length of the spinal cord, modeling mortality, the incidences of re-operation and scoliosis among the 3 groups. Conclusion The bevel and axial loads of the vertebrae can significantly enhance the incidence of vertebral rotation and the degree of kyphoscoliosis, respectively. The scoliosis model created by scapula-to-contralateral ilium tethering procedure in bipedal rats can preferably simulate the human's scoliosis.

ObjectiveTo observe the clinical effects of mirabilite external application and neostigmine injection at Zusanli acupoint combined with Xuebijing intravenous injection on patients with severe sepsis.Methods A history-prospective controlled study was conducted. Patients with severe sepsis admitted to the Fourth People's Hospital of Jiangsu University from January 2012 to November 2015 were enrolled. Twenty-one cases admitted from January 2014 to November 2015 were assigned as a research group and treated with application of mirabilite external application, Zusanli acupoint injection of neostigmine combined with intravenous Xuebijing injection; 22 patients with Xubijing treatment from January 2012 to November 2015 were included in a Xubijing group; 21 patients with routine therapy from January 2012 to November 2013 were included in a control group. The changes of white blood cell count (WBC), C-reactive protein (CRP), procalcitonin (PCT), mean arterial pressure (MAP), oxygenation index (OI), serum creatinine (SCr), acute physiology and chronic health evaluation Ⅱ(APACHEⅡ) score, sequential organ failure assessment (SOFA), gastrointestinal function score, blood platelet count (PLT), plasma prothrombin time (PT), activated partial thromboplastin time (APTT), and D-dimmer in three groups before and after treatment were observed; the length of stay in ICU and the 28-day mortality were compared among the three groups. Results Compared with those before treatment, WBC, PCT, CRP, SCr, APACHE Ⅱ score, SOFA score, gastrointestinal function score, APTT, PT, and D-dimer were all obviously lower after treatment for 7 days in various groups; OI, MAP, PLT were significantly higher, and the improvement degree of WBC, PCT, CRP, SCr, APACHE Ⅱ score, SOFA score, gastrointestinal function score, OI in research group was more significant than those of control group and Xubijing group [WBC (×109/L): 7.52±0.75 vs. 12.87±4.13, 10.88±0.66, PCT (μg/L): 1.14±0.55 vs. 6.32±1.39, 3.47±1.94, CRP (mg/L): 21.0±9.2 vs. 65.0±13.6, 35.0±13.9, OI (mmHg, 1 mmHg = 0.133 kPa): 357.0±20.4 vs. 295.0±20.4, 309.0±21.4, SCr (μmol/L): 7.89±2.35 vs. 14.33±9.17, 11.27±4.65, APACHE Ⅱ score: 10.38±0.75 vs. 18.27±2.57, 13.09±4.10, SOFA score: 1.05±0.66 vs. 6.01±2.33, 3.26±1.03, gastrointestinal function score: 0.31±0.11 vs. 2.01±0.46, 1.85±0.29, all P < 0.05], the improvement of PLT, APTT, PT, D-dimer indicators in the research group was more significant than those in control group, but the above improvement compared with that in Xubijing group, no statistically significant difference was found [PLT (×109/L): 220.32±32.44 vs. 105.56±44.03, 170.56±54.03, APTT (s): 30.16±5.23 vs. 39.09±10.11, 29.56±6.85, PT (s): 10.74±1.25 vs. 13.56±4.65, 10.05±1.44, D-dimer (mg/L): 1.13±1.98 vs. 4.03±2.65, 1.13±0.54]. The length of stay in ICU was shortened in research group than that in control group and Xubijing group (days: 13.23±9.45 vs. 25.22±15.46, 18.56±10.33, P < 0.05); the 28-day mortality was significantly lower in the research group than that in the control group [28.6% (6/21) vs. 47.6% (10/21), P < 0.05], but there was no statistically significant difference compared with that in Xubijing group [28.6% (6/21) vs. 31.8% (7/22), P > 0.05]. Conclusions The treatment of mirabilite external application and neostigmine injection at Zusanli acupoint combined with Xuebijing intravenous injection can obviously improve the clinical symptoms, blood coagulation indexes and organ functions, reduce the levels of inflammatory indexes, shorten the time of the length of stay in ICU and elevate the survival rate of patients with severe sepsis.

Objective To observe colorectal tumor's growth in hyperglycemia mice and its vascular endothelial growth factor (VEGF)'s expression, insulin-like growth factor-1 (IGF-1)'s variation of blood through the experiment, then to ascertain whether type 2 diabetes mellitus (T2DM) danger factors to promote colorectal cancer happen and progress or not. Methods The mouse model of colorectal cancer combined T2DM was established. The volume of tumor was observed. After 5 weeks, all mice were executed and IGF-1 in the blood and the expression of VEGF in the tumor tissue was examined. Objective To investigate the protective effect of matrine on lung injury associated with single lung ventilation during thorac-ic surgery,and to explore and consummate the prevention and control measures of single lung ventilation related lung injury.Methods To-tally 97 cases of non small cell lung cancer patients were randomly divided into the observation group ( 50 cases ) and the control group (47 cases) .The two groups of patients were given the same way of anesthesia.Patients of the observation group received intravenous drip of 2 mL matrine injection which were dissolved in 100 mL saline solution 30 min before anesthesia, while patients of the control group were merely given 100 mL saline solution 30 min before anesthesia.The pulmonary shunt fraction( Qs/Qt) ,xanthine oxidase( XOD) ,myeloperoxi-dase(MPO),superoxide dismutase(SOD) and nitric oxide(NO) of the following points in time were compared:before anesthesia induction (T0),the instant of OLV (T1),60 minutes after OLV(T2),120 minutes after OLV(T3),after lung inflation (T4),and 24 hours after opera-tion ( T5) .Results At the time of T1 to T4,pulmonary shunt fraction of the two groups were both significantly higher than that at T0 with sig-nificant difference ( P<0.05) ,but there was no statistical difference in terms of intra-group comparison at different time points ( P>0.05) .The PMN counts of the two groups at the time of T2 to T5 were significantly higher than that of T0 with significant difference (P<0.05),and the PMN counts at the time of T2 to T5 in the control group were significantly higher than that in the observation group with significant difference (P<0.05).The levels of serum XOD,MPO,and SOD at T2 to T4 in both of the two groups were significantly higher than that at T0 with signif-icant difference (P<0.05),and the serum levels of XOD,MPO and SOD at T2 to T4 in the control group were significantly higher than those in the observation group with significant difference (P<0.05).The levels of serum NO at T2 to T4 in both of the two groups were significantly higher than that at T0 with significant difference (P<0.05),and it was significantly lower than that in the observation group with significant difference (P<0.05).Conclusion The matrine pretreatment of lung injury in the patients with single lung ventilation has a protective effect, which can reduce the levels of oxidative stress and promote the NO release in patients by reducing PMN,XOD and MPO levels.
